Strengthening Equity through A...pplied Research Capacity Building in e-Health

|
There exists limited understan...ding of how e-Health solutions are perceived, designed, implemented and used. This goes beyond considerations related to automation, power supplies and affordability. Rather, it refers to context-specific, politically-aware and socially sensitive examination of how various ICTs are being appropriated, who is being included and excluded in the process, and whether the incorporation of ICTs is leading to more equitable health outcomes. The Strengthening Equity through Applied Research Capacity Building in e-Health (SEARCH) program will cultivate local research capacity to examine e-health and whether it leads to improved health equity and stronger health systems. Emphasis will be placed on methodological and analytical rigor, and policy dialogue. The first phase of SEARCH will involve scoping studies and workshops, with will elaborate the research agenda for the remainder of the program.

EQUITY IN HEALTH AND HEALTH FI...NANCING: BUILDING AND STRENGTHENING DEVELOPING COUNTRY NETWORKS

Equity in Health and Health Fi...nancing: Building and Strengthening Developing Country Networks
more
|
Equity in health is a pressing... global concern. Disparities in health status and access to health care within and across countries are both a cause and a consequence of social inequality. Access to health services continues to be largely determined by socioeconomic factors, and financial protection against catastrophic health costs remains inadequate. It is crucial to find evidence-based, and locally-produced and owned solutions, to address the issue of equity in health. Three IDRC-sponsored regional networks are separately investigating inequities in health, health financing and health delivery. The networks are: Strategies for Health Insurance for Equity in Less Developed Countries - SHIELD (103457), Equity in Asia-Pacific Health Systems - EQUITAP (105231), and the Latin American Research Network on Financial Protection (LANET), for Africa, Asia and Latin America and the Caribbean, respectively. This grant will allow the three networks to cooperate through a series of opportunities for capacity building, exchange, and cross-regional comparative and collaborative work. Such work is expected to enhance the evidence base and dissemination of research results for informed policy design.

Ecohealth and Communicable Dis...eases in Latin America: Improving Technical Writing Skills of Young Researchers
more
|
In 2006, IDRC launched a new c...ollaborative initiative to promote the use of the ecosystem approach to human health (ecohealth) in addressing the environmental and social determinants of vector-borne diseases in Latin America and the Caribbean. Focused on the prevention and control of Chagas disease, dengue, and malaria, this initiative supported seven research projects in nine countries until its conclusion in 2011. It led to the emergence of new young researchers in the field. Most of the research teams are still analyzing data and writing scientific articles. This research support project will build the capacities of junior researchers to write scientific articles. It will provide technical and editorial support to prepare a set of papers for publication in academic journals. It will also support the production of a synthesis paper discussing opportunities and challenges for using ecohealth approaches to prevent and control vector-borne diseases in the region.

PAN-AMERICAN PARTNERSHIP FOR I...NTEGRATIVE RESEARCH, POLICY, AND PRACTICE ON HEALTH, ENVIRONMENT, AND SOCIETY

Pan-American Partnership for I...ntegrative Research, Policy, and Practice on Health, Environment, and Society
more
|
The Americas is home to divers...e ecosystems that are vulnerable to increasing stress from environmental change, including changing climate, rapid urbanization, air and water pollution, and intensified agriculture and natural resources use. These stresses have direct impacts on the health and well-being of populations across the region. These are multi-faceted issues that are often only understood in the effort to solve or mitigate them. Ecosystems approaches to health provide a means to better understand multiple links between health and the environment. These approaches include more integrated and effective responses which promote collaboration guided by evidence. This project will leverage the best of ecohealth approaches in Canada, Latin America, and the Caribbean. This partnership aims to improve methods and means to influence and guide inter-sector policies and practices which contribute to healthy, just, and ecologically sustainable societies. The project team will develop and implement a Pan-American research and capacity-building program over a four-year period. The program will include training awards, intensive summer schools, professional development workshops, research fellowships, and multistakeholder dialogue sessions. The project will increase the interest and ability of policymakers, practitioners, and researchers to put into practice multi-sector responses for improving human health and ecosystem resilience (the ecosystem's ability to resist damage and to recover quickly). The partnership will help strengthen and embed ecosystem approaches to health in the Americas. It will also build sustainable communities of practice and reinforce their alliances with other groups and institutions.

