Introduction of Nutrition Serv...ices. It will support introduction of preventive nutrition
services in all 36 districts by scaling up the provision of micronutrients and deworming (see
details in Annex 2) and expanding the delivery of behavior change communication, focusing
on the prevention of malnutrition during pregnancy and in the first two years of life. In 12
districts and urban slums in 9 cities with the highest prevalence of acute malnutrition, therapeutic nutrition services will be delivered, including commodity support micronutrients
and Ready-to-Use Therapeutic Foods. The project will improve the capacity of health service
providers to deliver nutrition interventions at the facility and community level and
coordinating structures to implement multi-sectoral response to address nutrition.
iii) Expanded Coverage of HIV/AIDS Preventive Services. The component will support
expanding coverage of preventive, treatment, and care services for population subgroups
vulnerable to HIV infection [IDUs and Men Who Have Sex with Men (MSM)] in targeted
cities; and support provincial integrated biological and behavioral surveillance. Service
provision will be contracted out through a competitive process complementing the Global

Component 2. Improvement to ac...cessibility of maternal, nutrition and child health services
(US$14 million, including US$12 million IDA and US$2 million MDTFHRI)
39. This component will support several mechanisms whose main purpose is to subsidize
demand for healthcare and nutrition services. These mechanisms are (i) the Universal Health
Insurance Scheme (i.e. Couverture Maladie Universelle or CMU), including the Health
Equity Fund and the national identification system of the poorest; (ii) the national program for
improving demand for nutrition services; and (iii) the maternal health vouchers.
Sub-component 2.1. Support to the implementation of the Universal Health Insurance
scheme (US$7 million)
40. The objectives of this sub-component are to help the Government implementing the
Universal Health Insurance Coverage scheme, called Couverture Maladie Universelle
(CMU). As mentioned earlier, the CMU consists mostly in creating at least one mutuelle (i.e.
community-based health insurance scheme) in each commune of Senegal and in subsidizing
heavily (at a 50% rate) the premiums. An additional mechanism (i.e. Equity Fund) will ensure
that the households identified as poorest will pay neither a premium nor copayments. This
CMU is therefore based on an ambitious strategy of voluntary enrollment in communitybased

Component 2: Results-Based Monitoring, Impact Evaluation, Quality Assurance, Verification, and
Project Management (US$1.0 million equivalent total) which would include support for: (i) capacity
building activities for the Social Fund for Development (SFD), the implementing agency for the
project, in the area of results based financing; (ii) quality assurance of service provision; (iii)
monitoring of compliance with medical waste management; (iv) independent monitoring of project
targets and verification of project outputs; (v) project impact evaluation; and (vi) project

The Integrated Nutrition Proje...ct will initiate development of a national nutrition program whose long-term goal is to improve nutritional status so that malnutrition would eventually cease to be a public health problem. The project will have three components.. The National level nutrition activities component includes four sub-components: 1) program development and institution building, to develop national capacity in nutrition and to promote needed policy research and operation research; 2) information, education and communication (IEC) development to assess current behavioral aspects relevant to nutrition an develop appropriate messages and materials for use in IEC activities through inter-personal methods at the community level and through the mass media at the national level; 3) strengthening of existing nutition activities; and 4) project management, monitoring and evaluation. The community-based nutrition component focuses on growth monitoring and promotion activities with targeted and supervised supplementary feeding at the village level in community nutrition centers, which will serve as a locus for inter-personal IEC and mobilization of community actions to promote nutrition. The inter-sectoral nutrition program development will improve nutrition by emphasizing nutritional aspects of activities in various sectors and by supporting innovative actions with potential nutrition impact.

The project components are to:... (a) strengthen family planning (FP) services delivery; (b) strengthen health services delivery; (c) improve supportive activities to the delivery of FP and health services; and (d) continue women's and nutrition programs. FP service delivery will be strengthened by improving access to FP services, strengthening mother and child health (MCH) services, enhancing clinical service delivery and quality assurance, imparting in-service training of upazila and district staff, constructing and renovating facilities, and marketing contraceptives through the private sector. The project will strengthen health service delivery by increasing the range of maternal and neonatal health care services, strengthening nursing and medical education, introducing medical quality assurance, supporting medical research, strengthening disease prevention and control, developing urban primary health care, continuing and expanding school health programs, improving district and upazila health facilities, and improving the utilization of upazila health centers. The project will assist in strengthening information systems, improving FP and health management, expanding communications programs, supporting nongovernmental organization activities, and developing innovative projects. Finally, the project will assist with the continuation of three women's programs financed under previous projects and will strengthen and develop the National Nutrition Council.

