Moreover, poor mental health disproportionately affects people in Low & Middle Income Countries (LMIC) where there exists also a huge mental health workforce gap. This project focuses on mental health in LMICs, initially benefitting India & Indonesia where the partners are based although the preliminary version of the framework we aim to create will benefit all developing countries. Arguably, mental health is a right & tackling poor mental health is also a means of facilitating sustainable socio-economic development. Global Mental Health aligns with Sustainable Development Goal 3: 'Good Health & Well-Being,' specifically 3.4: 'By 2030, reduce by one third premature mortality from non-communicable diseases through prevention & treatment & promote mental health & well-being. Our ambition is to trigger a step-change in how the research community thinks about where, how & by whom mental health in LMICs can be impacted to benefit people experiencing poor mental health.

Tuberculosis (TB) is one of th...e deadliest diseases known to man. It has killed over 1 billion people in the last 2 centuries and is currently the biggest infectious disease killer globally. In 2016 there were over 10 million newly diagnosed TB cases and 1.7 million people died (worldwide 3 people die from TB every minute!). In some parts of the world, like Sub-Saharan Africa, the disease is out of control. TB most commonly affects the lungs and is transmitted through the inhalation of cough droplets, which enter the host's lung and eventually reach the air sacs (alveoli) where the infection takes root. However, if someone inhales TB bacteria it does not necessarily mean that they will develop active TB disease. In most people (~90 to 95%), the immune system is able to either kill or contain the bacteria before they develop disease. However, in ~5-10% of people, the bacteria multiply leading to TB disease. The immune system is complex with many interacting components. However, how these components work together in the lung to kill the bacteria and prevent disease development is poorly understood. Thus, it remains unclear why some people get the disease while others are protected. This is mainly because most research, up to now, involved animal models and cells from the human blood compartment, which poorly approximate what happens in the human lung. However, several lines of evidence now suggest that a type of white blood cell called a memory T-cell, if 'trained', can rapidly recognise and kill the TB bacteria. New research also suggests that antibodies, once thought to have no role in protection, can interact with other cells to kill TB bacteria. We aim to investigate these specific components and how they can protect against development of disease in the human lung. This will give us clues how to design protective interventions against TB. The best way to eradicate TB is by developing an effective vaccine. Yet the current vaccine used in many countries, BCG, only protects against TB in children and offers little protection in adults, especially in countries where TB is common. About 20 new vaccines are being evaluated but the development process is very long (10 to 15 years) and expensive (about £800 million from start to finish) and most vaccines will fail in the late stages of human testing. Thus, we need a new efficient and more affordable approach, involving small numbers of patients, to choose the best vaccines to move to larger human studies. Another unresolved issue is how best to administer the vaccine. Traditionally, vaccines are given by injection in the skin but inhaling it directly into the lungs may better activate the protective responses against airborne infections like TB. The proposed programme will su...pport a range of activities to:

Regarding Health, Nutrition & disease control in 2015: • During the period under review, two projects have been completed focusing access to low cost quality essential medicines. The recommendations emerging from these projects aim to influence policy levers in the sector of active pharmaceutical ingredient (API) manufacture and mergers and acquisitions in the pharma industry. Both the projects used secondary data combined with in-depth stakeholder consultations. Government of India has constituted a high level Katoch Committee to promote API manufacture in India. The findings from the M&A study counters arguments that an increase in prices of essential medicines projected is attributable to M&As.
• The work stream has reached out to countries like Ethiopia, Ghana, Kenya, Bangladesh and Nepal though its various partners
• An affordable, medical device (providing Phototherapy for Neonatal Jaundice) which KPP supported with seed funding is slowly gaining acceptance in Ethiopia, Ghana and Kenya. 1,168 Brilliance units have been installed across 15 countries, and after a competitive tender process the Government of Kenya now plans to install the device in every public hospital in the country.
• Two knowledge products, five communication briefs, and two achievement reports have been developed. These are in the public domain and have also been disseminated by email to focused stakeholders.
• The Health work stream participated in the 14th World Congress on Public Health in Kolkata over the 11th-15th February 2015. This provided a unique opportunity to disseminate KPP studies to professionals from many countries especially Africa/S.E. Asia. Over 200 public health professionals from countries like Nigeria, South Africa, USA, UK, Bulgaria, Nepal, Ethiopia, China and Taiwan visited the KPP exhibition stall. A presentation was made on the overall impact of Merger and Acquisitions in the pharma industry on access, production and manufacture of medicines in India.
• KPP presented findings from a KPP supported consultation on Artemisinin Resistance (AR) at the International Conference on “Antimicrobial Resistance, Novel Drug Discovery and Development: Challenges and Opportunities” 2nd-3rd March, 2015. This allowed outreach to over 50 participants, including international drug research experts.
• The DFID Health Adviser responsible for this work left early in the year and so DFID technical oversight and strategic direction has been more limited since then.

