The project will directly addr...ess poverty alleviation through the following objectives: (a) to improve the health and nutrition status of slum-dwellers in the urban areas of Metro-Manila, Metro-Cebu, and Cagayan de Oro; (b) to build the capacity of city and municipal governments in these areas to plan, finance and implement cost-effective slum health and nutrition programs; (c) to help slum communities to identify their own health, nutrition and environmental problems and participate in planning, implementing and monitoring appropriate interventions; and (d) to help the Department of Health develop policies and strategies which will improve the outreach and cost-effectiveness of health and nutrition delivery systems.The project will support the following components: (a) service delivery, to improve the outreach, range, quality and cost-effectiveness of priority health and nutrition services, through improvements in health risk assessment and client targeting system, additions/improvements to health staffing and facilities, and provision of additional equipment, contraceptives, drugs and supplies and food and micro-nutrient supplements; (b) institutional development, to improve the capacity of project local governments to plan, manage, monitor and evaluate their health programs, and of the Department of Health to support local governments, through strengthening management systems and planning and evaluation capacity; training technologies, programs and infrastructure; and communications technologies and programs; (c) community partnerships for health, to support community mobilization and community-based health and nutrition service delivery, through financing grants to strengthen local nongovernmental organizations (NGOs), to develop community/NGO/local government fora, and to implement joint community/local government/NGO sub-projects in health and nutrition; and (d) policy research and evaluation, to support operations research studies testing alternative service delivery models for key interventions, and studies to evaluate project progress.

Component 2. Improvement to ac...cessibility of maternal, nutrition and child health services
(US$14 million, including US$12 million IDA and US$2 million MDTFHRI)
39. This component will support several mechanisms whose main purpose is to subsidize
demand for healthcare and nutrition services. These mechanisms are (i) the Universal Health
Insurance Scheme (i.e. Couverture Maladie Universelle or CMU), including the Health
Equity Fund and the national identification system of the poorest; (ii) the national program for
improving demand for nutrition services; and (iii) the maternal health vouchers.
Sub-component 2.1. Support to the implementation of the Universal Health Insurance
scheme (US$7 million)
40. The objectives of this sub-component are to help the Government implementing the
Universal Health Insurance Coverage scheme, called Couverture Maladie Universelle
(CMU). As mentioned earlier, the CMU consists mostly in creating at least one mutuelle (i.e.
community-based health insurance scheme) in each commune of Senegal and in subsidizing
heavily (at a 50% rate) the premiums. An additional mechanism (i.e. Equity Fund) will ensure
that the households identified as poorest will pay neither a premium nor copayments. This
CMU is therefore based on an ambitious strategy of voluntary enrollment in communitybased

This PforR operation will supp...ort the Government’s primary health care (PHC) program for the
2015/16 – 2019/20 period, with a strong focus on a key government initiative, the Big Results Now in
Health (BRN in Health).
12. PHC under the Health Sector Strategic Plan IV (including BRN in Health) constitutes the
Government’s program. In the structure of the Tanzania health system, PHC is delivered at the district
level and below, involving district hospitals, health centers, dispensaries and community based health
services under the management of Council Health Management Teams (CHMTs). Over the next five
years, PHC will be guided by the HSSP IV, which will drive major reforms and implementation
including the BRN in Health.
13. The 2015-2018 BRN in Health program aims to accelerate the reduction of maternal and
neonatal mortality through improving performance, governance and accountability in PHC. It was
developed as part of Tanzania’s Development Vision 2025 and has four national key results areas
(NKRAs) as follows:
Performance Management: This result area aims to improve health workers’
performance. Interventions include: (i) a stepwise accreditation scheme for all PHC
facilities in the country (aka “Star Rating” initiative) which has both nation-wide
assessment and a subsequent facility improvement program (including incentives) to
help facilities improve their performances and star ratings, (ii) fiscal decentralization at
the facility level, (iii) performance contracts and targets at individual health worker
levels, and, (iv) social accountability mechanisms.
Human Resources for Health: This result area aims to improve the distribution of
skilled PHC workers especially in nine regions with critical shortages in human
resources for health (i.e. less than national averages). Interventions include: (i)
increasing PHC employment permits for such regions, (ii) engaging the private sector to
provide skilled HRH for public health facilities through PPPs, (iii) redistributing health
care workers within regions, and, (iv) optimizing the pool of new recruits through
“bonding” policy or compulsory attachments.
Health Commodities: This result area aims to improve the availability of essential
medicines in PHC facilities. Interventions tackle key issues along the health
commodities supply chain and include: (i) introducing new governance and
accountability mechanisms, (ii) developing new finance and business model for Medical
Stores Department (MSD), (iii) engaging private sector in procurement and distribution,
(iv) implementing quality improvement initiatives for inventory management, and, (v)
using innovative information and communication technology (ICT) to report stock-outs.
Maternal, Neonatal and Child Health: This result area aims to improve the coverage
and quality of MNCH along the continuum of care. Interventions include: (i) ensuring dispensaries and health centers meet Basic Emergency Obstetric and Neonatal Care
(BEmONC) requirements, (ii) expanding Comprehensive Emergency Obstetric and
Neonatal Care (CEmONC) to selected hospitals and health centers, (iii) strengthening
the corresponding satellite blood banks which serve facilities with CEmONC, and, (iv)
extending MNCH services to communities through the use of community health
workers (CHWs) and awareness campaigns. Five regions that are poorly performing on

