IMPROVING QUALITY OF MATERNAL ...HEALTH CARE FOR INDIGENOUS WOMEN IN ARGENTINA, MEXICO, AND PERU

Improving quality of maternal ...health care for indigenous women in Argentina, Mexico, and Peru
more
|
Latin America is home to the g...reatest inequities in the world, including inequality in access to quality health services. Indigenous peoples have the poorest health in the region, due to such factors as geographic isolation, limited access to safe water, sanitation and food, and a poor understanding of, and sensitivity to, cultural beliefs and practices. Among the indicators of greatest concern is the high rate of maternal mortality. Indigenous women consistently experience higher maternal mortality rates than non-indigenous women in Latin America. Efforts to improve maternal health for indigenous women and adolescent girls must be based on a clear understanding of how to provide and measure high-quality care that ensures adequate health outcomes with dignity. This project examines quality of care from a cultural and a technical perspective in three indigenous communities in Mexico, Peru, and Argentina. Each of these three case studies will explore perceptions of quality care among indigenous women, and will involve collection and analysis of data using a customized framework for quality of care indicators. The study will examine the implementation of integrated health service delivery networks in all the three countries. These networks are designed to coordinate different health providers and services and each country is implementing them differently. The three case studies will examine how the networks positively or negatively influence quality of care processes and outcomes. Results from the individual case studies and from the project as a whole are expected to inform strategies for scaling up efforts to improve quality of maternal care for indigenous populations. Expected outputs include scientific publications in Spanish- and English-language journals, and information sharing via community groups, the local press and social media.

This project aims to support t...he Global Financing Facility (GFF), a multi-donor financing platform that provides country-led, sustainable financing to end preventable maternal, newborn, and child deaths. The GFF finances national reproductive, maternal, newborn and child health (RMNCH) plans, supports countries in the transition towards sustainable domestic financing of RMNCH, finances the strengthening of civil registration and vital statistics (CRVS) systems, finances the development and deployment of global public goods, and contributes to a coordinated financing for RMNCH. Project activities will vary depending on the RMNCH priorities of the recipient government, but may include: (1) Clinical interventions such as increasing access to health services and health facilities from the pre-pregnancy stage onwards for women, newborns and children, (2) Health systems interventions such as strengthening health information systems, increasing access to medical products/technologies, and health systems strengthening; (3) Multi-sectoral interventions such as education, nutrition, and water and sanitation, in addition to strengthening CRVS systems. The GFF supports countries in developing individual RMNCH investment cases to align various sources of financing to national RMNCH priorities. The GFF leverages additional financing for RMNCH by matching grant funding from the GFF Trust Fund to financing from the International Development Association (IDA) and the International Bank of Reconstruction and Development (IBRD). GFF country selection is prioritized on the basis of criteria that includes available resources, need, population and income, eligibility for IDA/IBRD loans and the ability to achieve results.

RESULTS-BASED FINANCING FOR EQ...UITABLE ACCESS TO MATERNAL AND CHILD HEALTH CARE IN MALI AND BURKINA FASO (IMCHA)

Results-based financing for eq...uitable access to maternal and child health care in Mali and Burkina Faso (IMCHA)
more
|
In Mali and Burkina Faso, mate...rnal and child mortality rates remain high. The governments of both countries are trying to increase access to and use of health care for women and children. They have adopted policies to remove financial barriers and are now piloting results-based financing, which gives financial incentives to providers of health services for achieving predefined indicators. Despite these efforts, health inequalities persist. This project will contribute to increasing equitable access to health services for the poor by strengthening the implementation of health reforms in Mali and Burkina Faso. It examines the emergence, relevance, design, and development of results-based financing in both countries, and studies its implementation, sustainability, and efficacy. The project will also look at the impact of results-based financing on health providers' motivation and teamwork, local leadership, community management committees of health facilities, access to and use of health services, and financial protection of vulnerable populations. It will document any unexpected outcomes. The project will result in a number of scientific publications from established and young researchers. In addition, the results of the study will be disseminated to decision-makers across Africa and to international agencies involved in the design and scale-up of results-based financing for maternal and child health. This project is part of the Innovating for Maternal and Child Health in Africa (IMCHA) Initiative, a seven year, $CA36 million multi-donor partnership funded by Global Affairs Canada, the Canadian Institutes of Health Research, and Canada's International Development Research Centre.

