This project aims to improve t...he health of people in developing countries by supporting the introduction of health and financial innovations in developing countries. The project supports eligible researchers and scientists from Canada and the developing world to implement their proven innovations where people suffer disproportionately from poor health. The project aims to implement low-tech, low-cost innovations that have the potential to be disseminated widely and successfully, and that use novel approaches to increase the delivery and demand of local health services. The project also aims to explore, test, and validate new financial mechanisms to encourage greater innovation and improved health outcomes. Innovative financing mechanisms include approaches whereby recipient countries or organizations receive financial support based on the fulfillment of specific conditions, such as the improvement of the health of mothers and children.Project activities include: (1) launching a call for proposals to Canadian and developing-country researchers and scientists for the scale-up of health innovations in low- and middle-income countries; (2) selecting proposals to scale-up eligible and proven health innovations; and, (3) launching a call for proposals to international organizations with expertise in innovative financing mechanisms to present concrete models with potential for successful implementation in low- and middle-income countries for testing and validation.Grand Challenges Canada is a federally incorporated, not-for-profit organization launched in 2010 that supports the implementation of the Government of Canada's Development Innovation Fund.

Innovative Approaches to Birth... Registration for Maternal, Newborn and Child Health / Enregistrement des naissances pour la santé des mères, des nouvea

This project seeks to increase... the registration of children under the age of five, and improve the availability, quality, timeliness, and use of health information to assist in the planning of effective activities to improve maternal, newborn and child health (MNCH). The project also seeks to improve the availability and quality of information on child mortality to better inform planning and resource allocation, and increase accountability to MNCH efforts. The project assists developing countries to increase number of birth registrations by addressing access barriers, including reaching those who live in rural areas by mobilizing community health workers in the birth registration process. The project also seeks to use innovations, such as SMS technologies, to increase the number of births being registered. This initiative supports two key recommendations from the United Nations Commission on Information and Accountability for Women and Children s Health: (i) to take significant steps to establish a system for registration of births, deaths and causes of death, and have well-functioning health information systems; and (ii) to integrate the use of information and communication technologies in national health management information systems and health infrastructure.

The project supports a strateg...y developed and implemented by SIKIKA, a Tanzanian non-government organization, to increase transparency and accountability in the health and HIV/AIDS sectors by fostering effective engagement and interaction between health and HIV/AIDS policy-makers, service providers and service users. The strategy focuses on improving the awareness of citizens on key health policy issues, as well as increasing integration between community service organizations to coordinate and increase advocacy in areas of common priority. Expected results include: Tanzanian citizens have an increased capacity to demand transparency and accountability from their government, health systems are strengthened due to improved interaction within the civil society organization (CSO) sector to advocate to government; and the Government of Tanzania is held to a higher standard in response to the demand from citizens and CSOs.

This project supports the Nati...onal Evaluation Platform approach, which seeks to make maternal and child health and nutrition programs and policies more accountable, effective, equitable, and evidence-based. The main goal of the Platform approach is to develop central, comprehensive databases that house quality data on maternal, newborn and child health and nutrition. Governments and development partners then analyze and use this data to make evidence-based decisions in planning effective, high quality maternal and child health and nutrition initiatives that meet the needs of populations most at risk. The Platform approach addresses the challenges of traditional program evaluation methods and offers a cost-effective solution that gives national governments better tools to make decisions about their own health systems. It also helps development partners harmonize their efforts in planning effective programs. Project activities include: (i) developing a database containing recent, quality information on maternal and child health and nutrition activities and results that is updated annually; (ii) conducting data quality reviews and qualitative studies on gender differences in access to or quality of health services; (iii) mentoring and training in-country staff in data quality assessment, data management, analysis and communication of results; (iv) developing guidelines for application of the National Evaluation Platform in other countries; and (v) sharing findings with global programs seeking to increase accountability for women and children's health and nutrition. The Institute for International programs at Johns Hopkins University works in partnership with African research institutions. HEALTH SYSTEMS GOVERNANCE: COM...MUNITY PARTICIPATION AS A KEY STRATEGY FOR REALIZING THE RIGHT TO HEALTH

