The project development object...ive of the Health Project for the Comoros is to reduce mortality from common diseases by ensuring a better use of health facilities for the delivery of quality health care, and by organizing mosquito control activities to reduce the incidence of malaria. There are three components: 1) rehabilitating and equipping selected health facilities to facilitate the delivery of good quality health care to the population, which will also have a direct favorable impact on cost recovery by those facilities; 2) controlling malaria by improving the case management at all levels of the health service, educating the population for a better use of services and to improve case management at home, and preventive activities including mosquito control to reduce the incidence of the disease; and 3) institutional strengthening to facilitate and support the increased use of quality care by the population and to further implement health sector reforms, including a) training the staff of the public health and population ministry (MOH) and the health facilities staff in the delivery of quality health care, management, and accounting; b) studies to develop alternative mechanisms for health financing; and c) technical assistance, logistical support, and equipment for MOH's central and regional directorates and for PNAC (the agency in charge of marketing drugs).

The project has four major com...ponents: (i) Institutional strengthening of the Ministry of Health (MOH) to provide restructuring of the MOH services, training workshops and seminars for staff at all levels, health center reconstruction and MOH office rehabilitation works, furniture and medical equipment. Reforms aimed at management and financial autonomy for the main hospitals of Malabo and Bata are to be initiated in collaboration with French aid. (ii) Health strategy development and implementation establishing a management information system and a health planning unit in MOH. (iii) Human resource development to train health sector personnel and improve MOH's technical and management capacity at all levels of the health system. (iv) Strengthening of priority health programs to improve MOH's operational capabilities and testing of new approaches to health care delivery by strengthening field work in malaria control, mother and child health and family planning, sexually transmitted diseases, and by providing essential generic drugs, with a special focus on most vulnerable groups. The project finances the rehabilitation of four rural health centers in the remote rural districts of Micomiseng and Acurenam; equipment for the mother and child and family clinics in Malabo and Bata, and supplies of essential medical materials and drugs to all these facilities. Cost recovery is introduced through a differentiated fee structure.

The Health Sector Development ...Program Project for Ethiopia aims to develop a health system which provides comprehensive and integrated primary care services, primarily based at community health level facilities. It focuses on communicable diseases, common nutritional disorders, environmental health and hygiene, reproductive health care, immunization, the treatment and control of basic infectious diseases like upper respiratory tract infections, the control of epidemic diseases like malaria, and the control of sexually transmitted diseases especially HIV/AIDS. The project has eight components. They 1) expand primary health care access; 2) improve the technical quality of primary health care service provision; 3) expand the supply and productivity of health personnel; 4) ensure a regular and safe supply of effective, safe, and affordable high quality drugs while improving prescribing behavior by health providers; 5) improve awareness about personal and environmental hygiene and basic knowledge of common diseases and their causes as well as promote political and community support for health services; 6) transform the health system into a four-tiered system that is linked, equitably distributed, and managed in a decentralized, participatory, and efficient manner; 7) monitor improvements in service delivery, quality, and financial performance and evaluate the impact, effectiveness, and cost-effectiveness of the project's components; and 8) improve financial sustainability.

The immediate project beneficiaries are women and children dependent on primary health services for

The objectives of the Second S...tate Health Systems Development Project are to assist the governments of Karnataka, Punjab and West Bengal to: 1) improve efficiency in the allocation and use of health resources through policy and institutional development; and 2) improve the performance of the health care system through improvements in the quality, effectiveness and coverage of health services at the first referral level and selective coverage at the primary level to better serve the neediest sections of society. The ultimate goal of the project will be to improve the health status of the people, especially the poor, by reducing mortality, morbidity and disability. The project consists of three main components: 1) management development and institutional strengthening, which include: (a) improvement of the institutional framework for policy development; (b) strengthening of management and implementation capacity; and (c) development of surveillance capacity for major communicable diseases; 2) improvement of service quality, access and effectiveness at district, subdivisional and community/rural hospitals involving: (a) renovation and extension of district, subdivisional and community/rural hospitals; (b) upgrading of the effectiveness of clinical and support services and quality of services; and (c) improvement of the referral system; and 3) improvement of access to primary health care in remote and underdeveloped areas, which consists of: (a) upgrading of primary health centers in the Sunderban area of West Bengal; and (b) increase in the access to primary care services among scheduled castes/scheduled tribes population in Karnataka.

