Public Policy, Health Educatio...n and Female Empowerment in Uganda and Tanzania. The objective of this project is to investigate causal effects of public and non-governmental health interventions on family planning and female empowerment in East Africa. Two specific but related research questions will be analyzed in the project. First, we aim at investigating how nutritional supplements in early childhood affect female empowerment later in life through improved educational attainment in Tanzania. Second, we will analyze how family planning education transmitted through religious radio broadcasts affects female empowerment and sexual behavior in Uganda. Both research questions will use novel data from the Measure DHS surveys, and use statistical methods that focus on estimating causal effects of the interventions.

Poor fetal growth is associate...d with adverse health consequences for the individual, both short- and long-term. While the mothers nutritional status both before and during pregnancy is associated with fetal growth, underlying mechanisms are still not completely understood. The insulin-like growth factor (IGF) system and its components has been studied in pregnancy and suggested as potential mediators between the associations between maternal nutritional status and growth of the fetus. While several studies have found maternal IGF-1 concentrations to be associated with size at birth, other studies find no such associations. There is also little knowledge on associations between maternal IGF-system and later infant growth. This population-based study will be implemented in a rural setting in Bangladesh where poor maternal nutritional status is common. We aim to investigate maternal nutritional status and the IGF-system in relation to fetal growth, infant size at birth, body composition and infant growth. We will use different indicators of maternal nutritional status (body weight, length and body composition). Fetal growth will be measured by ultrasound and infant body composition by skinfolds. Our proposed study may give more insight into these complex relationsships and increase our understanding of fetal growth regulation. This will help designing appropriate nutrition interventions by identifying who benefits most, and when interventions would be most successful.

The project *Dynamics and Cons...equences of the Increasing Sex Ratio at Birth and the *Missing girls* in China* is collaborative research between Peking University of China and Nordic School of Public Health in Sweden. The field work had been performed in China, by Chinese collaborator, Mr. Shusheng Luo, Guided by Swedish researcher Dr. Petra Lofstedt. The following work is paper writing and publishing, and Mr. Shusheng Luo will pursue a doctoral degree at Nordic School of Public Health, Sweden, supervised by professor Max Petzold. This collaborative research will, in line with the recommendation form WHO, contribute to a further understanding and the magnitude of the problem with ?missing girls?, as a basis for policies and rising the public awareness of gender inequalities, discrimination of girls and of the long-term societal consequences,such as high suicide rate of women in rural areas, and so on. Both PhD Petra Löfstedt at The Swedish Institute of Public Health and Professor Max Petzold at Nordic School of Public Health has had a long-tem collaboration with the School of Public Health, Peking University, China. This bi-lateral project will definitely deepen the collaboration and communication between Swedish and Chinese institutions.

The aim of this project is to ...increase access to safe post-abortion care (PAC) and decrease maternal mortality and morbidity from unsafe abortions in Uganda. To date, there is no established medical regimen for 2nd trimester PAC, instead unsafe methods (sharp curettage) are used. Our previous study in Uganda confirm the efficacy of medical management by midwives in 1st trimester PAC. We now aim to study efficacy and safety of medical vs. surgical treatment for 2nd trimester PAC in a randomised controlled trial. 1030 women, seeking care for incomplete 2nd trimester abortion at six health facilities in central Uganda, will be included. The use of sharp curettage will be monitored and evaluated during implementation. Primary outcome is the efficacy of treatment to expel the retained tissue without any additional surgical treatment. Secondary outcomes include safety, acceptability and contraceptive uptake. A study coordinating centre will be established in Kampala. Close collaboration between partner-researchers will facilitate study conduct (2016-2018). Preparations and training will occur during 2016, followed by recruitment during 2016-17 and data cleaning and analyses in 2018. Throughout the study period, in-service training of medical management in PAC will be held, workshops and policy-dialogues with stakeholders will be held to tailor-make service guidelines and to lobby for inclusion of medical management in policy. Two dissemination seminars will be held in 2016 and 2018

