Sexual violence is an enduring... human rights violation, and its effects are major obstacles to the United Nations Sustainable Development Goals (SDG), particularly SDG5, which concerns gender equality and the empowerment of women. Survivors struggle to access post rape care services and prosecutions are extremely rare, particularly in Low- and Middle-Income Countries (LMICs). The lack of data, and weak statistical and technical capacity, pose major obstacles to sustainable development in LMICs. The proposed secondary data analysis will inform policy makers about barriers to service access and case attrition. The dataset provides urgently needed information about the experiences of adult and child survivors in Nairobi who attempted to access post rape care services from 2016 to present day. The results will enable rape survivors in Kenya to use the insights from the research to advocate for policy change while providing generalisable insights for other LMICs. Our vision is to show, via this project, how secondary data analysis can be creatively used to help Kenya and other countries address the global issue of sexual violence. Our ESRC GCRF IAA grant has prepared us to undertake this research and it culminated in a series of co-developed research objectives and questions. Our objectives are to conduct the secondary data analysis to deliver high-quality impactful data research on the nature of sexual violence, post rape care service gaps, and case attrition patterns, and to study sexual violence health and well-being impacts. Our research questions focus on understanding the nature of sexual violence, the barriers survivors face in accessing post rape care services (medical, police, legal), and the reasons why so few cases are prosecuted. We will also build the capacity of the WKF to update their results as new data are collected. The findings of the secondary data analysis will be discussed with policy makers and other stakeholders. Further, the secondary data analysis project will develop a capacity building research agenda that improves the ability of the Survivors' Network to research and document sexual violence cases. The project will engage and partner with rape survivors' networks in other LMICs to share results and demonstrate the value of secondary data analysis in delivering important insights to effectively prevent and respond to sexual violence, helping to achieve SDG5.

DETERMINANTS OF EFFECTIVENESS ...OF A NOVEL COMMUNITY HEALTH WORKERS PROGRAMME IN IMPROVING MATERNAL AND CHILD HEALTH IN NIGERIA
more
|
Determinants of effectiveness ...of a novel community health workers programme in improving maternal and child health in Nigeria
more
|
Improved mother and child heal...th (MCH) continues to be an issue of international priority, particularly for sub-Saharan African countries. Evidence suggests that schemes involving Community Health Worker (CHWs) can be effective in improving the health of mothers and children. Although such schemes are implemented in some developing countries such as Bangladesh, to guide further developments, much better understanding is needed on what makes CHW programmes successful and under what circumstances. In Nigeria, despite significant improvements, mother and child health remains an issue of concern, particularly in rural areas where most vulnerable groups live. In 2012, the Federal Government of Nigeria established the Subsidy Reinvestment and Empowerment Programme (SURE-P) to invest the revenue from fuel subsidy reduction into a social security programme to improve lives of most vulnerable populations. One SURE-P component, implemented in selected facilities in each State, focuses on maternal and child health (SURE-P/MCH). The idea is that recruitment of CHWs, combined with infrastructure development, and improved availability of supplies and medicines, will improve access to quality health services, and ultimately, improve mother and child health. Since December 2012, Conditional Cash Transfers (CCT) have also been added at selected sites ('SURE-P/MCH+CCT'). These incentive payments to pregnant mothers are linked to use of health services at different stages: e.g. for antenatal care visits and facility deliveries. The AIM of this project is to inform strengthening and scaling up of community health worker (CHW) programmes. This will be achieved by investigating two implementations (i.e. with and without conditional cash transfers) of a Nigerian CHW programme, to understand what factors, under what conditions, promote equitable access to quality services, and improve maternal and child health outcomes. We will do so by: 1.Developing an in-depth understanding of the context and the process of implementation of the interventions, including relations between health workforce and infrastructure and supplies, 2.Identifying, assessing and comparing the intervention outputs (e.g. skills and practices of CHWs and efficiency of primary health care facilities) and outcomes (e.g. equitable access to quality MCH services and attainment of MCH outcome targets), 3.Developing an empirically-based and theoretically-grounded model of complex relations between the people involved, context, implementation process, outputs and outcomes of the interventions, 4.Developing transferable best practices for scalability (expansion within a broadly similar context) and generalizability (expansion to different contexts) of the interventions. This five-year research and development project will be implemented in two States in Nigeria - Niger State in the North and Anambra State in the South, which were selected in consultation with the Federal MOH and SURE-P national program