National Nutrition Project

The National Nutrition Project... for Bangladesh will be the first of a series of investments which will support the Government's 15-year vision to extend community nutrition services to the entire country. The goal of the national program is to improve nutritional status to the extent that malnutrition will no longer be a public health problem. The project consists of two components. The first, the Service Component, is comprised of two sub-components: Area-based community nutrition services improve behavioral practices in critical areas such as breastfeeding, timely introduction of solid foods, and increased food intake during pregnancy. It also increases awareness and treatment of malnutrition in the primary care health system and provides food security and income-generating activities to the poorest households in the community. The second sub-component funds a foundation promoting and protecting breastfeeding; funds a program for national micro-nutrient supplements; and provides technical assistance to draft legislation on fortification and food quality standards. The second component, the Project Support and Institutional Development Component, consists of three sub-components: project management and institutional development; monitoring, evaluation, and operations research; and training and behavioral change communications. This component strengthens the management capacities of communities and nongovernmental organizations.

Project consists out of 3 comp...onents: Component 1: Cash transfers for beneficiary mothers (US$267 million). Component 2: Enhancing local level government capacity (US$32.87 million). Component 3: Monitoring and evaluation (US$3.5 million). COmponent 1 is by far the financially most important component and it is mainly concerned with child nutrition: This
component will finance quarterly cash transfers to eligible households. Eligibility will be
determined by the following two characteristics: (i) household will have to belong to the bottom
two expenditure quintiles,24 and b) such households must have pregnant women and/or mothers
of children below the age of 60 months. There would be four types of co-responsibilities
depending on the household demographic composition (see Table 2 below). Cash transfers will
be given conditional on utilizing the following services: (i) up to 4 antenatal care (ANC) visits by
pregnant beneficiaries; (ii) monthly GMP (Growth Monitoring and
Promotion) of children from 0-24 months; (iii) quarterly GMP for
children from 2 to 5 years of age; (iv) monthly attendance at CNCD awareness sessions by all
eligible mothers. The incentive structure emphasizes the relative importance of GMP among 0 to
24 month old children, and that of CNCD awareness25 among mothers for all children below 5.
There is a natural exit mechanism whereby the maternal benefit will end once the mother gives
birth, and the cash transfer linked to child growth and development will cease once the child
reaches 5 five years of age. Further details regarding program rules including possible exceptions

Sub-component A.3 – Nutritio...n Awareness and Support (Total US$ 3.7 million; IDA US$ 3.7 million): This sub-component would aim to raise awareness, improve attitudes and practices that enhance
nutritional outcomes for targeted beneficiaries in selected project areas and that support beneficiaries in
optimizing their livelihood activities. Increased household income is not necessarily translating into
improved nutritional outcomes and therefore can only partially address the under-, and mal-nutrition
challenges in Bangladesh. Activities proposed under the project would focus on awareness building,
behavioral change for personal hygiene, food preparation, food choices and mainstreaming nutrition
sensitive actions, particularly in selected income generating activities of beneficiaries. The target group for
the nutrition related activities comprises a sub-set of project beneficiaries that are among the core focus
group for nutrition interventions – pregnant and lactating mothers and young children under the age of five.
Behavioral change campaigns and awareness building would be facilitated by SDF using the established
community mechanism, while nutrition support activities would be outsourced to experienced
players/NGOs in the intervention areas. Activities to be covered under the support would be: (i) promoting
better hand washing practices amongst beneficiaries, particularly before food preparation, feeding of babies
and when using sanitation facilities; (ii) promoting better infant and child feeding practices for pregnant
and lactating mothers; and (iii) mainstreaming nutrition sensitive actions into selected income generating
activities. Additional support would be sought from Government institutions such as clinics and line
agencies for continuous community training on nutrition related issues and improved agricultural practices.
It is also expected that the recently negotiated Bank project on Income Support for the Poorest would reach

The principal objective of the... Comprehensive Maternal and Child Health (MCH) project is to reduce maternal and child morbidity and mortality in the poorest areas of China. This will be achieved by: 1) improving the quality and effectiveness of MCH care at the primary and county levels; 2) increasing equitable access to and utilization of the basic care; and 3) improving MCH management. The project has four main components: basic health care delivery; health worker's training; management improvement; and a national-level component. Under the basic health care delivery component the project will improve services at the village, township, and county referral levels. The health worker's training component will support in-service training of all health workers concerned with MCH care at the village, township, and county levels in basic MCH, clinical care and management skills. The management improvement component will improve management of comprehensive MCH programs at different levels through training, supervisory support and improved management information. The national level component will oversee the overall management and monitoring of the project, carry out interdepartmental and interprovincial coordination, provide technical support to the provinces, prepare and appraise the second phase of this project and disseminate the project experiences to nonproject areas.