Strengthening Distance Educati...on in the Caribbean / Renforcement de l éducation à distance dans les Caraïbes
more
|
This project aims to increase ...employment and other income-generation opportunities by improving access to post-secondary education for women and men living in remote and under- served communities of the Caribbean. The project seeks to strengthen the Open Campus of the University of the West Indies (UWI) to make high-quality education available to more people across the Caribbean through the use of distance education technologies. This is key in a region where countries are separated by the sea, making it difficult for many people to access the three traditional campuses of UWI (in Trinidad, Jamaica, and Barbados). The virtual Open Campus seeks to deliver expanded program offerings that are better aligned with labour market needs, and undertake stronger marketing outreach to students. Some project activities include: (i) designing, developing and delivering demand-driven, gender and environmentally sensitive post-secondary distance education programs to students across the Caribbean; (ii) training faculty, instructors and tutors in gender and environmentally sensitive course design, development and delivery; (iii) conducting a comprehensive analysis of program requirements to better respond to labour market needs., and (iv) setting up new library virtual services (online) that can be accessed by all Open Campus learning sites throughout the Caribbean.

|

Alternative Public Service Del...ivery Models in Health, Water and Electricity (Africa, Asia, Latin America)
more
|
The literature on public servi...ce delivery alternatives has to date been highly localized, sector specific and lacking in methodological consistency. This project seeks to analyze health, water and electricity delivery models in Africa, Asia and Latin America in order to identify and document successful alternatives to commercialization. Researchers will draw up a set of evaluation criteria (norms), carry out a mapping exercise, refine the conceptual model in a workshop, and identify a set of cases for in-depth study. The selected case studies will represent a cross-section of sectors, regions and models highlighted during the mapping exercise. In the final stage of the project, researchers will examine the findings as a whole, with an emphasis on their implications for service delivery options and health systems in South and Southern Africa.

EXPANDING PROVISION OF ESSENTI...AL SURGICAL SERVICES THROUGH TASK SHIFTING (UGANDA)

|
Expanding Provision of Essenti...al Surgical Services through Task Shifting (Uganda)
more
|
Surgically treatable problems ...are estimated to account for up to 11% of the world s disability adjusted life years. The World Health Organization (WHO) recognizes the challenges and benefits of a competency-based approach to surgical services in its publication, Global Initiative for Emergency and Essential Surgical Care, which provides critical guidance on how to strengthen health systems to take on the burden of surgical problems. In Uganda, the majority of surgical care is not provided by surgeons, but rather by medical (physicians) and health (non-physician) personnel. The use of task shifting, whereby lower-level health workers with specific training fulfill tasks normally provided by more highly-qualified personnel, is being considered as a way of addressing the severe shortage of physicians and other healthcare workers. This grant will finance the development of a training program to support task shifting in emergency and essential surgical services. The program will provide non-surgeon physicians and non-physician clinicians with formal training in surgical skills, as well as evaluation and critical examination of patient outcomes. If successful, the program is expected to lead to a process of certifying medical and health officers as qualified to provide emergency and essential surgical services.

