There is a long history of dis...connection and lack of trust between vulnerable communities (slums and favelas) and public authorities in Brazil, due to diverse reasons, including failures in service delivery (e.g., sanitation, education, health), overuse of violence by the police force, and the presence of militias (parastatal power). Many social interventions for COVID-19 prevention and control require co-operation from citizens to be effective. Local communities and their local leaders know their daily needs, their areas, and the social environment better than the government. To cope with COVID- 19, and similar future crises, members of the vulnerable community need to be engaged as part of the solution, and given voice. Our proposal aims to enact co-production with members of these communities and municipal councils. Such councils are already established all over the country. The project will promote an exercise on co-production to design tactics and solutions to prepare neighborhoods for facing future interventions, and in particular vaccine delivery (SDGs goals 3, 10,11,16). Such co-production space would open up opportunities to share information, give voice to local leaders and community members. It is about increasing resilience, reducing social vulnerability, rebuilding trust, through co-production to cope with pandemic outbreaks. The process includes four stages: (i) identification of 'successful collective strategies' in vulnerable areas of two Brazilian capital cities (Belem and São Paulo), looking at the determinants of such action, by interviews, (ii) extending the understanding through an extensive survey to others communities from the same cities, (iii) developing a mobile application to create a co-production milieu supporting people to jointly act with municipal councils, (iv) rolling out the initiative to others urban areas disseminating the results.

Families that lose a mother in... pregnancy or childbirth face many challenges. They are left with a gap that can lead to a loss of income and poverty. It can cause a lot of family pressures, especially for girls are left to care for younger children and are not able to go to school. Many of these families come from communities that are already poor, with very little infrastructure and limited access to decent healthcare. This can cause a much higher risk of death, both for mothers and babies, especially in poor countries. Opportunities are more limited because families lack money and education. Myanmar is a South East Asian country that has had many years of tension and conflict between the different people groups. Many of these groups live in remote mountainous areas where basic resources and infrastructure such as electricity are limited. There is often little healthcare in these areas with very few doctors, nurses or midwives to care for mothers and their families. One in 23 women in Eastern Myanmar either die or come close to death during pregnancy or childbirth. These challenges are all very typical in developing countries across the world today, but a lot can be changed with education that makes sense to mothers and families in their situations. Working together with our local partners we intend to learn more about the specific challenges facing families in four regions of Myanmar. From this we will co-develop three education programmes which will be especially adapted for each region. One of these programmes will help families and communities understand the processes of pregnancy and birth, recognise what is normal, what is unusual and what is dangerous and to know what to do and where to go to get help. Our second programme will further train health workers to understand the different signs and symptoms that are dangerous in pregnancy and birth and find ways to help mothers in time to prevent complications. Poverty is very high in parts of Myanmar, so our third programme is for community leaders and local people to support them to start and grow businesses. This way families can afford to eat well and get good healthcare when they need it. Our project will focus on remote regions in Myanmar and together we plan to learn how to find ways to overcome the legacy of many years of poverty and conflict. This project will include staff from our universities in Northern Ireland and Thailand as well as local partners who come from or have been working in the region for many years. Through this project we hope to help support the work of all our partners and improve education for mothers, families and communities. A key part of our project is making sure that the education they receive makes sense in their situations and includes their beliefs and customs leading to improved health, social and economic opportunities and accessible healthcare. Mental health challenges are f...aced by 1 in 4 people globally, but in Low and Middle Income Countries (LMIC), up to 80% of people do not receive preventive or acute care . Digital mental health is seen as a promising route to address the treatment gap, but also raises ethical, legal and policy challenges. In LMICs, lack of ethics, governance and policy guidance can slow down or prevent translation of digital mental health innovations into the healthcare system. This impinges on the rights of individuals to access mental health technologies that arise from digital innovations. A further consequence of the lack of attention to ethics and governance challenges is that the balance of harms and benefits of digital mental health have not been properly identified and analysed. The lack of analysis is likely to disproportionately affect young people, who are a key target for mental health interventions, and who also have distinctive individual and social vulnerabilities. To address this problem, the network will bring together stakeholders and experts in the ethics of digital mental health, digital innovators, mental health practitioners, and policy makers across Africa. The chief aim is to develop a robust framework for responsible and relevant digital mental health interventions for young people in African countries. The cross-disciplinary network will have a group of young people at its core, who will co-create all its work and outputs. The network will use a stakeholder mapping surveys, workshops and interdisciplinary training and exchanges to achieve its aims. It will contribute not only to scholarship, but also to research capacity, co-production, inter-sector mobility, and democratic participation among young people. Project activities will include: -Intensive training and capacity-building opportunities for early career researchers and young people (e.g. workshops, visiting scholar programmes), -Meetings and workshops to facilitate the transfer of knowledge among stakeholders, -Literature reviews to critically assess and synthesise existing work, identify gaps, and formulate research questions -Community engagement to inform empirical questions -With young people, co-create a sustainable engagement strategy to involve young people, with a focus on representation from vulnerable groups, such as LGBTQ+, sexual violence survivors, those living with HIV/AIDS Young people will be at the core of the Network, integrated with expert groups of researchers in ethics and social sciences, service providers (app developers, mental health practitioners and researchers), religious congregations, policy and governance. This will ensure the network outputs reflect the needs of young people and that the framework is scalable and sustainable as it is co-produced with young people, researchers, service providers, and policy makers. 1. Health in a changing climate: ...the dynamic challenge of snake bite in South Asia

