TB, together with HIV and AIDS... and malaria are the leading causes of illness and death in Kenya, having a disproportionate impact on women and children. The program supported by this grant aims to strengthen the provision of health care at the primary level by improving health care infrastructure, human resources, health planning, and management capacity including monitoring and evaluation to effectively deliver integrated TB/HIV services. The program targets TB patients, including those co-infected with HIV, and the general population.

Although there had been a redu...ction in TB cases between 1995 and 2001, the disease remains a major public health problem in Mauritania. The overall goal of the program supported by this grant is to reduce TB-related illness and death in the country. Grant funds are being used to train service providers and health staff; provide timely detection and quality treatment of cases, strengthen awareness and behavior change and communication activities; prevent HIV in TB patients; intensify detection among people living with HIV; and prevent opportunistic infections in people living with HIV who are co-infected with TB.

The program supported by this ...grant aims to ensure a reduction in the burden and human impact of TB and its transmission to such an extent that it no longer constitutes a public health problem in Nigeria. Increased access to DOTS facilities and improvements in the quality of care for TB patients are among the goals of the program, with targets of a 70 percent case detection rate and an 85 percent treatment success rate by 2010. A substantial portion of grant is being used for training in several areas: public and private sector health workers are being taught about DOTS, lab technicians are getting instruction in case detection, public health staff is receiving training in program management, and community volunteers are being educated about the cause transmission and curability of TB. Additionally, the program is actively identifying and treating patients with TB/HIV co-infection.

Built on the experiences and s...uccesses of the Round 2 grant, the Round 7 grant aims at attaining the goal of the Strategic Plan to Stop TB in the Western Pacific to halve prevalence and mortality from all forms of TB by 2010 relative to 2000, thereby contributing to the achievement of Millennium Development Goal 6. The following Pacific Island countries are benefiting from the program supported by this grant: Cook Islands, Kiribati, Marshall Islands, Micronesia (Federated States), Nauru, Niue, Palau, Samoa, Tonga, Tuvalu and Vanuatu. The program focuses efforts on sustaining and optimizing quality DOTS and exceeding the Stop TB Partnership targets for case detection and treatment; ensuring equitable access to high-quality TB care for all people with TB; and adapting DOTS to respond to multidrug-resistant TB and TB/HIV co-infection.

Although the incidence of new ...cases has been reduced since 1992, TB remains a significant public health problem in Peru, especially multidrug-resistant TB (MDR-TB). The overall goal of the program supported by this grant is to contribute to universal access to prevention services for the general and highly vulnerable populations, as well as to provide care, treatment and complete support for all those affected by TB, MDR-TB and extensively drug-resistant TB (XDR-TB) with a focus on social inclusion and human development. The program targets MDR-TB and XDR-TB patients and their families. Grant funds are being used to improve the impact of TB treatment in vulnerable populations; improve the laboratory network by upgrading the equipment and the information technology system; establish a quality sample transport system and training of health personnel; and scale up care and support to MDR-TB and XDR-TB patients by providing psycho-emotional support and drugs, surgery and support in case of adverse reaction to drugs, and training to health personnel. This grant is being implemented simultaneously with grants from Round 5 and Round 8.

Funded activities included strengthening peripheral diagnostic services, launching publicity campaigns, improving the management of information systems and carrying out operational research in relation to TB control strategies.

Tajikistan has the worst TB ep...idemic in Eastern Europe and Central Asia. The program supported by this grant aims to reach the targets of at least 70 percent detection of new sputum smear-positive cases with an 85 percent rate of successful treatment of identified cases. Program strategies including strengthening the management of the country's National Tuberculosis Control Program; implementing effective advocacy, communication and social mobilization of TB control; expanding DOTS in the prison system; introducing the management of drug-resistant TB; improving collaboration for control of TB/HIV co-infection; and strengthening the evidence for decision-making and action through operational research in TB. In addition, the program intends to scale up multidrug-resistant TB (MDR- TB) diagnostic and treatment services, including early case detection. It includes new interventions such as addressing TB and human rights issues among the prison population. The program targets TB patients, including prisoners; drug-resistant TB patients, migrant workers and their families; TB service providers; and the general population.