Simplified Clinical Tools and ...Educational Outreach for Health Workers
more
|
Developed in South Africa, sim...plified tools and training have been shown to improve outcomes for HIV, tuberculosis (TB) and primary care patients, and to enhance staff satisfaction and confidence. The following are key to their success: point-of-care training provided onsite in a clinical setting; case-based content; intermittent training sessions facilitated by peer trainers over time; and sustained educational support thereafter. This project will adapt the approach used in South Africa for Malawi, a more fragile health system where the challenge of integrating HIV services within primary care services is even greater. Working closely with the Ministry of Health and using a train-the-trainer model, researchers will test the approach, Simplified Tools and Training - Practical Approach to Lung Health, Malawi (STAT-PALM), in 31 rural health centres in Zomba district using a cluster-randomized trial. The training will focus on mid-level healthcare workers, that is, nurses, medical assistants and clinical officers. Outcomes measured will include staff retention and satisfaction, and patient outcomes with respect to HIV/AIDS, TB, malaria and other conditions seen in primary care. The overall goal is to improve access to high-quality HIV/AIDS care and primary care services.

BUILDING CAPACITY IN HEALTH SY...STEMS AND POLICY ANALYSIS IN SUB-SAHARAN AFRICA

Building Capacity in Health Sy...stems and Policy Analysis in sub-Saharan Africa
more
|
Since 2005, the Bill and Melin...da Gates Foundation has been financing the master's program in health and population at the Institut supérieure des sciences de la population (ISSP), Université de Ouagadougou, Burkina Faso. However, after completion of the program by five cohorts it has become apparent that certain areas of concentration need to be reinforced, mainly, health systems and policy analysis. This grant will support the design of a module worth six credits (120 hours) within the master's program in health and population, and a scholarship program for some 40 students to specialize in health systems and policy analysis. This will prepare them to undertake critical and comparative analysis of health systems in sub-Saharan Africa and to identify appropriate solutions.

INSTITUTIONALIZING ECOSYSTEM A...PPROACHES TO HUMAN HEALTH IN WEST AND CENTRAL AFRICA

Institutionalizing Ecosystem A...pproaches to Human Health in West and Central Africa
more
|
This project follows up on a e...arlier project on institutionalizing Ecosystem approaches to human health in West and Central Africa (102148). The earlier project saw the development of an Ecohealth curriculum and its integration in nine existing master s level training programs in seven universities and institutions in the region; 270 students completed the program and nine received scholarships to conduct Ecohealth research in the areas of water, transmissible diseases and climate change. However, the quality of the curriculums and the pedagogical tools developed did not meet expectations, and the level of appropriation of Ecohealth by the universities and institutes remained weak, dependent essentially on the voluntary commitment of a few teachers. This project aims to consolidate the accomplishments of the earlier project by improving the existing curriculums and training educators in schools of general medicine with a view to integrating Ecohealth into their institutions. In particular, the project will contribute to the creation of interuniversity doctoral-level training in public health that incorporates the link between health and the environment based on the Ecohealth approach. The project will involve expert members of the West and Central African Community of Practice in Ecohealth (WACA-COPEH), the responsible parties from the institutions involved, and international experts.

|
|

|

ASSESSING THE ATTITUDES AND PR...ACTICES OF PUBLIC HEALTH PROFESSIONALS REGARDING FUNDING FROM FOR-PROFIT CORPORATIONS
more
|
Assessing the Attitudes and Pr...actices of Public Health Professionals Regarding Funding from For-Profit Corporations
more
|
Over the past two decades, man...y corporations have begun philanthropic contributions to universities under the rubric of corporate social responsibility (CSR). In an era of dwindling support from governments and foundations, many universities have, in turn, begun to solicit funds from businesses. In the case of schools of public health, some potential corporate funders make products that may be considered harmful to health or a threat to the environment, creating a potential conflict of interest. The overall aim of this project is to inform a policy debate about possible guidelines for making decisions about accepting funds from for-profit corporations. At present, there are no generalized guidelines to govern the receipt of research and sponsorship funds from for-profit corporations - practices differ by schools and programs of public health. This project, managed by the American University of Beirut, will survey public health academics and practitioners around the world about their attitudes and practices around funding from for-profit corporations, particularly those whose products are associated with non-communicable diseases. The project will also review existing policies that govern schools of public health around the globe with respect to taking funds from for-profit corporations. The project will focus on schools and programs of public health in the Eastern Mediterranean region, North America, Africa, and Southeast Asia.

