The impact of stroke incidents... is immense: five million people die and another five million are permanently disabled every year due to stroke incidents, and stroke is placed third among reasons for acute death and first among reasons for neurological dysfunction in the western world. Its impact and consequences, however, are even more devastating in developing countries, where according to WHO estimates, death from stroke accounted for 85.5% of stroke deaths worldwide in 2001, and the number of disability-adjusted life years (DALYs), i.e. years of life lost and years lived with disability, was almost seven times higher than in high-income countries. In China, in particular, stroke is the leading cause of death, and it strikes earlier in life than it does in the western world. Its treatment relies to a great extent on the information provided by diagnostic methodologies, which are necessary to guide medical experts in choosing a treatment strategy and to assess its efficiency. This project will build on existing expertise by the project partners in the UK and China towards the development of a portable and low-cost system which can detect the occurrence, and monitor the evolution of stroke and its treatment using microwave technology. The proposed system will use low-power, non-ionizing radio waves (microwaves) to image the occurrence and evolution of stroke in an accurate manner, thereby removing the need for expensive imaging systems (such as CT scans) which would result in significant delays in treatment decisions. This approach can address several clinical needs. For example, the system can be used inside an ambulance to determine the type of stroke much earlier than CT scanners inside a hospital. This is particularly important for ischemic stroke patients (which account for over 80% of total cases), for which early detection is crucial for thrombolytic treatment (i.e. blood thinners that can dissolve clots in blood vessels due to ischemic stroke but would worsen the condition of a blood vessel rupture occurring in hemorrhagic stroke). Moreover, by serving as a point of care diagnostic tool for patients at their homes, the proposed approach can lead to more efficient management of stroke in the post-acute stage, which can improve the potential recovery of the patient. The scanner can therefore revolutionise stroke therapy and recovery in communities where CT or MRI equipment is scarce or not available, such as underdeveloped or rural areas in China and other ODA countries. The project will develop the microwave device relying on academic and industrial UK expertise in microwave imaging algorithms and instrumentation. In addition to offering academic expertise in microwave medical imaging, the project's ODA partners will accelerate the proposed device's pathway to clinical use through a network of hospitals and medical centres and cooperation with over 35 hospitals across 30 cities in China in the planning of clinical trials. Project consists out of 3 comp...onents: Component 1: Cash transfers for beneficiary mothers (US$267 million). Component 2: Enhancing local level government capacity (US$32.87 million). Component 3: Monitoring and evaluation (US$3.5 million). COmponent 1 is by far the financially most important component and it is mainly concerned with child nutrition: This
component will finance quarterly cash transfers to eligible households. Eligibility will be
determined by the following two characteristics: (i) household will have to belong to the bottom
two expenditure quintiles,24 and b) such households must have pregnant women and/or mothers
of children below the age of 60 months. There would be four types of co-responsibilities
depending on the household demographic composition (see Table 2 below). Cash transfers will
be given conditional on utilizing the following services: (i) up to 4 antenatal care (ANC) visits by
pregnant beneficiaries; (ii) monthly GMP (Growth Monitoring and
Promotion) of children from 0-24 months; (iii) quarterly GMP for
children from 2 to 5 years of age; (iv) monthly attendance at CNCD awareness sessions by all
eligible mothers. The incentive structure emphasizes the relative importance of GMP among 0 to
24 month old children, and that of CNCD awareness25 among mothers for all children below 5.
There is a natural exit mechanism whereby the maternal benefit will end once the mother gives
birth, and the cash transfer linked to child growth and development will cease once the child
reaches 5 five years of age. Further details regarding program rules including possible exceptions

Supporting Nutrition Service Delivery (appraisal, US$ 19.3 million, of which US$ 14.0 million from IDA and US$
5.3 million from the Government; actual Bank contribution, US$ 10.96 million). This component aimed to
strengthen community-based nutrition (CBN) and wider health services under the Health Extension Program’s
outreach services, through capacity enhancement of Health Extension Workers (HEWs) and their supervisors, as
well as mobilization of Volunteer Community Health Workers (VCHWs) to support HEWs in nutrition-related
outreach activities. It also planned to provide micronutrients to under-five children and pregnant and lactating
women principally in food-insecure regions with high malnutrition rates. The CBN interventions were designed
initially to target four diverse and highly food insecure regions (Amhara, Oromia, Tigray, and the Southern
Nations, Nationalities, and Peoples’ Region).
