Guatemala has a high level of ...tuberculosis (TB) incidence at 51 to 75 cases per 100,000 inhabitants (World Health Organization (WHO) 2009), and has still not reached the detection and cure goals established by WHO for the year 2005. The overall goal of the program supported by this grant is to reverse TB incidence, prevalence and mortality in Guatemala by increasing the case detection and treatment success rates, and decreasing the mortality rate from TB/HIV co-infection. Specific objectives were revised with the change in Principal Recipient at Phase 2 in 2010, but the program will continue to expand coverage of the DOTS strategy, focusing on poor and vulnerable populations, including indigenous people, people living with HIV and multidrug-resistant TB patients.

Haiti has high tuberculosis (T...B) prevalence at 331 cases per 100,000 population for all forms of the disease. The national TB program (PNLT) has been implementing DOTS – the basic package that underpins the Stop TB Strategy – since 1997. In 2009 the case detection rate for all TB forms was 62 percent. The most recent figure available for treatment success is 82 percent of smear-positive pulmonary cases notified (2007 cohort). The program supported by this grant aims to increase case detection and treatment success rates to meet the Stop TB targets (70 percent and 85 percent, respectively) by extending DOTS coverage with more diagnosis and treatment centers, and improving the quality of DOTS services. The fight against multidrug resistance will also be a core component of the program. DOTS quality will be improved by (1) strengthening human resources at all PNLT levels, and (2) supplying laboratories with technical equipment. Treatment of TB/HIV co-infection will be strengthened, and TB prevention in populations most at risk will be addressed through organizations that have extensive experience working with them. The program will build on progress made under Haiti's Round 3 TB grant.

In Honduras, Tuberculosis (TB)... is considered a public health issue, and it is among the 20 problems requiring mandatory notification. The program aims to increase case notification rate among new smear positive cases from 25.6 to 37.4 percent per 100,000 population by 2016 and to maintain treatment success rates from 2011 to 2015 above 85 percent. The program targets the population of 48 priority municipalities in the country as well as the at-risk population, multi-drug resistant TB (MDR-TB) patients and patients co-infected with TB and HIV. The program plans to improve DOTS strategy application at the institutional level to increase case detection and healing of TB patients, and to strengthen the laboratory network in order to improve diagnosis and response to new challenges proposed by the Stop the TB strategy. The program also plans to strengthen the inter-programmatic approach to decrease TB-HIV co-infection. By improving the clinical and programmatic management of MDR-TB and social support focused on the needs of the patient, family and community, the program increases the detection of cases and decreases the rate of abandonment. TB prevention, management and control of activities among prisoners and other high-risk populations is developed to decrease case incidence in these populations.

Tuberculosis (TB) control in J...ordan has improved, particularly in major cities and most of the rural areas; however, there are significant gaps in terms of quality of services for the most vulnerable groups of patients, particularly the non-Jordanian nationals who account for one-fifth of the residents of Jordan. This Program aims to pursue, optimize & sustain quality Directly Observed Treatment, Short Term (DOTS); adapt DOTS to respond to TB-HIV, multidrug-resistant TB (MDR-TB) and other special challenges; promote cross-border care and tracking of patients; engage all care providers; empower patients and communities; enable & promote operational research; and strengthen the health system response to TB. The goal of the Program is to improve the quality of TB care for these most vulnerable populations to scale up MDR-TB management, control and care, and strengthen the health system response to TB control.

Kazakhstan has one of the high...est TB burdens in Eastern Europe and Central Asia, including one of the highest rates of multidrug-resistant TB (MDR-TB). The country's Round 6 TB grant focuses on improving DOTS program quality by providing adequate diagnosis of and treatment for TB patients, as well as by expanding use of a national electronic surveillance and case monitoring system. The program focuses on those TB control activities that are not covered by the government budget or donor agencies, namely, infection control, MDR-TB, TB/HIV and inpatient treatment, as well as program monitoring. It also incorporates quality assurance initiatives for laboratory diagnostics, clinical training, health promotion and social support.

Tuberculosis (TB) is one of th...e leading causes of illness and death in Kenya, having a disproportionate impact on women and children. Since 1990, the absolute number of TB cases notified increased ten-fold in Kenya, as TB case notification rates for all cases has increased from 50 per 100,000 in 1990 to 329 per 100,000 people in 2008. Kenya developed a medium-term TB control strategic plan covering the period of 2006 to 2010, which was modelled on World Health Organization (WHO) Stop TB strategy. The strategic plan has steered the country towards the achievement of the global TB control targets of 70 percent case detection and an 85 percent treatment success rate. However, a number of challenges still remain for Kenya to achieve the Millennium Development Goals in TB prevalence and mortality, including reducing the delays in TB diagnosis and improving monitoring and evaluation systems. The Round 9 proposal is implemented by two Principal Recipients and aims to sustain and improve TB control gains in order to accelerate the reduction of TB incidence in Kenya. The current grant trains community health workers on the use of nutritional assessment tools and provides nutritional support to TB patients who are moderately and severely malnourished.

