Safe and affordable surgery is... a global health priority. For surgical procedures to effectively improve patient outcomes, health systems must also provide high-quality perioperative care (the care before, during and after surgery). There is now strong evidence that health systems failures around the time of surgery are a key factor limiting the net improvements in health which could be achieved through greater worldwide access to surgery. In this proposal, we will build the foundations of a major global health research unit to study and improve perioperative health systems. We will map perioperative care pathways in different resource limited hospitals. We will co-produce a list of research priorities, and a core dataset for quality assurance, clinical trials and implementation research. We will perform initial community engagement work, a research capacity needs analysis and develop the dissemination plans essential for a major research programme in perioperative care. This research will be for the direct and primary benefit of countries on the OECD DAC-list.

Many developing countries are ...experiencing rapid and unplanned urbanisation. Previous research has shown that this can create a variety of new health risks from communicable and noncommunicable diseases, and that newly-urbanised people can find it more difficult to access health services. Through this award, we will develop the research design, methods and partnerships for a large participatory project examining these issues in four countries across South Asia and sub-Saharan Africa. A pilot study in Nepal will be used to trial a series of participatory research methods (Participatory Video, Participatory Photo and Participatory Mapping) to capture the experiences and perceptions of both newly-urbanised people and providers of health services, to bring these into dialogue with health planners and policymakers, and to learn lessons for a larger-scale funding proposal. Meanwhile, preparatory work and network building will be carried out in Ghana, Bangladesh and one other African country.

Inequalities between different... groups in society can be created and maintained by public services such as healthcare, education and local government. Discrimination within these services results in poorer access to services, worse outcomes and non-representation in decision-making. This kind of exclusion from public services is in turn linked to higher levels of poverty and unemployment, poorer health and less capacity to influence how society is governed compared to other social groups. This project aims to produce knowledge on how a range of public services can contribute to the social inclusion of disadvantaged groups in ODA-eligible context, most notably Nigeria, Kenya, South Africa, Vietnam, India, Nepal, Bangladesh and Kazakhstan. This project will establish partnerships between communities that experience such exclusion, organisations that represent their interests, policymakers and academics to challenge the status quo and give these communities a voice in public service planning and decision-making. We will create a Partnership between five international research projects covering 21 countries in Africa, Asia and North America, to identify common solutions to the above issues. We will make recommendations from the evidence that is already available on how public services can become more inclusive of disadvantaged groups in society. We will provide funding to test out some of these solutions on a small scale and see whether these have the potential to reduce the inequalities that some groups experience. This will help lay the ground for larger-scale research that evaluates the most promising solutions that could work across a range of disadvantaged groups and a range of countries. This collaborative research will produce: 1) a policy briefing on evidence-based strategies that support public services to work fairly for everyone 2) an overview of recommended research methods and gaps in current evidence 3) two international workshops to discuss how and where inclusion strategies might work in practice 4) four to six pilot studies in low and middle-income countries supported by an international partnership 5) pilot reports describing methods and findings from each pilot study 6) recommendations about which potential solutions could be developed into a larger scale proposal and 7) an academic publication and accessible report bringing together knowledge from the individual pilots and highlighting a future research direction. Our Partnership aims to create a significant development in the field of research on equality and inclusion in society by bringing together researchers from different academic disciplines and by crossing public sector boundaries. We aim to produce new knowledge in and supporting contributions to diverse streams of evidence and publications with global relevance.