Institutional Strengthening and Capacity Building (appraisal, US$ 20.3 million, of which US$ 16.0 million from IDA
and US$ 4.3 million from the Government; actual Bank contribution, US$ 13.10 million). This component aimed to strengthen coordination and capacity for nutrition interventions by setting up a national coordination
mechanism for nutrition; strengthening human resources for nutrition, including researchers and nutrition
managers at various levels; and supporting capacity-building of institutions to implement nutrition interventions. It
also planned to support national advocacy and social mobilization messages on nutrition to build country
ownership, disseminate nutrition messages across various media, and complement practices of HEWs and
VCHWs in promoting appropriate practices. Finally, this component aimed to support operational research,
surveillance, and monitoring on nutrition, as well as overall monitoring and evaluation (M&E) for the National

For nutrition see GHA P145792....IDA54630.crs2 Ghana - Maternal; Child Health and Nutrition Project 2. The World-Bank supported Maternal and Child Health and Nutrition Improvement Project (MCHNP) (Cr No.4563-GH) started implementation in
February 2015. About 23% of project funds have been disbursed by Ghana Health Service (GHS) to the regions to enable service delivery at the
Community-Based Health Planning and Services (CHPS) level. The funds released have facilitated outreach services in the communities, home visits
to follow up on cases, durbars (community level meetings on health-related topics), meetings of community health committees and service delivery in
over 2,400 CHPS communities. The funds have also been used to support capacity building and enhance supportive supervision of the CHPS zones by
Sub District, District and Regional level officials in the country. Community-based Performance Based Financing (cPBF) has been launched in 8 pilot
districts in the Upper East, Upper West, Northern and Volta Regions to test the model of financial motivation as a tool for self-directed service delivery
improvement. An impact evaluation baseline has been concluded and contracts have been issued for service delivery to start.
Procurement and capacity building have been initiated under the project to enhance availability of logistics and personnel. This will include the provision
of vehicles, anthropometrics equipment, record books and the training of all community, Sub District, District and Regional level Health Teams. The

Component 1: Providing Materna...l and Child Health, Nutrition and Social Services
(US$61 million equivalent IDA Grant; US$20 million HRITF Grant: Total of US$81
million). This Component will finance two sub-components, namely: (i) Performance-based
Maternal and Child Health and Nutrition Service Delivery; and (ii) Results-oriented Family
Support for Poor and Vulnerable Families. Sub-component 1.1: Performance-based Maternal and Child Health and Nutrition
Service Delivery (US$44 million equivalent IDA Grant; US$20 million HRITF Grant - Total
of US$64 million). This sub-component will finance the following activities, namely: (a)
improving the quality and supply of maternal and child health services of selected public health
providers based on the eligibility criteria set forth in the MSPP Operations Manual through
small-scale rehabilitation, equipment, medical supplies, essential health commodities and
training of, and technical assistance to health personnel as well as the preparation of a
communication strategy through the carrying out of works and the provision of goods,
consultants' services and Training; (b) carrying out a program of activities to maintain and strengthen external controls in relation to, including carrying out of, third-party verification of,
the Packages of Maternal and Child Health and Nutrition Services in terms of quantity and
quality through the provision of consultants' services; and (c) using Results-Based Payments in
support of: (i) the delivery of a Package of Maternal and Child Health and Nutrition Services
including: (A) preventive services, such as immunization, micronutrient supplementation,
cholera prevention and promotion of insecticide-treated bed-nets; (B) promotion of health
services, such as increasing prevalence of exclusive breast-feeding and use of family planning;
(C) basic curative services, such as treatment of acute respiratory infections, cholera and other
diarrheal diseases, other childhood illnesses, and tuberculosis; and (D) reproductive health
services, such as family planning, prenatal care, emergency obstetrical care, and post-partum
care; and (ii) the delivery of Packages of Health Service-Related Activities including a program
of monitoring and supervision activities by select departmental health authorities and public
health supervisory units for the delivery of the Packages of Maternal and Child Health and
Nutrition Services. Results-based payment agreements will be signed with public and non public
