Building an Enhanced Cadre of ...Community Health Workers to Improve Maternal and Newborn Health in Rural Tanzania (IMCHA)
more

One woman dies in childbirth i...n Tanzania every hour. With such high rates of maternal and newborn mortality, this project will address the problem by giving community health workers greater skills to assess and treat women, while extending the healthcare system's reach to rural women and newborns. Rural obstacles to better healthIn the Mara region of Tanzania, nearly 60% of births occur outside of health facilities. A high proportion of these births result in poor outcomes for the mother, newborn, or both. Reducing maternal and newborn deaths in Tanzania involves overcoming barriers to accessing rural healthcare facilities and improving access to quality community health care. These barriers include: -lack of qualified human resources-long distances to health facilities-high cost and poor availability of transportation-lack of knowledge of the risks related to non-facility births-cultural beliefs about avoiding professional careExtending health care's reachThe project team will train community healthcare workers to improve how they assess and treat women and infants, and extend health care to rural areas. They will evaluate interventions to improve antenatal care and delivery, such as prenatal vitamins and travel vouchers to get women to health facilities for screening of hypertensive disorders, HIV, and other conditions. Community health workers will use mobile phones to improve routine processes such as birth registration and monitoring stocks and workflows. The research team will generate evidence on sustainable interventions for delivering community-based primary health care in Tanzania that improves access to antenatal care and skilled delivery. They will focus on ways to enhance the knowledge and role of community health workers and to increase their use of mobile health (mhealth). The project will help guide policymakers at all levels of government to explore scaling up interventions that can improve outcomes for mothers and newborns. Project partnersThis project is part of the Innovating for Maternal and Child Health in Africa program. The program is a seven-year $36 million initiative funded by Foreign Affairs, Trade and Development Canada (DFATD), Canada's International Development Research Centre (IDRC), and the Canadian Institutes of Health Research (CIHR).

This project will provide evid...ence for how systems-wide quality improvements can enhance maternal and newborn health outcomes at the district, health facility, and community levels in Tanzania. Healthy women, healthy babies Maternal and newborn health outcomes in southern Tanzania's Mtwara region are poor despite 70% of births occurring at health facilities. Recent research in the region found that maternal and newborn mortality rates were similar when comparing home births with health facility births. This suggests a clear need for improved quality of skilled care in health facilities. Another issue is poor health-seeking behaviours among women. Combined with weaknesses in the health system, these factors limit the use of high quality interventions that have been proven to reduce maternal and neonatal morbidity and mortality. Strategies to improve quality The Ifakara Health Institute, Tanzania's Ministry of Health, and Canada's SickKids Centre for Global Health have tested and applied quality improvement strategies for mothers and newborns across six districts in southern Tanzania. This project will explore if these strategies can be scaled up and integrated into district-level health systems. It will also examine whether these strategies for higher quality maternal and newborn health care services, better care-seeking, and improved health outcomes can be sustained at the regional level. Researchers will produce a model that can be streamlined and integrated into existing structures within the Tanzanian health system. It will generate evidence needed to inform national policy on quality improvement in Tanzania. Project partners This project is part of the Innovating for Maternal and Child Health in Africa program. The program is a seven-year $36 million initiative funded by Foreign Affairs, Trade and Development Canada (DFATD), Canada's International Development Research Centre (IDRC), and the Canadian Institutes of Health Research (CIHR).

Improving Access to Health Ser...vices and Quality of Care for Mothers and Children in Tanzania (IMCHA)

This project aims to reduce ma...ternal, newborn, and child deaths in Tanzania. The Government of Tanzania has placed a high priority on this issue. Healthcare service coverage has increased in recent years, but there are still significant geographic, financial, and socio-cultural barriers to services being accessed. Recent studies in Tanzania have shown that when healthcare systems are addressed (including barriers to health services and improving the quality of care), maternal and newborn deaths can be substantially reduced. Removing barriers to health care This project is designed to improve maternal, newborn, and child health. It consists of select interventions targeting users and providers of health care in four districts in Tanzania's Iringa region. Researchers will look at how poverty, gender relations, and living in rural areas act as barriers to accessing quality care. The project's interventions will address delays caused by: -decisions to seek care -transportation needs -availability of adequate and timely care at the facility The project team will evaluate the impacts and conduct a cost analysis of the interventions. Better access, increased demand This project is expected to increase demand and use of maternal and newborn health services. Researchers will share their findings with policymakers, program managers, and representatives of international organizations working in Tanzania. Their findings will be presented at national conferences and at the University of Dar es Salaam. They will also submit papers to peer-reviewed and academic journals. Innovating for Maternal and Child Health in Africa This project is part of the Innovating for Maternal and Child Health in Africa program, a seven-year $36 million initiative funded by Foreign Affairs, Trade and Development Canada (DFATD), Canada's International Development Research Centre (IDRC), and the Canadian Institutes of Health Research (CIHR).

