The Non-Communicable Disease (...NCD) control pilot in two districts: implementation of the NCD pilot
has started in the two selected districts (Dakshin Kanada and Dhavangiri). Staff, drugs, equipment and
contracts with local medical colleges for diagnostic services are in place. Procurement of drugs and
equipment is ongoing but interim arrangements have been put in place so the pilots can run smoothly while
this is being completed. The focus needs to shift to close monitoring of pilot implementation from both the
district and the state levels. Of particular importance is following up to ensure patients found positive after
screening receive adequate treatment. Also, it is critical to work with the technical support agency to
ensure routine data collection and analysis to understand trends and identify technical and operational
issues that need mid-course correction. A contract has been signed with an information, education and
communication (IEC) agency that will support the behavior change aspect of the pilot.
Investment in primary and maternal health care services (civil works and management): The project has
made progress in civil works contributing to primary and maternal health care services. Of the 336 works
under original credit, 333 works are completed and the remaining three are at final stage of completion. Of
the 59 civil works under the AF, 12 have been completed, and the remaining 47 are at different stage of
implementation and scheduled to be completed by the end of the proposed extension period. The
procurement of consultancy services for innovative/green technology for construction and renovation of
health facilities is completed. Procurement of services for health facility infrastructure assessment and
software for monitoring of civil works have also been completed.
Organizational development (Training & Quality Assurance): The procurement of an agency to conduct
soft-skills training for frontline health workers was completed and training of 10,000 staff across the state
is ongoing. Very good progress has been made to integrate the quality assurance functions started by the
project into the state National Health Mission, allowing sustainability post project completion. The project
continues to make progress in supporting the National Accreditation Board for Hospitals and
Healthcare Providers (NABH) accreditation of selected hospitals. Two hospitals have completed their
final assessment for full accreditation and four more are expected to complete it in the next few months.
An additional 30 hospitals (secondary and primary health facilities) are being supported towards entry
level accreditation. Road Safety: Implementation progress of the agreed activities under this component has improved. The
needs assessments and baseline surveys were completed for the three pilot districts, the Essential Standards
for Trauma Care for various levels of hospitals have been completed, training for 2,307 staff on advanced
and basic trauma care training has started, a paper-based injury information system is under
implementation and the blood bank networking process is ongoing. The equipment for emergency and
trauma care services identified during the needs assessment for 25 Taluk and district level Hospitals is
under procurement.
Health insurance: The scheme providing tertiary care coverage for people below the poverty line
continues to be implemented with significant success. The Suvarna Arogya Suraksha Trust (SAST) has
achieved all the milestones linked to the original credit, four out of seven milestones under the AF are
achieved. Several of the activities for the remaining AF milestones have been completed, with the costing
study also now ongoing and expected to be completed in the next two months. Milestones consisting of an
assessment of the frequency, geographic distribution, injury severity, insurance coverage and other

I. Improvement in Essential Re...productive and Child Health (RCH) Services: This component would
include (a) several activities which would be implemented by the Ministry o f Health and Family Welfare
(MOHFW), including procurement o f essential pharmaceuticals, medical supplies and equipment , inputs
for improving delivery o f routine immunization services, behavior change communication, training, and
innovations to enhance access such as publidprivate partnerships, and demand side financing; (b) all
innovative and developmental RCH related activities which are incorporated in the State Program
Implementation Plans (SPIPs), and which would be implemented by the states and territories. These
activities would be supported through the transfer of “flexible funds” from the GO1 to the states. This
sub-component represents the element o f creativity and flexibility the program would have at the state
level to improve access to care.
11. Technical Assistance. Monitoring; and Evaluation: This component would be implemented by the
MOHFW in close cooperation with the states and would comprise two sub-components: (a) technical
assistance to the states during implementation of the project. A major focus of this technical assistance
will be to address planning capacity in the Empowered Action Group (EAG) and North Eastern states,
Jammu and Kashmir and Himachal Pradesh; and (b) reliable Monitoring & Evaluation arrangements to
provide baseline values and track progress towards achievement o f the targets for the project’s indicators.
The information would be obtained through the district-based Reproductive and Child Health Rapid
Household Survey, complemented with data from service statistics, community consultations and
independent studies.