This project will help the Inn...ovating for Maternal and Child Health in Africa (IMCHA) program establish a communications and evaluation fund. It will also support the program in carrying out strategic, bilingual communications and knowledge translation activities, as well as monitor and evaluate its work. Improving maternal and child health IMCHA will improve maternal, newborn, and child health by using primary health care as an entry point to strengthen health systems, ensuring they are more equitable. The program is supporting 20 implementation research teams. The teams will generate new knowledge about how interventions work, for whom, and under what conditions, to ensure that mothers and their children have better access to the care they need. To promote the uptake of research findings so that they influence national and regional policies and practices, IMCHA is supporting two health policy and research organizations. These organizations are working closely with the implementation research teams. Communicating innovation and results This project establishes a communications and evaluation fund for the program. It will help facilitate administration of a budget line in the IMCHA program budget that was established and approved by its three funders: IDRC, the Canadian Institutes of Health Research, and Foreign Affairs, Trade and Development Canada, for the program's duration (2014-2020). The funds will allow IMCHA to communicate its approach and results, while helping to ensure that they are clearly understood and adopted by target audiences. The communications activities are listed in the IMCHA Communication Plan. The project will also allow the program to prepare for the IMCHA evaluations, and finance the mid-term and final evaluations. Full details are available in the IMCHA evaluation plan, logical framework, and program monitoring framework.

This project aims to support c...ommunity-based services that will improve maternal health in Ethiopia, a country with some of the worst health indicators in sub-Saharan Africa. Women's use of maternal and child health services has significant gaps between urban and rural areas. Healthy mothers, healthy babiesThe Government of Ethiopia is committed to improving maternal health. While some key indicators have improved, maternal mortality has not decreased significantly. The Ethiopian Health Department data suggests many of these maternal and neonatal deaths arise from home births in the absence of a skilled and experienced birth attendant. Strengthening community-based services is therefore important to ensure that mothers have access to services that result in safer deliveries.Technology, communication, skills developmentThis project will implement interventions such as: -training traditional birth attendants and equipping them with mobile phones-developing communication and education strategies for religious leaders and men -developing healthcare worker skills for safe deliveriesThe project team will evaluate the use of improved maternal waiting areas and health extension worker practitioners for their impact on maternal health outcomes. The research will be carried out in close partnership with the Ethiopian Health Department. Researchers will identify interventions that are the most effective at improving maternal and child health services with a view to scale up nationally. The project team will produce policy and media briefs, articles, and a project website to share information about the project and their findings.Innovating for Maternal and Child Health in AfricaThis project is part of the Innovating for Maternal and Child Health in Africa program, a seven-year $36 million initiative funded by Foreign Affairs, Trade and Development Canada (DFATD), Canada's International Development Research Centre (IDRC), and the Canadian Institutes of Health Research (CIHR).

The goal of this project is to... strengthen Ethiopia s health systems in order to deliver effective maternal, newborn and child health services to nomadic communities in the Omo Valley. Using a community-based approach, the project aims to: increase the number of pregnant women accessing health services; increase the proportion of health facilities providing comprehensive health care; and reduce nutritional deficiencies that affect safe childbirth and development. Activities include: training midwives and health workers on emergency obstetric and newborn care, and the diagnosis and treatment of HIV/AIDS, malaria, tuberculosis and waterborne diseases; training nurses on safe and clean childbirth; and providing ambulances and laboratory equipment to three health facilities. The African Medical and Research Foundation (AMREF) Canada is working with AMREF Ethiopia to implement this project. This project is part of Canada s Maternal, Newborn and Child Health commitment. The maximum CIDA contribution to this project includes $10,000 for monitoring purposes.

The project aims to reduce mat...ernal mortality in three departments of the country, through public health activities. It provides technical assistance, family planning, emergency obstetrical care, and the supply of inputs. The project is implemented by the United Nations Population Fund (UNFPA) and the United Nations Children's Fund (UNICEF) in support of the Ministry of Public Health and Population (MSPP). The project helps improve access to quality services for women and improve the management of high-risk pregnancies. It complements other health system development projects by ensuring that operating rooms of targeted hospitals and maternity wards are up and running, and that emergency blood transfusions can be performed.

The project aims to help reduc...e maternal mortality by improving maternal health at the national level, and particularly in the Nippes and Sud-Est departments. The project supports activities that improve family planning, childbirth in health institutions, increased access to essential medicines, and improved youth awareness of reproductive health issues. These activities gradually integrate the improved awareness and treatment of sexually transmitted infections, HIV/AIDS, and cervical and breast cancer screening. The maternal mortality ratio in Haiti is the highest in the Americas. The project contributes to reducing maternal mortality by promoting universal access to reproductive health, including childbirth and emergency obstetric care by qualified health professionals. The Pan-American Health Organization/World Health Organization (PAHO/WHO), the United Nations Population Fund (UNFPA), and the United Nations Children's Fund (UNICEF) implement the project in partnership with the Ministry of Public Health and Population (MSPP).