The development objective of t...he Maternal and Child Health (MCH) and Nutrition Services Support Project for Togo is to increase utilization of selected MCH and nutrition services for pregnant women and children. The project has two components. The first component is improved utilization of malaria and nutrition services to address malaria in pregnancy through the provision of intermittent preventive treatment for malaria in pregnancy (IPTp) with sulfodoxinepyrimethamine (SP) and free long-lasting insecticide nets (LLINs), to pregnant women attending antenatal care (ANC). It has following two sub-components: (i) support for: (a) malaria control in pregnancy through provision of LLINs and SPs as part of the basic package of ANC services; (b) community-based diagnosis and treatment of malaria; and (c) management, supervision, and behavioral changes to ensure effective utilization; and (ii) community- based nutrition services for pregnant women and young children under five years of age. The second component is strengthening health monitoring and evaluation systems; project management. It has following two sub-components: (i) strengthening the health monitoring and evaluation system, particularly the health management information system (HMIS), including the government's capacity to monitor its MCH programs; and (ii) project coordination and implementation support. The Nutrition and Early Childh...ood Development Project seeks to improve growth and development of children under six years of age, in terms of nutrition, health, psycho-social, and cognitive aspects. The project consists of three components. First, integrated community child care package will improve parental awareness on child care and development through parental education, child growth monitoring and promotion, training, and sensitization. Second, community support grants for child development will provide a) community support grants, which finance construction and operation of child care centers, or the production and marketing of weaning foods; and b) innovation grants, which support communities in implementing innovative ideas on improving the lives of children within their communities. Third, national program support for child development and nutrition will support program monitoring and evaluation; preventive program initiatives on micronutrients; early childhood development curriculum development; training; and information, education, and communications (IEC) for children's rights.

The Child Development Project,... aims at implementing the coordinated efforts of the pilot area-based program in Yemen, and improve the health, and nutritional status of children under five, and the educational status of girls in primary schools, specifically, in those districts currently under-served in health, and educational aspects. The components are the following: 1) Community readiness program, to provide training in assessing needs, planning, and social mobilization, to improve the community involvement in project participation. 2) Health, to strengthen the districts' health care system, in addressing relevant diseases, such as respiratory, and childhood diarrheal diseases, in addition to safe-motherhood and immunization. Construction works related to water supply, and sanitation, will be included. 3) Nutrition, to provide counseling, and nutrition education for improving children's nutritional status, and further address growth monitoring, and micro-nutrient supplements. 4) Education, to expand the girls' access to quality primary education, through innovative community-based schools. Women teacher training, and educational materials will be provided. 5) Early Child Development (ECD) pilot, to identify models for future expansion, towards improving access to education, among children in difficult circumstances, supported by studies on child rearing, and ECD interventions.

