The Health Sector Development ...Program Project for Ethiopia aims to develop a health system which provides comprehensive and integrated primary care services, primarily based at community health level facilities. It focuses on communicable diseases, common nutritional disorders, environmental health and hygiene, reproductive health care, immunization, the treatment and control of basic infectious diseases like upper respiratory tract infections, the control of epidemic diseases like malaria, and the control of sexually transmitted diseases especially HIV/AIDS. The project has eight components. They 1) expand primary health care access; 2) improve the technical quality of primary health care service provision; 3) expand the supply and productivity of health personnel; 4) ensure a regular and safe supply of effective, safe, and affordable high quality drugs while improving prescribing behavior by health providers; 5) improve awareness about personal and environmental hygiene and basic knowledge of common diseases and their causes as well as promote political and community support for health services; 6) transform the health system into a four-tiered system that is linked, equitably distributed, and managed in a decentralized, participatory, and efficient manner; 7) monitor improvements in service delivery, quality, and financial performance and evaluate the impact, effectiveness, and cost-effectiveness of the project's components; and 8) improve financial sustainability.

The objectives of the Particip...atory Health, Population, and Nutrition Project for the Gambia are to improve quality in reproductive health services, in infant and child health services, in nutrition services for women of childbearing age, infants, and children; and in management and implementation of a family health program. There are four components. The first helps improve maternal health services, prevents unwanted pregnancies, and prevents and controls STDs/HIV (sexually transmitted diseases and the human immunodeficiency virus). The second component aids in integrating vertical programs to combat childhood diseases through introducing and implementing the integrated management of childhood illnesses (IMCI) approach. The third component supports nutrition policy formulation and institutional strengthening; as well as community and micronutrient approaches to improve the nutritional status of women and young children. The fourth component supports the management and implementation of a family health program by capacity building and policy development, upgrading and maintaining health infrastructure--rebuilding selected primary health care facilities, establishing a blood supply system, procuring equipment, maintenance support, a local fund initiative fund, and project management. Capacity will be built in the areas of cost recovery; information, education, and communication; monitoring and evaluation; and research application.

The project's specific objectives are to: 1) reorient the health system towards cost-effective public health interventions like preventive and primary health care, assuring, in particular, better quality care for women and children, and focusing the Ministry of Health's (MOH) activities on regulation of public and private providers; 2) rehabilitate selected facilities and equipment that were down-sized and dedicated to cost-effective provision of maternal, perinatal and pediatric care; 3) develop human resources to support health reform, through continuing education for public health doctors, family practitioners, nurses, health administrators, and other personnel; and 4) support the development of an independent health fund with regional branches, in order to finance a package of basic clinical services in a cost-effective and equitable manner. The project consists of the following components: 1) health system reorientation, which includes: (a) modernization of public health services, health promotion and disease prevention; (b) healthy children and safe motherhood; and (c) realignment of MOH; 2) rehabilitation and maintenance of health facilities and equipment; 3) development of health human resources; 4) modernization of the health financing system; and 5) project administration.

The Primary Health Care Develo...pment Project aims to improve the coverage and utilization of quality primary health care (PHC) based on the model of family medicine/general practice, with an emphasis on reaching the poor and disadvantages. The project is an integral part of the Government of Georgia's health sector strategy and builds upon the investments in preventive health care supported through the Bank-funded First Health Project. The project consists of three components. First, the PHC service delivery component will support a two-phased development of PHC services in urban and rural areas through rehabilitation of facilities and provision of basic medical and office equipment. It will establish PHC clinics and referral laboratories, conduct a PHC referral pilot, and enhance community-based information, education, and communication. Second, the institutional development component will support capacity building and institutional development training, policy framework, and regulatory environment of PHC. It will also support the management of PHC services through an integrated health management information system. Third, the project management support component will support project implementation.

