ENHANCING PARTICIPATION OF IND...IGENOUS PEOPLE TO ADDRESS DISCRIMINATION AND PROMOTE EQUITY IN HEALTH SYSTEMS

Enhancing Participation of Ind...igenous People to Address Discrimination and Promote Equity in Health Systems

|
New research will help indigen...ous peoples in Latin America tackle the inequity, racial discrimination, and other barriers they face in accessing their right to health care.Geographic and language barriers, combined with ethnic and racial discrimination, are some of the reasons why indigenous peoples experience social exclusion. These factors impede their access to good health care. They also make it difficult for them to participate in the public sphere, and then to influence health policies and practices. Guatemala's progressive social reforms recognize the right to health for all citizens. This research project aims to strengthen the governance of the country's health system and enhance equity through the active participation of indigenous communities. Researchers will share their knowledge to implement a rights-based intervention in rural Guatemala. They will evaluate the intervention's impact on discriminatory treatment in health facilities, with a focus on providing communities with tools and strategies to monitor existing public policies and healthcare services. The impact evaluation will then help determine the effectiveness of the intervention in achieving better community participation in health policies, and reducing discriminatory practices in service delivery. Results will be disseminated throughout the project by means of development blogs, virtual networks, educational newsletters, screening of audiovisuals, meetings and workshops, educational radio programs, and policy briefs. Recipients will be local and national health policymakers and managers. The findings will also be shared with various stakeholders in Ecuador, a country that is interested in launching a similar intervention. Other expected outcomes include higher levels of empowerment among indigenous communities. Findings are expected to inform policy development in Central America and increase the capacities of the research teams as they share methods and knowledge.

The project aims to support th...e 6th National Survey on Maternal and Child Health in Guatemala, a key tool to influence decision-makers and support the formulation and monitoring of food security policies, programs and budgets by the government and donors. Guatemala's Ministry of Health and the National Statistics Institute lead the survey, with the technical support of UNICEF. Project activities include: (1) sharing information and analysis on food and nutrition security concerning children under five and men and women of reproductive age; (2) gathering information from women, men and households on their health and nutritional status, levels of mortality, fertility and reproductive patterns to highlight inequalities and determining factors, such as sex, age, location, urban-rural settlement, ethnicity, and education levels; and (3) supporting the Government of Guatemala to account to its citizens on the performance of the Zero Hunger Pact.

This project contributes to th...e construction of a new 200-bed provincial hospital in Gonaïves. It is one of three parts of a $30-million initiative. This component supports the design, construction of the majority of the hospital, its medical equipment and hospital management. As the partner agency, UNOPS is responsible for the implementation of these activities, including the $14.5 million contract for the construction of the hospital. The Gonaïves hospital initiative aims to improve access to and the quality of specialized health services for children, women, and men in Artibonite Department. It also aims to improve the health system in Haiti and to establish a replicable model for provincial-level hospitals in Haiti. The initiative includes the design, construction, and equipping of the new hospital. It also includes initial institutional support such as training in hospital management, hospital equipment usage, and maintenance, as well as specialized training in maternal, neonatal, and child health issues. The project is financially divided for administrative reasons. The sub-project I includes the majority of funds reserved for the design, construction and supervision; sub-project II includes the contribution allocated from Canada s Maternal, Neonatal and Child Health priority; and sub-project III represents funds set aside for monitoring and evaluation of the project.

