This grant represents CIDA's s...upport to the Global Health Research Initiative's (GHRI) secretariat. The GHRI uses these funds, along with funding from other Canadian government departments and agencies, to achieve its mandate. The GHRI is a research funding partnership of five Canadian federal agencies and departments whose mandates span health, research and development assistance: CIDA, Health Canada, the International Development Research Centre, the Canadian Institutes of Health Research, and the Public Health Agency of Canada. The GHRI aims to improve health and strengthen health systems by supporting collaboration between researchers and research end-users in Canada and low- and middle-income countries. The GHRI works to coordinate Government of Canada research efforts for effective, evidence-based interventions in the field of global health, to inform the work of CIDA and other Canadian departments and agencies. The GHRI Secretariat provides support to research programs and the general functioning of the Initiative.

This project aims to improve t...he health of people in developing countries by supporting the introduction of health and financial innovations in developing countries. The project supports eligible researchers and scientists from Canada and the developing world to implement their proven innovations where people suffer disproportionately from poor health. The project aims to implement low-tech, low-cost innovations that have the potential to be disseminated widely and successfully, and that use novel approaches to increase the delivery and demand of local health services. The project also aims to explore, test, and validate new financial mechanisms to encourage greater innovation and improved health outcomes. Innovative financing mechanisms include approaches whereby recipient countries or organizations receive financial support based on the fulfillment of specific conditions, such as the improvement of the health of mothers and children.Project activities include: (1) launching a call for proposals to Canadian and developing-country researchers and scientists for the scale-up of health innovations in low- and middle-income countries; (2) selecting proposals to scale-up eligible and proven health innovations; and, (3) launching a call for proposals to international organizations with expertise in innovative financing mechanisms to present concrete models with potential for successful implementation in low- and middle-income countries for testing and validation.Grand Challenges Canada is a federally incorporated, not-for-profit organization launched in 2010 that supports the implementation of the Government of Canada's Development Innovation Fund.

Alternative Public Service Del...ivery Models in Health, Water and Electricity (Africa, Asia, Latin America)
more
|
The literature on public servi...ce delivery alternatives has to date been highly localized, sector specific and lacking in methodological consistency. This project seeks to analyze health, water and electricity delivery models in Africa, Asia and Latin America in order to identify and document successful alternatives to commercialization. Researchers will draw up a set of evaluation criteria (norms), carry out a mapping exercise, refine the conceptual model in a workshop, and identify a set of cases for in-depth study. The selected case studies will represent a cross-section of sectors, regions and models highlighted during the mapping exercise. In the final stage of the project, researchers will examine the findings as a whole, with an emphasis on their implications for service delivery options and health systems in South and Southern Africa.

STRENGTHENING EQUITY THROUGH A...PPLIED RESEARCH CAPACITY BUILDING IN E-HEALTH

Strengthening Equity through A...pplied Research Capacity Building in e-Health
more
|
There exists limited understan...ding of how e-Health solutions are perceived, designed, implemented and used. This goes beyond considerations related to automation, power supplies and affordability. Rather, it refers to context-specific, politically-aware and socially sensitive examination of how various ICTs are being appropriated, who is being included and excluded in the process, and whether the incorporation of ICTs is leading to more equitable health outcomes. The Strengthening Equity through Applied Research Capacity Building in e-Health (SEARCH) program will cultivate local research capacity to examine e-health and whether it leads to improved health equity and stronger health systems. Emphasis will be placed on methodological and analytical rigor, and policy dialogue. The first phase of SEARCH will involve scoping studies and workshops, with will elaborate the research agenda for the remainder of the program.

Equity in Health and Health Fi...nancing: Building and Strengthening Developing Country Networks

Equity in health is a pressing... global concern. Disparities in health status and access to health care within and across countries are both a cause and a consequence of social inequality. Access to health services continues to be largely determined by socioeconomic factors, and financial protection against catastrophic health costs remains inadequate. It is crucial to find evidence-based, and locally-produced and owned solutions, to address the issue of equity in health. Three IDRC-sponsored regional networks are separately investigating inequities in health, health financing and health delivery. The networks are: Strategies for Health Insurance for Equity in Less Developed Countries - SHIELD (103457), Equity in Asia-Pacific Health Systems - EQUITAP (105231), and the Latin American Research Network on Financial Protection (LANET), for Africa, Asia and Latin America and the Caribbean, respectively. This grant will allow the three networks to cooperate through a series of opportunities for capacity building, exchange, and cross-regional comparative and collaborative work. Such work is expected to enhance the evidence base and dissemination of research results for informed policy design.