INTEGRATING A NEONATAL HEALTHC...ARE PACKAGE FOR MALAWI (IMCHA)

Integrating a neonatal healthc...are package for Malawi (IMCHA)
more
|
Malawi has the highest preterm... delivery rate in the world. This, paired with inadequate newborn care at health facilities, results in high rates of infant mortality. Managing the healthcare needs of preterm infants remains a challenge. They face complications during and after birth, and they have an increased risk of death, chronic medical problems, and malnutrition later in life. Many life-saving interventions have been successful in Malawi but they have not been scaled up into routine clinical practice. This project will determine whether a package of neonatal interventions, known as the Malawi Neonatal Package of Care, can be implemented at the health facility level to reduce neonatal mortality. Interventions include low-cost continuous airway support for breathing, breastfeeding support, Kangaroo Mother Care (skin-to-skin, mother-to-baby contact), hot cots to prevent hypothermia, management of hypoglycemia (low blood sugar), and phototherapy lights to treat jaundice. Researchers will investigate the best ways to implement these interventions in routine clinical practice. The project will directly train nurses, doctors, and other health care providers. It will also produce peer-reviewed journal articles, and provide presentations at hospitals and nursing organizations, interviews with media, and policy-relevant information to decision-makers. This is part of the Innovating for Maternal and Child Health in Africa (IMCHA) initiative,a seven year, $CA36 million multi-donor partnership funded by Global Affairs Canada, the Canadian Institutes of Health Research, and Canada's International Development Research Centre.

Results-based financing for eq...uitable access to maternal and child health care in Mali and Burkina Faso (IMCHA)

This pilot project aims to con...tribute to reducing maternal and neonatal mortality in four districts of the Kayes region. It supports the Government of Mali with the implementation of its Health and Social Development Plan (PRODESS), specifically to improve Maternal and Neonatal Health. The project is designed to complete the integrated community-level health system thereby strengthening the referral and evacuation plan, including the transport of pregnant women from villages to community health centres. The project comprises two complementary components: The first component takes the form of direct financial support to the four selected district authorities through the Regional Health Directorate. It aims to make new resources available for upgrading or building new health units, ensuring key services are available (e.g. emergency obstetric care, prevention of mother-to-child transmission of HIV/AIDS), having appropriate transportation in place, and ensuring availability and quality of qualified personnel, medicines and medical supplies. The second component is providing technical assistance by a Canadian support agency to assist the four districts with the implementation of their operational plans. It aims to help the local communities to get organized in order to set up in their villages a system to effectively reduce the delay in getting women in labour to the health centre. This component also contributes to strengthening the capacity of the local structures to plan and manage human and financial resources as well as to monitoring results.Part of the resources of this pilot project are being dedicated for the analysis of the results and the development and implementation of a results-dissemination strategy designed to inform the possible scaling up of this initiative. This project is part of Canada's Maternal, Newborn and Child Health commitment./ Ce projet pilote vise à contribuer à la réduction de la mortalité maternelle et néonatale dans quatre districts de la région de Kayes. This initiative is a key eleme...nt of the assistance provided by Canada to contribute to Mali reaching its development goals in the health sector, particularly in improving maternal, newborn and child health. It mainly aims to support the Ministry of Health of Mali in implementing is Ten-Year Health and Social Development Plan 2014-2023. As is the case for all budget support initiatives, Canada is working closely with other donors and the Government of Mali to improve the emphasis on accountability, transparency and effectiveness of national systems by increasing coordination and harmonization among donors and strengthening mutual accountability. Budgetary support also fosters greater policy dialogue between donors, government and partners, which helps to reinforce efforts to provide targeted and effective help and get results from long-term development. Monitoring and continuous evaluation of this initiative are made in conjunction with other donors.