The use of antibiotics in aqua...culture is a hazard associated with the development and spread of antibiotic resistance genes (ARG) in marine environments. Bacteria are carriers of ARGs and associated with planktonic organisms, and studies of plankton and oceanic circulation may provide new insight into the spread of resistance. However, to establish fair and effective governance of antibiotic use, especially in developing countries where aquaculture production is an important source of livelihood and revenue, it is also necessary to know the driving forces behind the usage. Our aim is to produce a synthesis, based on biophysical and socioeconomic factors, for policy recommendations for Asia and East Africa. The synthesis will be based on empirical results and we will address where and on which level remedies to prevent antibiotic resistance spread are most effective. More specifically, we will investigate the local economic role of, and need for antibiotics and produce a cost-benefit analysis. We will map the flow of antibiotics from production to use in order to identify points in the value chain where restrictions would be fair and effective. Further, we will use metagenomics to identify genetic fingerprints of the ARG pool from four aquaculture sites. The spread of specific ARG patterns are hypothesised to be linked to the dispersal of phytoplankton and will be projected by oceanographic connectivity models and verified with repeated sampling at different geographic scales.

The overall purpose is to impr...ove cattle health through increased understanding of the microbial community in cattle and ticks. Ticks are the most important vectors of disease-causing pathogens in domestic and wild animals. Tick-borne diseases (TBDs) are a major threat to both cattle and human health. Understanding the complex interactions within the microbiome is of great importance for understanding how tick borne pathogens spread and cause disease. We will study indigenous cattle and ticks in all 11 agro-ecological zones of Uganda using metagenomics. Metagenomics has proven to be a powerful tool for pathogen detection and for making in depth analysis of the microbial community. The microbiome of ticks feeding on cattle or living in its close proximity will be characterized and compared with the cattle microbiome. We propose to build a smart and user-friendly bioinformatics framework for comparison of metagenomes. Our results will give insights into the microbiome of cattle and its relation to tick microbiome and disease. Possibly, new disease causing agents will be identified. The study will also contribute with knowledge on how cattle owners understand the relationships between ticks, cattle and TBDs. The knowledge generated in this project can contribute to more appropriately designed interventions for diagnosis and prevention of TBDs.

In this project will target th...ree diseases where loss of pigment is evident and where the T cell responses may be the underlying cause of melanocyte destruction, namely; vitiligo, leprosy and post kala-azar dermal leishmanaisis (PKDL). Leprosy and PKDL primarily occur in populations of extreme poverty, while vitiligo, affects millions. In common is that the conditions can be highly stigmatizing. In India the stigma of vitiligo make patients choose to undergo reconstructive surgery to improve their appearance, a procedure rarely carried out in Sweden due to the risk of local recurrence of disease in the scar. The outward resemblance of vitiligo and leprosy contributes to the stigma, particularly in poor populations. Little is known about the causes of melanocyte destruction and if there possibly are any beneficial effects of the hypopigmentation. We aim to understand the role immune cells in the skin have in destruction of the pigment producing melanocytes. In particular, we are interested in how cells T contribute to melanocyte destruction in skin disorders of autoimmune and infectious origin. We will do this by assessment of skin immune cells. In both leprosy and PKDL the skin is a target for the infectious organism. However, there is limited knowledge about immune reactions and the immune cells in the skin. We want to increase the understanding we have of pathologies in these diseases something that can be of potential benefit for their management and their elimination.

We aim to introduce an innovat...ive new system, a MamaCard, to generate data on the quality of maternal and newborn care and mortality in Southern Tanzania. The MamaCard is an individual mother held booklet developed based on standard antenatal and delivery cards which includes detachable vouchers to document each visit and the content of care received. It is linked to an individual identifier. The information on the vouchers are scanned and electronically processed. This method has been developed by a Swedish group (Shifo) and is applied to support the recording of vaccination data in four districts in Uganda.We will adapt present technologies to capture data along the continuum of antenatal, intrapartum and postpartum care. We will assess health workers perception and attitude towards present and this new data capturing system. We will also assess the costs involved. We will compare and triangulate selected indicator estimates with those from other sources. We will apply obtained mortality and quality of care indicators to evaluate an ongoing funded quality improvement initiative called QUADS which is implemented in Southern Tanzania.Our hypothesis is that the new MamaCard system can potentially revolutionise the present health management information systems in low and middle income countries where data on quality of care received by mothers and babies as well as mortality is not available at local level and in timely intervals.

