The program supported by this ...grant focuses on strengthening existing TB services in Tanzania by increasing TB case detection and improving treatment success through increased uptake and decentralization of quality TB control services. The program is also scaling up quality and access to TB and TB/HIV services through public-private partnership and community involvement in TB control and patient empowerment. Control of MDR-TB, with strengthening of laboratory services & implementation of DOTS-Plus activities is a major focus. Grant funds are also being used to support prevention of HIV activities for TB patients and to provide antiretroviral therapy to co-infected cases. Under the program, community and health workers will be sensitized to TB and MDR-TB issues through community outreach, media programs and training to change attitude and behaviors.

Thailand ranks 17th out of the... world's 22 highest TB burden countries. Despite early successes in TB control, TB increased by about 40 percent in the 1990s due to Thailand's high HIV prevalence and large numbers of HIV/TB co-infections. About 40 percent of all TB cases in Thailand occur in persons of working age. The private sector now employs a larger proportion of the workforce than a decade ago. The workplace offers an opportunity for early TB diagnosis and treatment and interruption of transmission. The National TB Program, however, has limited penetration of private hospitals and clinics - the current principal health care providers to many working age persons, particularly in large cities. The program supported by this grant will strengthen TB services in three major areas: it will close the gap in the delivery of TB services to patients covered by the social security fund through workplace activities; strengthen the TB laboratory network, and targeted public health care facilities to deliver TB care to non-Thai migrants. The Principal Recipient will support continued advocacy work to strengthen political commitment in private hospitals and in enterprise sectors to ensure that effective systems are strengthened to maintain continuation of TB treatment to vulnerable populations.

Despite early successes in TB control, TB increased by about 40 percent in the 1990s due to Thailand's high HIV prevalence and large numbers of HIV/TB co-infections. TB remains one of the top ten causes of mortality in Thailand and, together with HIV, is a major obstacle to the country's economic expansion as some 40 percent of all cases occur in persons of working age. Multidrug-resistant TB (MDR-TB) is a growing concern, particularly among migrant populations where the real MDR-TB burden remains unknown. The program supported by this grant aims to reduce TB-related illness among non-Thai migrants in six border and adjacent provinces. Program strategies including establishing a TB collaboration mechanism for border areas for coordinated TB care; enhancing TB management with participation of communities and all relevant partners; systematic development of human capacity; and promotion and support of research and development.

Tuberculosis (TB) is a major p...ublic health problem in Thailand and the World Health Organization (WHO) ranks Thailand as one of the 22 high TB burden countries. Although there has been much improvement in Thailand's health system, there is still a gap among certain population groups i.e. poor Thai people, ethnic minorities, and non-Thai migrants, who have difficulties accessing health care services. There has been a general lack of resources to raise TB awareness and support TB case finding among these populations, which undermines Thailand's ability to control the spread of the disease and has resulted in anticipating multidrug-resistant tuberculosis (MDR-TB) in the country. This Program will directly result in care for both TB suspects and cases among vulnerable people, specifically among both migrants and mobile Thai citizens, living in 76 provinces. Its goal is access for all to reduce prevalence, morbidity and mortality of TB in line with the Millennium Development Goal (MDG). This is a consolidation of Round 10 and Round 8 TB grants.

The overall goal of the program supported by this grant is to reduce TB-related illness and death by improving the quality of DOTS services throughout the service delivery chain. The program targets TB patients and their families, prisoners and the wider community. Grant funds are being used to strengthen the community-based DOTS approach through training community health workers; provide TB treatment to patients in prisons, in the armed forces, clinics and enterprises; develop information and campaign material to sensitize the public, family member, civil society, private clinics and enterprises as well as key opinion leaders and the media; and increase prevention activities among vulnerable populations.

