Scoping Study for Regional Res...earch Hubs on Adolescent Sexual and Reproductive Health in the Middle East and North Africa and West Africa

|
The aim of this scoping study ...is to conduct a thorough review and analysis of appropriate models for research hubs – often referred to as centres of research excellence – on adolescent sexual and reproductive health in the Middle East and North Africa (MENA) and in West Africa. The largest cohort of youth in history is entering their reproductive age; however many do not obtain the information needed to acquire the knowledge and develop the skills to exercise their rights or to access the services required to protect themselves from harmful situations and their consequences. This situation is acute in several regions, including in the MENA and West Africa. In both regions, adolescents, particularly girls, face high risks of early marriage, sexual and gender based violence and transactional sex. Gender inequalities, and negative social norms and practices can compound risks related to sexual and reproductive health. There is a pressing need for robust, ethical, and participatory research on adolescent sexual and reproductive health behaviours, information and services. Exploring suitable models and approaches to support sustained regional research efforts and build research leaders can help address these knowledge gaps with context-specific and rigorous evidence. Outputs from the project include a final report to IDRC summarizing the analysis and presenting recommendations on institutions and institutional arrangements, a presentation of the results at a relevant regional or international conference, and an open access peer-reviewed publication.

PREVENTING TEEN PREGNANCY IN B...ENIN: DEVELOPMENT OF A SELF-ESTEEM-BASED INTERVENTION TO EMPOWER ADOLESCENTS

|
Preventing teen pregnancy in B...enin: development of a self-esteem-based intervention to empower adolescents
more
|
The goal of this project is to... reduce teen pregnancy in Benin by developing and promoting an innovative, evidence-based intervention combining sexual and reproductive health, self-esteem, and empowerment that targets adolescents. In Benin, adolescents and youth aged 10 to 24 make up 33% of the total population, and the teen fertility rate is high. This results in lower education rates, loss of economic potential, and short and long-term health consequences. Stopgap measures have been implemented to reduce the teen pregnancy rate. However, since psychosocial and economic factors are not considered, these measures have been largely ineffective in the general adolescent population. This action research project is based on multidisciplinary participative approaches uniting all stakeholders in adolescent sexual and reproductive health. It will use mixed methods, with evaluation research of the intervention based on a quasi-experimental pre and post-intervention study with testimonials. It will include a gender-based approach to clearly identify and take into account the power dynamics between men/boys and women/girls and the underlying social norms. The main expected outcomes of this study include identifying and analyzing intervention factors to propose a plan and a method for scaling up. In the short and medium terms, this project will change adolescents' behaviour, making them more self-confident and empowered. In the long term, this will help significantly reduce adolescent fertility. This is one of nine projects in a cohort of research projects focusing on two regions (West Africa and the Middle East) and addressing two thematic issues (adolescent sexual and reproductive health and health information systems). The project is closely aligned with Canada's new Feminist International Assistance Policy, the strategic priorities outlined in the WHO's Global Strategy for Women's, Children's and Adolescents' Health (2016–2030), and the global 2030 Agenda for Sustainable Development.

A number of studies in recent ...years have reported disturbingly high levels of violence against women in the workplace. An estimated 35% of women worldwide fall victim to direct violence at the workplace, and of these, between 40% and 50% are subjected to unwanted sexual advances, physical contact, or other forms of sexual harassment. A recent study on the working and living conditions of garment workers in Cambodia reported high levels of discrimination, harassment, and sexual abuse in the factories and another recent report reveals widespread sexual abuse suffered by garment workers in Karnataka, India. However, there is a lack of systematic evidence and understanding about the extent and practices of violence and the links of workplace violence with women's weak bargaining position at home. This research project explores these links to inform policies, interventions, and social dialogue at the community, enterprise, sector, and/or national level. It focuses on workplace violence against women workers in the garment sector in Bangladesh, Cambodia, India, and Indonesia. It will involve interviewing women workers using an ethical protocol developed to deal with sensitive matters, secondary data analysis, and identifying policy and legal interventions required to address violence at the workplace. Using this data, the project will provide capacity development workshops and training for women workers. The project will also engage in dialogue and negotiations with employers and relevant stakeholders to overcome and prevent violence. This research will also inform discussions at the international level towards a binding convention against gender violence in the workplace and various communications products, such as policy briefs, op-eds and articles, a documentary film, country studies, and a comprehensive legal paper that will contribute to general awareness. The project will be led by an Asian consortium of women's and labour rights researchers and organizations through a collaborative regional process across the four countries supported by an advisory group of academic, legal, and policy specialists. They include the Bangladesh Nari Progati Sangha (BNPS) and Nagorik Uddyog, also in Bangladesh, the Center for Alliance of Labor and Human Rights (CENTRAL) in Cambodia, Aakar in India, and the Sedane Labour Resource Centre/Lembaga Informasi Perburuhan Sedane (LIPS) in Indonesia, which will also coordinate the project.

