For twenty years now a tubercu...losis epidemic is going on in Ukraine. Ukraine was ranked second in Europe in tuberculosis incidence in 2018 (according to the Ministry of health of Ukraine). At the same time in Ukraine there are not enough possibilities for tuberculosis treatment. Hospitals do not have modern equipment and technologies to diagnose and treat tuberculosis. The TB dispensary Zaporizhzhia, like many other hospitals in Ukraine, uses outdated equipment produced in the days of the Soviet Union. The project makes it possible for the TB dispensary to buy a modern Medical x-ray processor which is going to improve and speed up the diagnostic process, as well as to reduce the consumption of electricity and water (at the moment 500 litres per day), previously required for the development of x-rays. This project is one of the micro-projects of the Consulate General Donetsk. The local implementer of the project is the charity fund 'HELPUS'.

1. Background and Summary Mala...ria is widely recognized to be a major public health priority in Afghanistan requiring continued international assistance. The disease is endemic in large areas below 2000 meters in elevation and is highly prevalent in river valleys used for rice cultivation. With an estimated population of 26.1 million in 2007 (WHO), approximately 80% of whom live in rural areas, it has been estimated that the annual incidence approaches 1.25 million cases per year . Recognizing the unacceptable burden this places on the health and economic development of the nation, the Government and the Ministry of Public Health (MoPH) are firmly committed to the control of the disease. In this context, the MoPH and the National Malaria and Leishmaniasis Control Programme (NMLCP) have endorsed the Global and Regional Roll Back Malaria (RBM) strategies and the Millennium Development Goals (MDGs). In support of this program, a detailed Gap Analysis was conducted in March 2008 and a revised National Malaria Strategic Plan (NMSP) was developed for 2008-2013. The current program is based on the revised Strategic Plan 2008-2013, considering the funding provided by the Global Fund in Round 5. A comprehensive spectrum of activities shall be carried out to achieve the stated Goal and Objectives of the NMSP. These initiatives will sustain and expand the set of activities supported during the Global Fund Round 5 grant period and propose innovative approaches to accelerate Programme implementation. Key strategies include improving access to early diagnostic services by expanding the network of laboratory services to rural areas, introducing Rapid Diagnostic Tests (RDTs) at the community level, upgrading case management policies through revised evidence-based National Treatment Guidelines (NTGs) and promoting their proper utilization in both the public and private sectors, engaging Community Health Workers (CHWs) to provide Home-based Management of Malaria (HMM), utilizing new distribution modalities to provide free long-lasting insecticide-treated bed nets (LLINs), activating creative Behavioural Change Communications (BCC) modalities to improve awareness and promote behaviour change, and strengthening Quality Assurance mechanisms in all aspects of Programme implementation. Specific measures will address efforts for the prompt detection and control of epidemics, to improve Monitoring and Evaluation (M&E) activities and to strengthen Programme capacity to sustain this ambitious set of initiatives. Selected operational research activities have been designed to add to the scientific basis for continued strategic planning and to implement an aggressive integrated set of carefully-planned interventions to eliminate Plasmodium falciparum (P. falciparum) from the border areas of Tajikistan. The malaria situation in Azerb...aijan deteriorated rapidly after 1990 due to dramatic worsening of socioeconomic conditions and a nearly complete cessation of preventive activities. In addition, malaria control efforts in the southwestern part of the country were complicated by high mobility of the population and the Nagorno-Karabakh conflict. New malaria cases hit a peak of 13,135 in 1996. This prompted the government to develop the National Malaria Control Program in 1998 in cooperation with World Health Organization (WHO) Regional Office for Europe. By 2006 Azerbaijan's malaria situation had improved dramatically, with only 143 new cases reported. Although the incidence is now relatively low - less than one infection per 1,000 people – the threat of renewed outbreaks remains high: 80 percent of the country is considered to be prone to malaria transmission and active foci are reported in many districts. The overall goal of the program the Round 7 grant supports is to reduce the transmission and incidence of P. vivax malaria and to prevent malaria outbreaks. The program is based on the National Malaria Elimination Strategy for the period 2008–2013 and aims to achieve its goals by training health staff in malaria-related planning, epidemic preparedness and monitoring, in line with the WHO's technical guidelines on malaria control and prevention, update existing legislation and regulations on malaria control, upgrade the country's laboratories in order to improve diagnosis of malaria cases and strengthen national capacities for timely response and prevention of malaria outbreaks.

