Concept Description
29. The ex...perience gained in the implementation of a sector-wide approach as well as the use of
pooled funding made the health sector the key entity for the multi-sectoral project management.
Tackling health, nutrition and population dynamics will require, among other factors: (i) improved
provision of quality RMNCHN services; (ii) expanded community-level interventions to overcome
demand-side barriers; and (iii) improvements in educational outcomes for boys and girls while
addressing gender gaps. Women’s education is negatively associated with infant mortality and
fertility and positively associated with female labor force participation, which in turn results in
increased opportunity costs of women's time and thus contributes to reducing family size. It is the
combination between improved empowerment of women and better access to RMNCHN services
which will eventually improve health, nutrition and population outcomes.
30. The project is organized around: (i) improving the supply of high quality RMNCHN services;
and (ii) improving the demand for RMNCHN services through SBCC and women's and girls’
empowerment.
31. Component 1: Improving the provision of high quality RMNCHN services. This
component aims to strengthen the delivery of health services for women of reproductive age
(including young women, pregnant women) and children under age five through improvements in
the quantity and quality of services. A gradual shift from financing inputs to paying for results,
making M&E a core function, and improving transparency and accountability in the sector will be
envisaged during the project life through a combination of results-based financing (RBF)
approaches. This component will benefit from complementary efforts of the regional Sahel
Women's Empowerment and Demographic Dividend (SWEDD) Project.
34. The component will also support the establishment of a PBF Technical Unit as well as the
preparation activities identified for the PBF strategy implementation in the sector. This PBF
Technical Unit will be strengthened with procurement, logistical and technical capacity to ensure
fluid implementation of PBF. The component will also support strengthening of the health
information system to improve M&E within the health sector and some initial start-up costs (in the
form of investment units) for material and equipment during the start-up phase to strengthen the
capacity of health facilities to deliver quality services.
35. Component 2: Increasing the demand for and utilization of RMNCHN services. This
component aims to increase demand for and utilization of RMNCHN services by promoting social
and behavior change among the main stakeholders and decision makers and empowering women.
Several synergies and complementaries have been identified between the proposed activities in this
component, the SWEDD Project, and the ongoing Social Safety Nets (SSN) Project and Education
and Skills projects in Niger.
• Sub-component 2.1. Promote Social and Behavioral Change. The sub-component will
support community-level SBCC activities on RMNCHN, population, and development issues using
performance contracts with NGOs and supported by community agents (relais communautaires).
SBCC will be used to increase the demand for RMNCHN services by addressing knowledge, social,
cultural and gender barriers.
• Sub-component 2.2. Reaching opinion leaders. The sub-component aims to expand peer education for behavior change among decision makers at the community level. The sub-component
will support the implementation of initiatives to reach opinion leaders, especially men in the
community and religious and traditional leaders.
• Sub-component 2.3. Women's and Adolescent Girls’ Empowerment. This sub-component
will improve community-level demand for RMNCHN services by empowering women and
adolescent girls with the knowledge and skills needed to make informed use of RMNCHN services
coupled with economic incentives for participation in key initiatives.
36. Component 3: Management, Monitoring and evaluation of the project. The component aims
to support project implementation by strengthening management and supervision. It will strengthen
MoH and MoP capacities in management and coordination through the provision of (i)
administrative, management and fiduciary support, (ii) capacity building at the central, regional and
local levels; (iii) technical support to enhance the design of policies and strategies and (iv)
implement monitoring and evaluation. The component will finance goods, consultants, training, and

Component 2. Improvement to ac...cessibility of maternal, nutrition and child health services
(US$14 million, including US$12 million IDA and US$2 million MDTFHRI)
39. This component will support several mechanisms whose main purpose is to subsidize
demand for healthcare and nutrition services. These mechanisms are (i) the Universal Health
Insurance Scheme (i.e. Couverture Maladie Universelle or CMU), including the Health
Equity Fund and the national identification system of the poorest; (ii) the national program for
improving demand for nutrition services; and (iii) the maternal health vouchers.
Sub-component 2.1. Support to the implementation of the Universal Health Insurance
scheme (US$7 million)
40. The objectives of this sub-component are to help the Government implementing the
Universal Health Insurance Coverage scheme, called Couverture Maladie Universelle
(CMU). As mentioned earlier, the CMU consists mostly in creating at least one mutuelle (i.e.
community-based health insurance scheme) in each commune of Senegal and in subsidizing
heavily (at a 50% rate) the premiums. An additional mechanism (i.e. Equity Fund) will ensure
that the households identified as poorest will pay neither a premium nor copayments. This
CMU is therefore based on an ambitious strategy of voluntary enrollment in communitybased