INTEGRATING GENDER AND EQUITY ...IN HEALTH RESEARCH AND POLICY IN INDIA
more
|
Integrating Gender and Equity ...in Health Research and Policy in India
more
|
In India, researchers and poli...cymakers have only partially succeeded in incorporating gender concerns in health research, policy and programs. There is need for a more indepth and nuanced gender analysis on women's health. And, gaps still exist in the issues covered, such as the intersection of gender with caste and economic class. This project will be carried out in two parts, the first being research and the second advocacy. Researchers will further develop an existing gendered framework for analyzing health systems, and apply it to ongoing field research on women`s health in Karnataka State. The research findings will then be used to pilot test innovations, reframe existing policy, and advocate for better services to reduce maternal morbidity and mortality, and violence against women. In this context, violence against women is understood, not as episodic, but as systematic, violation of women`s physical and psychological health through normalized patterns of deprivation and abuse.Specific outputs will include: a framework for understanding violence against women; adaptation of the 'Three Delays' model of maternal mortality to resource-poor and highly gender-biased contexts; and procedures for handling risks and complications in pregnancy. The results will be fed into ongoing work of the High Level Expert Group on Universal Health Coverage.

COMMUNITY HEALTH LEARNING PROG...RAM FOR HEALTH EQUITY IN INDIA

Community Health Learning Prog...ram for Health Equity in India
more
|
India has seen rapid economic ...growth over the past two decades. The country is poised to be one of the leaders in a multipolar world-where several countries strive for dominance but none retain world leader status for more than a few decades. While India's economic outlook continues to improve, the country's social sectors have not kept pace with economic growth. Growth has also not been equitable. Social and health inequities have increased. This is surprising given India's long history of democracy and rich culture of civil society activism India's existing and widening health inequities have led to discussions on addressing the health sector's challenges from a people perspective. This alternative paradigm to health and development contrasts with the dominant paradigm which is biomedically oriented. The Society for Community Health Awareness Research and Action (SOCHARA) has been working on addressing challenges in health using this alternative development approach. SOCHARA is a civil society-based professional resource group with a focus on community health action, innovative training and teaching, networking, research, and policy engagement. This project will enhance the skills and knowledge of 90 professionals through fellowships in community and public health. It will support training and research for participants in the one-year postgraduate-level teaching program. The research will provide a strong link to theory using an experiential learning approach. The project will strengthen SOCHARA's Community Health Learning and Information Centre's position as a knowledge centre with links to community, research, and policy/advocacy resources. It will also establish a community health resource network of practitioner scholar activists committed to an alternative public health approach based on equity, rights, social determinants, and 'health for all.'

PREPARING STATES IN INDIA FOR ...UNIVERSAL HEALTH COVERAGE

Preparing States in India for ...Universal Health Coverage
more
|
India scores poorly on many he...alth indicators. It has the highest number of malnourished children, the highest infant mortality rate, and one of the highest rates of maternal deaths in the world. This is concerning given that India's stellar economic growth in the last two decades, growth which is likely to continue. In recognition of these challenges, the Indian government will roll out universal health coverage by the year 2020 as set out under the Planning Commission's 12th Five-Year Plan. It also created a high level expert group to assist the process. Given India's population, geographical diversity, and health inequities, the path to universal health coverage is likely to vary widely across states. Using research evidence, policymakers will need to ensure a balance between prioritizing primary health care and expanding access to insurance for secondary and tertiary care. This expanded access has the potential to become a financial burden on households. This project aims to provide the evidence needed to support the rollout of universal health care in India. The Public Health Foundation of India, in collaboration with state-level institutions and decision-makers, will carry out the research. The Foundation is well-positioned to lead the project given its position as the Secretariat of the High Level Expert Group with a mandate to provide technical and logistical support to Indian states. India's universal health care rollout will take place in phases. The scale and scope of the reform is enormous for a country with a diversity of states with varying levels of economic and social development. Indian states manage health. They must be prepared to understand the costs and fiscal implications of achieving universal health coverage.Researchers will develop benchmarks to help states prepare for the rollout. They will work with states to develop an essential health package for primary, secondary, and tertiary health care services in tier one states. They will also examine how information technology can improve access to and delivery of quality health services in a transparent manner. This project will provide practical guidance that allows Indian states to implement and expand universal health coverage in a sustainable way. It will produce targeted reports for key stakeholders. The research team will share its results through media outreach and peer review publications. This important research will also help other countries moving toward universal health coverage.

