The project seeks to help redu...ce maternal and neonatal mortality in Haiti by improving the sexual and reproductive health of women and girls in four of the country's underserved departments (Grande Anse, Sud-Est, Nippes and Nord-Ouest). In addition to addressing gender inequality, the project is helping to build women's and girls' capacity to make more informed choices when exercising their sexual and reproductive rights. Project activities include: (1) training midwives at the Institut national supérieur de formation des sages-femmes (INSFSF) - the national midwifery training institute - and in two new satellite/practice schools, (2) deploying these newly trained midwives to improve the range and quality of services, as well as to spur demand for these services through a community approach, (3) strengthening the legal framework of the midwifery profession, particularly by regulating the profession in accordance with international standards. The project is expected to directly benefit 205,014 women and girls, 170,266 of which are women of childbearing age and 34,749 of whom are girls aged between 10 and 14 . / Le projet vise à contribuer à la réduction de la mortalité maternelle et néonatale en Haïti en améliorant la santé sexuelle et reproductive des femmes et des filles dans quatre départements du pays mal desservis (Grande Anse, Sud-Est, Nippes et Nord-Ouest). En plus de s'attaquer aux inégalités entre les sexes, le projet contribue au renforcement de la capacité des femmes et des filles à faire des choix plus éclairés dans l'exercice de leurs droits sexuels et génésiques. Les activités du projet comprennent : 1) former des sages-femmes à l'Institut national supérieur de formation des sages-femmes (INSFSF) et dans deux nouvelles écoles satellites/annexes, 2) déployer ces sages-femmes nouvellement formées pour accroître l'offre et la qualité des services, ainsi que pour stimuler la demande de ces services par une approche communautaire, 3) renforcer le cadre juridique de la profession de sage-femme, notamment en réglementant la profession conformément aux normes internationales. Le projet vise directement 205 014 femmes et filles dont 170 266 sont des femmes en âge de procréer et 34 749 filles de 10 à14 ans.

The project (also known by its... French acronym PRISMA - Prise en charge intégrée de la santé de la mère et de l'enfant dans l'Artibonite) aims to increase the coverage of sexual and reproductive health services and quality care for mothers, newborns and children in 8 of the 15 communes of the Artibonite department in Haiti. This is achieved by strengthening the health governance of the department, strengthening institutional emergency obstetric and neonatal care and community building. Project activities include: (1) constructing or rehabilitating spaces to improve services in seven basic emergency obstetric and neonatal health care centers for pregnant women, (2) training and mentoring managers, nurses and health workers in family planning and maternal and neonatal health care, 3) carrying out micro-projects for the renovation or construction of drinking water supply, sanitation and hygiene infrastructures in the health facilities of the project, (4) training and accompanying 150 members of local voluntary women's organizations and doubling the number of community health workers (269) responsible for sensitizing and referring populations to available health services, and (5) strengthening the functioning of the supply centers responsible for the distribution of medicines and medical equipment throughout the department. Overall, health facilities supported by the project seek to provide such services to approximately 146 000 women and 66 000 children under five. / Le projet vise principalement à augmenter l'offre de services de qualité en santé sexuelle et reproductive, maternelle, néonatale et des enfants de première ligne dans 8 des 15 communes du département de l'Artibonite en Haïti. Ce but est atteint par le renforcement de la gouvernance de la santé du département, le renforcement institutionnel des soins obstétricaux et néonataux d'urgence et le renforcement communautaire. Parmi les activités du projet, notons entre autres : 1) construire ou réhabiliter des espaces pour améliorer les services dans sept centres de santé de soins obstétricaux et néonataux d'urgence de base pour les femmes enceintes, 2) former et encadrer des gestionnaires, infirmières et travailleurs de la santé en méthodes de planification familiale et en soins de santé maternelle et néonatale, 3) réaliser des microprojets de rénovation ou construction d'infrastructures d'approvisionnement en eau potable, en assainissement et en hygiène dans les établissements de santé du projet, 4) former et accompagner 150 membres d'organisations locales de femmes bénévoles et doubler le nombre d'agents de santé communautaire (269) responsables de sensibiliser et référer les populations aux services de santé disponibles, 5) renforcer le fonctionnement des centres d'approvisionnement responsables de la distribution des médicaments et du matériel médical sur l'ensemble du département. This project seeks to encourag...e greater use of integrated sexual and reproductive health services by women and girls. The project works to increase policy makers' awareness of gender gaps in existing policies and programs, with the aim of strengthening these policies in Iraq. The project aims to increase awareness of the need to prevent and respond to gender-based violence (GBV) and other harmful practices. The project offers training to health managers to strengthen health systems to deliver quality integrated sexual and reproductive health services. / Le projet vise à encourager une utilisation accrue des services intégrés de santé sexuelle et reproductive par les femmes et les filles en Irak. Le projet travaille à améliorer les connaissances des décideurs en matière de genre et ce, en ce qui a trait aux politiques et politiques déjà existantes, de façon à les rendre plus efficaces. Ce projet comprend un programme de sensibilisation aux efforts nécessaires de prévention et d'intervention face à la violence fondée sur le genre et à d'autres pratiques néfastes. Le projet offre une formation à des gestionnaires travaillant dans le domaine de la santé afin renforcer les systèmes de santé pour fournir des services intégrés de santé sexuelle et reproductive de qualité.

