Cervical cancer is a leading c...ause of cancer mortality in Vietnam with over 80% of deaths among women aged over 45 years. In Western countries, regular cervical screening allows cancers to be detected and treated, but cervical screening in Vietnam is rare and less than 5% of the population have ever been screened. This means that cancers are detected later and are often fatal. Human papillomavirus (HPV) causes cervical cancer and is common in younger people in Vietnam. Research into how best introduce a vaccine to prevent future cervical cancers is currently being undertaken. However, little is being done to help women who are too old to be vaccinated and are now at highest risk of developing cancer. We propose to strengthen existing research partnerships between LSHTM and both the Pasteur Institute and Cancer Registry in Ho Chi Minh City (HCMC) in order to pilot an innovative HPV screening method among older Vietnamese women. Our research in the UK has indicated that the majority of invasive cancers could have been detected by sensitive HPV testing compared to very few by cytology (a smear test). This project will pilot HPV testing in two districts of HCMC in 300 women aged 45-64 years. The women testing positive will be invited for repeat testing after 1 year and those still positive will be invited to undergo a therapeutic biopsy with the aim of greatly reducing their future cancer risk. In addition, 100 former sex workers will be identified and also invited to take part. These women are estimated to be at much higher risk of developing cervical cancer than the general population and therefore are an important target group for HPV screening. This pilot project will allow us to assess the response rate and also the attitudes to screening in those that accept their screening invite compared to those who refuse. The cancer registry in HCMC is well organised but poorly resourced. Analysis of the data is required to understand unexpected changes in cancer rates which are likely to be due to migration in and out of the city. At least 50 cervical cancer patients identified by the registry will be interviewed in this pilot study to between understand their characteristics including whether they have moved from a rural area to HCMC.

In many LMICs, poverty correlates with poor health, we are working with partners in LMICs to convene researchers and stakeholders to generate independent, expert health policy advice, based on evidence from research.

Children growing up in many lo...w and middle income country (LMIC) contexts are at high risk of exposure to trauma, such as domestic or community violence, child abuse, or severe injury. Exposure to such adversity has been identified as a major contributor to psychological disorders in children. The negative impacts of childhood trauma exposure have been found to persist into adulthood, with wide ranging individual and societal consequences. Indeed, individuals exposed to childhood trauma are disproportionately represented among the most vulnerable groups in societies worldwide (e.g., the homeless). Despite this robust evidence of the detrimental effects of trauma exposure, there are key limitations to our understanding of this problem. First, the majority of relevant evidence derives from high income countries, rather than from LMICs, where exposure to childhood trauma is substantially more common and many children experience multiple adversities. Consequently, we have limited knowledge of the impact of childhood trauma as experienced in the types of contexts in which the majority of the world's children live. This, in turn, undermines capacity for targeted prevention or intervention in such settings. Second, there is extremely limited understanding of the underlying mechanisms that link early trauma with negative mental health outcomes, particularly considering biological processes. Our understanding of how trauma becomes embedded in the brain and body in children, and how that, in turn, confers risk of multiple psychological disorders, is particularly limited. We plan to address limitations in our understanding of the mental health consequences of trauma in children through capitalizing on and extending a key South African birth cohort, the Drakenstein Child Health Study (DCHS). Childhood trauma is a major mental health related concern in South Africa, particularly due to high levels of interpersonal violence (including high rates of community violence, intimate partner violence and child maltreatment). We propose to conduct an in depth study of the mental health impacts of childhood trauma in this cohort, which will address limitations in our understanding and achieve a major change in knowledge in an area of key importance for LMIC contexts. The DCHS is exceptional in having repeatedly measured child trauma exposure during the first years of life, and having simultaneously completed biological assessments and measures of child mental health at multiple times points. High rates of trauma exposure among children in the DCHS mean that it provides an opportunity to study the impact of childhood trauma which is not afforded by birth cohorts based in high income country contexts, in which very few children are exposed to severe or recurrent trauma. Cambodia is one of the world's... most landmine affected countries with over 64,000 casualties recorded since 1979 and over 25,000 amputees. Added to this there is also a rise in the number of amputations resulting from road traffic accidents. Currently around 10 million people in South East Asia, India and Sri Lanka need but do not have access to prosthetic and orthotic services and there is a deficit of circa 40,000 professionals. This project directly targets end-users (Prosthetists and amputees) in Cambodia with a view to future expansion into the Philippines, Myanmar, Indonesia and Sri Lanka through strategic partnership with Prosthetics and Orthotics NGO Exceed Worldwide and the Exceed Research Network. We have active collaborations with UK (and European) prosthetics manufacturers (Blatchfords, Otto Bock, Össur), NHS prosthetics and rehabilitation services, and the Defence National Rehabilitation Centre Headley Court. Recent developments in medical care have resulted in a surgical preference for through-knee amputation where previously above knee amputation was considered preferable. Through-knee amputations allow the socket for the prosthesis to fit on the stump so that the load through the artificial foot and knee is transmitted directly to the end of the stump, this maintains healthy, normal load through the thigh bone. An above knee amputation does not permit this normal loading, the socket has to transmit the loads all the way to the pelvic bone, partially bypassing the thigh, causing pressure sores, inhibiting the range of motion of the amputated limb, and producing bone fragility in the thigh. Therefore, through-knee amputations result in a reduction in pain, fewer incidences of bone formation within muscle (a highly debilitating complication), the