providers for the delivery of the Packages of Maternal and Child Health and Nutrition Services,
and with departmental health authorities and public health supervisory units for the supervision
and monitoring of service delivery. This sub-component will be implemented by the MSPP. Sub-component 1.2: Results-oriented Family Support for Poor and Vulnerable
Families (US$17 million equivalent IDA Grant). This sub-component will finance delivery of
social services through family support to poor and vulnerable families through: (a) the carrying
out of socio-economic surveys of families for generating the family development plans; (b) the
mapping of available social programs and services to develop the opportunity map and identify
the most needy families; (c) the recruitment and supervision of Kore Fanmi agents and of
municipal teams and verification of results; (d) the provision of Training to the Kore Fanmi
agents and municipal teams; (e) the setting-up of municipal offices; (f) the provision of necessary
supplies to support basic social services at the household level as specified in the FAES
Operations Manual; all through the provision of goods, consultants' services, Operating Costs
and Training; and (g) the provision of Conditional Cash Transfer Grants to Conditional Cash
Transfer Beneficiaries. This sub-component will be implemented by the Fonds d'Assistance
Economique et Sociale (FAES). Component 2: Strengthening the Stewardship and Management Capacity of
Government (US$9 million equivalent IDA Grant). This Component will finance two
subcomponents: (i) Strengthening MSPP's Stewardship and Management Capacity; and (ii)
Strengthening Social Protection Coordination and Management Capacity. Subcomponent 2.1: Strengthening MSPP's Stewardship and Management Capacity
(US$5 million equivalent IDA Grant). This sub-component will support strengthening MSPP's
stewardship and management capacity, increasing the capacity of the departmental health
authorities in supervision and monitoring of health service delivery, conducting surveys and
studies, and preparing a nationwide healthcare waste management strategy and plan, all through
the provision of goods, consultants' services and training. The capacity of the Ministry to support
the various contracting functions in the long-term will be strengthened through technical
assistance and training. These functions include planning, budgeting, public expenditure
tracking, coordinating and overseeing the effective provision of quality services, as well as
medical waste management. Finally, the Project will also finance surveys and studies as
necessary to measure the Project's results. This sub-component will be implemented by the
MSPP.
23. Subcomponent 2.2: Strengthening Social Protection Coordination and Management
Capacity (US$4 million equivalent IDA Grant). This sub-component will support strengthening
the institutional capacity of the Recipient at the central, municipal, and community levels, to
enhance coordination, organization, management, and social service delivery to families, by (a)
supporting the operation of Kore Fanmi and social protection steering and technical committee;
(b) carrying out capacity building activities for central and departmental authorities,
municipalities and major stakeholders at the municipal, departmental and national level in the
delivery of services or programs; (c) expanding the management information system of the Kore
Fanmi; (d) carrying out minor rehabilitation of municipal offices based on eligibility criteria set
forth in the FAES Operations Manual; (e) setting up a unified national beneficiary registry for
the identification and tracking of beneficiaries and social assistance programs; and (f) supporting
FAES for the management, supervision, monitoring and evaluation, procurement, and financial
management of sub-component 1.2 of the Project, through the carrying out of works and the
provision of goods, consultants' services, training and operating costs. This sub-component will

Component 1: Preserving Critic...al Education Services (US$23.5 million)
29. This component will preserve critical education services in the targeted vulnerable
regions and benefit approximately 974,300 primary school aged children and 10,000
community teachers in the public primary schools in the five targeted regions—currently
about 6,050 schools. Component 2: Preserving Critical Health Services (US$25.0 million)
30. The main objective of this component will be to preserve the provision of critical
health services—to pregnant women and children under five. Building on the MSPPII, the
proposed operation will expand support to 60 percent of health facilities in targeted areas in
delivering services and will address key supply- and demand-side barriers to access. Component 3: Preserving Critical Nutrition Services (US$10.5 million)
31. The main objective of this component is to increase access to nutrition services in the
community by expanding support to community nutrition sites, community nutrition
