Those most affected by malaria are the remote, vulnerable populations whose basic needs are often not met and who lack access to adequate health care services. The program supported by this grant aims to reduce malaria-related illness and death among at-risk populations, which include internally displaced persons, those affected by malaria, rural and indigenous populations and people of African descent in 44 targeted districts. The program is implemented by the Fundación Universidad de Antioquia based in Medellin and is in charge of the distribution of insecticide-treated nets, the provision of technical assistance to municipalities and advocacy activities targeting vulnerable populations.

Korea (DPR) had previously eli...minated malaria in the 1970s as a result of exerted vector control activities during a more economically stable period. In 1998 malaria re-emerged, with cases being reported from areas close to the border with Korea (Republic). Since the re-emergence of Plasmodium vivax malaria in Korea (DPR), it continues to undermine public health to varying degrees in seven out of ten provinces. The program supported by this grant aims to reduce malaria-related sickness by 50 percent by 2013 by enhancing case management through the maximization of confirmatory diagnosis and delivery of effective antimalarial therapies; scaling up the country's integrated approach to prevent and control malaria; and integrating community involvement. The program targets the general population.

Malaria is endemic in rural pa...rts of the Dominican Republic, and the country has recently seen growth in the number of cases reported, possibly due to the heavy rains brought on by tropical storms. The goal of the program supported by this grant is to reduce malaria-related sickness and death rates in the 14 municipalities highly endemic for P. falciparum malaria. The program is setting up an integrated and decentralized malaria epidemiological surveillance system that provides useful information for decision-making at the local level. The program is also providing early diagnosis, timely treatment and adequate management of severe cases of malaria in a decentralized manner. Grant funds are being used to implement a social communication and community participation strategy to address malaria prevention control and to distribute insecticide-treated nets to highly vulnerable populations. The program is targeting the total population in the 14 municipalities it covers, with a particular focus on Haitians and people of other nationalities living in those areas.

Natural disasters, in particul...ar the hurricane El Niño, have heavily influenced the epidemiological evolution of malaria in Ecuador, which is now resurgent. The peak of the epidemic occurred in 2001, with 106,641 cases registered, 47 percent of which caused by P. falciparum (PAHO Basic Health Indicator database). Since then, the number of malaria cases has been decreasing, with 31,523 registered cases in 2006, according to the World Health Organization's World Malaria Report 2008. The policy for first and second-line treatment for falciparum malaria has been modified twice due to the high resistance level of P. falciparum to chloroquine. The program will be implemented in 133 cantons in areas at risk of, or susceptible to, malaria transmission in 22 (out of 24) provinces in the country. These zones account for a population of 7,169,638 people, representing 47 percent of the country's total population (2008 data). The target population is located in at-risk zones with an emphasis on vulnerable groups, such as pregnant women and children under the age of 5, the indigenous population and immigrant workers. This grant is implemented by the Corporación Kimirina, which is in charge of increasing the capacity of health facilities to diagnose and treat malaria cases as well as training vulnerable populations (indigenous, students, migrants and internally displaced persons) in malaria prevention activities.

The program targets children under age five, pregnant women and families vulnerable to malaria. Grant funds are being used to expand indoor residual spraying to cover continental regions; expand access to and utilization of long-lasting insecticide-treated bed nets; expand the use of preventative medications among pregnant women and improve case management of both uncomplicated and severe malaria by using improved treatment protocols.

The program targets malaria patients, household and community members, public health officials and health workers.