TAKING ON VIOLENCE AGAINST WOM...EN: THE ARTS IN ACTION

Taking on Violence Against Wom...en: The Arts in Action
more
|
South Africa (SA) has one of t...he highest rates of gender-based violence (GBV) in the world: a woman is murdered every three-hours, and gendered assault is 5x higher than the global average (STATS-SA 2019). GBV is recognised not only as a serious violation of human rights, but also as a significant public health concern that includes injuries, gynaecological disorders, mental health disorders (including depression and post-traumatic stress disorder), adverse pregnancy outcomes, and sexually-transmitted diseases, including HIV and AIDS. GBV is also a barrier to social and economic development, rendering many survivors unable to work or otherwise move freely in society. On 9/11/2019, SA President Cyril Ramaphosa proclaimed GBV a national crisis, promising government debate to revise domestic violence laws and to initiate a national intervention plan that challenges patriarchal norms and engages at community levels. This project comes in response to his call, introducing arts-based interventions to provide support to GBV survivors, shift societal norms by promoting gender equality and behavioural change, strengthen law enforcement, improve economic power of women and take steps to enhance GBV legal and policy frameworks. It is the first primary preventative and support strategy for GBV in rural Eastern Cape, with actions taking place in the AmaZizi chiefdom, which, located within the AbaThembu Kingdom, Eastern Cape, comprises nine villages: kwaZangashe, Ngcobo, Ngqamakwe, Ugie, Elliot, Maclear, Cala, Idutywa and Gcuwa. Arts-based actions planned will help improve response services for survivors through capacity-building workshops, support communities to re-evaluate the social norms that lead to GBV through art exhibits and festival, strengthen legal and policy frameworks by facilitating evidence-base reform and amendments to current policies and providing training to local law enforcement, and improve the economic power of women through arts-based entrepreneurial training. Although AmaZizi is the target population, this project will serve as a test-bed for building support for wider GBV policy changes across South Africa, particularly in rural regions, and informing future prevention efforts globally. This project is led by ethnomusicologist and anthropologist Nanette de Jong (Newcastle University), who has worked extensively in South Africa for 15 years and is an expert on gender, culture and community, having carried out interventions that mitigate contextual influences on HIV/AIDS and GBV. FEASIBILITY OF IMPLEMENTING AN... INTEGRATED HYBRID MODEL OF SERVICE DELIVERY FOR AGYW SEXUAL AND REPRODUCTIVE HEALTH IN SOUTH AFRICA

Feasibility of implementing an... integrated hybrid model of service delivery for AGYW sexual and reproductive health in South Africa
more
|
Adolescent girls and young wom...en (AGYW) experience significant health challenges including HIV infection, unwanted teenage pregnancies and sexually transmitted diseases in South Africa. Challenges exist with the uptake and utilization of sexual and reproductive health services for AGYW and there are a number of missed opportunities with the health system to provide prevention and care to AGYW. There is a renewed drive for service integration by policy makers to integrate SRH services with newly emerging HIV prevention and treatment services, in addition to scaling up access to prevention, care and treatment of STI's at a primary healthcare level. Current isolated service delivery models adds time and cost to clinic facilities and may be seen as a barrier to accessing care, as young people may have less access to cash, competing demands and less control over their time. There is limited data on effective delivery models for integrated interventions for young populations, therefore this study aims to determine the feasibility of implementing an integrated hybrid model of service delivery for adolescent girls and young women (AGYW) sexual and reproductive health (SRH) at selected facilities in South Africa. The integrated service delivery model will be a hybrid model comprising of both integration of contraceptive, STI/HIV transmission prevention and mental health screening with linkage to care services for HIV treatment, PrEP and psychosocial support. The linkages to appropriate care should be facilitated by health connectors. In order to determine whether such an intervention would be feasible to implement at selected facilities, this study will 4 elements, namely acceptability, demand, practicality and adaptation using the following objectives: (1) determine client, provider and key informant acceptability of the integrated hybrid model for SRH service delivery for AGYW, (2) assess demand and preference for integrated SRH services amongst AGYW, (3) determine the practicality of delivering integrated hybrid model for SRH service delivery for AGYW in a resource constrained setting, and (4) determine current level of integration and the adaptations needed to implement the integrated hybrid model for SRH service delivery for AGYW.

A BEHAVIOURAL ECONOMIC ANALYSI...S OF REPRODUCTIVE HEALTH IN BURKINA FASO AND TANZANIA

A Behavioural Economic Analysi...s of Reproductive Health in Burkina Faso and Tanzania