The three project components, ...with appraisal and final costs, were :
1. Improve Awareness of Population Issues and Promote Adequate Reproductive Health Practices (US$4.5
million/US$1.9 million). (i) Increased awareness of population and development issues through training
workshops and dissemination of messages; (ii) Increased promotion of positive and safer behavior with
information, education and communication activities for youth, and support for behavior change of young
parents through training programs; and (iii) Improved equity and access to reproductive health services for
vulnerable groups including behavior change toward female genital mutilation, using a flexible funding
mechanism for NGOs (Population Support Fund) to undertake grass roots activities .
2. Improve Quality and Utilization of Priority Reproductive and Child Health Programs (US$6.0 million/US$8.0
million) (i) Rationalize management of support for reproductive and child health services, providing
management training for national programs to execute decentralized priority reproductive health programs plus
immunization. Included construction and equipping of two regional pharmaceutical warehouses, contraceptive
supplies, and rehabilitation of three prefectoral directorates of health . (ii) Provision of contraceptives,
STD/HIV/AIDS and family planning services that are culturally acceptable and medically safe, through
constructing and equipping two model clinics and ten health posts, and rehabilitating 15 health centers together
with training, supplies, and rehabilitation of the national referral laboratory for HIV /AIDS, plus equipment for
regional laboratories for screening of HIV /AIDS; and (iii) Support for safe motherhood and child health services
through training of obstetricians and their teams, with construction of a maternity hospital, rehabilitation of
operating theaters, intensive care units and post operative wards in eight prefectures, and 10 maternity waiting
shelters, together with vehicles, drugs, and obstetrical kits . During implementation it was agreed to reduce the
number of health posts from ten to four; construct three additional health clinics for US$ 1.5 million; and
purchase 28 vehicles instead of 14.
3. Strengthening Institutional Capacity to Manage and Coordinate Population and Reproductive Health Programs (US$1.5 million/US$2.5 million). (i) Strengthen institutional capacities to better manage population
programs through data collection and dissemination at policy workshops; and (ii) National coordination of

Components:
Two components wer...e supported under the project (planned commitments at appraisal and restructuring, and
actual commitments, are detailed in parentheses ):
1. Strengthening Health Care Services (Appraisal US$ 18.99 million, Actual US$ 17.62 million), with three sub
components aimed at: (a) improving quality of care through health personnel training, better standards and
referral systems, performance agreements between the Ministry of Health (MOH) and health centers and district
hospitals, and contracts with private clinics and NGOs ; (b) increasing demand for health services through
community mobilization activities; and (c) expanding GTZ-tested and evaluated quality assurance activities to
the project's targeted 18 districts. At the 2012 restructuring, sub component (c) was dropped.
2. Institutional Strengthening (Appraisal US$ 8.37 million, Actual US$ 5.91 million). Activities supported under
this component mainly aimed at increased coordination and better monitoring and evaluation within the MOH . In
addition, this component was to fund equipment at the Reproductive Health Training and Research Center in
Conakry and to support the improvement of medical waste management at the hospital, health center and health
post levels. Original sub components (as listed in the LA) included:
(a) Integration of maternal mortality into the health information system .
(b) Support to implementation of a Medical Waste Management Plan (MWMP).
(c) Support to the Reproductive Health Training and Research Center .
(d) Development of a national health insurance scheme .
(e) Carrying out of an annual tracking survey of health expenditures .
(f) Strengthening the leadership and policy -making capacity of the MOH and health sector reform .
(g) Strengthening the Directorate for Finance and Administration (DAAF) of the MOH and its decentralized
units in financial, procurement, and contract management .
(h) Project management and coordination.
At the 2012 restructuring, subcomponents (d), (e) and (f) were dropped, and subcomponents (g) and (h) were

Health System Reform Project

The Health System Reform Proje...ct aims to extend, and improve health care coverage, and quality, through implementation of the country's health reform strategy. The project components will: 1) develop strategies, programs, and systems needed to achieve regulatory, and institutional development, resource allocation objectives, financial sustainability, and health service delivery. Local and foreign technical assistance will be financed, as will information systems, equipment, and training. Monitoring and evaluation should support innovative health delivery models for the government to build on, and fine tune results, and, will assess as well the impact of project activities on indigenous populations. In addition, assistance to strengthen HIV/AIDS/STI surveillance systems, and improvements in blood banks will also be provided; 2) establish the linkage between policy objectives identified in the Policy Activity Schedule (PAS), and implementation of investments and innovative management changes. To this end, subprojects will extend coverage for the Honduran Social Security Institute and the Ministry of Health, focusing support on the expansion of integrated care models. Evaluation of the PAS will be carried out through three policy program reviews, to monitor progress, and assess compliance with policy changes; and, 3) finance project management and administration costs, namely technical assistance and consultants, office equipment and computerization, supervision and advisory services.

The objectives of the Tamil Na...du Integrated Nutrition Project are to: (a) reduce severe malnutrition among children 6-36 months in Tamil Nadu Integrated Nutrition Project (TINP) I areas; (b) increase the proportion of children 6-36 months in normal or mild malnutrition status in project areas; and (c) contribute to a reduction in the infant mortality rate to 55 per 1,000 births, and to a reduction in the incidence of low birth weight from present levels. The project has three components. The service delivery component will finance construction, furnishing and equipping of village nutrition centers; increased training for health and nutrition field workers and staff; supervisory vehicles; incremental maternal and child health drugs and medications; therapeutic nutrition supplements for malnourished younger children and at-risk pregnant and nursing women and incremental salary and other operating costs. The communications and community participation component will finance production and distribution of print and audio-visual materials for worker and community use. It would also fund training for community support groups. The project management and evaluation component will finance baseline nutrition surveys of project areas, mid-term and final project impact and process evaluations; vehicles, equipment, training and incremental operating costs of the project monitoring and evaluation system.