Despite efforts and successes,... indicators on sexual and reproductive health and rights in sub-Saharan Africa are among the worst in the world. To reverse this situation in Central and West Africa, a workshop was organized by WHO in November 2019 whereby stakeholders identified the major gaps in operational research capacities as a major issue that hinders the identification of effective approaches for improving adolescent sexual and reproductive health and rights. In West African countries, a great deal of effort has been made to standardize data collection and define the indicators produced by health information systems (HIS). However, these indicators are not always disaggregated to account for gender and age-related inequalities with regard to access to health services, nor are they necessarily representative of services at the community level and at health facilities. Moreover, even available data is rarely analyzed and used to inform programs and policies. Quality operational research that is supported by local experts to continuously strengthen HIS and their use in decision-making is therefore crucial for strengthening fair health systems. This project aims to strengthen and maintain operational research capacities through face-to-face training sessions and by mentoring program officers and researchers to improve adolescent sexual and reproductive health and rights and HIS in Benin, Burkina Faso, Côte d'Ivoire, Guinea, Mali, Niger, Senegal, and Togo. It will provide validated training modules that integrate gender and vulnerability considerations in general, as well as a pool of regional operational research experts and operational research protocols on adolescent sexual and reproductive health and rights and HIS. This will provide a solid foundation for future projects in the region and a resource for the cohort of projects that are already funded and that address sexual and reproductive health rights and sexual and gender-based violence. Efforts will be made to promote and facilitate the incorporation of the modules developed into existing university training courses.

Strengthening access to sexual... and reproductive health services for internally displaced people during COVID-19 in Burkina Faso
more
|
Refugee and displaced populati...ons, and the vulnerable communities with whom they share space, are at high risk for acquiring COVID-19 because their living conditions make it impossible to practice physical distancing and isolation or to access quality healthcare. Refugee women are particularly vulnerable because their limited access to sexual and reproductive health services and products is further interrupted and their duties as caretakers, especially in under-resourced refugee settings, are increased. This project is part of an initiative that will provide evidence and strengthen capacity for bridging the knowledge gap in responding to the growing COVID-19 health crisis in the short-term and the longer term. The initiative will support research on resilience building and preparedness to serve the needs of refugees and other populations on the move by promoting inter-sectoral approaches, including building bridges between humanitarian and development responses to reduce and control health risks for displaced populations. It will also leverage existing local opportunities to strengthen the overall initiative. This project will assess and respond to the needs of an internally displaced population in the fragile context of Burkina Faso, where growing conflict and terrorism may turn into a chronic development crisis. It focuses on increasing access to sexual and reproductive health services and related rights of internally displaced adolescents and women in the context of COVID-19. Firstly, it will identify the needs, constraints, and facilitating factors for using such services before developing and implementing an intervention to improve access to and awareness of sexual and reproductive health services and rights. The project will document the results and disseminate them as lessons learned to strengthen current and future responses to similar health crises.

The Maternal and Child Health ...(MCH) program supports implementation research projects that improve health equity and the quality, accessibility, and accountability of health services in low- and middle-income countries (LMICs), focusing on adolescent sexual and reproductive health and health information systems. Research is focused on populations in fragile contexts, specifically refugees and displaced populations in West Africa, the Middle East, North Africa, and other regions. In addition to addressing MCH's regular program focus, programming is pivoting to respond to the COVID-19 pandemic, as well as addressing persistent disparities in access to health services and rights in fragile contexts. Previous approaches supported LMIC researchers, policymakers, and implementers to influence national, regional, and global agendas through dialogues and the dissemination and use of research results. Given the context of the COVID-19 pandemic and restrictions around in-person meetings and travel, this project will catalyze innovative opportunities to support analysis, synthesis, and communication of research results. It aims to communicate the contributions of the MCH program and to produce greater research visibility and reach for impact. A range of outreach opportunities and communication products will be developed to highlight MCH supported-projects, disseminate their research results, and, when circumstances permit, to enable the strategic involvement of IDRC grantees in key events and conferences to promote knowledge exchange and to influence policies, practices, agendas, and funding priorities.

The Innovating for Maternal an...d Child Health in Africa (IMCHA) initiative seeks to improve maternal, newborn and child health by using primary health care as entry points to strengthen health systems, ensuring they are more equitable. It supports 19 research teams conducting 28 research projects, and 2 Health Policy and Research Organizations. This $36 million initiative is funded by Global Affairs Canada (GAC), the Canadian Institutes of Health Research (CIHR) and IDRC. IMCHA is ending on July 31, 2021. Knowledge translation, communication and dissemination activities are key at this stage to promote the uptake of research findings into national and regional policies and practices. They also allow increased visibility of the initiative, enriching discussions between donors about a future partnership. This Research Support Project (RSP) establishes a mechanism for IMCHA to authorize expenditures for knowledge translation, communication, and dissemination activities until the end of the initiative in July 2021.It also enables investments in strategic activities as opportunities arise to lay the groundwork for future programming. Specific activities have already been identified, including a special edition with the African Journal of Reproductive Health, consultancy contract(s) for the development of thematic synthesis pieces, translation of outputs, and events organization.