Project AbstractThe capacity t...o generate, adapt, and use scientific knowledge at national and regional levels is critical for improved health equity and governance in Asia. To meet this goal, there needs to be a pool of doctoral-level trained researchers working in institutions in low/lower- and middle-income (LIC/LMIC) countries in Asia. This project will address that gap through funding for fellowships.Filling the need for Asian doctoral researchersThere are too few researchers from LICs/LMICs in Asia trained at the doctoral level in health systems research; too often, those few are found outside the region. This has led to institutional capacity gaps in health systems research. In turn, these gaps have contributed to limited knowledge generation and even less use of knowledge to strengthen health systems.Enhancing local research and knowledgeTo help address these gaps, this initiative will support and fund fellowships for: -field work and dissertation for at least 10 eligible PhD students, in the process steering them to work on relevant topics in the region; -at least 13 recent PhD graduates, to support their integration and early careers in LIC/LMIC institutions in the region; -research grants for at least four LIC/LMIC institutions for collaborative cross-country studies; and -short-term training and dissemination to improve regional capacity in generating and using evidence for decision-making.The dissertation and re-entry grants will allow fellows to be hosted in a LIC/LMIC institution. These grants will create stronger ties for partnership between the fellows and institutions in poorer countries. They will also improve understanding of the weak institutions so that health research management properly reflects the poor enabling conditions in these institutions. Building better health systems research in AsiaThis project s ultimate objective is to build stronger individual and institutional capacities in health systems research in Asia, with a particular emphasis on LIC/LMICs. The project will also allow researchers and institutions to network on a region-wide basis. It is also expected that LIC/LMIC institutions will contribute more fully to regional knowledge production and policy influence.

This project will supports the... development of a global investment framework for civil registration and vital statistics (CRVS), which is critical to Canadian and global efforts in maternal, newborn, and child health; child protection; education; gender equality; accountability; and other areas. The purpose of this framework is to outline critical gaps and challenges in strengthening CRVS in developing countries, including by identifying potential opportunities and innovations for scale-up, estimating the global financial gap, and outlining options to mobilize partners financial contributions. The project will be led by the World Bank.

Governance for Urban Health Eq...uity: Mobilizing Demand for Primary Health Care among the Urban Poor in Bangladesh
more
|
The growth of urban slums in B...angladesh is creating a crisis in healthcare delivery. New research will identify opportunities to improve health care for the urban poor and involve communities more meaningfully in health governance. Rapid urbanization in developing countries is increasing pressure on cities to deliver public services, particularly in large urban slums. In Bangladesh, administrators of large cities are facing a governance crisis as they struggle to address the health needs of their citizens. Discord among the various levels of authority responsible for providing health services is having a negative effect on service delivery and creates confusion about the oversight and stewardship roles of municipal authorities. A plethora of non-governmental organizations and other stakeholders are also involved in delivering health services, compounding the situation. Addressing this governance crisis will be paramount to improving service delivery for slum residents and to redressing inequities in urban infrastructure and access to services in Bangladesh. This project will assess the current context, challenges, and opportunities around urban health governance and the delivery of primary health care to slum populations in Bangladesh. It will examine governance structures and identify opportunities to improve delivery of care. It will map the stakeholders involved in service delivery; engage community members with other stakeholders, particularly government, to develop policies and programs that respond to the needs of the urban poor; identify promising strategies for mobilizing communities around primary healthcare services; and strengthen the capacity of municipal authorities in urban governance and health equity. One expected outcome of this research is increased networking and engagement among all the actors involved in governing urban health in Bangladesh. The project will also foster greater involvement of communities in urban governance. Research findings will be communicated through several publications, including reports, policy briefs, and articles in scientific journals. In addition, the project will contribute to a local community of practice around urban health equity in Bangladesh.

This project aims to strengthe...n civil registration and vital statistics (CRVS) systems to enhance the health and well-being of women and children in developing countries. CRVS systems provide timely and reliable data for public health statistics that are necessary for the effective planning and monitoring of health systems. This project accelerates efforts to improve CRVS systems by: (1) performing in-depth case studies of specific CRVS needs; (2) developing investment plans that lay out the costs associated with developing effective CRVS systems; and (3) conducting a technical consultation to discuss findings and potential challenges for future investments in CRVS. This project also aims to provides a roadmap for potential future investments in CRVS in the four targeted countries. In addition, the project seeks to collect critical information for a high-level event on CRVS taking place in Canada in 2014 and to address the gaps in support for CRVS globally.