1. Background and Summary: M...alaria is endemic in all parts of Ghana and puts its population of 22.4 million at risk of malaria throughout the year. Over the past five years, between 3.1 and 3.5 million cases of clinical malaria were reported in public health facilities each year. Malaria accounts for 37.5% of all outpatient illnesses, 36% of admissions and 33.4% of all deaths in children under five years and 9.4% of deaths among pregnant women. The Program supplements the program supported through the fourth Round of Global Fund financing by focusing on two main interventions: 1) scaling up home based care in 123 rural districts to ensure that parents and caretakers recognize symptoms and signs of malaria and respond appropriately and promptly by seeking treatment from trained Community Drug Distributors; and 2) scale-up indoor residual spraying in 40 districts with the highest malaria burden. Universal coverage of bed nets is covered through the RCC program. Activities related to the universal coverage of bed nets are funded through the Global Fund grant number GHN-405-G04-M. In line with goals of global malaria control initiatives and the National Malaria Control Strategic Plan, this Program aims at contributing to the attainment of higher percentages of population in 40 targeted districts to be protected by indoor residual spraying, and a high percentage of children under five with malaria/fever receiving appropriate treatment at community level within 24 hours of fever onset. The Ministry of Health of the Government of the Republic of Ghana will implement the Home-Based Care (HBC) component, while AGAMC will implement the component of Indoor Residual Spraying (IRS). Both AGAMC and the Ministry of Health will conduct health care and community capacity building, monitoring and evaluation, operational research, and behavior change communication in their respective areas. Implementation of Indoor Residual Spraying will be based on the experience and best practices of AGA who has been implementing an IRS program in the Obuasi mining community since 2005. Results to date in Obuasi show over 74% reduction in malaria cases following two years of operation according to AGA's data sources. As of the effective date of this Agreement, the Program has two Principal Recipients who are jointly and severally responsible for all obligations of the Principal recipient under this Agreement, AngloGold Ashanti (Ghana) Limited ('AGA') and AngloGold Ashanti (Ghana) Malaria Control Limited ('AGAMC'). AGAMC shall be responsible for all aspects of the programmatic and financial management of the Program and AGA undertakes to indemnify the Global Fund and make good on the Principal Recipient's obligation in case of default by the AGAMC under this Grant Agreement.

According to WHO World Malaria... Report, Guatemala has the second highest number of Malaria cases in the Central America after Honduras, even though the total number of probable and confirmed cases has been steadily decreasing. Despite the significant progress made by the country, over 75 percent of the population remains at risk of contracting the disease, and 20 percent being a high-risk group. The program is implementing the Round 9 TB proposal aiming to significantly reduce morbidity due to Malaria in Guatemala and eliminate Malaria transmissions in health areas with the highest incidence of the disease. The funded activities include the distribution of insecticide-treated bed nets; training lab specialists on Malaria diagnosis, monitoring drug resistance, and providing prompt and effective Malaria treatment. The current grant will also aim to involve community leaders and the general population in the prevention and selective control of Malaria and seek to develop capacity and involvement of volunteer community groups, which will be useful in reducing Malaria transmission.

In Guyana, malaria is an endem...ic disease mostly limited in its geographical spread to the hinterland regions and is currently in its control phase, with a general trend to move towards pre-elimination. Nevertheless, it remains to date one of the most common causes of morbidity in the hard-to-reach regions where access to health care remains very challenging. This Program focuses its activities in the four most affected regions of Guyana (1, 7, 8 and 9) comprising 98% of all malaria cases. The Program aims at the achievement of the National Malaria Control Program Strategic Plan 2008-2013 by scaling up key prevention, diagnosis and treatment interventions to reduce the social and economic impact of malaria in the targeted regions, thus mitigating malaria's contribution to poverty in the country. The Program goal is to reduce the malaria morbidity by 70% at the end of August 2016 in these regions. This Program is a consolidation of the Round 7 and Rolling Continuation Channel (RCC) Round 3 malaria grants.

The island of Hispaniola, whic...h includes the countries of Haiti and the Dominican Republic, is the only Caribbean island where malaria remains a public health concern. According to WHO World Malaria Report 2010, Haiti's entire population is classified as at risk of malaria and almost half (47 percent) is at high risk. The program funded by this grant will support the Haitian national strategy goals to reduce mortality and morbidity due to malaria. Planned activities include behavioural change communication to increase knowledge of malaria among the population, scale up of diagnosis and treatment, strengthening of the surveillance system, designing of vector control activities, coordination with Dominican Republic to contribute to the elimination of malaria on Hispaniola and in the Caribbean. In 2012, three million long-lasting insecticidal nets were distributed.

Malaria is a major public heal...th challenge in India, accounting for about 800 million people considered at risk of Malaria. According to World Health Organization, 11 million Malaria cases and about 15,000 deaths due to Malaria occur annually in India. The Round 9 program aims to scale up effective preventive and curative interventions in those areas of the country where the intensity of transmission is highest and the health care delivery system constraints are the most severe. The program aims at universal coverage catalysing decline in Malaria related mortality and morbidity and contributing to achievement of national goals and Millennium Development Goals (MDGs); based on the experiences gained and lessons learnt during the implementation of the Intensified Malaria Control Project (IMCP) supported by the Round 4 grant since 2005 as well as the national program with domestic resources. Implemented by two Principal Recipients, under the Dual Track Financing scheme, the program plans to achieve universal coverage by 2015 by having effective preventive interventions for population living in high risk areas.