Neglect of sexual and reproduc...tive health and rights (SRHR) for adolescents results in poor health outcomes such as unwanted pregnancies, sexually transmitted infections, and violence. Community-based interventions aimed at strengthening SRHR for adolescents can help to reduce such health challenges. There is limited knowledge of the mechanisms that can effectively deliver such interventions, or of the processes that can bring about their integration into the health system. The purpose of this project is to identify the mechanisms that facilitate integration of SRHR interventions into community health systems, and establish whether such integration promotes (or not) the acceptability and adoption of SRHR services. This will be achieved by conducting a multiple case study embedded within an ongoing RCT. A theory-driven evaluation will be applied, which will, through a stepwise approach, develop and refine a causal theory resulting in policy recommendations, steering further research and informing teaching programmes. The 3-year project was conceived based in the partners´ mutual interest in health policy and systems research with a focus on community-based interventions to promote equity and the right to health. We aim for a long-term partnership through mutual development of methodological expertise and projects beyond this ResearchLink proposal. Regular workshops, study visits, and virtual meetings for senior and junior researchers will form the backbone of activities.

The School of Public Health at... the University of Zambia have requested financial assistance from Sida to fund an on-going research study (RISE) aiming to find evidence that financial support in combination with sexuality education empowers girls to avoid early pregnancies and drop out of school. Researchers at the University of Zambia (UNZA) School of Public Health, in partnership with researchers from the University of Bergen (UiB), Norway, are currently implementing the Research Initiative to Support the Empowerment of girls (RISE). This is a cluster randomized trial (running since 2016) which is examining whether early childbearing rates can be reduced and grade 9 completion rates increased in a rural Zambian context by:- provision of economic support to girls and their families, and- whether combining economic support with comprehensive sexual and reproductive health education and community meetings to address social norms, can have a greater effect.The trial has three study arms: 1) A control arm, 2) An economic support arm, and 3) A combined arm with economic support, comprehensive sexuality education, CSE, and community meetings. The study units are rural schools in twelve districts in Southern and Central provinces. The eligible participants were 5000 girls enrolled in grade 7 in 2016.The two universities are partners in a Centre of Excellence in Research funded by the Research Council of Norway and the University of Bergen. The primary objective of the Centre for Intervention Science in Maternal and Child Health (CISMAC) research consortium is to conduct excellent trials in low and middle income countries to generate evidence on how to effectively and equitably deliver health interventions to improve maternal and child health. In addition to generating evidence on the effects of the support packages, the RISE project is also strengthening the research capacity of Masters and PhD students in Public Health at the University of Zambia. Currently 7 master students from the University of Zambia are collecting/using data and being supervised from the RISE project. Sida has been requested to provide additional funding to the research project to enable the originally planned follow-up of the girls into 2020. With current funding, the project will have to end in 2018 due to insufficient funds which would not give sufficient evidence to validate the trial results.

Chemical exposure during criti...cal periods of early-life development may cause adverse health effects later in life. Women and children, especially in low-income settings, are exposed to several toxic compounds, including arsenic (As) and cadmium (Cd) from food and polycyclic aromatic compounds (PAHs) from burning of fossil fuels for cooking and heating, many of which have endocrine disrupting properties. We therefore aim to clarify if early-life exposure to As, Cd and PAHs is affecting growth and puberty development in adolescence. This will be accomplished by studies in a mother-child cohort (MINIMat) in rural Bangladesh in collaboration with several universities and icddr,b, Bangladesh. Exposure to As and Cd is already measured in urine from mothers in pregnancy and their children at 10 years. Measurements of PAHs will be conducted in the corresponding bio-banked urine samples (n=1000). Data on puberty from 11-14 years and data on growth from birth to pre-pubertal age, during puberty, and at 15 years is availible. In addition, we will also measure different hormones related to growth and reproduction in blood collected at 15 years. Consequences of co-exposures to metals and PAHs will be evaluated in combination with effect modification by gender and nutrition. This 4-year project will provide insight into common environmental exposures and effects on adolescent health in low-income settings, enabling future public health strategies to reduce exposures and improve health.