Health Systems Governance: Com...munity Participation as a Key Strategy for Realizing the Right to Health
more
|
New research will explore the ...potential of community participation in Uganda and South Africa to address inequities in health and promote health governance systems that give voice to the poorest and most marginalized.Realizing the right to health requires responsive and accountable governance of health systems. In Eastern and Southern Africa, several initiatives have promoted community participation in health systems and public policy-making as central to this challenge. For instance, in South Africa, a country that recognizes the right to health in its constitution, local health committees were set up to promote community participation in decision-making. In Uganda, village health teams and parish development committees were created as part of decentralizing health systems. However, despite these efforts, meaningful participation by communities has not been achieved. Community actors have limited capacity to access and use information for decision-making, lack a proper orientation to rights-based strategies, and don't fully understand the role they can play in governing health systems. This research project aims to develop models of good practice for community participation in Uganda and South Africa that can strengthen the governance of health systems and advance health equity. It will do this by building the capacity of communities in Uganda and South Africa using existing community participation structures. Research will assess the effectiveness of these structures in fulfilling their roles, particularly those related to processing patient complaints. It will link them with other civil society organizations to build a broader base of support. The project will also develop ways to engage with policymakers and health officials to lobby for effective policies that support the empowerment of health committees. The project will produce training, advocacy, and lobbying materials for community organizations, as well as teaching materials to strengthen public health curriculum. It will also produce a series of evaluation reports assessing the piloted models and their activities and disseminate good practices within the region. Overall, the project is expected to foster more participatory governance of health systems and strong local, national, and regional networking on the right to health.

AFRICA CENTRE FOR SYSTEMATIC R...EVIEWS AND KNOWLEDGE TRANSLATION

There is increasing global rec...ognition of the need to link research evidence to policy decisions and practice. However, health policy leaders have failed to fully apply research findings to formulate policies, particularly in low- and middle-income countries. Several initiatives focused on bridging the 'know-do' gap emphasize capacity building and the use of evidence briefs for policy, policy dialogues, and rapid response mechanisms. But researchers and policymakers have invested little effort in producing systematic reviews to support these initiatives, despite documented skills gaps in reading and understanding evidence. Previous reports found that systematic reviews are skewed toward findings from high-income countries, meaning that research evidence is not always relevant to health workers' and policymakers' concerns in low-income countries. This highlights the need to develop rigorous, contextually based evidence synthesis skills. This project will help researchers, policymakers, and institutions in Uganda and East Africa develop the skills and knowledge they need to craft and implement evidence-based health policies. The support will create a strong evidence-base relevant to national and regional priorities. It will also strengthen the links between researchers and policymakers. Activities will include:- priority-setting workshops;- customized training workshops; and, - hands-on mentorship opportunities.Through these activities, participants will produce and share reviews focused on Millennium Development Goals 4, 5, and 6, which address maternal health, child health, and specific communicable diseases.Training will be provided to participants from Uganda. Researchers from East Africa will also participate through existing networks and targeted support for young and emerging researchers. This grant will also fund the establishment of the Africa Center for Systematic Reviews and Knowledge Translation. The Center will build long-term capacity in evidence-informed health policy and action in Uganda and East Africa. It will also establish greater self-reliance for evidence synthesis and knowledge translation to bridge the gaps between research and health policy. The Center's training, publications, and activities will help integrate health equity into health systems, meaning greater equity along social and gender lines. The Center will help advance the field of health systems and policy research. Finally, this project will support a workshop to evaluate an innovative approach to policy influence, known as the 'Rapid Response Mechanism,' which provides timely evidence synthesis briefs at the request of policymakers. Four countries in sub-Saharan Africa have implemented Rapid Response Mechanisms. Each has its own challenges and innovations. The workshop will support collaborative work to develop the design and conceptual framework for evaluation across the four countries (Uganda, Burkina Faso, Cameroon, and Zambia).

ENHANCING NURSES ACCESS FOR CA...RE QUALITY AND KNOWLEDGE THROUGH TECHNOLOGY (ENACQKT)

Enhancing Nurses Access for Ca...re Quality and Knowledge through Technology (ENACQKT)
more
|
Although information and commu...nication technologies (ICTs) are increasingly available in the Caribbean, the health care culture tends toward traditional, paper-based, labour-intensive mechanisms. Increased use of ICTs could potentially facilitate point-of-care access to information and evidence-based decision-making regarding treatment. This project grew out of an earlier initiative (103687) that supported the use of a simple, proven ICT infrastructure and toolkit (personal digital assistant-PDA, wireless connectivity, relevant software) to address the clinical quality of care. This project will continue introduction of the technology through a train-the-trainer mechanism in rural hospitals in Barbados, Dominican Republic, Saint Kitts and Saint Lucia. Researchers will analyze the potential of the technology to help nurses respond to three of the main health challenges in the Caribbean: diabetes, hypertension and HIV AIDS. The project will also experiment with podcasting, which has the potential to bring relevant regional and general information to nurses using PDAs, and to provide nurses with the opportunity to create their own podcasts. It is anticipated that the intervention will result in improved patient outcomes over the course of the project.