Malawi has the highest preterm... delivery rate in the world. This, paired with inadequate newborn care at health facilities, results in high rates of infant mortality. Managing the healthcare needs of preterm infants remains a challenge. They face complications during and after birth, and they have an increased risk of death, chronic medical problems, and malnutrition later in life. Many life-saving interventions have been successful in Malawi but they have not been scaled up into routine clinical practice. This project will determine whether a package of neonatal interventions, known as the Malawi Neonatal Package of Care, can be implemented at the health facility level to reduce neonatal mortality. Interventions include low-cost continuous airway support for breathing, breastfeeding support, Kangaroo Mother Care (skin-to-skin, mother-to-baby contact), hot cots to prevent hypothermia, management of hypoglycemia (low blood sugar), and phototherapy lights to treat jaundice. Researchers will investigate the best ways to implement these interventions in routine clinical practice. The project will directly train nurses, doctors, and other health care providers. It will also produce peer-reviewed journal articles, and provide presentations at hospitals and nursing organizations, interviews with media, and policy-relevant information to decision-makers. This is part of the Innovating for Maternal and Child Health in Africa (IMCHA) initiative,a seven year, $CA36 million multi-donor partnership funded by Global Affairs Canada, the Canadian Institutes of Health Research, and Canada's International Development Research Centre.

In Mali and Burkina Faso, mate...rnal and child mortality rates remain high. The governments of both countries are trying to increase access to and use of health care for women and children. They have adopted policies to remove financial barriers and are now piloting results-based financing, which gives financial incentives to providers of health services for achieving predefined indicators. Despite these efforts, health inequalities persist. This project will contribute to increasing equitable access to health services for the poor by strengthening the implementation of health reforms in Mali and Burkina Faso. It examines the emergence, relevance, design, and development of results-based financing in both countries, and studies its implementation, sustainability, and efficacy. The project will also look at the impact of results-based financing on health providers' motivation and teamwork, local leadership, community management committees of health facilities, access to and use of health services, and financial protection of vulnerable populations. It will document any unexpected outcomes. The project will result in a number of scientific publications from established and young researchers. In addition, the results of the study will be disseminated to decision-makers across Africa and to international agencies involved in the design and scale-up of results-based financing for maternal and child health. This pilot project aims to con...tribute to reducing maternal and neonatal mortality in four districts of the Kayes region. It supports the Government of Mali with the implementation of its Health and Social Development Plan (PRODESS), specifically to improve Maternal and Neonatal Health. The project is designed to complete the integrated community-level health system thereby strengthening the referral and evacuation plan, including the transport of pregnant women from villages to community health centres. The project comprises two complementary components: The first component takes the form of direct financial support to the four selected district authorities through the Regional Health Directorate. It aims to make new resources available for upgrading or building new health units, ensuring key services are available (e.g. emergency obstetric care, prevention of mother-to-child transmission of HIV/AIDS), having appropriate transportation in place, and ensuring availability and quality of qualified personnel, medicines and medical supplies. The second component is providing technical assistance by a Canadian support agency to assist the four districts with the implementation of their operational plans. It aims to help the local communities to get organized in order to set up in their villages a system to effectively reduce the delay in getting women in labour to the health centre. This component also contributes to strengthening the capacity of the local structures to plan and manage human and financial resources as well as to monitoring results. Part of the resources of this pilot project are being dedicated for the analysis of the results and the development and implementation of a results-dissemination strategy designed to inform the possible scaling up of this initiative. This project is part of Canada's Maternal, Newborn and Child Health commitment. / Ce projet pilote vise à contribuer à la réduction de la mortalité maternelle et néonatale dans quatre districts de la région de Kayes. Il apporte un soutien au ministère de la Santé du Mali dans la mise en œuvre de son Programme de développement sanitaire et social (PRODESS), plus particulièrement en ce qui concerne l'amélioration de la santé maternelle et néonatale. Le projet est conçu pour compléter le système intégré de santé communautaire et ainsi renforcer le plan de référence et d'évacuation comprenant le transport de femmes enceintes des villages à des centres de santé communautaire. Latin America is home to the g...reatest inequities in the world, including inequality in access to quality health services. Indigenous peoples have the poorest health in the region, due to such factors as geographic isolation, limited access to safe water, sanitation and food, and a poor understanding of, and sensitivity to, cultural beliefs and practices. Among the indicators of greatest concern is the high rate of maternal mortality. Indigenous women consistently experience higher maternal mortality rates than non-indigenous women in Latin America. Efforts to improve maternal health for indigenous women and adolescent girls must be based on a clear understanding of how to provide and measure high-quality care that ensures adequate health outcomes with dignity. This project examines quality of care from a cultural and a technical perspective in three indigenous communities in Mexico, Peru, and Argentina. Each of these three case studies will explore perceptions of quality care among indigenous women, and will involve collection and analysis of data using a customized framework for quality of care indicators. The study will examine the implementation of integrated health service delivery networks in all the three countries. These networks are designed to coordinate different health providers and services and each country is implementing them differently. The three case studies will examine how the networks positively or negatively influence quality of care processes and outcomes. Results from the individual case studies and from the project as a whole are expected to inform strategies for scaling up efforts to improve quality of maternal care for indigenous populations. Expected outputs include scientific publications in Spanish- and English-language journals, and information sharing via community groups, the local press and social media.