The project will support the G...overnment's goal of improving the efficiency and effectiveness by which family welfare services are delivered in the States. The project will will be located in the states of Bihar, Gujarat, Haryana, Jammu, Kashmir, and Punjab. Specific objectives include : a) expanding the supply of family welfare services; b) increasing the demand for these services; c) improving the quality of services being delivered; and d) improving the efficiency by which the program is managed and operated. The project will consist of the following components : 1) upgrading and expanding the number of primary health centers and subcenters; increasing the medicine supplies, family planning supplies and equipment for maternal and child survival strategies; 2) expansion of the Social Marketing of Contraceptives, increasing the involvement of Private Voluntary Organizations in the Family Welfare Program, and strengthening the Information, Education and Communication efforts in each of these states; 3) improving the quality of family welfare services by upgrading the training of family welfare workers; and 4) improving management through the strengthening of each State's Directorate of Health and Family Welfare.

The project will include the f...ollowing components: increasing the supply of family welfare services to slum populations through improvements in outreach services using volunteer female health workers recruited from the slum communitities, and the upgrading of existing and construction of new health facilities; improving the quality of family welfare services provided to slum populations, by upgrading the supervisory, managerial, technical and interpersonal skills of all levels of new and existing medical and para-medical workers and increasing the availability of drugs, medicines and other health supplies; increasing the demand for family welfare services through expanded information, education and communication activities and increased participation of the community in the preparation and implementation of various project activities; improving the management and administration of the municipal Health Departments through appropriate upgrading of project supervision, management information systems, information, education, and communication functions, as well as integrating and/or strengthening co-ordination of health services; innovation schemes which cover a range of additional services including supplementary nutrition, creche programs, environmental sanitation drives, female education and skill training, and preparation of future projects.

The project's objectives are t...o improve the nutrition and health status of children under 6 years of age, with special emphasis on those 0-3 years old, and pregnant and nursing women. It will also help to improve the capacity of the Integrated Child Development Services (ICDS) to deliver services, including to tribal people, over the longer term. The project will comprise the following components: (a) service delivery, to increase the range, coverage and quality of nutrition and health services to target groups through improvements in the design and implementation of software systems, training for health and nutrition workers, provision of health referral services, and increasing the availability of medicines and equipment for maternal and child health; (b) communications, to provide innovative approaches and new messages to increase demand for the full range of project services and provide health and nutrition education; (c) community mobilization, to promote greater community ownership of ICDS services and involvement in meeting program objectives of better health and nutrition and women's and adolescent girls' development schemes; and (d) project management, monitoring and evaluation, to manage, monitor and evaluate the project and conduct operations research to test innovative activities and improve aspects of project design. The project will also include actions to strengthen ICDS among tribal populations.

The Integrated Child Developme...nt Services Project focuses on 301 tribal or otherwise disadvantaged blocks in Andra Andhra Pradesh and Orissa. Specific objectives are to reduce severe malnutrition by 50% among children 6 - 36 months, increase the proportion of children in normal or only mild malnourishment status, and to contribute to reductions in infant mortality and the incidence of low birth weight. The project consists of four components: A. Service delivery finances construction, furnishing and equipping of village child development centers and block offices; increased training for Integrated Child Development Services (ICDS) and health workers and supervisors; incremental drugs and medicines; vehicles for supervision, and incremental salary and other operating costs. B. Communications finances production and distribution of audio-visual materials, local consultancies for message design and development, training, surveys and incremental salary and other operating costs. C. Community mobilization finances support to village women's organizations and special educational programs for women and adolescent girls, including incremental worker honoraria and other operating costs. D. Project management and evaluation finances furnishing and equipping of a project management office in each state, its operating costs, local consultancies, surveys and operations research, including the costs of therapeutic nutrition supplements under optional delivery arrangements.