Violence against women and chi...ldren are serious global health issues. Despite seriousness of the problem, little is known about the pattern and the main risk factors of such violence particularly in low and middle income countries where both types of violence are highly prevalent. Current study aims to evaluate the pattern and determinant of women's exposure to Domestic Violence (DV) and/or Intimate Partner Violence (IPV), over a long period of time and estimate the casual relationship between maternal experience of IPV/DV and Violence against Children (VAC) using a novel longitudinal dataset from MINIMat study in rural Bangladesh. The original study implemented in 2001 in Matlab and since then 4436 pregnant women with 3267 singleton live births were repeatedly followed-up during pregnancy and after child birth (1-24m, 4.5y, 10y, & puberty) and comprehensive set of social and environmental exposures were collected from them in several occasions. Recruited women were interviewed about their lifetime experience of IPV/DV before child birth, during pregnancy and 10 years after childbirth. We plan to interview 1400 women and their children at (17 years of age) regarding their last year experience of violence using standard questionnaires. Multistage models will be used to to identify pattern and factors associated with women's experience of IPV/DV at different time points and also to evaluate temporal association between IPV/DV and VAC.

This project aims to assess th...e effect of the group-based IMPower intervention on reducing gender-based violence (GBV) and improving sexual and reproductive health and rights (SRHR) among urban poor adolescents in Cape Town, South Africa. Previous evidence indicates that IMPower can reduce sexual violence by 40-60%, but little is known about its effect on other types of GBV (physical, psychological) among boys as well as girls, or if it protects against other SRHR outcomes (e.g. unwanted pregnancy).We will conduct a cluster-randomized trial in Cape Town townships in collaboration with the Desmond Tutu HIV Foundation, Ujamaa-Africa, University of Western Cape and Karolinska Institutet. Twenty schools (N=1600 male and female students aged 10-19) will be randomized in a 1:1 ratio to intervention (7 weeks, 2-hour sessions on gender norms, empowerment, self-defense) or standard life-skills, with follow-up at 12 and 18 months. This is combined with a cost-effectiveness analysis (cost per averted case of GBV), and qualitative process evaluation to understand implementation facilitators/barriers.The project will generate new evidence on effective methods to prevent GBV and improve SRHR among adolescents in South Africa – ranked as the world's most unequal country with extreme rates of violence and HIV. Such evidence is urgently needed in order to enable sustainable scale-up in other settings of poverty and oppression where few effective programs currently exist.

AIM: The aim of this project i...s to increase access to highly effective and acceptable contraception at the time of post abortion care (PAC). The project will run over 3 years in Uganda.BACKGROUND: The importance of provision of highly effective reversible post abortion contraception in reducing maternal mortality and morbidity is being increasingly recognized. PAC is acknowledged as a priority intervention in order to achieve the SDGs especially in low income countries with a high burden of unsafe abortions. The use of medical post abortion care (mPAC) has increased access to care and optimized utilization of scarce medical, as well as human, resources.RATIONALE: While immediate insertion of IUC at surgical PAC is routine practice, a disadvantage with mPAC is the practice of delayed insertion of IUC (at 2-4 weeks post PAC), resulting in a high proportion of women not returning for the follow up and IUC insertion and thus risking a new unwanted pregnancy and unsafe abortion. Currently, data and guidelines on early post mPAC IUC insertion are lacking.METHOD: An initial evaluation of barriers to IUC uptake will be followed by a randomized, multicenter trial of the effectiveness, safety and acceptability of early insertion of IUC within 4 to 7 days after mPAC compared to IUC insertion after 2-4 weeks. We will randomize 2082 women in order to show non-inferiority with early insertion in device expulsion rates and higher use of IUC at 6 months post PAC.

