Malaria has become one of the ...major public health problems in Djibouti. The goal of the program supported by this grant is to reduce malaria-related sickness and death by 50 percent in the country by improving access to rapid, adequate and affordable treatment to 80 percent of patients; reinforcing prevention by integrating a larva destruction program with indoor residual spraying and the use of insecticide-treated nets; and establishing a comprehensive health information system able to provide reliable and quality data. The program targets pregnant women, children under 5 years of age and the migrant and refugee population.

Malaria is endemic in rural pa...rts of the Dominican Republic, and the country has recently seen growth in the number of cases reported, possibly due to the heavy rains brought on by tropical storms. The goal of the program supported by this grant is to reduce malaria-related sickness and death rates in the 14 municipalities highly endemic for P. falciparum malaria. The program is setting up an integrated and decentralized malaria epidemiological surveillance system that provides useful information for decision-making at the local level. The program is also providing early diagnosis, timely treatment and adequate management of severe cases of malaria in a decentralized manner. Grant funds are being used to implement a social communication and community participation strategy to address malaria prevention control and to distribute insecticide-treated nets to highly vulnerable populations. The program is targeting the total population in the 14 municipalities it covers, with a particular focus on Haitians and people of other nationalities living in those areas.

Malaria is the principal cause... of sickness and death in Equatorial Guinea, accounting for approximately one third of total sickness and death. The program supported by this grant seeks to reduce malaria-related illness and death on the mainland of Equatorial Guinea with an emphasis on vulnerable groups through an integrated set of interventions that include prevention and treatment. The program targets children under age five, pregnant women and families vulnerable to malaria. Grant funds are being used to expand indoor residual spraying to cover continental regions; expand access to and utilization of long-lasting insecticide-treated bed nets; expand the use of preventative medications among pregnant women and improve case management of both uncomplicated and severe malaria by using improved treatment protocols.

In Swaziland, malaria transmis...sion is more prevalent along the eastern, northern and southern borders, mainly in the Lubombo region. Plasmodium falciparum is responsible for 99 percent of malaria cases. The program supported by this grant aims to prepare Swaziland for malaria elimination. To enable the country to achieve this goal, the program focuses on four major intervention: case management, which includes interventions aimed at ensuring that all potential malaria cases are definitively diagnosed by rapid diagnostic tests or microscopy, and that chloroquine is replaced the artemisinin-based combination therapy for treating all confirmed malaria cases; integrated vector management, which involves extending vector control coverage to all households in malarious regions through a combined campaign for indoor residual spraying and distribution of long-lasting insecticide-treated bed nets; active surveillance, which involves establishing malaria surveillance sentinel sites around the country to conduct surveillance and develop an epidemic preparedness and response program to actively prevent and promptly respond to all epidemics; and a comprehensive and sustained information education and communication campaign to ensure continued strong engagement of all segments of Swazi society.

The program supported by this grant aims to reduce malaria-related sickness and death in the population at risk by 50 percent. The program is using an integrated approach: preventive measures in vector-targeted interventions through insecticide-treated bed nets, surveillance and treatment measures to reduce malaria transmission in relatively stable and malaria-free regions; control of epidemic outbreaks in areas where transmission is unstable; and support for communities in recognizing and treating the disease early before it causes harm. The program targets pregnant women, children under age five and the general population at risk for malaria.

In Guyana, malaria is an endem...ic disease mostly limited in its geographical spread to the hinterland regions and is currently in its control phase, with a general trend to move towards pre-elimination. Nevertheless, it remains to date one of the most common causes of morbidity in the hard-to-reach regions where access to health care remains very challenging. This Program focuses its activities in the four most affected regions of Guyana (1, 7, 8 and 9) comprising 98% of all malaria cases. The Program aims at the achievement of the National Malaria Control Program Strategic Plan 2008-2013 by scaling up key prevention, diagnosis and treatment interventions to reduce the social and economic impact of malaria in the targeted regions, thus mitigating malaria's contribution to poverty in the country. The Program goal is to reduce the malaria morbidity by 70% at the end of August 2016 in these regions. This Program is a consolidation of the Round 7 and Rolling Continuation Channel (RCC) Round 3 malaria grants.