Strengthening Distance Educati...on in the Caribbean/Renforcement de l'éducation à distance dans les Caraïbes
more
|
This project aims to increase ...employment and other income-generation opportunities by improving access to post-secondary education for women and men living in remote and under-served communities of the Caribbean. The project seeks to strengthen the Open Campus of the University of the West Indies (UWI) to make high-quality education available to more people across the Caribbean through the use of distance education technologies. This is key in a region where countries are separated by the sea, making it difficult for many people to access the three 'traditional' campuses of UWI (in Trinidad, Jamaica, and Barbados). The virtual Open Campus seeks to deliver expanded program offerings that are better aligned with labour market needs, and undertake stronger marketing outreach to students. Some project activities include: (i) designing, developing and delivering demand-driven, gender and environmentally sensitive post-secondary distance education programs to students across the Caribbean; (ii) training faculty, instructors and tutors in gender and environmentally sensitive course design, development and delivery; (iii) conducting a comprehensive analysis of program requirements to better respond to labour market needs; and (iv) setting up new library virtual services (online) that can be accessed by all Open Campus learning sites throughout the Caribbean./ Le but de ce projet est d'accroître l'emploi et autres activités rémunératrices en améliorant l'accès aux études postsecondaires des hommes et des femmes qui vivent dans des collectivités éloignées et mal desservies des Caraïbes.

STRENGTHENING THE ROLE OF PRIV...ATE SECTOR HEALTH PROVIDERS TOWARD UNIVERSAL HEALTH COVERAGE IN MENA

Strengthening the Role of Priv...ate Sector Health Providers Toward Universal Health Coverage in MENA
more
|
There is a growing global cons...ensus on the need to work toward universal health coverage in the post-2015 global development agenda. In parts of the developing world, public health systems provide only part of the answer and leave an unmet demand for health care. In the Middle East and North Africa (MENA), private sector healthcare providers appear to be thriving to meet this demand. However, little is known regarding the size and nature of this sector. The World Health Organization will convene a three-day consultation to share the results of recent assessments that map the status of private sector health providers across MENA. The consultation will also examine the extent to which regulations in Egypt and Yemen ensure both the safety of patients and competition among providers in this sector. This grant will cover the costs to convene a three-day consultation on strengthening the role of the private health sector toward universal health coverage. The consultation will examine the need for further study. It will also help establish the potential contribution and challenges facing the private health sector in a universal health coverage environment.

Adding a social science perspe...ctive to the field of health policy and systems research can help developing countries address health problems in a more comprehensive way. In recent years professionals have come to recognize that critical health problems need to be addressed in a more comprehensive way. Addressing health problems one disease at a time (a clinical or biomedical approach) no longer works. Instead, support for a more comprehensive health system involves creating more sustainable health delivery systems. This approach is important for developing countries where fragmented health systems cannot serve the population's needs and where the health research capacity of individuals and institutions is limited. The centrepiece of this research involves building the field of health policy and systems research with an explicit social science perspective. To achieve that goal, a series of postdoctoral research awards, sabbaticals for practitioner-scholars, and expert researcher residencies will be awarded in Africa. The awards will help to strengthen the knowledge base needed to create equitable health systems, while also upgrading methodologies. The University of the Western Cape and University of Cape Town in South Africa have the necessary experience and resources to launch this work, and enjoy strong linkages with universities across Africa as well as with other key stakeholders and institutions. The expected outcomes include building the field of health policy and systems research, not only in South Africa but also in low- and middle-income countries throughout Africa. Giving researchers the time and space to debate, consolidate, and further develop the field is expected to create a community of practice in health policy and systems research. Individual awards for postdoctoral research, scholarships, and residencies will complement this outcome. Other expected outcomes include publications, teaching material, podcasts, websites, and the dissemination of ideas through conferences and seminars.

November 2013 - The Americas r...egion, which includes Latin America and the Caribbean, is prone to natural disasters and other humanitarian crises, such as the 2010 Haiti earthquake and subsequent cholera epidemic. The frequency and impact of natural disasters in the Americas region has worsened over time. While some member states of the Pan American Health Organization (PAHO) region have extremely strong disaster preparedness, risk reduction, and response capacities, other member states have much weaker ability to plan for, mitigate, and respond to humanitarian crises. With DFATD support, PAHO aims to prevent death, illness and disability resulting from natural disasters and other humanitarian crises by strengthening the ability of national health ministries and regional health sectors to respond on a national scale in a more effective and timely way. This project aims to better equip national health experts responding to disasters and other humanitarian crises by providing them with training and by developing policies and guidelines to guide their work. The project also seeks to create an improved roster of health experts that can be deployed to respond immediately in the aftermath of a disaster or other humanitarian crisis.