Betel quid (Areca catechu) che...wing is prevalent in Asia including among pregnant women. Betel is known as carcinogenic but little is known about health risks for mother and off-spring. A few available studies suggest that health risks are similar to that of undernourishment in early life, adverse pregnancy outcome, poor foetal and infant growth and increased risk for metabolic syndrome in adulthood. Thus if poor nutrition and betel chewing are combined synergetic affects are plausible. Set in a rural Bangladesh where undernutrition is prevalent the aim is to evaluate the health consequences for mother and offspring of betel chewing. The first part of the study following 3466 community-based pregnant women and their infants to 6 wks is completed and showed negative effects of betel nut use on maternal biomarkers and infants' birthweight. Now at 3 y, the children will be followed-up to evaluate whether the observed adverse effects are lasting in early childhood. An estimated sample of about 2900 children will be available. At a clinic visit, the children will be assessed in terms of anthropometry, body composition, and blood pressure. A blood sample will be taken and analysed for biomarkers such as vitamin D, B-12, folate, lipids, inflammation, glucose and insulin. The results of the study will be presented for relevant stakeholders and it is our hope that if necessary, the research will rapidly be turned into policy and public health interventions.

Preeclampsia (PE) ispregnancy-...related syndromeaffecting 8.5 million women worldwide causing amaternal death every 11thminute, mainly in low- and middle-income countries(LMIC). It is well established that PE begins in the utero-placental unit, and is amplified by oxidative stress and inflammation. Recent findings have demonstrated an association between exposure to air pollutionand an increased risk of developing PE. Air pollution in LMIC is a tremendous health problem, where exposure to airborne particles 3 suggested by the WHO, thus posing a significant threat to pregnant. The overarching purpose of this project is to investigate the cellular mechanisms underlying the risk of developing PE when exposed to air pollution. Thespecific aimsof the project are: To collect, characterize and investigate Ethiopian air pollution particlesin vitro. To evaluate the role of air pollution particles as inducers of oxidative stress and inflammationin-vivousing animal models. To investigate the inflammatory maternal plasma profile in PE and intra uterine growth restriction (IUGR)correlated to air pollution exposure. To investigate placental DNA methylation pattern and histology correlated to air pollution exposure levels. Increased insight intoenvironmental risk factors for PE is crucial to take preventive measures protecting pregnant women from the negative effects of air pollution.

At least two million lives are... lost every year globally due to complications in the intrapartum period. We aim to initiate research to prevent this high number of stillbirths and neonatal deaths by forming a consortium of researchers working in large referral hospitals in East Africa. We aim to define and innovate interventions and perform implementation research focused on the recently published new intrapartum care guidelines of the World Health Organisations. Respectful intrapartum care shall be in the centre, and we will assess the potential of lactate point-of-care testing to diagnose hypoxia and fetal monitoring using the new Moyo Fetal Heart Rate Monitor.Network partners are Makerere University and Iganga hospital, Uganda, Muhimbili University Hospital and Aga Khan University, Tanzania, the College of Medicine in Blantyre, Malawi and Hospital Central Maputo, Mozambique. During the two-year period for which we apply for funding we aim to 1) innovate implementation packages to improve the provision of safe and respectful intrapartum care, 2) review present systems of perinatal data collection or e-registries in identified hospitals 3, investigate into the feasibility of improved diagnosis of peripartum hypoxia using umbilical cord lactate measurement, and 4) prepare jointly research questions and applications. This network is primarily targeting the funding opportunity of the upcoming implementation science call for maternal and child health of Horizon 2020.