Malaria is a major public heal...th issue in Madagascar with 92 percent of the population exposed to risk. The program supported by this grant aims to increase the correct use of affordable, high-quality artemisinin-based combination therapies (ACTs) by adolescents and adults to 75 percent by 2012. To reach this goal, the program will create publicity campaigns to raise awareness about the availability of affordable, high-quality ACTs; conduct behavior change communication activities among the population at large to encourage the use of ACTs; train doctors and nurses on the proper way of treating malaria, i.e. with ACTs and drug monitoring; build technical capacity for drug monitoring in 769 private health care facilities; and supervise private health care facilities, pharmacies and small dispensaries with an aim to promote ACTs and monitor the prevalence of antimalarial monotherapies.

The overall goal of the program supported by this grant is to contribute to the reduction of illness and death linked to malaria in Mali. Malaria continues to be a seri...ous health issue in Mozambique being responsible for up to 44% of all outpatient consultations, 57% of pediatric admissions and 30 % of all death registered in hospitals. The goal of the Program is to protect all populations at risk from malaria through long lasting insecticidal net (LLIN), indoor residual spray (IRS) and intermittent preventive treatment (IPT) by 2014; provide appropriate malaria case management to all persons at risk; and achieve effective malaria control management by National Malaria Control Program (NMCP), and provincial and district level authorities. The Program will target pregnant women and children, the rural population and residents of areas not yet covered by IRS. This Program is implemented by two Principal Recipients (PR) under the dual-track financing scheme. This PR will focus on procuring long lasting insecticidal nets (LLINs) and Rapid Diagnostic Tests (RDTs); conduct annual LLINs distribution, mass media campaigns and surveys of LLINs distribution; develop guidelines and train teams to conduct indoor residual spray (IRS); and conducting insecticide resistance testing.

Vulnerable populations include children under 5, pregnant women and HIV positive people who inequitably carry this burden. The overall goal of this program is to reduce malaria-related morbidity and mortality rates in PNG. Specifically, it will scale-up the successful universal insecticide-treated nets program and support positive behavior change in the broader population. The Program continues to build national capacity for the National Malaria Control Program (NMCP) and to improve the quality of preventive, diagnostic and treatment skills in the public health workforce, particularly concerning infants, children and adults, who have had episodes of malaria. The achievements gained through the universal insecticide-treated nets coverage program of the last six years supported through the Global Fund in Round 3 and Phase 1 of Round 8, the Government of PNG, the Rotary Club of Port Moresby (RAM) and other development partners have developed the foundation for Phase 2 of the Round 8 grant. It is a triple-track grant and this Principal Recipient, (Population Services International) focuses on home-management of malaria and as well on improving knowledge, behavior and participation of communities and individuals for effective malaria control.

The program supported by this grant aims to scale up efforts of the national control program in six epidemic-prone districts, focusing on poor and vulnerable populations at increased risk of malaria due to occupational factors — specifically, gem miners and farmers engaged in 'slash and burn' type cultivation. Because of the remote locations in which these occupations are usually practiced, the standard mechanism of disease surveillance fails to detect malaria patients in these groups at the early stages of the disease. The main planned activities include mobile malaria clinics; provision of rapid diagnostic tests and long-lasting insecticidal nets; an increase in entomological investigations to forecast and prevent malaria outbreaks; promotion of the use of larvivorous fish and refilling of abandoned gem pits to control the vector; and production and distribution of information, education and communication materials.

Sri Lanka is the first Southea...st Asia country to have successfully controlled malaria. Since 2,000, no patient has died from the disease. In the pre-eradication era, nearly 40 percent of the population was subject to malaria annually. The program supported by this grant aims to strengthen active case surveillance through the use of several interventions aimed at actively seeking parasite-carrying individuals in the community. For this purpose, the program will use mobile malaria clinics to detect symptomatic and asymptomatic malaria cases. In addition to strengthening active surveillance, the program will ensure total indoor residual spraying coverage in foci and the distribution of long-lasting insecticidal nets and other vector control measures as complementary measures in specific situations; implement an outbreak preparedness and rapid response strategy for early containment of outbreaks; contain imported malaria through the prevention of malaria in travelers; and develop appropriate web-based databases for illness and vector surveillance.

Malaria was eliminated in the ...1980s, but it reappeared the following decade when nearly 30,000 cases were reported in 1997. This was due to the economic upheaval triggered by the breakup of the Soviet Union, civil war and changes in agricultural practices, particularly the increase in rice cultivation, which spawned mosquito breeding grounds. The program supported by this grant aims to scale up efforts to prevent deaths caused by malaria, to interrupt P. falciparum malaria transmission, to reduce malaria-related illness and to contain and prevent malaria outbreaks in Tajikistan. Program objectives are to strengthen institutional capacities of the country's National Malaria Control Program and general health services and to enhance capacities for decision-making related to malaria; to improve capacities for and access to early diagnosis and adequate treatment of malaria; to promote cost-effective and sustainable vector control; to improve capacities for timely response to and prevention of malaria outbreaks and epidemics; to reinforce country surveillance mechanisms; to strengthen research capabilities; and to increase community awareness and participation in malaria prevention.