Nutrition services are offered... at the community level to poor households with pregnant women
and children 0-24 months old, addressing thus the first 1,000 days. Nutrition services for lactating
women and children under two include: (i) education and behavior change communication
(nutrition, health and hygiene promotion and hand-washing around critical junctures, e.g. before
eating, after defecation, before feeding a child, etc.), including cooking demonstrations; (ii)
growth monitoring, (iii)personalized counseling; (iv) micronutrient supplements (sprinkles) for
children 6-24 months old; (v) food supplementation during the lean season for all children 6-24
months old; and (vi) accompanied referral to health centers. Nutrition services offered to pregnant
women are (i) support in early identification of pregnancy and promotion of health seeking
behavior (ante-natal visits, use of iron and folic acids, vaccinations, etc.) ; (ii) adapted behavior

change communication (nutrition, health and hygiene promotion and hand-washing); (iii)
monthly follow up of weight gain for women; and (iv) accompanied referral to health centers and
free hemoglobin testing. Additionally, the project supports activities to stimulate early childhood
development for children between 3 and 5 years, through referral to and support of child care

The Health Sector Development ...Program Project for Ethiopia aims to develop a health system which provides comprehensive and integrated primary care services, primarily based at community health level facilities. It focuses on communicable diseases, common nutritional disorders, environmental health and hygiene, reproductive health care, immunization, the treatment and control of basic infectious diseases like upper respiratory tract infections, the control of epidemic diseases like malaria, and the control of sexually transmitted diseases especially HIV/AIDS. The project has eight components. They 1) expand primary health care access; 2) improve the technical quality of primary health care service provision; 3) expand the supply and productivity of health personnel; 4) ensure a regular and safe supply of effective, safe, and affordable high quality drugs while improving prescribing behavior by health providers; 5) improve awareness about personal and environmental hygiene and basic knowledge of common diseases and their causes as well as promote political and community support for health services; 6) transform the health system into a four-tiered system that is linked, equitably distributed, and managed in a decentralized, participatory, and efficient manner; 7) monitor improvements in service delivery, quality, and financial performance and evaluate the impact, effectiveness, and cost-effectiveness of the project's components; and 8) improve financial sustainability.

The objectives of the Particip...atory Health, Population, and Nutrition Project for the Gambia are to improve quality in reproductive health services, in infant and child health services, in nutrition services for women of childbearing age, infants, and children; and in management and implementation of a family health program. There are four components. The first helps improve maternal health services, prevents unwanted pregnancies, and prevents and controls STDs/HIV (sexually transmitted diseases and the human immunodeficiency virus). The second component aids in integrating vertical programs to combat childhood diseases through introducing and implementing the integrated management of childhood illnesses (IMCI) approach. The third component supports nutrition policy formulation and institutional strengthening; as well as community and micronutrient approaches to improve the nutritional status of women and young children. The fourth component supports the management and implementation of a family health program by capacity building and policy development, upgrading and maintaining health infrastructure--rebuilding selected primary health care facilities, establishing a blood supply system, procuring equipment, maintenance support, a local fund initiative fund, and project management. Capacity will be built in the areas of cost recovery; information, education, and communication; monitoring and evaluation; and research application.

The overall objective of the p...roject is to improve health and nutrition outcomes among
women and children by focusing on the least served Regions. The proposed changes will
support the long term objectives of reducing maternal and child mortality and undernutrition and
thereby contribute to the attainment of MDGs 1c, 4 and 5. Thus, the AF will contribute to the
Health, Nutrition and Population Global Practice goal of ending preventable deaths and disability
through Universal Health Coverage. Summary of Proposed Changes
The proposed AF would help finance the costs associated with scaled-up activities to enhance the
impact of the project and new activities included as part of project restructuring in response to a
looming food and nutrition security crisis as a result of poor and erratic rainfall in 2014. The
Additional Financing will:
a. Include issues of food security in the Social and Behavior Change Communication (SBCC)
Strategy and food security indicators in the community RBF contracts under Component 1;
b. Introduce household food and nutrition security interventions in the Baby Friendly
Community Initiative (BFCI) strategy in the current Project Regions (North Bank West, Upper
River and Central River Regions), and scale up the food security-enhanced BFCI and Primary
Health Care (PHC) additionally to North Bank East and Lower River Regions under Component
2;
c. Expand performance-based financing (PBF) for health and nutrition service delivery to the
North Bank East and Lower River Regions under Component 2; d. Replenish the funds allocated to Ebola preparedness and control under the restructuring of
December 2014; and
e. Increase scale and intensity of ongoing capacity building for service delivery and RBF due
to the geographic expansion and expansion of scope of activities under Component 3.
The Project Development Objective (PDO) of the MCNHRP remains unchanged and is to increase
the utilization of community nutrition and primary maternal and child health services in selected
regions in the Recipient’s territory. One outcome indicator, being the same as an intermediatelevel
Core indicator, has been deleted. One new PDO outcome indicator and three new
intermediate outcome indicators have been added to reflect the additional activities aimed at
enhancing household food and nutrition security.
The current project is being implemented in three regions, accounting for one third of the total
population with an expectation of reaching approximately 183,000 children under five and
180,000 women aged 15-49 years, yielding a total of 363,000 direct beneficiaries. The proposed
AF will reach these individuals with the additional food security interventions. In addition, the AF
will reach an additional 114,000 new direct beneficiaries (52,700 children under five and 61,800