The program supported by this ...grant focuses on strengthening existing TB services in Tanzania by increasing TB case detection and improving treatment success through increased uptake and decentralization of quality TB control services. The program is also scaling up quality and access to TB and TB/HIV services through public-private partnership and community involvement in TB control and patient empowerment. Control of MDR-TB, with strengthening of laboratory services & implementation of DOTS-Plus activities is a major focus. Grant funds are also being used to support prevention of HIV activities for TB patients and to provide antiretroviral therapy to co-infected cases. Under the program, community and health workers will be sensitized to TB and MDR-TB issues through community outreach, media programs and training to change attitude and behaviors.

The DOTS strategy was initiate...d in Zanzibar in 1987, but has only been available in urban areas. The program supported by this grant focuses on expanding health facility-based DOTS services to all ten districts of Zanzibar. As part of this process, the program is improving access to quality TB diagnostic services, strengthening TB/HIV collaborative activities and improving health workers' technical and managerial skills. The overall goal of the program is to raise case detection, improve patients' adherence to treatment, and to increase the cure rate. The program is also improving community awareness of TB. The program targets TB patients, people living with HIV and AIDS and the general population.

This stand-alone single stream... of funding (SSF) Round 10 tuberculosis (TB) grant has five objectives derived from Zanzibar's TB Strategy Plan 2010-2015. These objectives are: pursue high quality Directly Observed Treatment, Short Term (DOTS) expansion and enhancement; strengthen collaborative TB/HIV activities; prevent TB transmission in health facilities and other high risk congregate settings; engage all care providers in TB control; and empower people and communities in TB control. In an effort to achieve the two priority interventions of intensified case finding and alleviating the burden of TB in people living with HIV, this program aims to strengthen the ZTLP's(The Zanzibar TB and Leprosy Program) capacity so that it can carry out the Global Stop TB Strategy and its own TB Strategic Plan. This Program also aims at assessing and strengthening current monitoring & evaluation (M&E) practices and strategies present in the Zanzibar today. The Program targets TB patients, people living with HIV and AIDS and the general population.

The estimated Tuberculosis (TB...) incidence in Turkmenistan is 69 per 100,000 population. Despite Directly Observed Treatment, Short course (DOTS) being implemented country-wide, including in prisons, and the National TB Programme (NTP) having achieved notable results during recent years, important challenges still need to be addressed. The program funded under this grant has a country-wide scope and addresses the needs of both civilian and penitentiary sectors. The program is designed to sustain DOTS achievements in the country through strengthening NTP capacities, strengthening TB laboratory diagnosis, support to quality treatment of TB cases, advocacy, communication and social mobilization, and initiating TB/HIV collaborative activities. Health System Strengthening (HSS) is an integral part of the project. Interventions to be supported include continuing involvement of Primary Health Care (PHC) in TB control, and strengthening the key health system functions, such as financing, human resources development and service delivery. The program aims to decrease the burden of MDR-TB through introducing and scaling up the DR-TB management, expanding access to DR-TB diagnosis and treatment, providing social support for patients, and conducting operational research on DR-TB.

The oPt is a middle income ter...ritory composed of two separate geographical areas, the West Bank and the Gaza Strip, a narrow land strip with one of the highest population densities in the world. Despite high risk factors for the transmission of tuberculosis (TB) as a result of the ongoing conflict, the geographical isolation of the Gaza strip and the restriction policy on the movement of persons and goods in the West Bank, incidence rate remains at a low level (0.87%), comparable to neighboring countries such as Jordan and Israel. However, the extent of tuberculosis care is still limited, and thus case detection rate is considerably low (<5%; WHO Global Tuberculosis Report 2008). With the support of the Global Fund, the program will address a number of gaps in the existing efforts on tuberculosis care in the West Bank and Gaza Strip in a complimentarily and comprehensive way, strengthening capacities at the national level to better address TB, scaling up TB care through implementing the Stop TB Strategy in the Palestinian Territories, and developing a national plan of action.