While still under control, tub...erculosis (TB) prevalence, incidence and mortality rates have increased in Tunisia in recent years, affecting the country's prospects of realizing the Millennium Development Goal to halve TB prevalence and mortality by 2015 as compared with 1990. In addition, between 2002 and 2006 the prevalence of multidrug-resistant TB increased from 1.14 percent to 2.7 percent among new cases, and from 31 percent to 36 percent among retreated cases. The program supported by this grant aims to address gaps in the national TB strategy through strengthening DOTS (the basic package that underpins the Stop TB Strategy), improving prevention and care of multidrug-resistant TB, as well as by contributing to lower the incidence of TB among high-risk population groups (contact persons, prisoners and health care staff). Additional objectives are to decrease the lead time for testing suspected TB cases at the basic health care center level through reinforcement of the laboratory network; improve the TB monitoring and evaluation system; and strengthen national program management. The program is supported by two Global Fund grants implemented by a government and civil society Principal Recipient, respectively, working in partnership. This grant focuses on training-related activities.

The estimated Tuberculosis (TB...) incidence in Turkmenistan is 69 per 100,000 population. Despite Directly Observed Treatment, Short course (DOTS) being implemented country-wide, including in prisons, and the National TB Programme (NTP) having achieved notable results during recent years, important challenges still need to be addressed. The program funded under this grant has a country-wide scope and addresses the needs of both civilian and penitentiary sectors. The program is designed to sustain DOTS achievements in the country through strengthening NTP capacities, strengthening TB laboratory diagnosis, support to quality treatment of TB cases, advocacy, communication and social mobilization, and initiating TB/HIV collaborative activities. Health System Strengthening (HSS) is an integral part of the project. Interventions to be supported include continuing involvement of Primary Health Care (PHC) in TB control, and strengthening the key health system functions, such as financing, human resources development and service delivery. The program aims to decrease the burden of MDR-TB through introducing and scaling up the DR-TB management, expanding access to DR-TB diagnosis and treatment, providing social support for patients, and conducting operational research on DR-TB.

Ukraine has the biggest tuberc...ulosis (TB) control problem of any country in Europe with the exception of the Russian Federation. The goal of the program supported by this grant is to contribute to reducing the national TB burden through expanding and enhancing access to high-quality TB services. Specific objectives include preventing multidrug-resistant TB, improving treatment effectiveness, institutionalizing the training and retraining of health care workers, and improving laboratory diagnosis. These ends will be achieved by building national TB program management capacities at central and regional levels; consolidation of the laboratory structure and introduction of external quality analysis; strengthening provision of DOTS (the basic package that underpins the Stop TB Strategy) supported by case management and outreach; rollout of internationally approved treatment for multidrug-resistant TB; and systematic human resource management and introduction of TB-relevant content in courses for TB specialists and primary health care physicians. This program will benefit the vulnerable and the poor, the populations most at risk of TB.

Viet Nam ranks 12th out of the... 22 countries that account for 80 percent of the world's Tuberculosis (TB) burden. The program supported by this grant aims to reduce the transmission of TB by scaling up essential TB program components such as multidrug resistant tuberculosis (MDR-TB), TB control in closed settings and Advocacy, Communication, and Social Mobilization (ACSM) through new partnerships for expanded impact. The program targets people living with HIV, intravenous drug users, commercial sex workers, and prisoners and people living in Training and Education Centres (TEC). The current grant is based on the Round 9 TB proposal and supports the National Tuberculosis Program for 2011 to 2015 (NTP) and the next Mid-Term Development Plan (MTDP). The activities focus on gaining political commitment to increase human and financial resources and make TB control a nationwide political commitment to increase human and financial resources and make TB control a nationwide priority integral to the national health system, on granting access to quality assured TB sputum microscopy, standardized and quality TB treatments and on improving reporting, monitoring and evaluation.