|
Our research aims to generate ...new insights that explain the low or inadequate uptake of reproductive health services in sub-Saharan Africa. By applying a behavioural economics lens to reproductive health and engaging African researchers as full research partners in this research we will not only make an important academic contribution, we will also build up research capacity in an area in which expertise is still limited. The generated knowledge would then be used to identify and test innovations to existing reproductive health initiatives that would increase the uptake of the services offered. Engaging policy-makers and practitioners in this research would increase the effectiveness of policies and ultimately benefit the lives of poor people.More specifically, we envisage the following groups of beneficiaries from the findings of our research:1. Development economists, experimental economists and social scientists in development studies, gender and reproductive health. They will benefit from our research for the reasons given in the section Academic Beneficiaries. We intend to publish about six papers in top academic journals in the fields mentioned and present our findings widely at appropriate academic seminars and conferences.2. Researchers in Burkina Faso and Tanzania in an area that is fast-growing, but in which expertise is still very limited: behavioural development economics. To this end, we will engage researchers from both African research partners in co-production of knowledge to reach the full potential of the proposed research, and we plan to employ and thoroughly train large teams of local assistants in the use of experimental methods. We also expect the new expertise to spread beyond the African research institutes, via existing local collaborations (for example Centre MURAZ collaborates with Africsanté, and MITU with Ifakara Institute).3. Public and private providers of reproductive health services in Burkina Faso and Tanzania. Through providing new insights in the low or inadequate uptake of reproductive health services and testing innovations to existing initiatives, our proposed research can lead to the improved design of existing policies and initiatives.4. International development organisations involved in research and advocacy for combating poverty and stimulating gender equality and reproductive health (such as the United Nations' UN Women, United Nations Population Fund, United Nations Development Programme, World Health Organisation and the World Bank, EngenderHealth, Marie Stopes International) as well as international organisations or networks of organisations that translate research into useful information for policymakers (e.g. Population Reference Bureau, the Population and Poverty Research Network). Our research should help predict responses to policy interventions that these organisations recommend or implement, and could therefore help modify/sharpen their recommendations or increase the effectiveness of their i

STRENGTHENING INTERDISCIPLINAR...Y RESEARCH IN TECHNOLOGY TRANSLATIONAL PATHWAYS FOR IMPROVED MATERNAL AND CHILD HEALTH IN UGANDA

Strengthening Interdisciplinar...y Research in Technology Translational Pathways for Improved Maternal and Child Health in Uganda
more
|
Every year, over 303,000 women... die during pregnancy and childbirth, 2.7 million babies die during the first 28 days of life and 2.6 million babies are stillborn. 81% of all still-births occur in sub-Saharan Africa with Uganda ranked 28th among countries with the highest pre-term births. Uganda and other sub-Saharan countries have committed to halving preventable deaths of pregnant women and newborns within the next 5 years. There are numerous examples of technological innovations that have been co-developed by engineers and clinicians, for example the preeclampsia diagnostic strip. Whereas these solve unmet clinical needs and overcome technological challenges, few have been translated to clinical practice. Clinicians and engineers alone cannot solve the complex healthcare systems especially in Low- and Middle-Income countries (LMICs). This will only be achievable by developing novel, sustainable interdisciplinary partnerships spanning clinical and engineering excellence through to expertise in implementation science and policy change. We are therefore proposing a much broader interdisciplinary research approach to solving problems relating to maternal and child health in Uganda. Our aim is to expand and strengthen our existing collaborations beyond engineering and medicine in order to foster innovations that are useful, practical and implemented in the complex health systems that exist in Uganda and LMIC. We shall achieve this by building a network of researchers including social scientists, data scientists, health services researchers, business researchers and implementational scientists in order to improve innovations and use of appropriate medical technologies for maternal and child health for use in Uganda

Through the Research for Decis...ion Makers activity, USAID supports implementation research on priority health areas, provides advisory services to the Government of Bangladesh (GOB) for health policy work, and builds health sector research capacity. This activity will initiate eight implementation research studies on family planning, maternal and newborn health, nutrition and tuberculosis. These studies will inform USAID and the GOB on programmatic strategies to improve utilization of health services and health outcomes. The results of these studies will provide the evidence base to support policy analysis and programmatic guidance to assist the GOB, USAID, and other partners in strategic decision-making on health. The activity supports Bangladesh's national health policy and its investments in health.

Prevention and Treatment of Pr...e-Eclampsia and Eclampsia (Ending Eclampsia)
more
|
The Ending Eclampsia activity ...contributes to the reduction of maternal mortality and morbidity by increasing access to appropriate prevention and treatment of pre-eclampsia (PE) and eclampsia (E) in Nigeria. The program improves the early detection and management of PE/E and increases the use of magnesium sulphate, an underutilized but essential primary health commodity that reduces seizure risks in women with PE.

BREAKTHROUGH - Research
|
Breakthrough-RESEARCH will con...vene and engage a broad range of health and development stakeholders, supporting them in developing, promoting, and operationalizing visionary, consensus-driven agendas for Social and Behavior Change (SBC) research that contribute to measurable global health impact by providing the evidence needed to increase the practice of priority health behaviors and enable social norms for improved health and development outcomes. This may include, for example, synthesizing evidence and building consensus around SBC research agendas, designing and implementing high-priority SBC studies, and developing research utilization strategies and products tailored to end-users. Breakthrough-RESEARCH may also conduct performance and impact evaluations of SBC activities implemented through Mission bilateral or other central projects.
