The Need: Cryptococcus neofor...mans (C. neoformans) is a yeast like fungus which manifests as meningitis and less frequently pneumonia within the human body. In sub-Saharan Africa C. neoformans causes more deaths annually than tuberculosis. C. neoformans is the most common cause of fungal infection in patients with AIDS and there are an estimated 1 million cases of cryptococcal meningitis worldwide each year. The current drugs used to treat this disease are a 2 week course of amphotericin B and 5-flucytosine followed by a long term course of fluconazole. Amphotericin B and 5-flucytosine are associated with toxicity and fluconazole suppresses the growth of the fungus but does not completely remove it from the body. There is a therefore a great need for new drugs to treat C. neoformans infections. These drugs must have less toxicity and work better against the disease. The aim of this proposal is to complete the early stage drug discovery activities required to ultimately develop new drugs for cryptococcal meningoencephalitis that can be used in sub-Saharan Africa. The Concept: It has previously been reported within scientific papers that a class of compound called the benzimidazoles (widely used to treat worm infections in humans and as fungicides in agriculture) have good activity against C. neoformans in a laboratory and this has subsequently been confirmed by work here at the University of Liverpool (UoL). Benzimidazoles have been shown to interfere with cell division in C. neoformans through binding to beta-tubulin. Beta-tubulin is a protein that is essential for cell division and growth, when benzimidazoles bind to beta-tubulin the fungal cell can no longer grow. More recently, unpublished studies carried out at UoL have shown flubendazole to be active against C. neoformans in the mouse model. Humans also have beta-tubulin so it is essential that any new drug developed against the beta-tubulin target is selective for C. neoformans beta-tubulin. Medicinal chemistry (modification of the chemical structure of a drug) can be used to improve both selectivity for beta-tubulin and activity against C. neoformans. The Solution: Identification of an active class of compound, against a known target, with established whole cell C. neoformans and animal model activity provides an excellent starting point for medicinal chemistry modification. To date benzimidazole compounds have never undergone medicinal chemistry optimisation to establish beta-tubulin selectivity or to improve C. neoformans activity. The two most commonly use benzimidazole drugs (Flubendazole and albendazole) are extensively metabolised within the body and have a limited ability to move from the stomach into the blood stream. New compounds will be designed using information from computer models to predict how they are likely to behave within the body. Concerns regarding the relatio...nship between climate change and food production have called the attention of scientific, governmental and non-governmental institutions. The accumulation of anthropogenic greenhouse gases (GHG) is well established as the main cause of climate change, with over 20% of these emissions coming from food production. Even if clean energy targets are met, greenhouse gases will rise due to methane and nitrous oxide generated by agriculture. This, coupled with the increasing demand for high protein food, will arguably drive food to top priority on the climate change mitigation agenda. Furthermore, food production is likely to be affected by climate change, compromising food security and jeopardising achievement of the second United Nations Sustainable Development Goal: 'End hunger, achieve food security and improved nutrition and promote sustainable agriculture', with major ramifications for the global poor. Brazil is one of the most populated and agriculturally productive countries in the world and as such has an important contribution to greenhouse gas emissions. At the same time, Brazil is one of the first to consider sustainability in its National Dietary Guidelines. The third of five principles underpinning the guidelines focuses on 'the interdependence between healthy diets and the social and environmental sustainability of the food system''. The guidelines are meant to advise and empower people to make better food choices. However, due to the lack of metrics to assess the impact of foods on the environment, it was not possible at that time to directly incorporate the information about GHG emissions in the recommendations given by the guidelines. Changing what Brazil eats has a large impact on the environment, population health, and on the Brazilian economy. Brazil is the third biggest agricultural producer worldwide, with a forecast value of US$183 billion by 2021, yet agriculture and land use change account for about 55% of Brazil's GHG emissions. Simultaneously, 57% of the Brazilian male population is overweight or obese, and the number of people with diabetes has more than doubled over the past three decades (15%+ of the population), leading to obesity healthcare costs rising to $330 billion over the next 40 years. Managed sustainable healthy dietary and food systems change is needed if Brazil is to meet its National Policy on Climate Change (38.9% GHG emissions reduction relative to 2020 emissions projections), sustain its agriculture sector and reduce healthcare spending. This project will offer tools and map pathways, which public policymakers can call on to shift towards healthy sustainable diets, thus promoting the economic development and welfare of Brazil. Epstein-Barr virus (EBV) accou...nts for more than 200,000 cases of cancer each year and 1.8% of all cancer deaths are due to EBV-attributable malignancies including nasopharyngeal carcinoma (NPC) and gastric carcinoma which have higher incidence in low-middle-income-countries (LMICs), e.g. NPC in southern parts of china with >25 times higher incidence than the rest of the world. However, the mechanisms of cancer causation by EBV remain an area of active investigation and controversy. Since the clinical symptoms of NPC are nonspecific and no known pre-cursors exist, diagnosis is often delayed until the advanced stages of disease when five-year survival is less than 50%, as opposed to 90% survival when NPC is identified at an early stage. To date, there are no specific effective treatments for EBV-associated diseases, thus identification of different biomarkers present in subsets of EBV-positive individuals to aid early detection of disease is particularly important. In addition to well-established biomarkers, ten more EBV antigens that best discriminated NPC status have recently been identified through protein microarray technology. However, the output from this technology does not directly translate to an exact amount of antibody circulating in the blood. The proposed study will develop and validate a panel capable of measuring and quantifying the amounts of EBV biomarkers, using a new technology that is high-throughput with low-cost and low volume of samples required. We also propose to use this panel in a pilot case-cohort study including 200 each of NPC, gastric cancer cases and population-based subcohort individuals in the well-established China Kadoorie Biobank (CKB) - a prospective study of 0.5 million individuals recruited at age 30-79 years from 10 diverse regions in rural and urban China, with stored blood samples. The aim of this pilot case-cohort study is to evaluate whether the EBV serology measured several years prior to disease diagnosis could be used to identify high-risk individuals and also to assess the association between EBV biomarkers, their joint relationships with other lifestyle and environmental risk factors and the risk of NPC and gastric cancer. Given the high throughput and low cost features of the technology together with preliminary data identifying high-risk populations provided by this research, this EBV panel may be suitable for large epidemiology studies which can comprehensively investigate EBV-related disease mechanisms, and could potentially be used for early detection/screening among high-risk individuals who experienced other lifestyle or environmental risk factors such as smoking or with a family history of cancer. It may then help to inform EBV-related disease prevention and control policy making by treating infection or vaccinating high-risk individuals, i.e. those infected with highly-disease-associated EBV biomarkers also with other lifestyle and environmental risk factors such as family history of cancer