CLIMATE CHANGE, VULNERABILITY,... AND HEALTH IN COLOMBIA AND BOLIVIA

Climate Change, Vulnerability,... and Health in Colombia and Bolivia
more
|
Climate change is affecting th...e world's natural ecosystems, according to the Fourth Assessment Report of the Intergovernmental Panel on Climate Change. In Latin America, researchers have observed substantial changes in precipitation, temperature, climate variability, and the number and severity of extreme events. This project aims to assess population vulnerability and climate change impacts on waterborne and water-related diseases in Bolivia and Colombia. It will use a participatory, multistakeholder, and transdisciplinary research approach in two Andean Region watersheds, the Lauca River Basin in Bolivia and the Cauca River Basin in Colombia, to develop ecosystem and human health policy adaptation strategies. These river basins are home to indigenous communities and large urban centres, such as Cali. Although on different development paths, communities in both regions are dealing with changes in water availability and quality due to climate change. This affects commercial and subsistence agriculture. It also has important health consequences as waterborne and water-related diseases, such as diarrhea and dengue, are on the rise. Researchers will use an interdisciplinary ecosystem approach to assess population vulnerability to climate change and its impacts on human health in these two watersheds, specifically to determine population vulnerability to waterborne and water-related diseases. They will apply a watershed approach to study social and ecological systems and their interactions. This approach examines systems at a territorial level, bridging the gap between local and national. It will help ensure that researchers consider the range of environmental services to benefit people and engage multiple levels of government and social actors. It will lead to a more effective assessment of vulnerability and adaptation issues. This work will contribute knowledge to the national climate change adaptation policy process in Bolivia and Colombia. Based on the results of the two multistakeholder action research studies, researchers will propose and test adaptation strategies to improve ecosystems and human health. They will work with decision-makers and key stakeholders, especially women and indigenous people. This capacity-building approach will increase the likelihood that solutions are sustainable over the long term. These solutions will translate scientific findings into actions that transcend the health sector, improving the quality and reach of community services. They may also offer support and insight to other regional climate change adaptation initiatives by providing a better understanding of climate change vulnerability in river basins along with improved institutional research capacities.

Strengthening Equity in China ...through Policy Evaluations and Capacity Building

Under 'Healthy China 2020,' un...iversal basic healthcare coverage will be provided to all Chinese by 2020. To realize the reform's goals, the Chinese government's 2011-2015 five-year plan builds on previous efforts. One of the key objectives is to improve equity in health care. China's 18th Communist Party Congress has made this a priority, with the aim of establishing institutions in each province to ensure equity. This research project will shed light on the current level of health equity within the Chinese health system. It will also assess recently implemented interventions to reduce health inequities in China. The China National Health Development Research Center will develop and lead the project in collaboration with St. Michael's Hospital. The newly established China Network on Health Equity (CNHE) will also provide support to strengthen China's capacity for health systems and policy analysis. The research team will assess the performance of selected health policies by conducting case studies. Their goal will be to improve implementation approaches for better health equity outcomes in areas including: - integrated management for health services; - health insurance through the three major insurance schemes; - maternal health; and, - payment reform in rural China. This research builds on a first phase of an earlier project that raised the awareness and built capacity for health policy evaluation in China. This second phase will analyze health policies and programs to understand what works. Researchers will consider the health system as a whole and use a multi-sectoral and multidisciplinary approach. The case studies, combined with training, will help build research capacity at all levels of decision-making. The project is expected to result in: - health policy analysis guidelines for China; - a knowledgeable and effective China Network on Health Equity; and, - enhanced culture of evaluative thinking and evidence-based decision-making at all levels, from provincial to national.