The program supported by this ...grant focuses on introducing artemisinin-based combination therapy as the treatment of choice for malaria case management in Tanzania. Progressive resistance to other first-line treatment option s for malaria including chloroquine has contributed to the continued high rates of malaria-related deaths, particularly in children. The program is supporting the revision of malaria treatment guidelines and public awareness campaigns aimed at sensitizing health care providers and the general population of the policy change. The program targets the entire population of Tanzania, including children under age five and pregnant women.

1. Background and Summary: Th...e Program is intended to scale up the existing malaria interventions and consolidate on the gains made by the program to further reduce malaria transmission in the general population. This will be achieved by maintaining a strong focus on case management and diagnosis as well as preventive measures. The prevention strategy will emphasize 'scaling up for impact' by ensuring high coverage of LLINs in the general population. Indeed the wide scale distribution of insecticide treated nets has been a significant contributing factor to the 35% decline in mortality of children under five between 2000 and 2004 (Masanja et al. 2008). The Program will be managed by the Ministry of Health and Social Welfare (MOHSW) through the National Malaria Control Program, a department under the MOHSW. The objectives of the program are: a) To improve early recognition of malaria and access to effective diagnosis and case management through proper diagnosis of malaria and provision of Prompt, effective anti-malarial treatment; b) To prevent infection with malaria by maintaining high coverage of long-lasting insecticidal nets (LLINs), with specific emphasis on vulnerable groups, complemented by other preventive community outreach methods such as control of malaria during pregnancy and Behavioral Change Communication (BCC); and c) To strengthen the surveillance system, operational research and program management. Additionally, the cross-cutting HSS interventions in the Program will strengthen the MOHSW and community support for HIV/AIDS, tuberculosis and malaria through improved service delivery, program integration and information systems. The cross cutting interventions will include a well defined and responsive BCC strategy in line with the need to reinforce key malaria control messages as well as tailor these to a changing context of reduced malaria transmission and its potential impact on behaviors and vulnerabilities. 2. Goal: To contribute to the reduction of morbidity and mortality attributed to malaria by increasing and sustaining the demand for and utilisation of proven malaria interventions in the population of Zanzibar. 3. Target Group/Beneficiaries: • Entire population of Zanzibar (specifically, children under five years of age and pregnant women) 4. Strategies: • Malaria Case Management including Malaria in Pregnancy using ACTs • Integrated Vector Control • Malaria Surveillance 5. Planned Activities: Prompt effective antimalarial treatment and diagnosis. • Procure 2nd line (quinine injectableS and tabs) • Procure 1st line ACTs • Central Medical Stores (CMS) clearing, storage and handling costs for medicines and diagnostics • Train health workers on case management • Supportive supervision • Equip and maintain Zanzibar Malaria Control Programme (ZMCP) referral laboratory • Train laboratory technicians / staff • Provide equipment to public laboratories • Procure lab consumables and Rapid Diagnostic Tests (RDTs) • Provide support

Malaria is endemic throughout ...Togo, peaking during the rainy season. According to the National Health Information System (NHIS) 2007 report, it accounts for half of all hospitalizations in the country and is the primary cause of mortality in health care facilities. The program supported by this grant aims to reduce the malaria burden by increased coverage of antimalarial activities, building on results financed by three previous Global Fund grants (from Rounds 3, 4 and 6), with the specific objective of achieving universal coverage of long-lasting insecticidal nets by early 2012. Strategies include a mass distribution campaign for nets, and enabling correct diagnosis and treatment of malaria at community level. The program is supported by two Global Fund grants implemented by a government and civil society Principal Recipient, respectively. This grant focuses on provision of rapid diagnostic kits and treatment (including quinine, as well as artemisinin-based combination therapy), supplying laboratories with equipment and consumables, and strengthening transportation means for health personnel as well as organizing their technical and administrative supervision. The entire population of Togo will benefit from the program, which targets population groups most at risk from malaria: pregnant women and children under 5.

The program is also focusing on ensuring demand for and correct and efficient use of highly effective malaria treatment and implementing artemisinin-based combination therapy in communities using the home-based management of fever strategy. The program targets children under age five, pregnant women and the general population.