CLOSING THE GAPS: HEALTH EQUIT...Y RESEARCH INITIATIVE IN INDIA
more
|
Closing the Gaps: Health Equit...y Research Initiative in India
more
|
India's rapid economic progres...s has its costs. There has been a significant increase in social and economic inequalities by class, caste, ethnicity, gender, and location. This project will address these inequities through sound, actionable, and measurable evidence. The project team's research will aim to provide insight that will encourage government and civil society to make equal access to health care a priority in their initiatives. India's shortage of research on health inequities The Commission on Social Determinants of Health highlighted the value of evidence in overcoming inequities. India has a shortage of rigorous evidence related to health inequities, and much of it is aggregated at the state or country level. The evidence is often fragmented across academic disciplines, practitioner circles, and civil society actors. Most of the studies fail to examine the combination of factors, pathways, and frameworks that influence how health equity issues are conceptualized, measured, and addressed through public policies. Garnering a more integrated and shared understanding of the state of health inequities in India is a step toward redressing inequities through evidence-informed programs, practices, and policies. Research to overcome the challenges This research project will bring together ideas, actors, and disciplines working to address health inequities in India. The research team will use a multidisciplinary and multi-stakeholder approach to create a health equity research agenda. This agenda will guide emerging health equity researchers in their work to -develop a series of bibliographies; -write working papers; and -conduct original research. Cross-disciplinary network to bridge gaps The project's activities will also include establishing a network of health equity researchers. The network has the potential to continue beyond the project's timeframe, as long as the demand for cross-disciplinary, rigorous evidence remains a priority for decision-makers, researchers, practitioners, and CSO groups. The researchers will provide much-needed analysis and results that inform current health equity challenges. The network can be called upon for ongoing analysis and translation of health equity research to address emerging and persistent challenges. Strength in collaboration and numbers The project will help increase the connections between previously disconnected researchers, civil society organizations, decision-makers, and practitioners working on health equity issues. The researchers will share their findings at national and international seminars, conferences, and workshops. They will be encouraged to publish their findings in journal articles, policy briefs, fact sheets, and news stories. The project will also serve to enhance the Achutha Menon Centre for Health Science Studies' reputation as a centre with expertise on cutting-edge, multidisciplinary research on health inequities in India.

IMPROVING HEALTH DEVELOPMENT A...ND SERVICES MONITORING TO ADDRESS HEALTH INEQUITIES IN INDONESIA

Improving Health Development a...nd Services Monitoring to Address Health Inequities in Indonesia
more
|
Indonesia is undergoing rapid ...demographic and epidemiological changes. Its large and diverse population is changing, as are the patterns, causes, and effects of health and disease conditions in the country. As a result, the Indonesian health system is struggling to deliver the services its people need. The Ministry of Health (MOH) is aware of the growing disparities in health services across the country's 500-plus districts and cities. It has introduced new financial and human resources policies and programs to ensure coverage and quality of care in disadvantaged areas. This three-year project aims to contribute to better and more equitable health development in Indonesia. It will do so through improved district-level monitoring of health services and Indonesians' health status. Researchers will refine and introduce an innovative measurement tool to judge whether the new measures are effective in improving service delivery and reducing health gaps. The MOH National Institute of Health Research and Development (NIHRD) developed the Public Health Development Index (PHDI). It tracks health development progress in every district, ranks the districts, and then identifies areas that are lagging behind. This will help create a better understanding of the reasons for different levels of progress. It will also produce evidence to inform health policy and programs. With IDRC support, NIHRD will enhance the index usability through consultations with experts and key stakeholders. The research team will test the selected index components and their aggregation in pilot districts. Expected results include: - an improved and more reliable Public Health Development Index; - better integration of the index into existing information monitoring systems; - better district government capacity to use the tool; -greater collaboration between district and central government; and, - greater use of sound evidence for priority-setting and decision-making at the district and national levels.