Labouring women and violence: ...Building workplaces free of violence

BETTER SEXUAL AND REPRODUCTIVE... HEALTH AND RIGHTS FOR ADOLESCENT GIRLS IN SENEGAL – CAPACITY BUILDING
more
|
Better sexual and reproductive... health and rights for adolescent girls in Senegal – capacity building
more
|
Adolescent girls in Senegal fa...ce significant challenges in reaching their full development potential. Exposure to sexual and gender-based violence, compounded by limited access to sexual and reproductive health services, restricts these young women's rights. Many are being coerced into unwanted sex or early marriage. Gender-based violence also perpetuates a cycle of disempowerment and poor health outcomes for adolescent girls, especially among those already vulnerable because of poverty, location, and low education. The Senegalese government has passed laws and developed policies and programs that are aimed at creating an enabling environment for family planning and reproductive health. However, challenges remain that limit effective approaches to address the needs of adolescent girls in Senegal, including harmful social, cultural, and religious norms, beliefs, and practices that limit effective care. This project will help address the knowledge gap about how best to act upon the root causes and means to address the compounding factors that limit the realization of young women's rights. It will be complemented by five implementation research projects and a set of national and local youth-led organizations aimed at building a powerful leadership response centered on the needs of adolescent girls in Senegal and informed by evidence.

Better Sexual and Reproductive... Health and Rights for Adolescent Girls in Senegal (ADOS), co-funded by IDRC and Global Affairs Canada, seeks to generate Senegal-specific insights. The aim is to better understand and address the interface between adolescent sexual and reproductive health and rights and sexual and gender-based violence, and to support policymakers' and community leaders' uptake of implementation research and of youth's inclusion. This project will support technical assistance for the program's grantees, the evaluation of the program, and communication with and engagement of key stakeholders. Local experts in gender analysis, implementation research, ethics, and knowledge translation will be recruited to support the selection/evaluation of research projects, build the capacity of recipients, and provide ongoing follow-up to support research and youth organizations in effectively implementing their respective projects. An evaluation expert will also be retained to facilitate the communication of the program's logic model and performance management framework with key stakeholders, establish the program's baseline, and design tools for effective and ongoing data collection. The capacity of both research teams and youth teams will be strengthened to address the program's monitoring and learning needs. Finally, the program will regularly engage with various stakeholders and produce knowledge and communication products for broader outreach.

Efforts will be made to promote and facilitate the incorporation of the modules developed into existing university training courses.

more
|
Betel chewing is prevalent in ...Asia including among women in pregnancy. Betel is known as carcinogenic but little is known about health risks for mother and fetus. Studies suggest that health risks are similar to that of undernourishment in early life; adverse pregnancy outcome, poor fetal and infant growth and increased risk for metabolic syndrome in adulthood. Thus if poor nutrition and betel chewing are combined synergetic affects are plausible. Set in a rural Bangladesh the aim is to evaluate the health consequences for mother and offspring of betel chewing. A community based cohort of 4700 pregnant women will be followed repeatedly from gestation wk 8-10 until infant is 1y. Outcome data on maternal nutrition (wt, ht, body composition, skinfolds, circumferences) and metabolic status (insulin, glucose, lipid profile, blood pressure) fetal growth (ultrasound), adverse pregnancy outcome, size at birth, growth and infant nutritional status (wt, ht, skinfolds, circumferences) be collected. Proposed mechanisms for the toxicity of betel chewing will be evaluated by biomarkers (vit D, vit B-12, folate, crp, agp) in blood samples in pregnancy, at delivery (cord) and infancy. Exposure assessment will include the different ingredients in betel chew and indicators developed that will enable dose-effect analyses of exposure on the outcomes. As the research team includes capacity of large scale interventions it is expected that research results rapidly may be turned into action

Conflict-Related Sexual Violen...ces: Understanding What, When and Why
more
|
Finally, sexual violence in wa...r has been recognized as a pressing global security problem in policy and research. Yet, much still needs to be done to better understand its dynamics and to more effectively work towards its prevention. The overarching category of sexual violence remains imprecise as a legal and policy tool, and potentially misleading as a common category for a vast array of acts in disparate contexts, driven by different logics, and with different effects. Drawing on the unique expertise of the research team and focusing its inquiry to the Great Lakes region in Africa, the aim of this research project is to further query and comprehend the variations in conflict-related sexual violence against women/girls and men/boys in this site. The project asks four interrelated research questions: 1) What are the dominant forms; 2) prevailing contexts; 3) logics of sexual violence?; 4) How is sexual violence delineated in legal, policy and academic texts? It will answer these questions by: analysing documented sexual violence cases; conducting individual screening, in depth interviews and participatory group research with refugee populations; interviewing ex-combatants and soldiers/combatants convicted of sexual violence; analyzing legal, policy and academic texts. Ultimately, it will contribute to general knowledge about conflict-related sexual violence, and provide a more nuanced picture against which comparisons with other conflict and non-conflict settings can be made.