Improving Maternal and Child H...ealth in Underserved Rural Areas of Nigeria through Implementation Research
more
|
Maternal and child health is a... priority for Nigeria, but there are significant challenges and opportunities at state levels that influence efforts to reduce deaths. This project will contribute to government efforts in Delta State to improve delivery and use of maternal and child healthcare services in three marginalized rural communities. Maternal and child health concerns According to the World Health Organization, Nigeria's maternal mortality ratio in 2013 was 560, down from 610 in 2010 but still higher than sub-Saharan Africa's ratio of 510. Newborn and child mortality rates show high ratios as well. Structural barriers related to gender and poverty, knowledge, traditional cultural beliefs and practices, lack of social support networks, and health systems weaknesses influence governments' ability to promote better delivery and use of health care services in vulnerable rural communities. Project leadership The Centre for Population and Environmental Development (CPED), a Think Tank Initiative-funded organization, proposes to undertake this study and intervention program. The project team will conduct implementation research on innovative maternal and child health care approaches. They will also promote knowledge translation and scaling-up. This project will improve CPED's research and advocacy work on health systems research related to maternal and child health. CPED and its partners will recommend strategies to improve maternal and child health services in the study area's communities. Research support The Think Tank Initiative is a multi-funder program dedicated to strengthening independent policy research institutions, or think tanks, in developing countries. The program aims to enhance their ability to provide sound research that informs and influences policy. This project also has the support of the Innovating for Maternal and Child Health in Africa program, a seven-year $36 million initiative funded by Foreign Affairs, Trade and Development Canada, Canada's International Development Research Centre, and the Canadian Institutes of Health Research.

The project aims to accelerate... the reduction of maternal, newborn and child mortality in 15 states and the federal capital territory of Nigeria. It is designed to strengthen the delivery of maternal, newborn and child health services through evidence-based, gender-responsive interventions, using existing health and community structures in the focus states. It also supports the procurement and distribution of equipment such as newborn resuscitation devices, HIV testing kits, communication equipment, and reproductive health supplies to cover 60% of the annual requirements of these states. Finally, the project provides technical and financial support to enhance the skills of health workers - midwives, doctors and Community Health Extension Workers - through updating training guidelines under the Midwifery Services Schemes, the Community Health Extension Workers Program and doctors serving in the Nigeria Youth Service Corps. The project is implemented in alignment with the Government of Nigeria's Integrated Maternal, Newborn and Child Health Strategy. The project is implemented by UNICEF in collaboration with the World Health Organisation (WHO), and the United Nations Population Fund (UNFPA). This project is part of Canada's Maternal, Newborn and Child Health commitment.

|

This project will examine how ...to strengthen the Bajenu Gox Initiative's contribution to improving maternal and child health across Senegal. Former President Abdoulye Wade founded this community-based health worker program to train women to be leaders in reproductive health. Based on the Wolof term for godmother, the Bajenu Gox Initiative provides support to women during the prenatal, delivery, and post-natal periods. It also provides advice about caring for children under 5. Community support for womenThis project aims to improve the Bajenu Gox Initiative's long-term stability at the community level and establish systems that will allow the program to scale up nationally. Researchers will use a community development approach based on a gender and social equity perspective to examine how to enhance the initiative's contribution. The research is structured in two phases: -literature searches on holistic community-based maternal and child health approaches that have been tested in Senegal and other African countries, and possibly in Canada-intervention research based on the findings in this literatureThe project team will share the results through-situation assessment and economic feasibility studies-training materials-a guide on how to effectively manage the initiative-documentary to highlight the initiative's impacts -scientific publications -presentations at international conferencesInnovating for Maternal and Child Health in AfricaThis project is part of the Innovating for Maternal and Child Health in Africa program, a seven-year $36 million initiative funded by Foreign Affairs, Trade and Development Canada (DFATD), Canada's International Development Research Centre (IDRC), and the Canadian Institutes of Health Research (CIHR).

This project aims to support t...he Global Financing Facility (GFF), a multi-donor financing platform that provides country-led, sustainable financing to end preventable maternal, newborn, and child deaths. The GFF finances national reproductive, maternal, newborn and child health (RMNCH) plans, supports countries in the transition towards sustainable domestic financing of RMNCH, finances the strengthening of civil registration and vital statistics (CRVS) systems, finances the development and deployment of global public goods, and contributes to a coordinated financing for RMNCH.Project activities will vary depending on the RMNCH priorities of the recipient government, but may include: (1) Clinical interventions such as increasing access to health services and health facilities from the pre-pregnancy stage onwards for women, newborns and children, (2) Health systems interventions such as strengthening health information systems, increasing access to medical products/technologies, and health systems strengthening; (3) Multi-sectoral interventions such as education, nutrition, and water and sanitation, in addition to strengthening CRVS systems.