This project supports the Nati...onal Evaluation Platform approach, which seeks to make maternal and child health and nutrition programs and policies more accountable, effective, equitable, and evidence-based. The main goal of the Platform approach is to develop central, comprehensive databases that house quality data on maternal, newborn and child health and nutrition. Governments and development partners then analyze and use this data to make evidence-based decisions in planning effective, high quality maternal and child health and nutrition initiatives that meet the needs of populations most at risk. The Platform approach addresses the challenges of traditional program evaluation methods and offers a cost-effective solution that gives national governments better tools to make decisions about their own health systems. It also helps development partners harmonize their efforts in planning effective programs. Project activities include: (i) developing a database containing recent, quality information on maternal and child health and nutrition activities and results that is updated annually; (ii) conducting data quality reviews and qualitative studies on gender differences in access to or quality of health services; (iii) mentoring and training in-country staff in data quality assessment, data management, analysis and communication of results; (iv) developing guidelines for application of the National Evaluation Platform in other countries; and (v) sharing findings with global programs seeking to increase accountability for women and children's health and nutrition. The Institute for International programs at Johns Hopkins University works in partnership with African research institutions. The project aims to raise the ...economic potential of poor and marginalized people in Malawi through integrated activities that create sustainable bases for increased income at the household level. The core of the project mainly targets women and provides both women and men with skills to increase their productivity either through vocational training leading to employment, micro-enterprise development, or increased agricultural production. In order to maximise its results, the project also invests in education to help children have the necessary foundation for further training in the future, and in basic health to help reduce the incidence of health related issues that affect productivity, especially for women.

This project aims to improve a...ccess to clean water and sanitation for 80,000 women, men, girls and boys in 92 communities in Zomba and Machinga districts in rural Malawi. Some project activities include: (i) digging 32 new wells (eight boreholes and 24 shallow wells); (ii) rehabilitating and protecting 60 failed or unprotected water points; (iii) building 3,576 improved sanitation facilities to ensure the proper treatment and disposal of sewage wastewater and prevent illness; and (iv) providing training to women and men to help them maintain and manage water and sanitation facilities in a sustainable manner, including training on operating and maintaining simple hand-pumps. The project also supports community groups to develop and implement locally tailored fundraising approaches to finance the upkeep of the water and sanitation facilities, and to develop group leadership and management skills, especially among women.Malawi is considered a water-stressed country, with only half the rural population having access to clean drinking water and adequate sanitation. Water-borne diseases are among the major causes of death among young children in the country. This, together with pressure from population growth, makes urgent the provision of safe water, improved sanitation and the teaching of healthy hygiene habits.

This project focuses on the de...velopment of bilateral programs and projects benefiting Mali. The project also aims to build the Mali program's capacity to engage in well-founded and well-articulated policy dialogue with Malian authorities and with other technical and financial partners. The project allows to hire external resources (consultants or partner organizations) to carry out assignments with specific objectives. The project also provides, alone or with other partners, funding for events or initiatives aimed at acquiring and sharing knowledge that benefits Mali, and therefore contributes to strengthening the quality of Canada's bilateral programming in Mali.

Innovative Approaches to Birth... Registration for Maternal, Newborn and Child Health / Enregistrement des naissances pour la santé des mères, des nouvea

This project seeks to increase... the registration of children under the age of five, and improve the availability, quality, timeliness, and use of health information to assist in the planning of effective activities to improve maternal, newborn and child health (MNCH). The project also seeks to improve the availability and quality of information on child mortality to better inform planning and resource allocation, and increase accountability to MNCH efforts. The project assists developing countries to increase number of birth registrations by addressing access barriers, including reaching those who live in rural areas by mobilizing community health workers in the birth registration process. The project also seeks to use innovations, such as SMS technologies, to increase the number of births being registered. This initiative supports two key recommendations from the United Nations Commission on Information and Accountability for Women and Children s Health: (i) to take significant steps to establish a system for registration of births, deaths and causes of death, and have well-functioning health information systems; and (ii) to integrate the use of information and communication technologies in national health management information systems and health infrastructure.