The malaria situation in Bangl...adesh has worsened in recent years, particularly in the hill tract districts located in remote forested border areas. Health infrastructure in these areas is inadequate and access to health care services is poor. The seasonal influx of non- immune labourers into these areas further exacerbates malaria transmission. The program supported by this grant combines funding from grants in Rounds 6 and 9 and aims to scale up and strengthen malaria control activities in Bangladesh. The program's three major objectives are to provide quality diagnosis and effective treatment to 90 percent of estimated malaria cases in the 13 highly endemic districts by 2012, to promote the use of long-lasting insecticidal nets (LLINs) in 80 percent of households in these districts and to use selective indoor residual spraying for the containment of outbreaks. In close collaboration with the other Principal Recipient (PR), the current grant will focus mostly on procurement and supply management of LLINs/ITNs, treatment or Ordinary Mosquito Nets, diagnosis, training, monitoring and evaluation, and information management.

With a goal of reducing malaria-related sickness and death among pregnant women and children under the age of five in the Mono and Couffo regions, the program supported by this grant will extend the implementation of malaria control activities, by promoting the use of insecticide-treated bed nets through women's groups and health facilities; and by increasing access to early diagnosis and treatment with artemisinin-based combination therapies. In addition, the program aims increase awareness about malaria prevention and treatment.

Malaria is a major public heal...th concern in Bhutan, affecting more than 70 percent of the country's population and putting people in 15 of the country's 20 districts at risk of becoming infected. Malaria is considered endemic in five districts which border on the Indian states of West Bengal and Assam. An additional ten districts are epidemic prone. The program supported by this grant provides scale-up and continuation of existing program activities with an increased health systems strengthening component. The overall goal of the program is to contribute to a 50 percent reduction in malaria-related sickness and death by 2013 as compared with 2005. The grant has been consolidated with BTN-405-G01-M.

Malaria is endemic to the Amaz...on region of Bolivia. The prioritized area for this program comprises 36 of the 75 municipalities where malaria transmission was verified during 2007. Ninety-five percent of the malaria load is concentrated in these 36 municipalities, of which 5 have an annual parasite index (API) of more than 50, and 14 municipalities have an API of less than 10. The rest have an API of less than 1. Populations susceptible to malaria include indigenous peoples, mobile, agricultural and rural populations and armed forces personnel. The goal of the program is to accelerate the process of reducing malaria mortality in the country, prioritizing the elimination of malaria by P. falciparum on a national level. The strategy consists of improving the services for promotion, prevention and management of malaria cases and consequently extending coverage in the country's most endemic area. The program focuses its activities on the country's 36 municipalities where the disease is highly endemic, accounting for 95 percent of the country's malaria cases and 100 percent of malaria cases by P. falciparum. Implementation of the program, recruitment of additional staff and strengthening of the voluntary collaborators' network in the Brazil nut harvesting area is expected to improve and increase timely diagnosis and treatment of malaria by at least 50 percent in the program area.

Malaria is the prime cause of ...morbidity and mortality in Burundi. Between 1991 and 2000 the country experienced an unprecedented malaria epidemic with over a threefold increase in the number of malaria cases. The program supported by this grant will expand the implementation of existing malaria control activities, including antivectorial strategies and epidemiological surveillance systems. The objective of the program, which is aimed at pregnant women, children under five and non-immune populations from the high plateau region, is to reduce the illness rate linked to malaria in health units by 50 percent. The Round 2 Program contributed to the reversal of the gradual upward trend of malaria cases seen in Burundi every year. Prevalence of malaria went from 46% in 2000 down to 29% in 2006.

Malaria is one of the leading ...public health problems in Cambodia and the primary cause of hospitalization and hospital deaths. The program supported by this grant will build on and further strengthen the decentralization policy of the government of Cambodia; scale up the piloted approaches to malaria control, such as the introduction of women change agent educators, participatory theater, malaria school health education and the child-to-child approach as well as social marketing of insecticide treatment kits for hammock and insecticide-treated nets in a phased manner. The program will also fill existing gaps in the country's National Malaria Control Program, especially by expanding the coverage of behavior change communications approaches and by providing free distribution of long-lasting insecticidal nets in remote and rural areas and islands.

While morbidity has been stead...ily declining during the last ten years, malaria remains one of the leading causes of mortality in Cambodia. In 2009 more than 150,000 cases of malaria were reported. But the true burden is likely to be much greater, as two-thirds of those who seek treatment for fevers do so in the unmonitored private sector. The overall goal of the program financed by this grant is to move toward pre-elimination of malaria across Cambodia, with special efforts to contain artemisinin-resistant P. falciparum malaria. Strategies include improving access to early diagnosis with the objective of detecting all malaria cases; improving access to artemisinin-based combination therapies in both the public and private sectors, so as to prevent use of monotherapies and substandard drugs; measures for vector control; implementation of an information, education and communication strategy to promote behavior change among the population at risk and mobilize the community to help eliminate the malaria vector. The program will also provide effective management (including information systems and surveillance) to enable rapid and high quality implementation of the pre-elimination strategy. The program targets village malaria workers, mobile populations, village health volunteers, pregnant women, provincial health departments, health centers and private facilities.