Snakebite is a neglected tropi...cal disease (NTD) and despite being less well publicised and studied, has a far greater impact than many other NTDs such as dengue or leishmaniasis. There are estimates of up to 120,000 deaths per year globally with considerable additional morbidity resulting, for example, from limb damage or renal failure. This burden of disease is relatively hidden as snakebite not only predominantly affects poorer countries in the tropics and subtropics, but it also mainly affects the rural poor, particularly agricultural workers and subsistence farmers in lower-middle income countries (LMICs). In many countries, snakebite is seasonal and is distributed unevenly across a country because of complex interactions between human behaviour, climate, varying geography and other factors affecting snake distributions. This means that there are substantial challenges in estimating the number of deaths and complications of snakebite. National surveys are rare because they are so difficult to carry out. The variation in numbers of snakebite across countries and over any one year means that it is difficult to estimate national and regional numbers from time-limited small local studies. This absence of accurate numbers for many countries and regions means that the problem does not get the international attention that it requires and makes it extremely difficult for local public health authorities to plan appropriate health services. Improved methodological approaches for mapping snakebite risk are therefore urgently needed, particularly when considering the potential for global environmental and climate change to exacerbate snakebite impacts. Examples such as the peak of snakebite deaths in India during the monsoon and in Bangladesh during floods illustrate the potential for environmental factors to influence disease. In Sri Lanka, bite and envenoming patterns also vary between climatic zones, with bite and envenoming incidence changing with altitude and rainfall. Climate change is thus likely to be an important yet currently unrecognised contributor to altering snakebite risk in affected areas, potentially impacting on snakebite incidence by altering snake abdundance, distributions and behaviour, altering human abundance and behaviour or both. This study aims to improve the ways in which the epidemiological burden of snakebite can be estimated and mapping risk using modelling of the interactions between snake and human distributions, behaviours and environmental factors and to investigate the extent to which climate and land-use change will impact upon this burden. The study will develop and validate methodologies using data from Sri Lanka and, in future, use these approaches to improve estimations of the snakebite burden and map risk over wider geographical areas in South Asia, and predict how they may change in the future. Malaria is a major public heal...th challenge in India, accounting for about 800 million people considered at risk of Malaria. According to World Health Organization, 11 million Malaria cases and about 15,000 deaths due to Malaria occur annually in India. The Round 9 program aims to scale up effective preventive and curative interventions in those areas of the country where the intensity of transmission is highest and the health care delivery system constraints are the most severe. The program aims at universal coverage catalysing decline in Malaria related mortality and morbidity and contributing to achievement of national goals and Millennium Development Goals (MDGs); based on the experiences gained and lessons learnt during the implementation of the Intensified Malaria Control Project (IMCP) supported by the Round 4 grant since 2005 as well as the national program with domestic resources. Implemented by two Principal Recipients, under the Dual Track Financing scheme, the program plans to achieve universal coverage by 2015 by having effective preventive interventions for population living in high risk areas.

To improve maternal, new-born ...and reproductive health services for the urban poor in Bangladesh by unifying and expanding quality clinical and community outreach services which will benefit over a million couples with modern family planning methods and wi

DFID proposes to support expan...ded services for the urban poor through 65 new and up-graded clinics providing maternal and new-born care. These include 27 new comprehensive facilities opened 24/7, equipped to manage complicated deliveries, and free to the poor. This would expand services and provide assistance to approximately 42% of urban births among the urban poor. . The programme could also provide 48 mobile and satellite clinics each month for homeless, street dwellers and factory workers.
The clinics would adhere to WHO standards and provide a full range of essential maternal and primary services including:
• maternal nutrition: control of anaemia and distribution of Vitamin A
• Training of 2500 health workers in nutrition messages and intervention,
• ANC services including tetanus toxoid,
• neo-natal and post-natal care within 48 hours of giving birth,
• modern and long acting family planning methods,
• hygiene promotion for all, including focused efforts on birth attendants and new mothers,
• adolescent reproductive health services,
• emergency support to victims of gender based violence ,
• menstrual regulation services (through Marie Stopes)

To provide increased coverage ...with essential reproductive, maternal and child health services in DFID-supported health zones. To increase coverage with essential health services in 52 DFID-supported health zones in five provinces (Nord Ubangi, Kasai-Centrale, Kasai, Maniema, Tshopo). This will benefit over 9 million Congolese citizens, and contributes towards achievement of MDGs through the delivery of services to improve reproductive, maternal and child health

ASSP (Accès aux Soins de Sant...é Primaires) is implemented by a consortium of NGOs led by IMA World Health (see table 1) and currently supports 52 health zones (out of 516 nationally) in five of the 26 provinces in DRC. It provides an estimated 9 million people with access to essential primary and (to a lesser extent) secondary healthcare services. The design phase began in October 2012 with implementation expected to run for five years from April 2013 to March 2018.
- At community level, ASSP aims to generate demand for services, and strengthen the voice of communities and service users in holding service providers to account for their delivery. The programme also includes a large WASH component and a nutrition package for the prevention and treatment of malnutrition.
- At health facility level, ASSP is supporting the construction and renovation of health centres and hospitals and provides facilities with essential equipment, drugs/supplies, training and an operational allowance. These inputs enable services to be provided and user fees to be substantially reduced, or removed altogether for vulnerable groups.
- At health zone level, ASSP strengthens government capacity to manage health zones by enabling Équipes Cadres de Zones de Santé (health zone management teams, ECZS) to supervise health facilities and to use planning tools for the development of the health zone.
- At provincial level, ASSP supports the Directions Provinciales de Santé (provincial health divisions, DPS), and is rehabilitating and enhancing the capacity of Centrales de Distribution Régionales (regional drug distribution centres, CDRs).
- At central level, technical assistance is being provided to the Ministry of Health through ASSP to improve public financial management, and the programme is helping to set up and roll out new information systems for both health and human resources management. It also supports the Ministry to develop and disseminate norms and standards, and has facilitated policy dialogue with faith-based providers of health services.