Alternative Public Service Del...ivery Models in Health, Water and Electricity (Africa, Asia, Latin America)
more
|
The literature on public servi...ce delivery alternatives has to date been highly localized, sector specific and lacking in methodological consistency. This project seeks to analyze health, water and electricity delivery models in Africa, Asia and Latin America in order to identify and document successful alternatives to commercialization. Researchers will draw up a set of evaluation criteria (norms), carry out a mapping exercise, refine the conceptual model in a workshop, and identify a set of cases for in-depth study. The selected case studies will represent a cross-section of sectors, regions and models highlighted during the mapping exercise. In the final stage of the project, researchers will examine the findings as a whole, with an emphasis on their implications for service delivery options and health systems in South and Southern Africa.

The purpose of the Real Accountability: Data Analysis for Results (RADAR) project is for the Institute for International Programs of Johns Hopkins University to work with Canadian institutions and organizations, in-country evaluation partners, governments and development partners in a subset of countries where Canada has maternal, newborn and child health (MNCH) programming.Project activities include: (1) support CAN-MNCH in the training of a cadre of Canadian technical experts that are available to assist in implementing the MNCH results tools in DFATD programs with MNCH programming; (2) support the design of an external, Canadian-based, data repository to ensure data quality, comparability and availability in order to generate regular results reports; (3) assist in the implementation of these tools in a select number of Canada's MNCH countries in order to validate results, refine the methodologies and ensure proper implementation. The ultimate beneficiaries for this project are mothers and children, as this initiative will develop tools to assess the quality of care provided to mothers and children, in the areas of nutrition and newborn health programs./ Ce projet vise à élaborer, mettre en œuvre et peaufiner des outils et approches pour renforcer la surveillance des pays et l'établissement de rapports sur la santé des mères, des nouveau-nés et des enfants (SMNE) et la nutrition. This project is part of the De...partment of Foreign Affairs, Trade and Development (DFATD) International Youth Internship Program (IYIP), funded by the Government of Canada's Youth Employment Strategy (YES). The IYIP contributes to the YES by providing a large spectrum of Canadian graduates with valuable international development work experience abroad, needed to launch successful careers. The Atlantic Council for International Cooperation offers 40 Canadian interns internships in nine countries: Bhutan, Bolivia, Gambia, Ghana, Haiti, Jamaica, South Africa, Kenya, and Uganda. The internships focus on increasing food security, stimulating sustainable economic growth and capacity-building. Interns assist in preparing educational and communications materials, facilitating and designing workshops, organizing events and managing projects.

This Volunteer Cooperation Pro...gram allows for the sending of 60 volunteer advisors, mainly dentists, optometrists and homeopaths mission. The program aims to 1) strengthen the individual capacities of local partners; 2) improve the practices of health and education professionals; and 3) improve the day-to-day practices of managers and staff of partner organizations. These health professionals will aim to help alleviate, on a volunteer basis, some of the most urgent health deficiencies among the poor, starting with children and school-aged children and their mothers, in regions lacking access to care. This will significantly increase the learning capacity, job performance and income of beneficiaries. In addition, the Canadian volunteers contribute to community management of certain health problems by promoting the establishment of fruitful and lasting collaboration between organizations from within the community and the services provided by public health institutions. The program also supports a public engagement program to form a body of Canadian citizens who are more educated, informed and engaged./ Ce programme de coopération volontaire permet d'envoyer 60 conseillers volontaires, principalement des dentistes, optométristes ou homéopathes, en mission.

Strategies to Improve the Qual...ity of Health Care - Learning from Experiences in Uruguay and Chile
more
|
Improving access to primary he...alth care and the quality of services in Latin American countries is urgently needed to address high health inequities in the region. Lessons learned from two successful campaigns promoting maternal and child health in Chile and Uruguay could provide insights for further health reforms and improved service delivery across the region. Better access to primary health care and service delivery in Latin America is urgently needed to address high health inequities in the region. This involves implementing reforms, such as those undertaken in Uruguay and Chile where programs such as 'Good food and nutrition practices' and 'Chile grows with you' focused on improving maternal and child primary healthcare.' This research aims to identify and share those programs' success factors. Researchers in each country will examine how the programs addressed maternal and child health, focusing on their design and implementation. Teams will also conduct a comparative analysis of the framework guiding their respective policies and systems. Results will be disseminated at regional and national levels, encouraging reflection and discussion among various stakeholders. Findings will also inform program development and policy-making for greater health equity in the region. Two universities are working with a team of researchers that includes graduate level students in Chile and Uruguay.Expected outcomes include reports that will be shared widely in hopes of scaling up the programs, and making any needed refinements to the reforms in Uruguay and Chile. Better collaboration among researchers and decision-makers within and between both countries is also expected. This will help build capacity for and raise interest in this kind of research.