Malaria is the leading cause o...f illness and death in Cameroon. It accounts for 40 to 50 percent of medical consultations, 30 percent of hospitalizations, 40 percent of deaths among children under five years, and 35 to 40 percent of hospital deaths. The overall goal of the program supported by this grant is to reduce malaria-related sickness and death by 50 percent among children under five years of age and pregnant women. Among the planned activities are: the collection of baseline date on malaria and the distribution of insecticide-treated bed nets to families with children under the age of five. The program also plans to increase the supply of malaria drugs for intermittent presumptive treatment, train university graduates and health providers on the country's malaria management strategy, equip health centers and district hospitals for diagnosis and follow-up of malaria cases and develop information, education and communication campaigns on prevention and treatment.

In the Central African Republi...c (CAR), malaria is the cause of 58% of outpatient consultations, and 54% of inpatient deaths and incidence is rising, according to the 2010 National Health Data System (SNIS) data. Malaria is also the most significant factor in high maternal and child mortality rates. The overall goal of the Program is to contribute to the reduction of malaria morbidity and mortality rates by 50% in the general population, and specifically in children under five and pregnant women. The grant targets the entire population due to the disease epidemiology in the country. The goal will be achieved by increasing both malaria prevention by 80% in the general population, and through improving the appropriate management of simple malaria cases to 50%, in targeted heatlh centres by 2016. The aim is also to strengthen the operational capacities of the National Malaria Service (Service de Lutte contre le Paludisme) and ensure proper monitoring and evaluation of the programme by 2016. The International Federation of Red Cross and Red Crescent Societies was nominated as the Principal Recipient to implement the Phase 2 of the Round 8 proposal, given their proven competencies in operating in both fragile states, and complex humanitarian emergencies.

The five Special Regions of My...anmar (Myanmar's Kechi Special Region I, Kechi Special Region II, Kanggong Special Region, Shan Special Region II and Shan Special Region IV) have a high burden of malaria. Given the weakness of public health infrastructures in these regions, the implementation of a program managed from within China is deemed an effective strategy to ensure access to malaria services for these populations. Due to its specific geographical scope, it is maintained as a separate funding agreement from grant number CHN-S10-G13-M which supports China's national malaria program. The goal of the Program is to improve the health status and life expectancy of people living in the five Special Regions of Myanmar bordering China, to contribute to the regional effort to prevent the further development and spread of artemisinin resistance, and to indirectly support China's national malaria program. The Program will target 586,000 local residents of the five Special Regions of Myanmar; 100,000 long-term migrants from China based in this target area; and an estimated 1.5 million short-term migrants who frequently cross the Sino-Myanmar border into the target area.

Malaria is endemic in two sout...hern provinces of China: Yunnan – particularly along the border with Viet Nam, Lao (People's Democratic Republic) and Myanmar – and the island province of Hainan. Both areas have falciparum malaria and high levels of resistance to antimalarial drugs. The program supported by this grant aims to reduce the malaria burden of frequently mobile Chinese workers crossing the common border with Myanmar from 12 project counties in Yunnan; and to reduce the malaria burden of Myanmar local residents in four Kachin special regions of Myanmar bordering with Yunnan. Planned activities include the establishment of 40 new health posts and the strengthening of 30 existing posts; training of 140 malaria control health workers in comprehensive malaria care; the organization of community-based health education activities for malaria control; and the establishment of a cross-border malaria surveillance system and information exchange mechanism.

Malaria incidence in China has... declined to the point where elimination is within reach. This grant is the result of a successful National Strategy Application for funding to eliminate malaria in China by the end of 2015, except in border counties in Yunnan and Tibet. The strategy targets the general population in the malaria endemic regions and populations at risk such as migrant workers. The overall goal is zero mortality due to malaria by 2013. The main interventions funded by the grant are aimed at ensuring access to early and accurate diagnosis, and prompt, effective and safe treatment in all sectors – private as well as public. Other objectives include full coverage of the population at risk with appropriate vector control measures, better health education and community mobilization efforts. The malaria surveillance system will be strengthened through improved case detection, case reporting, entomological and antimalarial resistance monitoring, and ensuring adequate outbreak response capability. The elimination strategy builds on Global Fund support for the 2006-2015 National Malaria Control Program provided under Rounds 1, 5 and 6.