Implementing comprehensive, co...mmunity-focused primary health care is a major challenge in post-conflict regions such as South Sudan and neighbouring Northern Uganda. South Sudan is recovering from two decades of civil war that have destroyed education and health infrastructure. Torit Hospital, one of the major hospitals in the region, is poorly staffed and lacks specialists and basic equipment. The situation is similar in Northern Uganda. In the District of Gulu, the burden of disease has largely been managed through St. Mary's Hospital Lacor. This region has not been able to achieve the progress in maternal and child health that has been reached in the central part of Uganda. This project assesses the implementation and scale of community-focused reproductive and child health interventions in post-conflict settings. It focuses on establishing evidence to promote sustainability and resilience, with the goal of ensuring access to essential health services by vulnerable populations. The researchers will explore strategies for improved maternal care and child nutrition services, cervical cancer screening, and support for women's empowerment through outreach efforts from the two hospitals. Research findings will be validated through ministries of health at the local, district, and national levels to optimize engagement and to encourage adoption of the project's recommendations to strengthen systems. The results of the study will directly inform the ministries of health in South Sudan and in Uganda on related interventions, including a scalable model for improved community-based services. This will address barriers to interventions, reduce maternal and child mortality, and extend the reach of health services to communities that have faced shocks and stressors. This is part of the Innovating for Maternal and Child Health in Africa (IMCHA) initiative, a seven year, $CA36 million multi-donor partnership funded by Global Affairs Canada, the Canadian Institutes of Health Research, and Canada's International Development Research Centre. Objectives /

Quality Improvement for Matern...al and Newborn Health in Mtwara Region, Tanzania (IMCHA)
more
|
This project will provide evid...ence for how systems-wide quality improvements can enhance maternal and newborn health outcomes at the district, health facility, and community levels in Tanzania.Healthy women, healthy babiesMaternal and newborn health outcomes in southern Tanzania's Mtwara region are poor despite 70% of births occurring at health facilities. Recent research in the region found that maternal and newborn mortality rates were similar when comparing home births with health facility births. This suggests a clear need for improved quality of skilled care in health facilities. Another issue is poor health-seeking behaviours among women. Combined with weaknesses in the health system, these factors limit the use of high quality interventions that have been proven to reduce maternal and neonatal morbidity and mortality.Strategies to improve quality The Ifakara Health Institute, Tanzania's Ministry of Health, and Canada's SickKids Centre for Global Health have tested and applied quality improvement strategies for mothers and newborns across six districts in southern Tanzania. This project will explore if these strategies can be scaled up and integrated into district-level health systems. It will also examine whether these strategies for higher quality maternal and newborn health care services, better care-seeking, and improved health outcomes can be sustained at the regional level. Researchers will produce a model that can be streamlined and integrated into existing structures within the Tanzanian health system. It will generate evidence needed to inform national policy on quality improvement in Tanzania.Project partnersThis project is part of the Innovating for Maternal and Child Health in Africa program. The program is a seven-year $36 million initiative funded by Foreign Affairs, Trade and Development Canada (DFATD), Canada's International Development Research Centre (IDRC), and the Canadian Institutes of Health Research (CIHR).

This project aims to reduce ma...ternal, newborn, and child deaths in Tanzania. The Government of Tanzania has placed a high priority on this issue. Healthcare service coverage has increased in recent years, but there are still significant geographic, financial, and socio-cultural barriers to services being accessed. Recent studies in Tanzania have shown that when healthcare systems are addressed (including barriers to health services and improving the quality of care), maternal and newborn deaths can be substantially reduced.Removing barriers to health careThis project is designed to improve maternal, newborn, and child health. It consists of select interventions targeting users and providers of health care in four districts in Tanzania's Iringa region.Researchers will look at how poverty, gender relations, and living in rural areas act as barriers to accessing quality care. The project's interventions will address delays caused by:-decisions to seek care-transportation needs-availability of adequate and timely care at the facility The project team will evaluate the impacts and conduct a cost analysis of the interventions.Better access, increased demandThis project is expected to increase demand and use of maternal and newborn health services. Researchers will share their findings with policymakers, program managers, and representatives of international organizations working in Tanzania. Their findings will be presented at national conferences and at the University of Dar es Salaam. They will also submit papers to peer-reviewed and academic journals.Innovating for Maternal and Child Health in AfricaThis project is part of the Innovating for Maternal and Child Health in Africa program, a seven-year $36 million initiative funded by Foreign Affairs, Trade and Development Canada (DFATD), Canada's International Development Research Centre (IDRC), and the Canadian Institutes of Health Research (CIHR).