This project will address high... maternal and newborn mortality in Tanzania by adapting and implementing a maternal and newborn health intervention approach that follows the MamaToto process.Health care in rural TanzaniaThe rural regions of Geita and Mwanza have some of the worst maternal and child health indicators in the country. Local health facilities are under-resourced and possess limited management capacity. While the national policy calls for a community health-worker program, the reality is that there are gaps in coordinating and implementing community health services at local levels. Community health-workers have limited training and supervision. The Government of Tanzania's new commitment to improving mothers and children's well-being needs evidence that will support the creation of stronger health systems through community-based interventions. The MamaToto approachThis project draws on the low-cost MamaToto (mother-baby) process implemented in Uganda, where substantial gains in maternal and child health were achieved. These gains took place through an approach that strengthened services at health facilities and within communities. This project will implement and evaluate a modified version of MamaToto in rural Tanzania, aligned with the country's current policy guidance for community health services. The project team will engage district leaders, strengthen facility services, and develop the role of community health workers. They will demonstrate if and how community-based interventions can reduce maternal and child mortality by effectively bringing low-cost program efforts to scale under government leadership. This project aims to scale up national maternal and child health programs that will improve survival rates for mothers and children. Based on these interventions, the project team will develop an implementation package that includes lessons learned, tools, and other materials to allow other jurisdictions to undertake similar initiatives.Project partnersThis project is part of the Innovating for Maternal and Child Health in Africa program. Mama na Mtoto: Barriers and en...ablers to gender, equity and scale-up in Tanzania (IMCHA)

In Tanzania, a project popular...ly known as MamaToto (mother-baby) is building local capacity to improve maternal, newborn, and child health (MNCH). It is testing a comprehensive MNCH package that builds on district-led implementation, evidence, and best practices from similar efforts in Uganda. It aims to reduce maternal and child mortality by increasing deliveries by skilled birth attendants, increasing antenatal and postnatal care attendance, developing an implementation model for MNCH districts, and establishing strategies for future scale-up in Tanzania, targeting the most vulnerable populations. However, as this project demonstrates, barriers to scaling up population-based MNCH life-saving interventions must be removed to reduce maternal and child deaths and morbidity — including engaging men and community stakeholders. In the Misungwi district of Tanzania, where the project is being implemented, only 13% of pregnant women attended antenatal care in the first trimester and only 47% had four or more of the minimum recommended visits. Contraceptive prevalence rate is 17% and unmet need for family planning is 27%. This project, a partnership between two Tanzanian institutions, the Catholic University of Health and Allied Sciences and the Mbarara University of Science and Technology, will enhance local capacity for qualitative research design, delivery, analysis, synthesis, and knowledge translation (moving from theory to practice). It aims to increase understanding of opportunities to foster male/adolescent/vulnerable population involvement, hence increasing access to antenatal care, delivery, and post-natal care. More generally, it will increase understanding of what factors encourage and discourage the scale-up beyond project-funded districts, based on similar experiences with MamaToto in Uganda. Finally, it provides opportunities for East African researchers working in a South-South partnership. Qualitative methodology using focus groups and key informant interviews will be used to gather information on potential barriers and enablers to address current implementation gaps. Stakeholders and end users will be engaged throughout the research cycle using an integrated approach to translating this knowledge into action. Early input of research priorities will be sought through meetings with district health teams and district and national policymakers in the Ministry of Health. This project is funded by the Innovating for Maternal and Child Health in Africa program. It is a seven-year $36 million initiative funded by Global Affairs Canada, IDRC, and the Canadian Institutes of Health Research.