The Family Welfare project wil...l strengthen and improve the functioning of the Government of India's Family Welfare (FW) program in Assam, Rahasthan and Karnataka, with the objective of thereby lowering current levels of fertility and maternal and childhood mortality in the three states. The FW program will be strengthened through five components: (1) strengthening of FW service delivery, including extension and upgrading of infrastructure through construction of sub-centers and Primary Health Centers to serve as first referral units for obstetric emergencies, and strengthening of outreach and community linkages through setting up of mobile clinics and establishment of community linkages through setting up community volunteer networks; (2) rationalization of training institutions and planning, improvement of program logistics, and promotion of Non-Governmental Organization and private sector involvement; (3) strengthening of demand generation activities through improved information, education, and communication planning and activities; (4) strengthening of program management and implementation capacity; (5) a fund for innovative schemes aimed at improving service quality, and for preparation of FW investment proposals for the heavily tribal North-Eastern states.

The Orissa Health Systems Deve...lopment Project aims to assist the government of Orissa to 1) improve efficiency in allocating and using health resources; and 2) improve the performance of the health care system by improving quality, effectiveness, and coverage at the referral level and select coverage at the community level. There are three components. The first, Management Development and Institutional Strengthening, strengthens planning, management, and implementation capacity at the state, district, and facility levels; and strengthens the health management information system (HMIS) and the capacity for surveillance of major communicable diseases. The second, Improving Service Quality, Access, and Effectiveness at the Secondary Level, upgrades area, sub-divisional, and district hospitals; and improves effectiveness of clinical, managerial, and support services at the facility level. The third component, Improving Access to Basic Health Services at the Community Level, selectively upgrades community health centers; promotes health services for tribal areas and disadvantaged groups; and improves referral mechanisms and strengthens linkages between the different tiers of the health care system.

The Rajasthan Health Systems D...evelopment Project for India will assist Rajasthan in improving the health status of it s population, in particular the poor and underserved population. Specifically, the project will increase access to and improve the quality of health care. The project has three main components, one of which mainly focuses on the poor and tribal population. Component 1 improves the state's institutional capacity for health policy development and planning by establishing a Strategic Planning Cell that focuses on designing and implementing strategies for public-private partnership, and developing contracting mechanisms and carrying out a diagnostic assessment, and designing a regulatory framework. The component also improves the health management information system, and supports a variety of training activities on health management, clinical aspects, waste management, equipment maintenance and behavior change communication. Component 2 physically renovates and upgrades district hospitals and health centers, implements a health care waste management system, establishes an institutional framework, trains staff and workers, improves the quality of clinical services, and strengthens the current referral system. Component 3 supports interventions to improve access to health care among disadvantaged populations, especially the tribal population and these households below the poverty line. Specifically, it carries out information, education, and communication campaigns; targets a new strategy aimed at improving health services for tribal and poor populations that focuses on improved service delivery, health camps, non-financial incentives to recruit medical personnel, and incorporates tribal medicine; and uses innovative mechanisms to reduce financial barriers to health care.

The Uttar Pradesh Health Syste...ms Developments Project aims at establishing an appropriately managed health system, through the delivery of effective services, stemming from policy reform, institutional, and human resources development, in addition to investments in health services. The two main components call for: 1) developing a strategic management capacity, through formulation, and review of health systems performance, and the establishment of a drug policy, public-private partnerships, and manpower activities, as well as pursuing policy reform analysis, and research in the health sector. In addition, accurate health management information systems will be developed, and, managerial appraisal systems implemented to improve actions concerning individual performance, in addition to conducting management training to enable critical project interventions in changing management practices. The project management unit capacity will be strengthened to coordinate and implement project issues; and, 2) improving the quality of clinical practice in public health services, and of access to health services. Quality assurance systems will be introduced in an effort to improve skills, and monitoring practices will ensure adoption of best clinical practices through systematic review of medical, and technical records. Programs and services will focus on disease prevention, and health needs. Both components include the financing of civil works, equipment, and medical supplies, as well as training, and consulting work as needed.

I. Improvement in Essential Re...productive and Child Health (RCH) Services: This component would
include (a) several activities which would be implemented by the Ministry o f Health and Family Welfare
(MOHFW), including procurement o f essential pharmaceuticals, medical supplies and equipment , inputs
for improving delivery o f routine immunization services, behavior change communication, training, and
innovations to enhance access such as publidprivate partnerships, and demand side financing; (b) all
innovative and developmental RCH related activities which are incorporated in the State Program
Implementation Plans (SPIPs), and which would be implemented by the states and territories. These
activities would be supported through the transfer of “flexible funds” from the GO1 to the states. This
sub-component represents the element o f creativity and flexibility the program would have at the state
level to improve access to care.
11. Technical Assistance. Monitoring; and Evaluation: This component would be implemented by the
MOHFW in close cooperation with the states and would comprise two sub-components: (a) technical
assistance to the states during implementation of the project. A major focus of this technical assistance
will be to address planning capacity in the Empowered Action Group (EAG) and North Eastern states,
Jammu and Kashmir and Himachal Pradesh; and (b) reliable Monitoring & Evaluation arrangements to
provide baseline values and track progress towards achievement o f the targets for the project’s indicators.
The information would be obtained through the district-based Reproductive and Child Health Rapid
Household Survey, complemented with data from service statistics, community consultations and
independent studies.