West Africa has many of the lo...west development indicators in the world - 10 of the 15 member states of the West African Community number among the world's 35 low-income countries. The World Health Organization reports that 14 of the member states have a high maternal mortality ratio, defined as 300 or more maternal deaths per 100,000 live births. These dismal statistics tell the story of a region facing dysfunctional health and social services with a limited capacity to conduct research that informs sound health policies and practices. Of the health research that is undertaken in the region, much is done in isolation, and findings are not shared or disseminated across governments or health systems. The result: synergies across findings are not made and links between researchers and decision-makers are often weak. This project sets the foundation for producing and using rigorous and relevant evidence to strengthen equitable and sustainable health systems in West Africa.Four teams will be funded through a call for proposals focusing on research on how to strengthen health systems in West Africa. A group of leaders with regional expertise will be mobilized and supported to advise on the call and guide the overall project. The project will also help the West African Health Organisation, a respected stakeholder and the mandated body coordinating efforts to improve health in the region, promote interest in the call and encourage the use of research findings by ministers of health from member countries.Research results will be disseminated through a special issue of a peer-reviewed journal, policy briefs, and articles to further inform health policy and practice in West Africa.

NEGLECTED ISSUES RELATING TO A...FRICAN HEALTH SYSTEMS: AN INCENTIVE FOR REFORM
more
|
Neglected Issues Relating to A...frican Health Systems: An Incentive for Reform
more
|
Inequalities in access to and ...quality of health services result in poorer health for disadvantaged groups. In West Africa, poor access to health care has been linked to increased mortality of children under five. Several issues that affect access to and quality of health care for the poor and vulnerable in West Africa are neglected because of systemic 'blind spots' - issues that are not covered by regular investigative protocols, are not questioned because they are part of well-established routines, or conflict with other interests. They include practices that don't comply with official standards, bottlenecks within healthcare organizations, and shortcomings in implementing healthcare policies in the field. Even when these issues are discussed, the lack of reliable data makes it difficult to propose and implement workable solutions. These neglected issues need to be properly documented, shared, and discussed publicly to foster reform that will improve the quality of health care, especially for the most vulnerable. Funding will support six research projects on issues in the health care system that affect access to and quality of care. Results will then link to reform efforts. Researchers will tackle key issues that have direct implications on the quality of care available to vulnerable populations, including daily service delivery, women's health, decentralization, and sustainability. Research will be undertaken in Niger to build a comprehensive picture of what is needed to reform a national health system. Local health professionals and public health officials will participate in studies and provide an insider's perspective on how the system is actually functioning. The project will strengthen the skills of West African researchers in the social anthropology of health, promote mentoring of young African researchers, and improve the quality of empirical social sciences research conducted in Africa. It will also provide researchers, health care professionals, and community activists with a forum for discussion and will bring these issues to the attention of public health officials and health professionals. Several training activities involving students, PhD students and health care workers will be incorporated in the ongoing work. Conferences will be held at local, district and national levels to share the results of the six research projects. Results will also be released internationally to francophone and anglophone scientific and academic communities, and to the media through reports, articles, and other publications.