Overall progress toward projec...t development objective remains Satisfactory. The project is implemented in all 52 Local Government Areas (LGAs) of
the three states - covering 1470 primary and secondary health facilities. There is significant improvement in service utilization particularly for skilled
delivery, family planning, and immunization. Quality of care has also improved over the last time of reporting. The Performance based financing (PBF)
component of the project is currently implemented in 26 LGAs. Compared with the last reporting, outpatient consultation, normal skilled delivery,
focused antenatal care, completely vaccinated child and family planning utilization show steady improvement both at the primary and secondary
health care level. In terms of quality of care, there was improvement in general in the three states. However, some the team in agreement is working
with the National Primary Health Care Development Agency (NPHCDA) to introduce quality improvement plan and management for some health
facilities in four (4) LGAs per state using the Safecare model. The progress in the decentralized facility funding (DFF) arm is also encouraging in most

Strategic Health Development Plan 2010 to 2015. It gives renewed priority to a package of high
impact, evidence-based, cost-effective health interventions known as the six pillars: (i) maternal,
newborn and child health (Antenatal,
obstetric, &
post-natal
care; Family
planning;
Deploy
midwives;
VVF
prevention); (ii) childhood essential medicines and increasing treatment of
important childhood diseases (Community
treatment of
malaria,
pneumonia,
diarrhea); (iii) improving child nutrition (Growth
Monitoring &
Promotion;
Treatment of
acutely
malnourished
children;
Micronutrient
supplementation); (iv) immunization (Routine
Childhood
Immunization;
Tetanus toxoid
for mothers;
polio eradication); (v) malaria
control (ITN
distribution;
diagnosis &
treatment
with ACTs); and (vi) the Elimination of Mother to Child Transmission (EMTCT) of HIV. The
objective is to dramatically improve the coverage of these interventions that currently suffer
from poor access and utilization. In addition, to its six “pillars” the SOML Program also includes
two “enablers”: (i) promoting innovation and the use of information and communications

Building on decades of USAIDs ...work procuring health commodities and strengthening supply chains, the Global Health Supply Chain-Procurement and Supply Management project is working to transform global and national supply chains for health commodities. The project integrates two former USAID programs into one efficient supply chain that serves many of the worlds most vulnerable and difficult-to-reach communities. The project is designed to meet todays critical global health challenges eliminating HIV and AIDS, providing universal malaria coverage, and helping women meet their family planning and reproductive health needs. By bringing together advanced technical solutions, a team of highly qualified experts, and proven commercial processes and principles, USAID Global Health Supply Chain works to reduce costs and increase efficiencies in global and national supply chains. In addition to optimizing supply chains, the project is strengthening national supply chain systems and fostering collaboration among supply chain stakeholders worldwide.

This PforR operation will supp...ort the Government’s primary health care (PHC) program for the
2015/16 – 2019/20 period, with a strong focus on a key government initiative, the Big Results Now in
Health (BRN in Health).
12. PHC under the Health Sector Strategic Plan IV (including BRN in Health) constitutes the
Government’s program. In the structure of the Tanzania health system, PHC is delivered at the district
level and below, involving district hospitals, health centers, dispensaries and community based health
services under the management of Council Health Management Teams (CHMTs). Over the next five
years, PHC will be guided by the HSSP IV, which will drive major reforms and implementation
including the BRN in Health.
13. The 2015-2018 BRN in Health program aims to accelerate the reduction of maternal and
neonatal mortality through improving performance, governance and accountability in PHC. It was
developed as part of Tanzania’s Development Vision 2025 and has four national key results areas
(NKRAs) as follows:
Performance Management: This result area aims to improve health workers’
performance. Interventions include: (i) a stepwise accreditation scheme for all PHC
facilities in the country (aka “Star Rating” initiative) which has both nation-wide
assessment and a subsequent facility improvement program (including incentives) to
help facilities improve their performances and star ratings, (ii) fiscal decentralization at
the facility level, (iii) performance contracts and targets at individual health worker
levels, and, (iv) social accountability mechanisms.
Human Resources for Health: This result area aims to improve the distribution of
skilled PHC workers especially in nine regions with critical shortages in human
resources for health (i.e. less than national averages). Interventions include: (i)
increasing PHC employment permits for such regions, (ii) engaging the private sector to
provide skilled HRH for public health facilities through PPPs, (iii) redistributing health
care workers within regions, and, (iv) optimizing the pool of new recruits through
“bonding” policy or compulsory attachments.
Health Commodities: This result area aims to improve the availability of essential
medicines in PHC facilities. Interventions tackle key issues along the health
commodities supply chain and include: (i) introducing new governance and
accountability mechanisms, (ii) developing new finance and business model for Medical
Stores Department (MSD), (iii) engaging private sector in procurement and distribution,
(iv) implementing quality improvement initiatives for inventory management, and, (v)
using innovative information and communication technology (ICT) to report stock-outs.
Maternal, Neonatal and Child Health: This result area aims to improve the coverage
and quality of MNCH along the continuum of care. Interventions include: (i) ensuring dispensaries and health centers meet Basic Emergency Obstetric and Neonatal Care
(BEmONC) requirements, (ii) expanding Comprehensive Emergency Obstetric and
Neonatal Care (CEmONC) to selected hospitals and health centers, (iii) strengthening
the corresponding satellite blood banks which serve facilities with CEmONC, and, (iv)
extending MNCH services to communities through the use of community health
workers (CHWs) and awareness campaigns. Five regions that are poorly performing on