Improving quality of maternal ...health care for indigenous women in Argentina, Mexico, and Peru

Latin America is home to the g...reatest inequities in the world, including inequality in access to quality health services. Indigenous peoples have the poorest health in the region, due to such factors as geographic isolation, limited access to safe water, sanitation and food, and a poor understanding of, and sensitivity to, cultural beliefs and practices. Among the indicators of greatest concern is the high rate of maternal mortality. Indigenous women consistently experience higher maternal mortality rates than non-indigenous women in Latin America. Efforts to improve maternal health for indigenous women and adolescent girls must be based on a clear understanding of how to provide and measure high-quality care that ensures adequate health outcomes with dignity. This project examines quality of care from a cultural and a technical perspective in three indigenous communities in Mexico, Peru, and Argentina. Each of these three case studies will explore perceptions of quality care among indigenous women, and will involve collection and analysis of data using a customized framework for quality of care indicators. The study will examine the implementation of integrated health service delivery networks in all the three countries. These networks are designed to coordinate different health providers and services and each country is implementing them differently. The three case studies will examine how the networks positively or negatively influence quality of care processes and outcomes. Results from the individual case studies and from the project as a whole are expected to inform strategies for scaling up efforts to improve quality of maternal care for indigenous populations. Expected outputs include scientific publications in Spanish- and English-language journals, and information sharing via community groups, the local press and social media.

Mozambique has made it a prior...ity to address its high maternal death rate and its high adolescent pregnancy rate. Access to quality sexual and reproductive health information, services, prenatal services, and delivery services is limited, particularly in rural areas across the country. This project will help find sustainable solutions to these challenges in Nampula province's rural Natikiri district. Researchers collaborated closely with the surrounding community, the local hospital, and provincial and national ministries of health to prioritize health problems and develop a set of innovative interventions. The project will cost and assess the impact of a community-owned motorcycle ambulance service and of scaled-up access to reproductive health outreach services and community hospital emergency delivery services. Strengthening the capacity of maternity staff, enhancing emergency obstetrical and outreach services, and advancing efforts to eliminate illicit charges to clients should contribute to strengthening and improving the quality of provincial maternal health services. An action research methodology based on intervention implementation and ongoing monitoring will be applied to cost interventions, identify lessons to improve ongoing implementation, and assess impact. Results will be shared with clinics and officials in the province and at the national level to promote wider application and adoption. Scientific publications will be produced and findings will be shared at national and international conferences. This project will be implemented with the collaboration of the University of Saskatchewan and the Universidade Lúrio in Mozambique. It is part of, and will contribute to, the goals of the Innovating for Maternal and Child Health in Africa program by improving the scale of access to services for pregnant women in Mozambique through innovation and community engagement. The Innovating for Maternal and Child Health in Africa (IMCHA) program is a seven-year, $36 million initiative funded by Global Affairs Canada, IDRC, and the Canadian Institutes of Health Research.