AFRICA CENTRE FOR SYSTEMATIC R...EVIEWS AND KNOWLEDGE TRANSLATION
more
|
Africa Centre for Systematic R...eviews and Knowledge Translation
more
|
There is increasing global rec...ognition of the need to link research evidence to policy decisions and practice. However, health policy leaders have failed to fully apply research findings to formulate policies, particularly in low- and middle-income countries. Several initiatives focused on bridging the 'know-do' gap emphasize capacity building and the use of evidence briefs for policy, policy dialogues, and rapid response mechanisms. But researchers and policymakers have invested little effort in producing systematic reviews to support these initiatives, despite documented skills gaps in reading and understanding evidence. Previous reports found that systematic reviews are skewed toward findings from high-income countries, meaning that research evidence is not always relevant to health workers' and policymakers' concerns in low-income countries. This highlights the need to develop rigorous, contextually based evidence synthesis skills. This project will help researchers, policymakers, and institutions in Uganda and East Africa develop the skills and knowledge they need to craft and implement evidence-based health policies. The support will create a strong evidence-base relevant to national and regional priorities. It will also strengthen the links between researchers and policymakers. Activities will include:- priority-setting workshops;- customized training workshops; and, - hands-on mentorship opportunities.Through these activities, participants will produce and share reviews focused on Millennium Development Goals 4, 5, and 6, which address maternal health, child health, and specific communicable diseases.Training will be provided to participants from Uganda. Researchers from East Africa will also participate through existing networks and targeted support for young and emerging researchers. This grant will also fund the establishment of the Africa Center for Systematic Reviews and Knowledge Translation. The Center will build long-term capacity in evidence-informed health policy and action in Uganda and East Africa. It will also establish greater self-reliance for evidence synthesis and knowledge translation to bridge the gaps between research and health policy. The Center's training, publications, and activities will help integrate health equity into health systems, meaning greater equity along social and gender lines. The Center will help advance the field of health systems and policy research. Finally, this project will support a workshop to evaluate an innovative approach to policy influence, known as the 'Rapid Response Mechanism,' which provides timely evidence synthesis briefs at the request of policymakers. Four countries in sub-Saharan Africa have implemented Rapid Response Mechanisms. Each has its own challenges and innovations. The workshop will support collaborative work to develop the design and conceptual framework for evaluation across the four countries (Uganda, Burkina Faso, Cameroon, and Zambia).

This project will help establi...sh an Ecohealth Chair in Human and Animal Health in Protected Ecosystems to improve the sustainability of conservation areas and the health of local communities, livestock, and wildlife in Central and East Africa. Threats to African wildlife habitats As more and more people move in and around Africa's wildlife conservation areas, there is increased land degradation, biodiversity loss, and health risks for both humans and animals. In East and Central Africa, a number of factors are causing social and environmental change for traditional pastoralist communities. These include population pressures and climate change. Agricultural policies are also an issue, as they have reduced access to former grazing lands and watering points. These changes are leading to sedentary settlements in conservation areas. To add to the problem, fishing communities have grown in and around national parks to exploit lake resources. This has led to more frequent interaction and conflict between human populations and wildlife. As a result, the health risks are magnified. The need for research on human and animal health Few scientific studies have validated or quantified the extent of these problems. There are also significant knowledge gaps on potential long-term effects on human and animal health, ecosystem sustainability, and costs and benefits of potential mitigation strategies. Through the Ecohealth Chair position, this project will foster a body of evidence and knowledge, research capacity, and collaboration to tackle these important challenges. The Chair will target diseases of animal origin that threaten agro-pastoralists and fisher communities living within and around the Queen Elizabeth National Park in western Uganda, a UNESCO biosphere reserve and a recognized Ramsar Convention site of global importance currently under threat by human activity. Applying ecohealth research to benefit local communities The Chair's program of research and partnership-building will be embedded within One Health Central and Eastern Africa (OHCEA), a regional network of public health and veterinary schools. A key objective is to use the Queen Elizabeth National Park as a long-term demonstration site for implementing applied ecohealth research studies that benefit local communities and the park ecosystem. This will help to develop the field of ecohealth in East and Central Africa. Research findings will contribute to evidence-based policy recommendations and ensure increased awareness among communities, local health authorities, and community-based organizations about health vulnerabilities, risks, and possible responses. The Chair will also expand One Health's disease control work to encompass relevant social, economic, and environmental dimensions. This will give the organization the expertise needed to guide more sustainable policies and livelihood practices in and around conservation areas in sub-Saharan Africa. It will also aim to scale up the effort t