Leadership and managerial capa...city strengthening for quality pregnancy and newborn outcomes in Tanzania (IMCHA)
more
|
Success in the implementation ...of maternal and newborn health interventions at the country level has been generally attributed to three main interlinked factors: leadership and management, resources, and end-user-related factors. Leadership and managerial capacities are critical for transformational change and sustainability, including gender-related dimensions of health systems leadership. These capacities also play a significant role in introducing and/or scaling up implementation of emergency obstetric and neonatal care in health facilities. This project, implemented by the Tanzanian Training Centre for International Health, working in collaboration with Dalhousie University, complements current programming for accessing safe deliveries in Tanzania. It assesses the identified gaps in leadership and managerial capacities as well as the skills gaps in comprehensive emergency obstetrical and newborn care (CEmONC) in health facilities in Morogoro, Tanzania. This project employs a mixed-methods, participatory approach to research and capacity building that examines results over the longer term. It will be implemented in the same intervention health centres selected for the safe deliveries project, along with control health centres. Through enhancing leadership capacity of health centre managers and providers and CEmONC service provision, this project aims to produce research results that can contribute to improving maternal and child health service delivery. By sustaining and scaling up use of these services at the health centre level in Tanzania, it will also contribute to the optimum use of existing resources (non-human and human) for maternal, neonatal, and child health. This project is funded by the Innovating for Maternal and Child Health in Africa program. It is a seven-year $36 million initiative funded by Global Affairs Canada, IDRC, and the Canadian Institutes of Health Research

The project seeks to reduce ma...ternal and newborn mortality in Tabora region, Tanzania, by improving the availability of quality maternal and newborn health services and increasing the utilization of maternal and newborn health services by women and their families at health facilities and within the community in targeted districts. The project increases the number of safe deliveries taking place in health facilities, the number of women who receive quality antenatal care during pregnancy and the contraceptive prevalence rate. It also decreases the number of women with unmet family planning needs. Project activities include: (1) building the capacity of regional and district council health management teams in data management and usage, supply chain management, budgeting and leadership to plan, deliver and manage maternal and newborn health services, (2) refurbishing and upgrading health centre and hospital maternity wards to provide life-saving Basic Emergency Obstetric and Newborn Care (BEmONC) and Comprehensive Emergency Obstetric and Newborn Care (CEmONC) equipment and develop an emergency transportation system, (3) training and mentoring health care workers in BEmONC, CEmONC and reproductive health services including family planning, (4) training and supporting at least 1000 community health workers to deliver quality maternal and newborn health education and promote utilization of health care services, (5) developing and delivering youth friendly sexual and reproductive health services in health facilities, and, (6) leveraging existing village savings and loans programs which utilize a social fund for maternal and newborn health expenditures to increase women's financial independence and decision making related to household resources. The project is being implemented in collaboration with the Society of Obstetricians and Gynaecologists of Canada, the Canadian Society for International Health, McGill University, CARE Tanzania, Association of Gynaecologists and Obstetricians of Tanzania, Ifakara Health Institute and the White Ribbon Alliance for Safe Motherhood Tanzania.

This project addresses the mai...n causes of maternal and newborn mortality and morbidity through a two-pronged approach. On the demand-side, it is increasing utilization of sexual and reproductive, health, and rights SRHR services by women, adolescent girls, newborns and their families and on the supply side, it is improving the availability of quality SRHR services. The project is also working to improve utilization of, and access to, SRHR services for women of reproductive age and adolescent girls that are both gender responsive and adolescent friendly, and that support women and adolescent girls to have greater decision making power to seek SRHR services independently through gender transformative strategies. The project works closely with regional and national level governments to ensure sustainable delivery of high quality, gender sensitive health services. Project activities include: (1) training healthcare staff on high-impact maternal and newborn health interventions such as basic emergency obstetric and newborn care (BEmONC), comprehensive emergency obstetric and newborn care (CEmONC), Helping Babies Breathe program, Kangaroo Mother Care, post-partum care and family planning, (2) equipping select facilities and hospitals with BEmONC and CEmONC equipment, Helping Babies Breathe and Kangaroo Mother Care supplies, reproductive health commodities and gender sensitive/adolescent friendly information, education and communication materials, (3) training community health workers and supporting outreach on gender equality, reproductive health services including family planning, and adolescent friendly maternal and newborn health, (4) conduct Village Health Days to provide maternal and newborn health services, including vitamin A, deworming and immunization for children under five, (5) develop new village savings and loans associations for adolescents and support existing ones for adults and community health workers to improve the financial independence of women and girls and promote the allocation of emergency funds. The project is implemented in collaboration with Jhpiego, Africare and White Ribbon Alliance for Safe Motherhood Tanzania. The Uzazi Uzima II project see...ks to reduce maternal and newborn mortality and morbidity in all five districts of Simiyu region, Tanzania by improving the availability of quality adolescent, maternal and newborn health services in underserved districts through three mutually supportive mechanisms. The project will: improve the knowledge and skills of healthcare providers on maternal, newborn and sexual/reproductive health to ensure they can provide critical services to their beneficiaries in a gender sensitive and responsive way, improve the infrastructure and supply-chain for health facilities to encourage women, girls and their families to use services, and improve gender responsive health management systems (such as planning, prioritization and management of services). The project will also increase awareness and understanding of sexual, reproductive, maternal and child health with special focus on men's participation in reproductive, maternal and newborn health and decision making. Project activities include: (1) training health workers from four health centres and 24 dispensaries on: essential maternal and newborn health skills, such as Basic Emergency Obstetric and Newborn Care (BEmONC), Comprehensive Emergency Obstetric and Newborn Care (CEmONC), family planning, nutrition counselling, and newborn resuscitation, (2) refurbishing four health centres and 24 dispensaries to provide CEmONC, BEmONC, water, sanitation and hygiene and family planning services, including a satellite safe blood center at the regional hospital, (3) equipping targeted health facilities with the necessary stocks of equipment and commodities to provide BEmONC, CEmONC, family planning and safe blood services, (4) establishing a referral system for maternal and newborn health care and family planning services across all tiers of the health system, (5) training and supporting Ministry of Health and facility-based managers, and health workers to improve community to facility linkages (enhanced data collection, consolidation, analysis and reporting), and (6) training and equipping community health workers, traditional birth attendants, traditional healers, and local leaders to provide community-level health promotion services (including maternal and newborn health, water, sanitation and hygiene, family planning, nutrition, gender issues and use of mobile health technology to mobilize communities). The project is implemented in collaboration with Amref Health Africa in Tanzania, Marie Stopes Tanzania and Deloitte.