Women from low and middle-inco...me countries (LMICs) are at increased risk of both intimate partner violence (IPV) and mental health disorders. Earlier interventions to address mental health consequences of IPV in high-income settings have poor transferability in LMICs since they require specialists and treatment durations that are unaffordable. To address this, WHO has developed a brief behavioural intervention called Problem Management Plus (PM+) for IPV victims.We plan a cluster randomised trial to assess the effectiveness of PM+ in Nepal with high prevalence of both IPV and poor health outcomes among women. A total of 72 health centres (clusters) (N=1420 individual women) will be randomly allocated in a 1:1 ratio providing PM+ versus standard care (control). There will be three data collection points over 18 months preceded by formative research to adapt the PM+ module to the local context. Qualitative interviews will explore feasibility and acceptability among service providers and participants. Descriptive statistics, multilevel analysis and intention to treat analysis will be applied. Qualitative data will be thematically analyzed.This trial will contribute real-life evidence of scalability for informed decision-making by providing innovative large-scale evidence of task-shifting mental health service provision to lower cadre workers, for more affordable, culturally acceptable and effective management to mitigate the huge unmet need in IPV-exposed women in this region.

The overall objective is to de...velop a long-term partnership between Lund University (Sweden), Gulu University (Uganda) and University of Juba (South Sudan) to strengthen research capacity on conflict-related sexual violence, and thereby contribute to an improved knowledge-base to inform the response. Despite the growing research in this field, there is a lack of standardized definitions and methods, which constitutes a challenge in comparing and relating findings across studies. Thus, we will focus on identifying methodological challenges, proposing new and improved methodologies, testing these through a small pilot project, and based on the results, develop joint research applications for large-scale research. Guiding research questions are: what are the challenges and limitations in research methodologies, how can tools and processes be strengthened to better ensure that survivors feel safe and secure about reporting conflict-related sexual violence, and how can the improved tools and processes be used to access hard-to-reach groups? During year 1 we will arrange joint workshops/conferences, and design and implement the pilot study at a refugee camp. During year 2 we will assess the results from the pilot study, disseminate these through workshops/conferences at all 3 locations, and develop joint applications. The collaboration will yield increased knowledge and data regarding methodological research challenges related to conflict-related sexual violence.

At least two million lives are... lost every year globally due to complications in the intrapartum period. We aim to initiate research to prevent this high number of stillbirths and neonatal deaths by forming a consortium of researchers working in large referral hospitals in East Africa. We aim to define and innovate interventions and perform implementation research focused on the recently published new intrapartum care guidelines of the World Health Organisations. Respectful intrapartum care shall be in the centre, and we will assess the potential of lactate point-of-care testing to diagnose hypoxia and fetal monitoring using the new Moyo Fetal Heart Rate Monitor.Network partners are Makerere University and Iganga hospital, Uganda, Muhimbili University Hospital and Aga Khan University, Tanzania, the College of Medicine in Blantyre, Malawi and Hospital Central Maputo, Mozambique. During the two-year period for which we apply for funding we aim to 1) innovate implementation packages to improve the provision of safe and respectful intrapartum care, 2) review present systems of perinatal data collection or e-registries in identified hospitals 3, investigate into the feasibility of improved diagnosis of peripartum hypoxia using umbilical cord lactate measurement, and 4) prepare jointly research questions and applications. The aim of this collaborative ...project is to further develop a network of researchers, clinicians, policy makers and others to ensure access to safe and comprehensive abortion care (CAC), including post abortion care (PAC) and provision of effective and acceptable post abortion contraception to reduce maternal mortality and morbidity from unplanned pregnancy and unsafe abortions in Uganda. Our previous study confirms the efficacy of PAC provided by midwives in 1st trimester pregnancy. An on-going trial will provide the evidence for safe medical management of 2nd trimester PAC. The network led by researchers from Karolinska Institutet-Makerere University and the planned activities will provide a platform for implementation of evidence to practice by i) task shifting to midlevel providers in PAC, ii) by evaluating telemedicine provision of CAC and, iii) by providing information on the safety, efficacy and acceptability of post PAC provision of intrauterine contraception. Close collaboration between partner-researchers will facilitate study planning, implementation, analysis and dissemination to relevant stakeholders. Throughout the study period, in-service training of medical management in CAC, PAC and provision of intrauterine contraception will be held, and workshops and policy dialogues with stakeholders will help to tailor-make service guidelines and to lobby for inclusion of CAC in policy. Comprehensive Sexuality Educat...ion (CSE) promotes health and well-being, respect for human rights and gender equality. Integration of CSE in schools in Zambia, just like in many other low and middle-income countries (LMICs) has not been optimal. Research that explores how such integration shapes sexuality decisions among adolescents is very scarce if not absent. Further few studies take a gender lens to evaluate mechanisms shaping integration of the programmes. This community based participatory research driven realist evaluation project aims to ascertain: why, how, and under what conditions the integration of sexuality education into the schools will lead (or not) to improved knowledge and capacity to make positive sexuality decisions among adolescents, including more equal gender relations.We will adopt at step-wise approach to the realist evaluation and embed it in participatory research phases to 1) map and analyse challenges towards integration 2) develop an initial program theory that proposes an explanation of how the integration of a sexuality education into the school setting can promote positive sexuality decisions among adolescents, 3) test the programme theory through multiple-case studies applying a quantitative survey, photovoice elicitation, observations as well as qualitative interviews and 4) finally propose a refined programme theory and set of recommendations which will provide useful lessons to guide integration processes and actions.