The oPt is a middle income ter...ritory composed of two separate geographical areas, the West Bank and the Gaza Strip, a narrow land strip with one of the highest population densities in the world. Despite high risk factors for the transmission of tuberculosis (TB) as a result of the ongoing conflict, the geographical isolation of the Gaza strip and the restriction policy on the movement of persons and goods in the West Bank, incidence rate remains at a low level (0.87%), comparable to neighboring countries such as Jordan and Israel. However, the extent of tuberculosis care is still limited, and thus case detection rate is considerably low (<5%; WHO Global Tuberculosis Report 2008). With the support of the Global Fund, the program will address a number of gaps in the existing efforts on tuberculosis care in the West Bank and Gaza Strip in a complimentarily and comprehensive way, strengthening capacities at the national level to better address TB, scaling up TB care through implementing the Stop TB Strategy in the Palestinian Territories, and developing a national plan of action.

TB is a major threat to health... in Zambia. The annual risk of infection is very high as is HIV prevalence among TB patients. The program supported by this grant aims to reduce illness, death and the socio-economic burden associated with TB in the Zambian population. Grant funds are being used to pursue high-quality DOTS enhancement and expansion; address TB/HIV, MDR-TB and other challenges; contribute to health systems strengthening; engage all care providers; empower people with TB and communities; and enable and promote research. The program targets TB patients, people co-infected with TB and HIV and AIDS, healthcare workers, community treatment supporters and general and vulnerable populations.

A drug-resistant survey, condu...cted in 2007, showed multidrug-resistant TB (MDR-TB) prevalence of 22 percent among new TB cases and 56 percent among previously treated cases. Over 14 percent of all MDR-TB cases are extremely drug-resistant TB (XDR-TB). Azerbaijan is among the 25 priority MDR-TB and XDR-TB countries identified by the World Health Organization based on estimated MDR-TB burden (more than 10 percent among both new and retreatment cases combined). The consoidated program encompasses the activities financed under Phase 2 of the Round 5 TB grant and the Round 7 TB grant. The program aims to reduce the burden of TB in Azerbaijan in high-risk population groups (including people living with HIV), prisoners, labor migrants, refugees and internally displaced persons) by strengthening and expanding the DOTS framework, scaling up the management of drug-resistant TB and preventing the further spread of drug-resistant TB including XDR-TB. This consoidated program also aims to improving the detection, diagnosis and quality treatment of TB cases (including MDR-TB) and contributing to the universal access of the population to diagnosis and treatment. These objectives are achieved by strengthening the management and coordination of the National TB Control Program, involving the primary health care network in TB control, introducing and scaling up drug-resistant TB management, sustaining drug resistance surveillance throughout the country, strengthening DOTS in penitentiaries and upgrading the laboratory services.

Over the years, Bhutan has ach...ieved a significant breakthrough in scaling down the TB infection rate due to the establishment of the Bhutan National Tuberculosis Control Program in 1976. However, DOTS coverage of certain population groups is still incomplete — in particular, women, youth under the age of 18 and people living in rural areas. The government is strongly committed to eradicating the TB threat, directing its efforts at the most vulnerable groups — the poor and remote populations. The national program has been pursuing a pro-poor policy by treating 70 percent of TB patients free of charge. The program supported by this grant builds upon the work being done under the Round 4 grant with the overall goal of preventing the emergence of further drug resistance. The program is geared particularly towards enhancing access to DOTS services for women, youth, the rural population, migrant workers and prisoners. In addition to DOTS, the strategy includes management of multidrug-resistant TB, including building capacity for drug-susceptibility testing and HIV sentinel testing in all TB patients during three months per year. The strategy is being implemented through all health care providers in the country.

TB is a major public health pr...oblem in Botswana, responsible for more than 10 percent of all hospital admissions and more than five percent of outpatient visits. This grant supports a program that aims to increase the treatment success rate from 72 percent to 85 percent by 2007. The program targets the country's entire population with particular focus on health workers, current TB patients and people living with HIV and AIDS. To achieve its goal, the program will scale up community TB care in all health districts, strengthen TB/HIV collaborative activities and strengthen supervision and monitoring and evaluation at all levels.