The Maharashtra Health Systems... Development Project seeks to assist the local government to: 1) improve efficiency in the allocation and use of health resources; and 2) improve the performance of the health care system at the first referral level and selective coverage at the community level. There are three project components. First, the management development and institutional strengthening component will improve the institutional framework for policy development; strengthen the management and implementation capacity at the state, divisional, district, and facility levels; and develop surveillance capacity for major communicable diseases and strengthening Health Management Information System. Second, improving service quality and effectiveness at district and sub-divisional hospitals will renovate/extend district hospitals and upgrade selected community health centers (CHCs) to sub-divisional hospitals and construct training centers at four remaining district hospitals. The third component, improving access and innovative schemes, will a) renovate/extend and upgrade clinical effectiveness at 35 CHCs and enhance their outreach functions; b) improve the referral mechanisms with the primary and tertiary levels and with private health care; c) promote health services in tribal areas and for disadvantaged groups; and d) develop a superspeciality hospital as an innovative scheme for closer cooperation between the public and private sectors.

The Health System Reform and M...alaria Control Project will improve the health status of the Loas population, and to reduce morbidity and mortality rates in selected parts of the country. The project will specifically improve: (a) the quality of basic health care, particularly for infants, children and women through health system reform for priority health programs, including family planning; (b) strengthen the malaria control program in the country; and (c) improve health awareness and health behavior in the population by strengthening the Ministry of Health (MOH) Institute of Health Education to develop health education messages; and (d) build the capability of the MOH and of participating health systems to implement project activities. The project will consist of the following components: (a) policy reform; (b) service delivery; (c) institutional development; and (c) community mobilization.

The Health Sector Development ...Project, will assist the Government of the Solomon Islands improve rural communities' health, by strengthening reproductive health and malaria programs, through new testing methods, and improved planning, management, and monitoring of priority health programs. The project components call for: a) improved reproductive health, in provincial services, focusing on training for medical, and nursing staff, including clinical supervision, and upgrading of rural health facilities. International assessment, and monitoring guidelines for health quality will be adapted, and, an inter-sectoral task force, with provincial representation will provide feedback to the Ministry of Health and Medical Services (MHMS); b) improved malaria prevention and control, which will provide funds for the National Malaria Control Program, while improving analysis and monitoring, of factors affecting program success, while improving its effectiveness. An inter-sectoral task force will support, and fund pilot studies for an improved community response; c) capacity building, to strengthen the MHMS institutional capacity in health planning, emphasizing health management information systems, monitoring evaluation/research, and, donor coordination; and, d) project coordination and management, which will provide support functions, including financial, and procurement management.

The development objective of t...he Maternal and Child Health (MCH) and Nutrition Services Support Project for Togo is to increase utilization of selected MCH and nutrition services for pregnant women and children. The project has two components. The first component is improved utilization of malaria and nutrition services to address malaria in pregnancy through the provision of intermittent preventive treatment for malaria in pregnancy (IPTp) with sulfodoxinepyrimethamine (SP) and free long-lasting insecticide nets (LLINs), to pregnant women attending antenatal care (ANC). It has following two sub-components: (i) support for: (a) malaria control in pregnancy through provision of LLINs and SPs as part of the basic package of ANC services; (b) community-based diagnosis and treatment of malaria; and (c) management, supervision, and behavioral changes to ensure effective utilization; and (ii) community- based nutrition services for pregnant women and young children under five years of age. The second component is strengthening health monitoring and evaluation systems; project management. The National Health Support Pr...oject's overall objective is to improve the health status of the rural population of Viet Nam in the poorer areas of the country. Specifically, it will aim to: 1) provide high quality, reliable primary health care on a sustainable basis in fifteen of the poorer provinces; 2) reduce nationwide mortality and morbidity due to malaria, tuberculosis and acute respiratory infections, as well as the adverse socio-economic impacts associated with these diseases; and 3) strengthen the capacity of the ministry of health (MOH) in policy-formulation, planning and management. The development objectives of ...the Health Reform Support Project (HRSP) are to: 1) increase access for women and children to a package of integrated maternal and child health services provided in district hospitals and health centers in selected districts; and 2) to improve the effectiveness of national public health programs and resource allocation within the public health sector in Yemen. There are three main project components. The first component strengthens health services delivery at the district level, as part of the policy of the Ministry of Public Health and Population to operationalize a District Health System (DHS) by upgrading district hospitals and health centers in eight districts and ensuring the effective delivery of a package of integrated maternal and child (PIMAC) health services together with supporting the decentralization of resource management. The second component improves the effectiveness of public health programs by strengthening two critical programs, namely, malaria and health education. It finances civil works for minor renovations of referral laboratories, proivides equipment and supplies for laboratories and vector control and disease surveillance, computers and vehicles, emergency stocks of essential drugs and insecticide treated materials, staff training, etc. The third component supports human resources development in the areas of financial analysis and health systems management.