Project consists out of 3 comp...onents: Component 1: Cash transfers for beneficiary mothers (US$267 million). Component 2: Enhancing local level government capacity (US$32.87 million). Component 3: Monitoring and evaluation (US$3.5 million). COmponent 1 is by far the financially most important component and it is mainly concerned with child nutrition: This
component will finance quarterly cash transfers to eligible households. Eligibility will be
determined by the following two characteristics: (i) household will have to belong to the bottom
two expenditure quintiles,24 and b) such households must have pregnant women and/or mothers
of children below the age of 60 months. There would be four types of co-responsibilities
depending on the household demographic composition (see Table 2 below). Cash transfers will
be given conditional on utilizing the following services: (i) up to 4 antenatal care (ANC) visits by
pregnant beneficiaries; (ii) monthly GMP (Growth Monitoring and
Promotion) of children from 0-24 months; (iii) quarterly GMP for
children from 2 to 5 years of age; (iv) monthly attendance at CNCD awareness sessions by all
eligible mothers. The incentive structure emphasizes the relative importance of GMP among 0 to
24 month old children, and that of CNCD awareness25 among mothers for all children below 5.
There is a natural exit mechanism whereby the maternal benefit will end once the mother gives
birth, and the cash transfer linked to child growth and development will cease once the child
reaches 5 five years of age. Further details regarding program rules including possible exceptions

Sub-component A.3 – Nutritio...n Awareness and Support (Total US$ 3.7 million; IDA US$ 3.7 million): This sub-component would aim to raise awareness, improve attitudes and practices that enhance
nutritional outcomes for targeted beneficiaries in selected project areas and that support beneficiaries in
optimizing their livelihood activities. Increased household income is not necessarily translating into
improved nutritional outcomes and therefore can only partially address the under-, and mal-nutrition
challenges in Bangladesh. Activities proposed under the project would focus on awareness building,
behavioral change for personal hygiene, food preparation, food choices and mainstreaming nutrition
sensitive actions, particularly in selected income generating activities of beneficiaries. The target group for
the nutrition related activities comprises a sub-set of project beneficiaries that are among the core focus
group for nutrition interventions – pregnant and lactating mothers and young children under the age of five.
Behavioral change campaigns and awareness building would be facilitated by SDF using the established
community mechanism, while nutrition support activities would be outsourced to experienced
players/NGOs in the intervention areas. Activities to be covered under the support would be: (i) promoting
better hand washing practices amongst beneficiaries, particularly before food preparation, feeding of babies
and when using sanitation facilities; (ii) promoting better infant and child feeding practices for pregnant
and lactating mothers; and (iii) mainstreaming nutrition sensitive actions into selected income generating
activities. Additional support would be sought from Government institutions such as clinics and line
agencies for continuous community training on nutrition related issues and improved agricultural practices.
It is also expected that the recently negotiated Bank project on Income Support for the Poorest would reach

For nutrition see GHA P145792....IDA54630.crs2 Ghana - Maternal; Child Health and Nutrition Project 2. The World-Bank supported Maternal and Child Health and Nutrition Improvement Project (MCHNP) (Cr No.4563-GH) started implementation in
February 2015. About 23% of project funds have been disbursed by Ghana Health Service (GHS) to the regions to enable service delivery at the
Community-Based Health Planning and Services (CHPS) level. The funds released have facilitated outreach services in the communities, home visits
to follow up on cases, durbars (community level meetings on health-related topics), meetings of community health committees and service delivery in
over 2,400 CHPS communities. The funds have also been used to support capacity building and enhance supportive supervision of the CHPS zones by
Sub District, District and Regional level officials in the country. Community-based Performance Based Financing (cPBF) has been launched in 8 pilot
districts in the Upper East, Upper West, Northern and Volta Regions to test the model of financial motivation as a tool for self-directed service delivery
improvement. An impact evaluation baseline has been concluded and contracts have been issued for service delivery to start.
Procurement and capacity building have been initiated under the project to enhance availability of logistics and personnel. This will include the provision
of vehicles, anthropometrics equipment, record books and the training of all community, Sub District, District and Regional level Health Teams. The

The objective of the Project i...s to improve the utilization of maternal, child health and nutrition services at
the primary level of care in Target Regions. This will be achieved by supporting the government (a)
strengthen supply, (b) strengthen demand, and (c) strengthen supervision and management capacity, for
maternal and child health services, in the poorest regions of Guinea.
The project target regions are two of the poorest in Guinea, Faranah and Labe, which were selected based
on a number of criteria detailed in section A of Annex 2 of this Project Appraisal Document (PAD). Both
regions cover a total of nine out of 34 districts in Guinea: the Labe Region encompasses five districts
(Mali, Koubia, Tougue, Lelouma, Labe) and the Faranah region encompasses four districts (Faranah,
Dinguiraye, Dabola, Kissidougou).
The immediate project beneficiaries are women and children dependent on primary health services for
their needs. By targeting two of the poorest regions in Guinea, and focusing on service delivery at the
primary level of the health system, the project will target the poorest. The Public Expenditure Review
(PER) has shown that health facilities at the lower end of the health system (health posts, and health
centers), are much more likely to serve females, rural residents, the young, the poor, and less educated