The Effective Health Care RPC ...will provide high quality reviews to help the global development community accelerate progress in the areas of the MDGs for communicable diseases (malaria, TB, diarrhoea and neglected tropical diseases), organisation and financing of care, nutrition and aspects of food security, child health, maternal health, growth, aspects of HIV, poverty and gender.
There will be four main areas for the results:
i) Rigorous systematic reviews relevant to the MDGs. The RPC will build on its existing research synthesis capacity in Cochrane reviews, broaden the scope, and include organization and financing of care;
ii) Relevant, accessible messages in reviews and dissemination products. The RPC will improve access, build on existing products, diversify for specific target audiences, and use a variety of multimedia approaches and Web tools to maximise uptake and use;
iii) Stronger policy networks that demand and use evidence. The RPC will build links with policy networks through dialogue and products and facilitate the use of evidence in decisions;

Tuberculosis (TB) incidence am...ong ethnic populations in Macedonia is significantly high. The data for Multi-Drug Resistant TB (MDR-TB) in the country is incomplete and data on TB/HIV is largely known especially among high risk groups such as prisoners. The overall goal of the program is to ensure high quality and sustainable Directly Observed Treatment Short-course (DOTS) interventions in the country. The target beneficiaries for this program are all inhabitants of Macedonia that visit the Primary Health Care (PHC) services for respiratory symptoms and populations at high risk for TB like ethnic minorities, prisoners and Injecting Drug Users (IDUs) with special emphasis on MDR-TB and TB/HIV co-infection among them. The program builds on achievements and experiences gained through the implementation of Round 5 TB grant in Macedonia. The current grant will scale-up activities to address MDR-TB, TB/HIV and other challenges with special consideration to vulnerable groups and assure high quality TB control by enhancing the DOTS strategy at all levels of health care.

The Trust’s avoidable blindn...ess programme comprises four initiatives:
1. Trachoma management in Africa (£40.1m) and the Pacific (£2m). This is managed through a partnership with International Coalition for Trachoma Control (ICTC) who appointed Sightsavers to manage the initiative in 6 African countries working to support the elimination of trachoma in Malawi, Kenya and Uganda, and significant reductions in the burden of trachoma in Mozambique, Nigeria, and Tanzania. Trachoma is a neglected tropical disease which affects the eyes, if left untreated, it can lead to blindness. Elimination of trachoma is possible through the adoption of the World Health Organisation’s SAFE strategy (Surgeries, Antibodies, Facial cleanliness and Environmental components). In the Pacific, the Trust it is working with ICTC and the Fred Hollows Foundation to, support elimination in four countries; Fiji, the Solomon Islands, Vanuatu and Kiribati (the Trust also supports work in Australia but not with DFID funds).
2. Diabetic retinopathy (DR). An increase of 60% in DR is predicted in the Commonwealth by 2030 . Diabetes increases the risk of many eye health problems, with DR being the main cause of blindness. The Trust is supporting a partnership between the Public Health Foundation of India (PHFI) and the London School of Hygiene and Tropical Medicine (£4.2m) to improve prevention, detection and treatment of DR in India. The Trust is also supporting the Caribbean Council for the Blind to reduce rates of DR in the Caribbean (£1.4m), and the Fred Hollows Foundation to reduce rates of DR in Bangladesh, Pakistan and the Pacific (£2m).
3. Retinopathy of prematurity (ROP). The major stream of work is in India through a partnership with the Public Health Foundation of India (PHFI) and the London School of Hygiene and Tropical Medicine (£3.5m), who are working with federal and state governments to integrate services to prevent, detect and treat ROP within Special Newborn Care Units. These units are being rolled out at district level with a target of 1000 units operational in 2017. The initiative covers supporting strategy and guideline development, training physicians, new patient record and management systems and the development and testing of more affordable equipment for ROP diagnosis.
4. Strengthening of knowledge, tools and people to improve eye health care across the Commonwealth managed by the International Centre for Eye Health (CEHC), hosted by the London School of Hygiene and Tropical Medicine (£11.5m). The Consortium supports fellowships to increase eye care expertise in low income Commonwealth countries, provides a knowledge platform to facilitate learning and information exchange, and a technology programme to develop, validate and rollout low cost eye health diagnostic and management tools.