This PforR operation will supp...ort the Government’s primary health care (PHC) program for the
2015/16 – 2019/20 period, with a strong focus on a key government initiative, the Big Results Now in
Health (BRN in Health).
12. PHC under the Health Sector Strategic Plan IV (including BRN in Health) constitutes the
Government’s program. In the structure of the Tanzania health system, PHC is delivered at the district
level and below, involving district hospitals, health centers, dispensaries and community based health
services under the management of Council Health Management Teams (CHMTs). Over the next five
years, PHC will be guided by the HSSP IV, which will drive major reforms and implementation
including the BRN in Health.
13. The 2015-2018 BRN in Health program aims to accelerate the reduction of maternal and
neonatal mortality through improving performance, governance and accountability in PHC. It was
developed as part of Tanzania’s Development Vision 2025 and has four national key results areas
(NKRAs) as follows:
Performance Management: This result area aims to improve health workers’
performance. Interventions include: (i) a stepwise accreditation scheme for all PHC
facilities in the country (aka “Star Rating” initiative) which has both nation-wide
assessment and a subsequent facility improvement program (including incentives) to
help facilities improve their performances and star ratings, (ii) fiscal decentralization at
the facility level, (iii) performance contracts and targets at individual health worker
levels, and, (iv) social accountability mechanisms.
Human Resources for Health: This result area aims to improve the distribution of
skilled PHC workers especially in nine regions with critical shortages in human
resources for health (i.e. less than national averages). Interventions include: (i)
increasing PHC employment permits for such regions, (ii) engaging the private sector to
provide skilled HRH for public health facilities through PPPs, (iii) redistributing health
care workers within regions, and, (iv) optimizing the pool of new recruits through
“bonding” policy or compulsory attachments.
Health Commodities: This result area aims to improve the availability of essential
medicines in PHC facilities. Interventions tackle key issues along the health
commodities supply chain and include: (i) introducing new governance and
accountability mechanisms, (ii) developing new finance and business model for Medical
Stores Department (MSD), (iii) engaging private sector in procurement and distribution,
(iv) implementing quality improvement initiatives for inventory management, and, (v)
using innovative information and communication technology (ICT) to report stock-outs.
Maternal, Neonatal and Child Health: This result area aims to improve the coverage
and quality of MNCH along the continuum of care. Interventions include: (i) ensuring dispensaries and health centers meet Basic Emergency Obstetric and Neonatal Care
(BEmONC) requirements, (ii) expanding Comprehensive Emergency Obstetric and
Neonatal Care (CEmONC) to selected hospitals and health centers, (iii) strengthening
the corresponding satellite blood banks which serve facilities with CEmONC, and, (iv)
extending MNCH services to communities through the use of community health
workers (CHWs) and awareness campaigns. Five regions that are poorly performing on

The development objective of t...he Maternal and Child Health (MCH) and Nutrition Services Support Project for Togo is to increase utilization of selected MCH and nutrition services for pregnant women and children. The project has two components. The first component is improved utilization of malaria and nutrition services to address malaria in pregnancy through the provision of intermittent preventive treatment for malaria in pregnancy (IPTp) with sulfodoxinepyrimethamine (SP) and free long-lasting insecticide nets (LLINs), to pregnant women attending antenatal care (ANC). It has following two sub-components: (i) support for: (a) malaria control in pregnancy through provision of LLINs and SPs as part of the basic package of ANC services; (b) community-based diagnosis and treatment of malaria; and (c) management, supervision, and behavioral changes to ensure effective utilization; and (ii) community- based nutrition services for pregnant women and young children under five years of age. The second component is strengthening health monitoring and evaluation systems; project management. Sub-component A.3 – Nutritio...n Awareness and Support (Total US$ 3.7 million; IDA US$ 3.7 million): This sub-component would aim to raise awareness, improve attitudes and practices that enhance
nutritional outcomes for targeted beneficiaries in selected project areas and that support beneficiaries in
optimizing their livelihood activities. Increased household income is not necessarily translating into
improved nutritional outcomes and therefore can only partially address the under-, and mal-nutrition
challenges in Bangladesh. Activities proposed under the project would focus on awareness building,
behavioral change for personal hygiene, food preparation, food choices and mainstreaming nutrition
sensitive actions, particularly in selected income generating activities of beneficiaries. The target group for
the nutrition related activities comprises a sub-set of project beneficiaries that are among the core focus
group for nutrition interventions – pregnant and lactating mothers and young children under the age of five.
Behavioral change campaigns and awareness building would be facilitated by SDF using the established
community mechanism, while nutrition support activities would be outsourced to experienced
players/NGOs in the intervention areas. Activities to be covered under the support would be: (i) promoting
better hand washing practices amongst beneficiaries, particularly before food preparation, feeding of babies
and when using sanitation facilities; (ii) promoting better infant and child feeding practices for pregnant
and lactating mothers; and (iii) mainstreaming nutrition sensitive actions into selected income generating
activities. Additional support would be sought from Government institutions such as clinics and line
agencies for continuous community training on nutrition related issues and improved agricultural practices.
It is also expected that the recently negotiated Bank project on Income Support for the Poorest would reach

When CDC’s help is requested... – either by the U.S. or other governments, international agencies, or non-governmental organizations – our public health experts answer the call. Our teams have a broad range of skills in applied epidemiology, health assessments, surveillance activities, water and sanitation, and field investigations.
Our work focuses on 5 main areas:
• Investigations: We provide epidemic investigations, rapid health and nutrition assessments, public health surveillance, and disease prevention and control to monitor, track, and contain outbreaks at the source.
• Planning: We use findings from operational research projects to develop more effective public health interventions that can prevent the spread of disease. We also offer program evaluation and emergency preparedness support.
• Training: We help strengthen technical skills across CDC and U.S. government agencies; other governments; international and private volunteer organizations; and with students to build critical public health capabilities.
• Guidelines: We develop guidance internally and with partners on public health issues and technical areas, such as best practices in tracking outbreak-prone diseases in crowded settings.
• Partnerships: We build and maintain relationships with strategic multinational, international, and non-governmental partners, working together to strengthen global health security and protect people worldwide.