TB is a significant health pro...blem in Cambodia, primarily affecting adults of working age. The program supported by this grant aims to reduce TB-related illness and death in line with the Millennium Development Goals and the Stop TB Partnership strategy.. The program targets people suspected of having TB and those that have been confirmed as having TB; TB patients with limited access to DOTS; TB/HIV co-infected patients; and multidrug-resistant TB (MDR-TB) patients. Grant funds are being used to ensure high quality TB services nationwide by strengthening laboratory services, providing quality assured anti-TB drugs and implementing activities for advocacy, communication and social mobilization; expand close-to-patient DOTS services through community DOTS and public-private mix services; address TB/HIV co-infection by building capacity of staff to plan and implement TB/HIV collaborative activities; and to address MDR-TB by building the capacity of the National TB Program to provide MDR-TB services in three selected sites in country and to provide MDR-TB treatment to a growing number of patients.

The Democratic Republic of Con...go (DRC) ranks 11th out of 22 countries most affected by Tuberculosis (TB). TB is a serious public health problem despite the policy of free medication being made available across the country. The program supported by this grant aims to ensure a treatment success rate of new TB smear-positive patients at 87 percent by the end of 2012. The goal of the program is to reduce the morbidity and mortality caused by TB in the Congolese population. The program will treat all TB patients with first-line drugs and focus on people affected by multi-drug resistant (MDR) TB. The program is also a follow-up of the Global Fund Round 2 TB and Round 5 and 6 consolidated grants in the DRC, which will be implemented by two Principal Recipients under Dual Track Financing scheme. The current grant will implement Practical Approach to Respiratory Health (APSR), apply Public-Private Mix (PPM) strategies, and strengthen awareness through advocacy and community-based TB care by extending community-based Directly Observed Treatment, Short Course (DOTS) therapy, and setting up associations of former TB patients and research activities.

According to the World Health ...Organization (WHO), the estimated incidence rate of sputum positive pulmonary tuberculosis (TB) in the Dominican Republic fell from 54 to 40 per 100,000 inhabitants between 2003 and 2006, and the mortality rate from 17 to 15 per 100,000 inhabitants (WHO Global Tuberculosis Control 2005 and 2008 respectively). This downtrend in the incidence and mortality rate is a result of the marked improvement in the country's national TB control response. The main objective of the Rolling Continuation Channel program is to contribute to the reduction of TB-related morbidity and mortality by sustaining and expanding the actions implemented in the program supported by the Round 3 grant among vulnerable populations in 22 provinces of the Dominican Republic. In order to meet this goal, most of the project interventions are related to the need for strategic National Tuberculosis Control Program (NTCP) guidelines within the context of the health sector reform process taking place in the country, as well as a review and update of the tools and planning and operational systems of the NTCP. In addition, the program focuses on the need to promote the closure of detection gaps and to implement a number of necessary studies for the reorganization of NTCP interventions.

Due to the economic downturn, ...political instability, armed conflicts and population displacements, Tuberculosis (TB) re-emerged as a major public health threat in Georgia. Despite remarkable progress achieved since 1995 under the National TB Control Program (NTP), serious challenges remain related to resistance to TB drugs. In 2008 to 2009, MDR-TB in Georgia was found in 10.6 percent to 11.3 percent of new cases and 32.5 percent to 40.3 percent of previously treated cases. The Country Coordinating Mechanism (CCM) submitted a TB proposal in Round 10 and agreed to consolidate the existing Round 4 Rolling Continuation Channel (RCC) TB grant with Round 10 grant to align activities and reporting. In sustaining universal access to multi/extensively-drug resistant (M/X-DR) TB, the program targets to enrol 3, 730 DR-TB patients in treatment over the next 5 years, ensuring coverage of not less than 90 percent of the needs. The NTP Central Unit through the network of public health care institutions and providers will be responsible for implementation of the majority of the planned activities. The current grant will support provision of technical assistance to the NTP in priority areas of TB control and provide IT support.