The program supported by this grant aims to increase the correct use of affordable, high-quality artemisinin-based combination therapies (ACTs) by adolescents and adults to 75 percent by 2012. Malaria is the leading cause o...f morbidity and mortality in Malawi and it accounts for 38 percent on all Out-Patient Department (OPD) attendance. The Malaria inpatient death rate is 20 percent in under 5 years and 3 percent in those over 5 years of age. According to Malawi's Multiple Indicator Cluster Survey in 2006, 21 percent of the population have access to anti-malarial drugs within 24 hours.. The Insecticide Treated Nets (ITNs) usage was at 24.7 percent and 25.6 percent for children under five and pregnant women respectively. The program supported by this grant focuses on the diagnosis of suspected Malaria cases with rapid diagnostic tests and distribution of Long Lasting Insecticide Nets (LLINs) as these are major gaps identified and in line with universal access to Malaria interventions. The goal is to reduce the burden of Malaria to a level of no public health significance and to achieve universal coverage for all Malaria control interventions, particularly diagnosis and prevention. The activities include procurement, distribution, and storage of LLINs, community mobilization, and behaviour change communication.

Malaria continues to be a seri...ous health issue in Mozambique being responsible for up to 44% of all outpatient consultations, 57% of pediatric admissions and 30 % of all death registered in hospitals. The goal of the Program is to protect all populations at risk from malaria through long lasting insecticidal net (LLIN), indoor residual spray (IRS) and intermittent preventive treatment (IPT) by 2014; provide appropriate malaria case management to all persons at risk; and achieve effective malaria control management by the National Malaria Control Program (NMCP), and provincial and district level authorities. The Program will target pregnant women and children, the rural population and residents of areas not yet covered by IRS. This Program is implemented by two Principal Recipients (PR) under the dual-track financing scheme. This PR will focus on developing training materials and to train community health workers on malaria prevention and treatment plus organizational skills and M&E; performing participatory assessment of capacities at community level; developing income generation activities at the community level; conducting KAP survey; producing materials for IEC and behavior change activities; and supporting several coordination meeting between community and provincial levels.

Malaria is one of the top thre...e priority public health problems in Myanmar. The program supported by this grant aims to reduce Malaria morbidity and mortality by at least 50 percent by 2015 and to contribute towards socio-economic development and the Millennium Development Goals. The proposal was based on the National Strategic Plan (NSP) for Malaria prevention and control, following international best practices. The proposal is implemented based on a Dual Track Financing scheme; it builds on and takes into account lessons learned from existing national Malaria Control Program and from the Malaria projects of other partners. The program targets the population at risk of Malaria in the areas of programme implementation and high risks groups including migrant workers and communities in remote poorly served populations. The objective of this grant is to prevent Malaria using long lasting insecticidal nets (LLINs) and insecticide-treated mosquito nets (ITNs) in high and moderate-risk villages in 180 townships and strengthen technical and administrative management capacity for Malaria control at all levels. The activities include the training of health staff and Village Health Volunteers (VHV) and expansion the Malaria microscopy and provision of rapid diagnostic tests (RDTs).

Over 80 percent of Nepal's pop...ulation is at some risk of contracting malaria every year. The true prevalence is believed to be high but realistic estimates are hindered by poor diagnostic techniques and low awareness of the disease among the population, of which only about 40 percent has access to public health service outlets. The overall goal of the program supported by this grant is to reduce the malaria burden via effective prevention and treatment of the disease. Targeting the country's 13 high-risk districts, the program is building capacity in both public and private health care facilities for the improvement of the coverage and quality of early diagnosis, distributing insecticide-treated nets and ensuring prompt and effective treatment of malaria right down to the community level. In addition to the provision of training and the supply of antimalarial drugs, the program's objectives include regular surveys of antimalarial drug quality, as well as studies of therapeutic efficacy and resistance to inform the updating of national malaria drug policy.