In Kyrgyzstan, Tuberculosis (T...B) re-emerged as an important public health problem after the independence and its burden remains high in the country. The case notification rate is 117 per 100,000 populations and is the third highest among the 53 countries of the World Health Organization European Region. According to the Drug Resistance Survey in 2007, Kyrgyzstan faces a very high multi drug-resistant TB (MDR-TB) prevalence of 25 percent among new positive cases and 54 percent among previously treated cases. The program consolidates Round 6 TB grant by strengthening TB information system, patient incentives, support to local authorities and community involvement in TB control. The goal of the program is to reduce the burden of TB by strengthening of Directly Observed Treatment Short-term (DOTS) framework and by scaling-up the management of Drug Resistance-TB (DR-TB). The program will be implemented using a Dual-Track Financing approach under 2 Principal Recipients, Project Hope and United Nations Development Programme (UNDP). This grant will focus on several activities such as strengthening the human and infrastructural capacities, establishing routine drug resistance surveillance throughout the country, upgrading the laboratory services and providing up-to-date treatment of DR-TB cases with appropriate patient support to ensure adherence.

The World Health Organization ...(WHO) estimated the prevalence and incidence of tuberculosis (TB) in Lao PDR in all forms to be 289/100,000 and 151/100,000 respectively. The goal of this Program is to reduce TB prevalence and mortality by half by 2015 relative to 1990, contributing to the achievement of the overall Millennium Development Goals (MDGs). The Programs four objectives are: ensuring universal and equitable access to TB diagnosis and treatment for all people suffering from TB; considerably scaling up programmatic management of drug-resistant TB (PMDT); strengthening TB-HIV collaborative activities; and strengthening managerial capacity of the National TB Program (NTP) and partners. Its target groups include all TB patients, contacts of TB patients, people living with HIV and AIDS (PLHIV) and ethnic minority groups. This is a consolidation of Round 10 and Round 7 TB grants.

To achieve its goal, the program will be increasing access to high quality DOTS by establishing additional microscopy centers in existing public sector laboratories, two prisons and DOTS centers in the private sector; establishing effective TB and HIV collaborating mechanisms; and empowering the community and people living with TB by increased and sustained advocacy, communication and social mobilization activities in all 15 counties. The program targets the general population ages 15-54, prisoners, underserved communities and people living with HIV and AIDS.

The program supported by this ...grant focuses on scaling up activities for prevention and control of TB. Its key interventions include strengthening laboratory capacity, training health personnel, expanding DOTS and improving the referral system between the community and the health centers where chronic cough is detected and the laboratory services. Sensitizing the general population to TB prevention and control is an integral part of the program and is being achieved through information, education and communication campaigns. The program targets people infected and affected by TB and the general population.

The program supported by this grant aims to reduce the rate of TB in the population by maintaining the detection rate above 70 percent; increasing the rate of treatment success from 76 percent to a minimum of 85 percent by 2014; and halving TB prevalence by 2015 with respect to 1990 levels. The program will focus on key populations that are most at risk of TB infection, including: populations in remote and poor areas; prisoners in the country's 30 largest penitentiaries; and people living and working in mining and extraction zones. The program will also support people suffering from multidrug-resistant TB in 20 priority areas.

To accelerate the reduction of... the morbidity burden of TB in the Moroccan population
more

Tuberculosis (TB) remains a ma...jor public health problem in Morocco. According to the 2009 Report of the National Tuberculosis Program (NTP), 70% of the detected TB cases were detected in the more urbanized and populated regions. This Program aims to overcome the weaknesses identified at the level of the anti-TB campaign and expand its geographic coverage throughout the country targeting urban population and high risk groups. The main objectives of the Program are to: increase the detection rate by 50% in 2012 compared to 2011; reach a therapeutic success rate of 90% by 2015; and increase the systematic TB screening activities amongst individuals at high-risk. The goal of the Program is to accelerate the reduction of the burden of TB in the Moroccan population, including vulnerable populations. The target groups are; urban population in 19 Regions; high risk groups (persons living with HIV and AIDS [PLWHA], prison inmates, contact persons, etc.); and care providers and organizations supporting the NTP.

The program supported by this grant aims to reduce the prevalence, illness and death related to TB in the country. The program targets the rural population and populations that usually do not have access to health services. Grant funds will be used to detect smear-positive cases through extension of DOTS services; counsel and test for HIV in TB patients; provide preventive therapy for HIV-positive TB patients; and diagnose and treat MDR-TB.

In 2007, an estimated 124,416 TB cases were notified in all forms, corresponding to a case notification rate of 265 cases per 100,000 population. The program is implemented through a Dual Financing Track scheme and focuses on TB and high risk patients. The proposal for this grant builds on the National TB Strategic Plan 2011 to 2015, and coordinated with both the National HIV/AIDS and Malaria proposals. The program aims to reduce the burden of TB by 2015 in line with the Millennium Development Goals (MDG) and Stop TB components of the new Stop TB Strategy. The activities under current grant will enhance the quality and expand services to all TB patients, sustain and further improve case detection and treatment success rate.