TB is one of the main health p...roblems in Guinea-Bissau. The National Health Development Program for 2008-2017 (PNDS II) lists the anti-TB campaign as a national priority. The overall goal of the program supported by this grant is to reduce TB morbidity and mortality by extending the coverage of DOTS, the basic package that underpins the Stop TB Strategy, throughout the country. The program will focus on expanding access to treatment and follow-up in remote areas through community health workers, strengthening the country's diagnostic capacities and responding to the growing threat of multidrug-resistant TB. The program targets the entire population, with particular emphasis on most-at-risk groups such as people living with HIV and prisoners. Grant funds will be used to provide first- and second-line drugs, HIV screening for TB patients, training for health staff, and to rehabilitate and equip community health posts and laboratories. The program supported by this grant broadly pursues the same strategies and activities that were funded under the two Round 3 grants.

After years of success in cont...rolling tuberculosis (TB) in Guyana in the 1970s, the disease re-emerged in the 1990s as an important health problem. Contributory factors include the HIV epidemic, and lapses in national control and prevention efforts. Integration of TB prevention, treatment and care activities into general health care services is critical to expanding access to TB services, but integration has been slow in 2000-2010. This is mostly due to a lack of capacity at local authority level, despite continued investment in health infrastructure and national capacity. The overall goal of the program supported by this grant is to reduce the transmission of TB and the emergence of multidrug-resistant strains by improved case detection and treatment success rates. Strategies include management of HIV/TB co-infection; expansion of DOTS in the prison system to all prisons; strengthening human resources at the central unit of the National TB Program; and developing and implementing regional programs. Additional activities include the development of a strategy for information, education and communication with a focus on materials concerning HIV/TB co-infection; and development and implementation of a public-private mix program to promote collaboration across sectors, and of TB treatment programs for hard-to-reach high-risk populations.

In Honduras, Tuberculosis (TB)... is considered a public health issue, and it is among the 20 problems requiring mandatory notification. The program aims to increase case notification rate among new smear positive cases from 25.6 to 37.4 percent per 100,000 population by 2016 and to maintain treatment success rates from 2011 to 2015 above 85 percent. The program targets the population of 48 priority municipalities in the country as well as the at-risk population, multi-drug resistant TB (MDR-TB) patients and patients co-infected with TB and HIV. The program plans to improve DOTS strategy application at the institutional level to increase case detection and healing of TB patients, and to strengthen the laboratory network in order to improve diagnosis and response to new challenges proposed by the Stop the TB strategy. The program also plans to strengthen the inter-programmatic approach to decrease TB-HIV co-infection. By improving the clinical and programmatic management of MDR-TB and social support focused on the needs of the patient, family and community, the program increases the detection of cases and decreases the rate of abandonment. TB prevention, management and control of activities among prisoners and other high-risk populations is developed to decrease case incidence in these populations.

Kazakhstan has one of the high...est TB burdens in Eastern Europe and Central Asia, including one of the highest rates of multidrug-resistant TB (MDR-TB). The country's Round 6 TB grant focuses on improving DOTS program quality by providing adequate diagnosis of and treatment for TB patients, as well as by expanding use of a national electronic surveillance and case monitoring system. The program focuses on those TB control activities that are not covered by the government budget or donor agencies, namely, infection control, MDR-TB, TB/HIV and inpatient treatment, as well as program monitoring. It also incorporates quality assurance initiatives for laboratory diagnostics, clinical training, health promotion and social support.

TB, together with HIV and AIDS... and malaria are the leading causes of illness and death in Kenya, having a disproportionate impact on women and children. The program supported by this grant aims to prevent the development and transmission of MDR-TB in Kenya and to halt and begin to reverse the incidence of TB in Kenya by 2015. The program targets patients with MDR-TB, TB/HIV infected patents and the general public. Grant funds are used to provide refresher training of laboratory personnel in Acid-Fast Bacillus smear microscopy; collect and transport sputum samples from the peripheries to the Central Reference Laboratory for culture; procure protective gear for health care workers; construct an isolation facility at Kenyatta National Hospital ; and to procure second line anti-TB drugs for the treatment of patients with MDR-TB.