Health Project

The objectives of the Health S...ector Reform Project are the following: 1) reform and improvement of the health care system in Kyrgyz Republic; 2) improvement of the health status of the population; 3) improvement of the clinical effectiveness of the service delivery system; 4) improvement of the economic efficiency of the delivery system; and 5) extension of access of appropriate care, enhancement of the quality of care, and assurance of the long-run financial viability of the system. The project consists of the following four major components: 1) primary health care including: (a) strengthening of women's reproductive health management; (b) acute respiratory infection and diarrheal disease control; and (c) tuberculosis control; 2) facilities rehabilitation; 3) medical care provider payment; and 4) pharmaceutical management, which will support the following: (a) national drug regulatory organization; (b) essential drugs program; (c) procurement of essential drugs; and (d) national drug policy.

Health Sector Reform 2 Project
|
The Second Health Sector Refor...m Project will improve performance, and long-term financial viability of the health system in the Kyrgyz Republic, by adjusting the delivery system to available means, and, focusing on health risks, and diseases. Project components will: 1) support strategic developments, to reduce inappropriate utilization of hospital services, by strengthening primary health care. In addition, it will contribute to rationalizing, and modernizing the provision of hospital services, through buildings rehabilitation, and equipment supply, including management of physical, and human resources; 2) continue the development of sustainable health care financing systems, and, develop a health information system, to provide quality data for monitoring, and improving the performance of health care; 3) promote quality improvement of health services, by creating the appropriate infrastructure, conducive to enhanced, and sustainable quality improvement capacity in the health sector, and supportive of the reform program, and of an increased role of primary care; 4) create a restructured system of disease prevention, and health promotion, aligned with recent epidemiological changes in the country, and with the continuously changing profile of the main health problems; and, 5) ensure administrative procedures, i.e., procurement, disbursement, and financial management.

The project will introduce fin...ancial and institutional policy reforms and inject needed investment resources. The project supports the following six components: (i) development of a national population policy, staff training and provision of contraceptives; (ii) expansion of ongoing TB control and launching of STD control activities, supply of drugs and equipment; (iv) development of a pilot system of urban filter clinics and improvements to facilities, equipment, and management of the central hospital; (v) technical and logistical support to a pilot household food security scheme and to a food consumption monitoring system; and (vi) training, advisory services, and logistical backup for the Ministry of Health (MOH) financial and information management, health planning and education, and manpower development, and for the National Health Training Center.

Health Sector Reform Project
|
The Health Sector Reform Proje...ct, will increase access to sustainable quality of health care services, particularly in the areas of rehabilitation, and prevention, within the framework of the health program in Lesotho. The components will: 1) improve financial management, and information systems, through training in accounting procedures, formulation of reporting formats, and responsiveness to information requirements in the Ministry of Finance on health planning, and statistics, and, includes the procurement of supportive information technology. Regional training will be provided, to attain a restructured, and comprehensive integrated budget, within a medium-term expenditure framework; 2) establish procedures, responsibilities, and skills for procurement planning, and implementation within the Ministry of Health and Social Welfare, and, provide technical assistance through training, addressing monitoring, and evaluation of financial, programmatic, and impact indicators; 3) develop a strategic human resources plan, to rationalize staff deployment, and recruitment/retention of health workers; 4) coordinate donor support to acquire expertise regarding annual work-plans, and budgets among developing partners; 5) include analytical activities in planning for Phase II implementation plan; and, 6) support HIV/AIDS prevention, and control policy.

The Health Sector Development ...Program Project's objective is to improve the health outcome of the population, with a focus on meeting the health needs of the underserved segments of the population, as well as accelerating the country's demographic transition toward slower population growth. There are three project components. The first component, expanding access to affordable quality preventive, social, and curative services, will improve coverage for primary health care; improve services in selected secondary and tertiary hospitals; improve overall technical efficiency, increase initial stocks of drugs and medical consumable; and provide support to nongovernmental organizations for health-related activities and action against social exclusion. The second component, strengthening the health system management capabilities, will cover management training; recruitment and redeployment of health personnel; reforms of pharmaceutical sub-sector; logistic support for managerial functions; and support to the Management Information Systems. The third component, exploring and developing sustainable financing schemes for the health sector, will be done through expansion of cooperative cost sharing schemes; policy reforms on benefits; community participation; and cost recovery mechanism.