ability to bear significantly higher loads, and maintain bone health. The cost (>£50,000 per device) and maintenance required make modern sockets and powered knee mechanisms designed for through-knee amputees inappropriate for use in low and middle income countries. Current low cost solutions for this provided by the adaptation of Red Cross knee joint prostheses suffer from major limitations such as an inability to be locked in extension, severe compromises on cosmetic appearance resulting in social exclusion, and a very low prosthetic knee joint (due to the long thigh component) producing other functional deficits. In this project we will develop a low-cost through-knee prosthesis the initial concept for which has been developed by the applicants through prior work with partners in Cambodia. This will be developed further to create a pathway to support the translation of future frugal technology projects (projects that are low cost in terms of manufacture and maintenance) and we will populate this frugal technology pathway with a series of follow on prosthetics and orthotics projects for amputees. Lung diseases are a major caus...e of ill health and premature death globally, with a particularly large burden in Africa. Asthma and chronic obstructive pulmonary disease (COPD) are very common, and COPD is the third biggest killer in Africa. African patients develop more severe asthma and COPD, and at a younger age, compared to the rest of the world. Low level of lung function in young adult age is an important risk factor for the development of COPD. Furthermore, low lung function increases the likelihood of early death from all causes as early as the third decade of life. Studies from high-income countries have shown that lung function tracks from school age to old age, and that lung function in early childhood is an important determinant of COPD in adulthood. Childhood asthma and lower respiratory tract infections in early life reduce lung function through childhood. The burden and the type of respiratory infections and asthma, as well as environmental factors which adversely affect development of lung function (such as cigarette smoke, biomass exposure, allergens or psychosocial stressors), are markedly different in Africa compared to the high-income countries. However, despite the high frequency and severity of childhood asthma, high incidence of respiratory infections, and many harmful environmental exposures, to date there are no data on early-life factors associated with poor lung function and its trajectory in Africa. We have shown that there is a scope to intervene in early childhood to improve lung function, and reduce long-term consequences of low lung function in childhood. In order to develop interventions to reduce the risk of low lung function in the needy population in Africa, we have to identify childhood lung function trajectories, and discover their early life environmental determinants, which are specific to this part of the world. This information is crucial to develop novel preventative strategies in this part of the world, which will impact on COPD, and the other adverse consequences of diminished lung function. Our overall aim is to investigate lung function trajectories in African children from birth to 8 years of age, and to identify early-life risk factors associated with low lung function trajectory. We will focus on early-life exposures and respiratory outcomes during childhood, as the trajectory of long-term lung health is established in early life. To achieve this, we will build on the unique South African birth cohort of 1000 mother-child pairs, with detailed measures of infectious diseases and non-infectious exposures through pregnancy and childhood, extensive biobank of samples, and longitudinal measurements of lung function and respiratory diseases from birth to age 5 years. In the course of this project, we will measure lung function and the progression or severity of clinical symptoms through to age 8 years, with ongoing collection of environmental exposures. As the world goes through one ...of the most turbulent periods in its recent history, the health, social & economic consequences associated with 'post-conflict disability' arising through major limb loss (MLL) has gained importance as a significant public health issue. Our preliminary observations and community engagement activities in two post-conflict societies - the Sub-Acholi Region in Northern Uganda and Northern/Eastern provinces in Sri Lanka, have demonstrated this to be an under-appreciated complex and multi-faceted problem characterised by: a: Lack of suitable services capable of addressing the complex needs of the affected populations b:Inability of the affected population to access the existing services due to poor transport links, poverty and ignorance. c: Ad-hoc service development using non-sustainable funding sources at the time (or the immediate aftermath) of the conflict with inadequate characterisation of the needs of the local population. d: Social-cultural factors and belief systems that adversely affect the day-to-day experiences of the victims and prevent their reintegration into the society. Influencing and engaging with future policy making in this field therefore requires an integrated approach involving multi-disciplinary teams. This proposal aims to bring together the 'Humanitarian and Conflict Response Institute (HCRI)' - an institution specialising in global health and 'In Place of War (IPOW)'- an organisation with expertise in conducting cultural programmes in post conflict settings, to focus on the multi-faceted problem of disability associated with MLL in two post-conflict societies - Uganda and Sri Lanka. The project will link HCRI and IPOW with Gulu Medical Faculty (GMF)-established specifically to address the health needs of Northern Uganda, the 'Gulu Referral Orthopaedic Workshop (GROW)' - the only centre providing prosthetic limb services in Gulu district in Northern Uganda, and 'Meththa Foundation UK' (MF)-a charity that pioneered prosthetic services in post-war Sri Lanka using recycled components from the UK and an innovative outreach model for service delivery. The involvement of Geographical Information Scientists (GIS) from University of Manchester (UOM), surgeons/public health officials from Northern Uganda and the Uganda-UK Health Alliance (UUHA) - the official intergovernmental body with a mandate to create sustainable partnerships between the two countries, will strengthen the proposed network. Research output from this network will: a. Evaluate the impact of arts and social theatre in challenging entrenched cultural views and bias associated with MLL b: Highlight the harm caused by ad-hoc service development in conflict environments c: Provide the forum, pilot data, policy options and critical appraisals of delivery systems in low-middle income countries to deal with the rising tide of MLL cost-effectively d: Fabricate and deliver 50 prosthetic limbs using an outreach service delivery model (succ