This project aims to improve g...ender equality in nine districts in Manica and Tete provinces such that adolescent girls and young women can exercise their rights to autonomy and self-determination over their own body, as well as to sexual and reproductive health. Project activities include: (1) conducting outreach, training and small group mentoring of adolescent girls and young women to increase their confidence, knowledge, skills and civic action on sexual and reproductive health, (2) establishing community feedback and accountability mechanisms on quality of health services and progress on gender equality, (3) facilitating community dialogues for young and older men to increase their engagement and responsibility and develop action plans on sexual and reproductive health, gender violence, early marriage, and gender equality, (4) training community healthcare workers who counsel family members on the harmful effects of sexual and gender-based violence, as well as child, early and forced marriage, (5) broadcasting radio shows that address harmful and discriminatory norms about sexual and reproductive health, (6) establishing services for survivors of sexual and gender-based violence, including screening and treatment of sexually transmitted infections and psychological support, (7) training and mentoring health providers and managers to offer quality, safe and legal contraceptive and abortion services, and (8) conducting outreach to provide family planning and other sexual and reproductive health services and referrals to the most remote populations. / Le projet vise à améliorer l'égalité des sexes dans neuf districts des provinces de Manica et de Tete au Mozambique pour que les adolescentes et les jeunes femmes puissent exercer leurs droits à l'autonomie et à l'autodétermination sur leur propre corps, ainsi qu'à leur santé sexuelle et reproductive. Les activités du projet comprennent : 1) faire de la sensibilisation, donner de la formation et fournir un encadrement en petits groupes pour que les adolescentes et les jeunes femmes puissent acquérir une plus grande confiance, des connaissances et des compétences, en plus de pouvoir intervenir au niveau de la collectivité, en ce qui concerne la santé sexuelle et reproductive, 2) mettre en place des mécanismes de rétroaction et de responsabilisation communautaires en ce qui concerne la qualité des services de santé et les progrès en faveur de l'égalité des sexes, 3) tenir des dialogues dans la collectivité pour amener les hommes – jeunes et moins jeunes – à participer davantage à la promotion de la santé sexuelle et reproductive, à la lutte contre la violence sexiste et les mariages précoces ainsi qu'au renforcement de l'égalité entre les sexes, en plus d'assumer une plus grande part de responsabilité en domaine, et pour élaborer des plans d'action à cette fin, 4) donner de la formation aux travailleurs en santé communautaire chargés de sensibiliser les familles aux conséquences de la violence sexu

Nigeria has one of the highest... rates of maternal mortality in the world. Maternal mortality is especially high in Bauchi State, where, according to a recent survey, only 16% of women had a qualified attendant for their last delivery, and only 40% had four or more visits to a government facility before delivery (four visits is the minimum standard set by UNICEF and the World Health Organization). Both national and state governments have identified maternal mortality as a priority issue, with a particular focus on strengthening primary healthcare services to improve maternal and child health.One of the key strategies for reducing maternal and child mortality is child spacing. This project, a partnership between the Royal Institution for the Advancement of Learning/McGill University and the Federation of Muslim Women Association in Nigeria, will develop and test a culturally appropriate home-visit intervention focusing on child spacing and the use of contraception. This home-visit approach, using portable video-education technologies, is now in use to reach the most vulnerable population. It will be expanded to all pregnant women and their spouses. Visits will cover information on safer pregnancy and delivery and will support couples to adopt safer behaviours in child spacing using culturally relevant practices. The project will further work with state governments, in particular the Bauchi State Primary Health Care Development Agency, to equip a critical mass of government officers with the necessary skills and tools for effective management, supervision, and monitoring of such a program.This project is funded by the Innovating for Maternal and Child Health in Africa program. Responding to the challenge of... adolescent perinatal depression

Adolescents experience a numbe...r of major neurobiological, hormonal, physical, and psychological changes, which present important risks to their health and well-being. Pregnancy compounds this experience for adolescents, and society's expectations often add further pressures well beyond those experienced by the average pregnant adult. However, health systems are not attuned to the particular care needs of pregnant adolescents. Evidence shows that a critical gap exists in the management of perinatal adolescent depression, a condition that brings significant long-lasting consequences for both the mother and the infant. Through this project, researchers will complement their ongoing work in maternal mental health by addressing the critical knowledge gap regarding the mental health needs of adolescents. Specifically, they will develop and evaluate an intervention program for adolescent perinatal depression, and determine the factors required for its integration in the delivery of routine primary care. They will collaborate with the Nigerian National Mental Health Action Committee to promote the uptake of their findings into policies and practices related to the management of perinatal depression. This work will contribute to the much-needed body of evidence on how to develop policies and scale up programs for integrated adolescent mental health care in Nigeria and beyond. The project will build on the established partnership between the University of Ibadan (Nigeria) and McGill University (Canada). By focusing on adolescent mental health, it addresses an important under-researched and strategic niche area in maternal and neonatal health. This project is funded by the Innovating for Maternal and Child Health in Africa program. It is a seven-year $36 million initiative funded by Global Affairs Canada, IDRC, and the Canadian Institutes of Health Research.