Despite the well-established n...otion that sexual and gender-based violence runs along a continuum, there is little empirically based scholarship that explores how peacetime rape and wartime rape differ and/or are similar. Does it matter to the ways we understand, prevent and redress the harms of rape that some occur in zones of relative peace, and some in zones of active armed conflict? Indeed, there has been a marked divergence in the explanatory frames for explaining rape in peacetime in relation to wartime rape among scholars and advocates alike. In this project, we address this question through in-depth studies of the forms, contexts and logics of sexual violence in four different sites, two of which are clearly deeply entrenched warzones (Uvira Provence in the Democratic Republic of the Congo and Kashmir, in India) and two that are characterized by long-time peaceful relations and the lack of armed conflict (Mai-Ndombe in the DRC and South Delhi, India). Drawing on the uniquely grounded experience of the investigators, the project will thus produce novel insights on the continuum and non-continuum of sexual violence between war and peace. In addition to providing original empirical data on how the forms, contexts and logics of sexual violence differ and converge by collecting original data in two countries (the DRC and India), it also seeks to enhance our understanding of the frames that limits our abilities/willingness to note and address continuances of violence.

Background: Maternal anemia is... among the underlying causes of maternal mortality in low-income settings such as Uganda. In South Africa 40% of maternal deaths are associated with anemia. Majority of adolescent girls who become pregnant have underlying depleted nutritional reserves. The World Health Organization has recommended preconception anemia care with micronutrients supplementation. Evidence is lacking on the feasibility, acceptability, use, effectiveness and health system impact of preconception anemia care in low-income settings despites being a beneficial strategy. Objectives: To establish and evaluate a pilot research on prevention of maternal anemia through preconception anemia care with the aim of reducing maternal and newborn mortality in low-income countries. Specifically, to determine the feasibility, acceptability, use, effectiveness and health system impact of preconception anemia care for young women in low-income settings.Methodology: After a baseline assessment the preconception anemia care will be set up with a minimum package for preconception care as health education and counsellinig. Quantitative and qualitative research methodologies will measue its effects, patient exit interviews, key informant interviews of health workers and leaders, in-depth interviews with patients and health workers, focus group discussions with women and clinical record abstraction. The members of this network will participate in data collection, evaluation and dissemination.