TB is still endemic in Nicarag...ua, where it is focused in vulnerable populations. By 2014 the program aims to detect 93 percent of the sputum smear-positive cases, to treat successfully 90 percent of these cases and to reduce treatment abandonment to 4 percent. The strategies include broadening the access to high quality DOTS, with a focus on community participation, and widening the coverage of prevention and control activities that started with the Round 2 grant. Target populations include the groups most exposed to the risk of infection - prisoners, people living with HIV and people living in poor and marginal areas – as well as those with multidrug-resistant TB. The Round 2 TB grant contributed to the reduction of the TB in seven selected departments. The Rolling Continuation Channel (RCC) program aims to strengthen and consolidate the results achieved by the program funded by the Round 2 grant. The RCC program will widen the scope of the interventions to the most vulnerable groups, improving prevention and control activities and changing the programmatic approach to be consistent with the National TB Control Strategy 2008 – 2015.

Built on the experiences and s...uccesses of the Round 2 grant, the Round 7 grant aims at attaining the goal of the Strategic Plan to Stop TB in the Western Pacific to halve prevalence and mortality from all forms of TB by 2010 relative to 2000, thereby contributing to the achievement of Millennium Development Goal 6. The following Pacific Island countries are benefiting from the program supported by this grant: Cook Islands, Kiribati, Marshall Islands, Micronesia (Federated States), Nauru, Niue, Palau, Samoa, Tonga, Tuvalu and Vanuatu. The program focuses efforts on sustaining and optimizing quality DOTS and exceeding the Stop TB Partnership targets for case detection and treatment; ensuring equitable access to high-quality TB care for all people with TB; and adapting DOTS to respond to multidrug-resistant TB and TB/HIV co-infection.

Although the incidence of new ...cases has been reduced since 1992, TB remains a significant public health problem in Peru, especially multidrug-resistant TB (MDR-TB). The overall goal of the program supported by this grant is to contribute to universal access to prevention services for the general and highly vulnerable populations, as well as to provide care, treatment and complete support for all those affected by TB, MDR-TB and extensively drug-resistant TB (XDR-TB) with a focus on social inclusion and human development. The program targets MDR-TB and XDR-TB patients and their families. Grant funds are being used to improve the impact of TB treatment in vulnerable populations; improve the laboratory network by upgrading the equipment and the information technology system; establish a quality sample transport system and training of health personnel; and scale up care and support to MDR-TB and XDR-TB patients by providing psycho-emotional support and drugs, surgery and support in case of adverse reaction to drugs, and training to health personnel. This grant is being implemented simultaneously with grants from Round 5 and Round 8.

Each year, Sao Tomé and Princ...ipe's National Tuberculosis Program records an average of 150 cases of all forms of TB. The program supported by this grant aims to reduce TB-related illness and death in the country by improving and expanding effective DOTS implementation, fighting TB/HIV co-infection and MDR-TB, and by giving people living with TB and their communities the ability to take action. The program targets the general population, youth, people living with HIV and AIDS, and the national healthcare system.

The World Health Organization ...estimated the incidence of TB in Serbia at 32 per 100,000 people in 2007. Figures for multidrug-resistant TB in 2007 were estimated at a low 0.4 percent of new cases and 4.1 percent of those previously treated. The program supported by this grant will continue and expand activities started under the program financed by a Round 3 Global Fund grant. The interventions to be implemented reflect the new epidemiological situation in the country and are focused on strengthening the DOTS framework, the management of drug-resistant TB and HIV/TB co-infection as well as on TB control in risk groups and vulnerable populations.

At 533 per 100,000 inhabitants... in 2007, the estimated incidence of new smear-positive tuberculosis (TB) cases in Suriname is the fifth highest in the Americas and estimated HIV prevalence among TB cases is the third highest in the region (WHO report Global Tuberculosis Control 2009). The goal of the program supported by this grant is to introduce quality-assured DOTS (the basic package that underpins the Stop TB Strategy) in Suriname in order to reduce TB morbidity, mortality and disease transmission while preventing the development of drug resistance. The program – which is aligned with the interim national TB strategy and the global Stop TB Strategy – will focus on fighting TB/HIV co-infections, improving infection control in health care settings and providing TB control activities for high-risk groups such as people living with HIV, prisoners and indigenous peoples. This is Suriname's first Global Fund grant for TB.

As in many other countries in ...the region, TB continues to be a major communicable disease in the Syrian Arab Republic. This grant will be used to address the low case detection rate and to contribute to reducing the gaps identified in the Syrian Arab Republic's ten-year strategic plan by improving access to TB care, particularly for poor and vulnerable populations; addressing multidrug-resistant TB (MDR-TB); engaging all care providers; strengthening the health system; and encouraging advocacy, communication and social mobilization. The program targets poor urban and rural areas of the five northeastern governorates; poverty pockets in the rural areas of the middle and coastal governorates; vulnerable groups at high risk of developing TB, in particular TB contacts and prisoners; and patients with MDR-TB.