LINKING EMERGING INFECTIOUS DI...SEASES RESEARCH AND POLICY NETWORKS IN SOUTHEAST ASIA AND CHINA: APEIR PHASE II

Linking Emerging Infectious Di...seases Research and Policy Networks in Southeast Asia and China: APEIR Phase II
more
|
The Asian Partnership on Emerg...ing Infectious Diseases Research (APEIR) is a multi-country, multi-disciplinary and multi-sectoral research network that enables researchers and experts from several sectors, including agriculture and health, to explore joint research and policy questions in animal and public health. The partnership uses an ecohealth approach to address the complexity of emerging infectious diseases. This project presents a new phase of APEIR research on emerging threats from antimicrobial resistance and wildlife trade. It aims to strengthen the field of ecohealth and enhance emerging infectious disease control in Asia through research, capacity building, social and policy advocacy, and stronger knowledge and response networks. The wide use and abuse of antimicrobials as anti-infective drugs and non-therapeutic treatments are key contributors to persistent infections caused by resistance to antimicrobials. This use and abuse is also a major cause of morbidity and mortality around the world. In China and Southeast Asia, the lack of policy or regulation enforcements means that the use of antibiotics is not well-controlled. This research will provide evidence and guidance to develop country-specific and regional strategies to improve the responsible use of antibiotics and to reduce the emergence of resistant infectious agents in China, Indonesia, Laos, Thailand, and Vietnam.Southeast Asia's thriving wildlife trade supports disease transmission to humans. It also threatens livestock, international trade, rural livelihoods, native wildlife populations, and ecosystem health. Little is known about the zoonotic disease risks involved in transmitting diseases between animals and humans. This study will assess the pathogens that circulate and will explore possible pathways of transmission to humans, specifically in China, Laos, Thailand, and Vietnam. It will help prevent emerging zoonotic infectious diseases. It will also decrease wildlife consumption, while educating people about the safe and wise use of wildlife as a source of food protein for poor people. Finally, it will support wildlife diversity and mitigate illegal wildlife trade in the region.Building on past research on avian influenza and ongoing knowledge-translation activities in the health field, this new phase of work will help APEIR build and support regional capacity to communicate and share knowledge to promptly address emerging infectious disease threats.

Climate Change, Vulnerability,... and Health in Colombia and Bolivia

The project aims to build the ...capacity of the Ministry of Public Health of the Democratic Republic of Congo in budget preparation and execution. Canada's support complements and reinforces the Democratic Republic of Congo's ongoing efforts towards improving public financial management in the health sector at the central level by further extending those efforts to the central and provincial levels. The project is designed to establish an efficient gender responsive public financial management system for the health sector at central level, and develop provincial level gender-responsive health medium term expenditure frameworks in all provinces. Key activities include capacity building for budget preparation within the Ministry of Health at both levels. The project focuses on training and the provision of technical support to adapt the methods and techniques developed at the central level to the provincial, so that the templates and tools used are coherent but adapted to the specific conditions of the provinces.

The project contributes to imp...roving maternal, newborn and child health, protecting children and preventing sexual and gender-based violence by supporting the production and nationwide broadcast of radio serial dramas. These dramas provide information, tools and resources to stimulate social change, raise the awareness and improve the overall health and well-being of the audience. The dramas use characters and settings that reflect people's daily lives and current impressions of health and social issues. Produced in the most commonly spoken languages in the Democratic Republic of Congo, these dramas use the country's most economical and far-reaching mass media mechanism. The project promotes positive behavioural changes related to themes such as: immunization for all children and pregnant women; improving child nutrition; the proper use of treated bed nets to prevent malaria; child spacing and delaying marriage and onset of childbearing to promote the health of mothers; birth registration; and positive behaviours for both women and men with regard to gender equality and HIV/AIDS prevention.

The project improves access to... basic education, health and livelihoods for 200,000 individuals living in 10 villages in the territories of Walikale and Lubutu located East of the Democratic Republic of Congo. The project also supports good governance in the communities and their understanding of environmental issues. To improve children's access to basic education, particularly for girls, the project includes building or renovating eight schools and provides equipment as well as training on good teaching techniques to 80 teachers. The project also involves youth in development projects to build their awareness about environmental and development issues. In addition, the project aims to achieve better access to public health services by building or renovating and providing equipment to seven health centres in six villages, as well as latrines in seven villages. The project also provides health training on illness prevention, reproductive and early childhood health services to 35 community health workers, and provides training in illness prevention and children's health to 200 volunteers in the target communities. Other activities include protecting six natural water springs, creating and training village committees to protect these springs, and ensuring that at least 40% of committee participants are women. Finally, the project aims to ensure improved livelihoods and access to sufficient healthy nutritious foods by increasing agricultural production. Activities include: distribution of equipment to farmers; training in animal husbandry for 150 women farmers; training in agroforestry for communities to improve soil quality and crop yields; establishing seven tree nurseries in the villages and ensuring that at least 40% of participants in the newly-established village development committees are women. The project emphasises in particular gender equality so that women can gain access to positions of greater influence and of greater responsibility within their community. The local Partner is Union of Associations for Gorilla Conservation and Community Development in Eastern Democratic Republic of Congo (UGADEC).