As national health systems in ...developing countries make progress toward achieving universal health service coverage, many face ethical challenges. In its 2010 World Health Report, the World Health Assembly called on the World Health Organization (WHO) to develop a plan of action to support countries seeking to enhance universal coverage (UC). Such coverage is seen as having three interrelated dimensions: coverage of people, with needed health services and financial risk protection. The WHO asked a group of health ethicists from India, Zambia, Thailand, Uganda, Ethiopia, Norway, the UK, and the United States to develop guidelines to help countries address ethical issues while expanding universal coverage. In order to make the guidelines relevant to different national contexts, this project will conduct national case studies in Ethiopia, Uganda, and Zambia. Researchers will explore the country-specific ethical implications of choices made along each of the three UC dimensions. The study will examine ethical choices involved in providing essential health services in the three countries. Researchers will emphasize the equity implications, including:-the proportion of the population the various service benefit packages cover; -the types of prioritized services provided to those covered; -how governments finance various schemes; and,-whether the financing and the distribution are fair. More specifically, the research team will:-describe the composition of essential health service benefit packages and the selection criteria for the different countries;-examine equity in coverage with the various service benefit packages;-examine health financing institutional arrangements and governance structures;-analyze revenue generating mechanisms and their implications for fairness and equity in access to health care services;-describe the ethical challenges for policymakers and service providers implementing universal health coverage; and,-share the research findings with each country.

STRENGTHENING IN-COUNTRY HEALT...H SYSTEMS RESEARCH CAPACITY IN WEST AFRICA
more
|
Strengthening In-Country Healt...h Systems Research Capacity in West Africa
more
|
Research DescriptionHealth out...comes are generally poor in West Africa. The region lags far behind its Millennium Development Goals with some of the worst maternal and neonatal mortality indicators globally. This initiative will aim to strengthen capacities to conduct and use local health policy and systems research (HPSR) to address these important problems. Why local health research mattersThis locally driven and locally generated knowledge is needed to inform and adapt health interventions and ongoing political economy approaches to emerging issues, particularly in light of goals to achieve universal health coverage. These issues include leadership, governance, and accountability. The challenge is that health systems can only be changed and strengthened within countries themselves. To influence policies intended to improve health outcomes at the local level, there is a need to strengthen the institutional and environmental contexts in which HPSR occurs to create an enabling environment that can support policy change. Growing the field of health policy and systems researchVery often, the actors (often external) who know how to do HPSR may not understand the depth of country contexts. At the same time, those who know the contexts may not have the skills they need to undertake HPSR. HPSR is an emerging and growing field. Partners involved in building the field have acknowledged that there is a need to grow the numbers of people who can undertake research independently. Until then, those who are developing the field are overstretched. This project will set out to address the need by helping to produce a critical mass of individuals and institutions working in a supportive context. Implementing a multi-faceted approachBecause of the region's different approaches to conceiving and applying conceptual frameworks, the project team will develop an approach that draws on this diversity. They will build bridges across groups, rather than imposing a single external approach. To do so, they will identify and engage with those active in strengthening HPSR capacity. They will build partnerships through a series of three face-to-face meetings, combined with email, Skype, and/or phone contact to support continuing engagement and dialogue. The focus of the meetings will be to develop an implementation plan.Efforts will focus on identifying stakeholders involved in building HPSR capacity in Lusophone, Francophone, and Anglophone countries, along with key regional actors such as the West African Health Organisation. Expected resultsThe project's results include strengthened regional relationships to support HSR, a regional vision how to build HPSR skills and knowledge, and a regional implementation plan to strengthen capacities. Other planned results include:-a regional research implementation, monitoring, and evaluation proposal;-a resource mobilization plan to obtain funding; -a policy dialogue with the West African Health Organisation and the Economic Community
