This grant represents part of ...Canada's contribution to the Global Drug Facility (GDF), a program of the Stop TB Partnership. It provides access to anti-tuberculosis (TB) drugs for governments that agree to introduce, expand, or maintain the diagnostic, treatment, and monitoring policies of the DOTS (directly observed treatment, short-term) strategy. Launched in 2001, the GDF works to improve the supply, distribution, and monitoring of anti-TB drugs in developing countries and is the only global bulk procurer of anti-TB drugs. Canada was the founding donor of the GDF and to date (2010) has been the single largest donor country for first-line TB drugs.The GDF is a four-pronged mechanism whereby: (1) countries who cannot afford to procure all the anti-TB drug they need are eligible to apply for drugs for up to six years (DFATD provides funding for this component); (2) countries who can pay for the anti-TB drugs they need but cannot assure the quality of the drugs can directly procure them through the GDF at reduced bulk prices (DFATD funding does not contribute to this component); (3) countries where the procurement system is weak can receive technical assistance to strengthen their capacity for drug procurement (DFATD supports this component); and (4) Global Fund recipient countries can procure anti-TB drugs directly through GDF, using Global Fund monies, to ensure a stable and high-quality drug supply./ Cette subvention représente une partie de la contribution du Canada au Dispositif mondial pour l'approvisionnement en médicaments, un programme du Partenariat Halte à la tuberculose. This project aims to reduce il...lness and death from tuberculosis (TB) in low-income countries by supporting TB REACH, an initiative of the Stop TB Partnership. TB REACH provides grants to partners for testing innovative approaches which aim to increase the number of people diagnosed and treated for TB, decrease a patient's time to start appropriate treatment, and improving treatment success rates in key populations and communities with limited access to care. It combines fast-track, results-based financing, and external monitoring and evaluation to produce results, so other donor agencies and national governments can scale-up successful approaches, and maximize their own investments. Project activities include: (1) funding of projects which deliver innovative and gender-sensitive approaches for TB diagnosis, treatment and reporting, (2) providing guidance and tools for monitoring activities, and evaluating impact of funded projects, (3) delivering technical assistance to improve the outcome and impact of project activities, (4) supporting analysis of project data and disseminating results, and (5) aiding in the development of national TB strategic plans while advocating for inclusion of successful approaches funded by TB REACH. / Ce projet vise à réduire les cas de maladies et de décès attribuables à la tuberculose (TB) dans les pays à faible revenu. Pour ce faire, il appuiera TB REACH, une initiative du Partenariat Halte à la tuberculose. TB REACH fournit des subventions à ses partenaires pour qu'ils fassent l'essai d'approches innovantes qui visent à augmenter le nombre de diagnostics posés et de traitements accordés en cas de TB, à diminuer le temps d'attente avant que le patient initie son traitement et à améliorer le taux de réussite du traitement auprès des populations clés qui ont un accès limité aux soins de santé. Pour produire des résultats, l'initiative mise sur un financement accéléré fondé sur les résultats ainsi qu'une surveillance et une évaluation externes, de sorte que les autres organisations donatrices et les gouvernements nationaux peuvent mettre à l'échelle les approches qui fonctionnent bien et maximiser leurs investissements. The programme aims to promote ...the well-being of children in the Mosoq Ayllu are by developing pre-school education, literacy, nutrition, life skills and youth entrepreneurship. The programme is implemented in the rural area of Cusco, Pitumarca and Checacupe municipalities in Peru. Most of the inhabitants get their income from agriculture, other sources for income are sparse. Poverty and the poor quality of school education and health services affect the quality of children's life in the area. Child protection violations also occur. Within the programme, the capacity of community members and local partners on child protection are enhanced. Children's nutrition status is being enhanced by promoting livestock breeding and entrepreneurship. Innovative teaching methods are introduced in order to promote the quality of pre and primary education. The programme benefits 11.000 people in 34 rural villages. The main partner is World Vision Peru, which is responsible for the programme implementation and for the utilization of the programme resources.