The objective of the Project i...s to improve the utilization of maternal, child health and nutrition services at
the primary level of care in Target Regions. This will be achieved by supporting the government (a)
strengthen supply, (b) strengthen demand, and (c) strengthen supervision and management capacity, for
maternal and child health services, in the poorest regions of Guinea.
The project target regions are two of the poorest in Guinea, Faranah and Labe, which were selected based
on a number of criteria detailed in section A of Annex 2 of this Project Appraisal Document (PAD). Both
regions cover a total of nine out of 34 districts in Guinea: the Labe Region encompasses five districts
(Mali, Koubia, Tougue, Lelouma, Labe) and the Faranah region encompasses four districts (Faranah,
Dinguiraye, Dabola, Kissidougou).
The immediate project beneficiaries are women and children dependent on primary health services for
their needs. By targeting two of the poorest regions in Guinea, and focusing on service delivery at the
primary level of the health system, the project will target the poorest. The Public Expenditure Review
(PER) has shown that health facilities at the lower end of the health system (health posts, and health
centers), are much more likely to serve females, rural residents, the young, the poor, and less educated

Component 1: Providing Materna...l and Child Health, Nutrition and Social Services
(US$61 million equivalent IDA Grant; US$20 million HRITF Grant: Total of US$81
million). This Component will finance two sub-components, namely: (i) Performance-based
Maternal and Child Health and Nutrition Service Delivery; and (ii) Results-oriented Family
Support for Poor and Vulnerable Families. Sub-component 1.1: Performance-based Maternal and Child Health and Nutrition
Service Delivery (US$44 million equivalent IDA Grant; US$20 million HRITF Grant - Total
of US$64 million). This sub-component will finance the following activities, namely: (a)
improving the quality and supply of maternal and child health services of selected public health
providers based on the eligibility criteria set forth in the MSPP Operations Manual through
small-scale rehabilitation, equipment, medical supplies, essential health commodities and
training of, and technical assistance to health personnel as well as the preparation of a
communication strategy through the carrying out of works and the provision of goods,
consultants' services and Training; (b) carrying out a program of activities to maintain and strengthen external controls in relation to, including carrying out of, third-party verification of,
the Packages of Maternal and Child Health and Nutrition Services in terms of quantity and
quality through the provision of consultants' services; and (c) using Results-Based Payments in
support of: (i) the delivery of a Package of Maternal and Child Health and Nutrition Services
including: (A) preventive services, such as immunization, micronutrient supplementation,
cholera prevention and promotion of insecticide-treated bed-nets; (B) promotion of health
services, such as increasing prevalence of exclusive breast-feeding and use of family planning;
(C) basic curative services, such as treatment of acute respiratory infections, cholera and other
diarrheal diseases, other childhood illnesses, and tuberculosis; and (D) reproductive health
services, such as family planning, prenatal care, emergency obstetrical care, and post-partum
care; and (ii) the delivery of Packages of Health Service-Related Activities including a program
of monitoring and supervision activities by select departmental health authorities and public
health supervisory units for the delivery of the Packages of Maternal and Child Health and
Nutrition Services. Results-based payment agreements will be signed with public and non public
providers for the delivery of the Packages of Maternal and Child Health and Nutrition Services,
and with departmental health authorities and public health supervisory units for the supervision
and monitoring of service delivery. This sub-component will be implemented by the MSPP. Sub-component 1.2: Results-oriented Family Support for Poor and Vulnerable
Families (US$17 million equivalent IDA Grant). This sub-component will finance delivery of
social services through family support to poor and vulnerable families through: (a) the carrying
out of socio-economic surveys of families for generating the family development plans; (b) the
mapping of available social programs and services to develop the opportunity map and identify
the most needy families; (c) the recruitment and supervision of Kore Fanmi agents and of
municipal teams and verification of results; (d) the provision of Training to the Kore Fanmi
agents and municipal teams; (e) the setting-up of municipal offices; (f) the provision of necessary
supplies to support basic social services at the household level as specified in the FAES
Operations Manual; all through the provision of goods, consultants' services, Operating Costs
and Training; and (g) the provision of Conditional Cash Transfer Grants to Conditional Cash
Transfer Beneficiaries. This sub-component will be implemented by the Fonds d'Assistance
Economique et Sociale (FAES). Component 2: Strengthening the Stewardship and Management Capacity of
Government (US$9 million equivalent IDA Grant). This Component will finance two
subcomponents: (i) Strengthening MSPP's Stewardship and Management Capacity; and (ii)
Strengthening Social Protection Coordination and Management Capacity. Subcomponent 2.1: Strengthening MSPP's Stewardship and Management Capacity
(US$5 million equivalent IDA Grant). This sub-component will support strengthening MSPP's
stewardship and management capacity, increasing the capacity of the departmental health
authorities in supervision and monitoring of health service delivery, conducting surveys and
studies, and preparing a nationwide healthcare waste management strategy and plan, all through
the provision of goods, consultants' services and training. The capacity of the Ministry to support
the various contracting functions in the long-term will be strengthened through technical
assistance and training. These functions include planning, budgeting, public expenditure
tracking, coordinating and overseeing the effective provision of quality services, as well as
medical waste management. Finally, the Project will also finance surveys and studies as
necessary to measure the Project's results. This sub-component will be implemented by the
MSPP.
23. Subcomponent 2.2: Strengthening Social Protection Coordination and Management
Capacity (US$4 million equivalent IDA Grant). This sub-component will support strengthening
the institutional capacity of the Recipient at the central, municipal, and community levels, to
enhance coordination, organization, management, and social service delivery to families, by (a)
supporting the operation of Kore Fanmi and social protection steering and technical committee;
(b) carrying out capacity building activities for central and departmental authorities,
municipalities and major stakeholders at the municipal, departmental and national level in the
delivery of services or programs; (c) expanding the management information system of the Kore
Fanmi; (d) carrying out minor rehabilitation of municipal offices based on eligibility criteria set
forth in the FAES Operations Manual; (e) setting up a unified national beneficiary registry for
the identification and tracking of beneficiaries and social assistance programs; and (f) supporting
FAES for the management, supervision, monitoring and evaluation, procurement, and financial
management of sub-component 1.2 of the Project, through the carrying out of works and the
provision of goods, consultants' services, training and operating costs. This sub-component will