Implementing comprehensive, co...mmunity-focused primary health care is a major challenge in post-conflict regions such as South Sudan and neighbouring Northern Uganda. South Sudan is recovering from two decades of civil war that have destroyed education and health infrastructure. Torit Hospital, one of the major hospitals in the region, is poorly staffed and lacks specialists and basic equipment. The situation is similar in Northern Uganda. In the District of Gulu, the burden of disease has largely been managed through St. Mary's Hospital Lacor. This region has not been able to achieve the progress in maternal and child health that has been reached in the central part of Uganda. This project assesses the implementation and scale of community-focused reproductive and child health interventions in post-conflict settings. It focuses on establishing evidence to promote sustainability and resilience, with the goal of ensuring access to essential health services by vulnerable populations. The researchers will explore strategies for improved maternal care and child nutrition services, cervical cancer screening, and support for women's empowerment through outreach efforts from the two hospitals. Research findings will be validated through ministries of health at the local, district, and national levels to optimize engagement and to encourage adoption of the project's recommendations to strengthen systems. The results of the study will directly inform the ministries of health in South Sudan and in Uganda on related interventions, including a scalable model for improved community-based services. This will address barriers to interventions, reduce maternal and child mortality, and extend the reach of health services to communities that have faced shocks and stressors. This is part of the Innovating for Maternal and Child Health in Africa (IMCHA) initiative, a seven year, $CA36 million multi-donor partnership funded by Global Affairs Canada, the Canadian Institutes of Health Research, and Canada's International Development Research Centre. The Aga Khan Development Netwo...rk's Health Action Plan for Afghanistan (HAPA) is a five-year, multi-donor initiative that aims to improve the health status of the population in selected provinces of Afghanistan (Badakhshan, Baghlan and Bamyan), with a focus on women of reproductive age and children under five. HAPA works at the national level with health education and training institutions and the Ministry of Public Health. Global Affairs Canada is supporting this initiative in collaboration with the 'Agence Française de Développement' (AFD) [French Development Agency]. Project activities include: (1) improving the quality and expanding the range of health services in remote and rural areas; (2) strengthening the availability and quality of health human resources; and (3) improving health practices among rural population, especially in the area of nutrition. HAPA aims to directly benefit 1,264,022 people, including 495,598 women, 525,732 men, 123,773 girls and 118,919 boys (below five years of age). This initiative aims to build ...local and facility-level capacity and accountability in order to reduce barriers that prevent pregnant women and mothers from utilizing sexual and reproductive health services in one province of Afghanistan and two regions in Malawi. The populations in both Afghanistan and Malawi face many challenges when it comes to accessing necessary medical care, particularly for pregnant women, and children. This has led to high maternal and neonatal mortality rates. Major factors that contribute to high mortality rates in both countries include long distances to health centres, a lack of skilled health professionals, and a lack of information regarding sexual and reproductive health. Project activities include: (1) improving the quality and supply of services at the facility level by improving the skills and approaches of health-care workers, as well as ensuring that the facilities and tools themselves are of standard quality and availability; (2) expanding demand for health services, and addressing gender gaps by increasing women's decision-making for their own and their children's health; and (3) increasing male support for women's decision making, increasing community participation in sexual and reproductive health, and changing behaviour that puts women and girls at risk. As a result, vulnerable groups will proactively seek and advocate for health services. This project targets 174,701 direct beneficiaries and 622,132 indirect beneficiaries. The Organization is implementing this project in collaboration with Community World Service Asia in Afghanistan, and in Malawi with Embangweni Mission Hospital (EMH) of Livingstonia Health Department of the Church of Central Africa Presbyterians (CCAP), and Mulanje Mission Hospital (MMH) of the CCAP Synod of Blantyre /Le projet vise à développer la responsabilisation, la capacité locale et celle des établissements à réduire les obstacles qui empêchent les femmes enceintes et les mères d'utiliser les services de santé génésique dans une province d'Afghanistan et deux régions du Malawi.