The continuation project build...s on the previous projects achievements and lessons learned. Tuberculosis continues to be one of the main health problems in Somalia. In addition more and more people are diagnosed or suspected of carrying multi-drug resistant tuberculosis (MDR-TB) a form of TB caused by bacteria strains that do not respond to the medicines used in standard TB treatment. PSR is one of the leading agencies working on detecting MDR-TB cases in Somalia and the aim is to increase the local capacity on MDR detection through cultures and drug susceptibility tests. In addition PSR will continue to facilitate the operations of two TB Centres offering WHO recommended DOTS Protocol for TB patients/suspects and their families. These clinics are located in Hargeisa and Mogadishu respectively. The centres will continue to offer voluntary HIV-testing for TB patients.The objectives of the continuation project are similar to those set for the previous project. The project has been designed in such a way that it supports and strengthens Somalia?s own National Tuberculosis Programmes which are part of the Ministries of Health. This project also supports the implementation related to the so-called Somalia Compact which includes the key features of the Peace and State-building Goals (PSG) of Somalia. In particular this project supports the realization of Goal 5 namely Revenue and Services. At the same time it is in line with other key elements of the Compact such as inclusiveness and increased participation of citizens. In addition to the support from the MFA Finland PSR?s TB work in Somalia is supported by the Global Fund to Fight AIDS Tuberculosis and Malaria. All the GF projects support the implementation of the NTPs meaning that this project is also closely linked to the overall GFATM TB Programme. Another component of PSR?s TB work is related to WFP?s Nutrition Program. With the support of WFP PSR continues to distribute food products for TB patients and their families with the aim of helping some of the most vulnerable people of the country. The main beneficiaries of the project include the local people in Hargeisa and Mogadishu suspected or diagnosed with TB ? and their families. The project will ensure that there is no discrimination whatsoever when it comes to access to treatment and services.

The project builds on the prev...ious TB intervention implemented by LSV. However in order to increase effectiveness totally new strategies will be applied.TB continues to be an essential health problem in Somalia. The disease hasn't been beaten yet but rather new problems emerge such as multi-drug resistant forms of TB resulting in much more complex treatment regimens and bigger danger to the population.LSV will continue to diagnose and treat TB patients in Hargeisa and Mogadishu. As a new strategy the private sector health care providers (clinics and pharmacies) and local communities will be engaged in the effort in a systematic way in order to strengthen a holistic public-private-people partnership approach to tackling the disease. In addition collaboration with the local maternity and child health facility network will be strengthened. The overall objective is to contribute to the reduction of TB burden and suffering in Somalia. The immediate objectives of the project include 1) To increase TB awareness and participation of communities in TB control. 2) To increase TB awareness and participation of private practitioners and pharmacists in TB control in. 3) To diagnose and treat patients at two TB centers according to WHO's DOTS protocol. 4) To provide TB culture services and drug susceptibility tests (DST) for suspected MDR cases.Regarding the cross-cutting themes the project will empower women and facilitate their access to TB services. In addition the project will reduce inequality by providing an access to TB services for internally displaced persons (IDPs). When it comes to climate proofing there are no negative impacts on environment. For example thanks to community DOTS there will be less daily traveling to TB Centers.The main project partners include NGO called PSR and National TB Programs (NTPs). LSV will strengthen their capacity in all the fields described above.The main beneficiaries include local people of Hargeisa Mogadishu and nomads living in nearby areas who have TB or belong to risk groups. The project will help the poorest of the poor. For example many cases are found among people who suffer from malnutrition.

The project is founded on PSR'...s previous projects in Somalia which have developed new strategies to tackle tuberculosis (TB) ? especially in community and private sector engagement. A special focus will be placed on women and vulnerable community members: disabled persons IDPs and prisoners. Tuberculosis is a central health problem in Somalia. The increase of multi-drug resistant tuberculosis (MDR-TB) is a global threat ? fighting against MDR-TB in Somalia benefits all countries. Another central problem is the low treatment coverage rate of about 37% leaving far too many cases untreated further transmitting tuberculosis to others. The impact targets of the project are to: a) tackle multi-drug resistant tuberculosis, and b) increase the tuberculosis case detection rate. The long-term objective of the project is to contribute to the reduction of the tuberculosis burden in Somalia with the following means:1. Increase the access of women and vulnerable groups to TB services (such as disabled personsIDPs and prisoners)2. Increase TB referrals from the private sector3. Improve the quality of TB treatment in both the public and private sector4. Improve the quality of TB laboratory work.The central partners of the project are the National Tuberculosis Programmes of Somaliland and Somalia. PSR has a central role in developing the national tuberculosis diagnostic infrastructure in Somalia. The project will support the National TB Reference Laboratories in Somaliland and South-Central Somalia as well as will treatment of tuberculosis patients in 3 clinics in Hargeisa and Mogadishu. These clinics will develop innovative community public and private sector engagement models to increase case detection improve treatment quality and to increase public awareness and decrease stigma/discrimination. The clinics will also provide HIV services and nutritional support for malnourished patients (with WFP).Together with the local administration the tuberculosis clinics will be transformed into health centers providing specialized tuberculosis services by the end of 2022 to support integration with basic health services and sustainability after the project has ended.

Contribuer à l'élimination d...e la maladie en Afrique en appui aux programmes nationaux de lutte contre la tuberculose de 8 pays d'AOC. Le projet permettra de renforcer les actions de prévention, de dépistage, et prise en charge des malades, en favorisant l'implication des acteurs communautaires. Le projet visera plus spécifiquement à renforcer l'accès à la prévention de la tuberculose chez les populations vulnérables.