A collaboration between Sweden... and Somaliland to build a research program for capacity development of midwives: This collaboration is a response to The Somali Midwifery Strategy key area: Strengthening and investing in midwifery education and research.The initiative, involving academia in Sweden and Somaliland, begins the process of building an agenda for midwifery research in the Somali region. The research agenda aims to contribute with scientific knowledge being an foundation for the future design and testing of innovative models of care for the development of midwifery-led care in the Somali context. The expected output is a research agenda in line with the research ideas to describe the magnitude of life-threatening, abortion-related complications and women's access to quality PAC and contraceptives in post-conflict Somaliland.A core group of researchers will be jointly responsible for activities in the collaboration plan. The project, starting in January 2021, will run for two years, during which the following activities are planned: build dialogues with the Somali Health Authorities and key UN health agencies, and engage them through workshops on evidence-based findings related to midwifery care, conduct research-agenda workshops and set research agenda/priorities, write joint publications and proposals, and disseminate the research agenda and proposals. The long-term goal of this project is to contribute scientific evidence to scale up the quality of midwifery care to reduce maternal mortality and morbidity in relation to giving birth in Somaliland.

Sida is planning a second phas...e of the RMNCAH/N Health programme on reproductive, maternal, newborn, Child, adolescent health and nutrition in Zambia 2015-2019, extended up to June 2020 and with a planned additional extension up to December 2020 adjusting to COVID-19. A new programme is expected to start during the first half of 2021. The next phase will be assessed during the autumn of 2020 and will be a follow-up to Phase I but with a stronger focus on supporting high impact interventions and systems strengthening to ensure continued progress to reduce maternal and newborn deaths and the high number of teenage pregnancies and malnourished children. Community Health and adolescent SRHR will be further enhanced. In line with this and to ensure that appropriate data is available for the right type and direction of interventions, it is proposed to conduct an analysis of the recent Demographic Halth Survey 2018, DHS, from a more systematic Equity perspective. During the spring of 2019, a Mid-Term Review of the RMNCAH/N programme was conducted by Sida's Health and SRHR framework agreement consultancy firm KIT. Oe major take-away from the MTR findings was that Sida needs to better address and programme interventions and activities from an Equity perspective. This includes improved targeting based on inequities stemming from socio-economic, gender, geographic, age-related factors. The Ministry of Health has also requested more in-depth analysis to identify pockets of inequities in service provision and quality of care across the country. When the RMNCAH/N programme Phase I focused on more generalized interventions to improve service delivery, it has been assessed that it is now critical to support tailor-make interventions based on more individual capacities, contexts and needs in various districts in the provinces supported. In 2019, WHO African regional office and Zambia's Ministry of Health published a report on Health Equity and Social Determinants of Health in Zambia to gather evidence to support a comprehensive policy move towards Universal Health Coverage in Zambia. The Zambian Government is committed to fulfil its aim to reach the SDGs Before 2030 and to leave no one behind - a phrase which is often heard in various policy and health-decision making fora. The Embassy has contacted the School of Public Health at the University of Zambia, SoPH, as they have been responsible for conducting a recent study of the DHS 2013/14 from a Health inequality perspective earlier in 2019, based on the WHO study. A dissemination of results took place in August 2019. As the Embassy started planning Phase II of the Health programme, SoPH were approached to consider repeating a similar study on the DHS 2018 to ensure analysis of recent data. the goal of the Kenya Health P...rogramme is to reduce health inequalities and reverse the downward trends in health outcome and impact indicators in Kenya. It aims to increase access to affordable, quality, basic health services.
The funding will support:
1) Free distribution of bed-nets to pregnant women and children under 5
2) Social Marketing of condoms to reduce HIV infection
3) Increased and consistent use of Family Planning commodities to improve family planning
4) Improving Health systems and accountability mechanisms
5) Improving the efficiency of government health facilities
6) Piloting Health Financing mechanisms to improve access to government health services.
The UK funding is expected to
1) Distribute 6.48 million bed-nets to pregnant women for malaria
2) Treat 522,400 pregnant women for malaria
3) Provide 662,000 women with family planning
4) Distribute 173.5 million condoms
This will avert more than 36,000 malaria deaths, 16,765 HIV infections and 770,800 unwanted pregnancies