The goal of this project is to... improve the health services available to women in Afghanistan. The project maintains the operational activities of the 58 already established family health houses in Daikundi province and establishes 115 new family health houses. Each house has a delivery room and an examination room in which a trained community midwife can safely work. The communities choose the location for the houses and are responsible for building them. The project also establishes a referral system for women and babies who require emergency care, women with high-risk pregnancies, and women who require delivery and recovery care beyond the basic care provided at the family health houses. The houses also receive support from mobile health teams from existing health facilities that bring supplies and provide supervision and mentoring for the midwives. Some project activities include: (1) training the midwives and equipping each family health house with the diagnostic and medical equipment required to ensure safe childbirth; and (2) setting up telecommunication and transport services between facilities to assist with the referrals.

Maternal and child mortality r...ates in Mali and Burkina Faso remain unacceptably high and the use of healthcare services in many parts of both countries is limited. This project focuses on community-level interventions that have the potential to address this priority issue.Better health care, better outcomesDespite recent increases in services, there is inadequate coverage for the most poor and vulnerable populations. Strengthening health service delivery includes placing a priority on: -increasing access to services-improving quality of care-ensuring good continuity of care for mothers and children-addressing financial and geographic challenges associated with seasonal malariaFrom pregnancy to birthIn both countries, researchers will assess the role of four interventions on maternal and child morbidity and mortality. In Mali, the project will evaluate the expansion of village health solidarity units, known as Caisses villageoises de solidarite¿-sante¿. The health units address financial and geographic inequities that women face when accessing care. The project team will implement and assess emergency obstetric and neonatal care services protocols at community health centres. They will also create an audit cycle of maternal and neonatal deaths. In Burkina Faso, researchers will test the use of mobile phone applications in extending information and services to expectant mothers. They will monitor the mother and child through a mobile technology for community health initiative used by site staff in the villages to facilitate communication with the health centres. Their work will also inform the rollout of a large-scale chemoprevention program to address seasonal malaria. This project is part of the Innovating for Maternal and Child Health in Africa initiative, a seven-year CA$36 million partnership funded by Foreign Affairs, Trade and Development Canada, the Canadian Institutes of Health Research, and IDRC.

Support to the Global Financin...g Facility (GFF)/Appui au mécanisme de financement mondial (MFM)
more
|
This project aims to support t...he Global Financing Facility (GFF), a multi-donor financing platform that provides country-led, sustainable financing to end preventable maternal, newborn, and child deaths. The GFF finances national reproductive, maternal, newborn and child health (RMNCH) plans, supports countries in the transition towards sustainable domestic financing of RMNCH, finances the strengthening of civil registration and vital statistics (CRVS) systems, finances the development and deployment of global public goods, and contributes to a coordinated financing for RMNCH. Project activities will vary depending on the RMNCH priorities of the recipient government, but may include: (1) Clinical interventions such as increasing access to health services and health facilities from the pre-pregnancy stage onwards for women, newborns and children, (2) Health systems interventions such as strengthening health information systems, increasing access to medical products/technologies, and health systems strengthening; (3) Multi-sectoral interventions such as education, nutrition, and water and sanitation, in addition to strengthening CRVS systems. The GFF supports countries in developing individual RMNCH investment cases to align various sources of financing to national RMNCH priorities. The GFF leverages additional financing for RMNCH by matching grant funding from the GFF Trust Fund to financing from the International Development Association (IDA) and the International Bank of Reconstruction and Development (IBRD). GFF country selection is prioritized on the basis of criteria that includes available resources, need, population and income, eligibility for IDA/IBRD loans and the ability to achieve results.