The main project beneficiaries... will be India's pregnant women and children under six
years of age, with a particular focus on 0-3 year old children. In addition, the project will benefit
key stakeholders in the implementation of the ICDS Program and nutrition initiatives, including
the staff of MWCD at the center, state, district and block levels as well as civil society
organizations active in nutrition. Likewise, the project will benefit other relevant ministries and
agencies involved in nutrition at the center and state levels. Lastly, the project will provide direct
support to communities in eight project states by financing the development, testing and

This Component is expected to support the expansion and continued improvements to the district
health management information system (DHIS2); support to train internal Ministry of Health
(MOH) staff in access and utilization of the system is also covered. This system is utilized by the
MOH to report on their health indicators and provides key management information to inform
policy direction and resource allocation.

Component 2: Service Delivery
Comments (optional)
This Component is expected to support the GOL’s aim to strengthen reproductive, maternal, and
child health, and nutrition services. Financing will be provided based on results which are tracked
through a set of DLIs. DLIs are a set of tracer indicators aimed at measuring performance against
service delivery outcomes and health system strengthening actions.

Component 3: Nutrition Social and Behavior Change Communication
Comments (optional)
This component is expected to support the design of a national social and behavior change campaign
(SBCC) which will include determinants of health and nutrition that lie outside of the health sector
such as indoor air pollution, water, and sanitation. It is also expected to finance implementation of
this SBCC in approximately 800 villages located in districts with high number of children with poor
nutrition outcomes. Activities to be supported include technical assistance, and costs related to
designing the campaign, and support for village level sanitation in high priority districts.

Component 4: Management, Monitoring and Evaluation
Comments (optional)
The component is expected to support Program management, including fiduciary tasks and
monitoring and evaluation of the Program. Financing from IDA will cover technical assistance for
Provinces to achieve the DLIs, contracting of a firm to carry out independent verification of the
DLIs, and selected studies.

Component 5: Contingent Emergency Response
Comments (optional)
The objective of the contingent emergency response component with a provisional zero allocation is
to allow for the reallocation of financing in accordance with the IDA Immediate Response
Mechanism in order to provide a rapid response to disaster or emergency events, as needed. This component would finance expenditures on a positive list of goods and/or specific works and services
required for emergency recovery. An Emergency Response Manual (ERM) will apply to this
component, detailing streamlined financial management, procurement, safeguard and any other