Preventable and treatable dise...ases cause a huge amount of illness and a huge number of deaths in Africa. Infectious diseases, like malaria, are some of the biggest killers. Not only do these diseases cause major health problems for those infected, they are also a major financial burden to individuals and communities, and they hinder economic and societal development for whole countries. The poorest and most marginalized communities are usually the worst affected, and so these diseases exacerbate inequalities associated with geography, gender and ethnicity. Current strategies to tackle infectious diseases in the poorest settings rely on rudimentary approaches to diagnosis and treatment, such as reliance on a few clinical features (like fever, difficulty breathing or pallor) and very few simple tests to decide the treatment someone should receive. This approach is necessary because there are rarely diagnostic laboratories nearby to provide comprehensive testing, and even if there are, their services are often too expensive. Unfortunately, the simple methods of diagnosis often result in incorrect diagnosis, and provide little data from which to learn how to improve. A wrong diagnosis can mean the wrong treatment is given, resulting in prolonged illness or death, whilst at the same time encouraging overuse of some treatments like antibiotics or anti-malaria drugs, which can drive resistance to these drugs. A revolutionary solution to this problem is to develop a new generation of digital diagnostic tests which can be used at the point-of-care in even the hardest-to-reach communities. These diagnostic tests are not only more accurate and faster than current alternatives but potentially cost saving. Digital diagnostics have the potential to transform the situation by linking the precision typical of an advanced laboratory with the portability, connectivity, analysis and support that can be provided through a modern smart phone. This means tests can be administered anywhere and anytime by a wide range of healthcare workers with minimal training. For example, a drop of blood collected from the tip of the finger of child by a health worker in their community, could be applied to a tiny microchip powered by a mobile phone battery, and within 15-20 minutes a result could be available to make a diagnosis. The results are transmitted from the microchip to a smartphone via Bluetooth so no internet connection is required. If 'Malaria' is detected, the device can notify the healthcare worker of the type of malaria and the correct treatment. The smartphone could also send the result and location data via the mobile network to a remote computer system, which could determine whether there is an unusually high level of malaria in that area and, if so, notify the national Malaria control program that extra resources are required to tackle an outbreak before it gets worse. Well-funded medical labs are h...ighly automated, detailed digital images and results are recorded by computerised instruments. This improves throughput, quality control, and record-keeping, and would enable training and telemedicine for rural contexts where an expert technician is not available. Currently, automated diagnostic devices are expensive, manufactured in rich countries. Most of the Global South is left behind, not only because of the high up-front cost, but because equipment cannot be maintained locally, due to proprietary technologies, conservative regulations, and poor supply chains for spare parts and consumables. This network will bring digital diagnostics to laboratories and clinics across Africa, in sustainable and responsible partnership with local clinicians and engineers. Our vision is of integrated, modern, digital healthcare laboratories that are supported by a thriving local ecosystem of biomedical engineers and developers. The crux of our approach is the use of 'open source hardware', where designs for easily replicated, high-quality diagnostic tools are shared under a license permitting their use, sale, and modification. Crucially, open source projects share not only technical know-how, but also ownership of innovations. Local entrepreneurs are not required to enter costly licensing deals in order to make use of these designs - the only condition is that they share any improvements they make to the design, for the benefit of similar companies elsewhere. To reach our vision we must build a body of knowledge and skills that enable a new generation of medical instruments that can be repaired and customised without relying on a handful of rich countries. Our aim is to test the potential of open-source hardware as a new business model to establish and scale digital diagnostic solutions in LMICs, using the OpenFlexure Microscope as a case study. The links we build between engineers, healthcare scientists, medical professionals, and social scientists will not only help us achieve this aim, but will provide local, pan-African, and intercontinental links that help to build capacity in diagnostic innovation where it is needed most. We will spend the first year of this project planning, identifying barriers to open source innovation, and forming networks of local connections to overcome these barriers. In the following years, we will study the process of taking a locally produced, 3D printed microscope from working prototype to usable product for malaria diagnostics in three different African countries. We will use the framework of Implementation Science to study the 'diffusion of innovation' as the technology is taken through regulatory approval and adopted by healthcare providers. We will engage with a number of other projects, for example low cost retinal imaging devices, to compare across different technologies, and to share what we have learned in malaria microscopy with other application areas. The private sector is a major ...and growing source of treatment in low- and middle-income countries (LMIC), but there is considerable concern about the safety and quality of care. Recent years have seen fast growth in small and medium sized private clinics, but their regulation is often poor, and relatively little is known about the effectiveness of interventions to improve the quality of care they provide. This project aims to address this evidence gap. The research takes place in the context of an innovative intervention developed by the international NGO PharmAccess that extends the benefits of clinical standards and stepwise certification to small and medium-sized facilities in LMIC, using standards developed with international accreditation bodies. The PharmAccess model seeks to improve the quality of care that facilities provide as well as to shape the broader healthcare and finance markets and policy environment. There are several components to the model. Health facilities are assessed on a set of 'SafeCare' structural quality standards, trained on quality improvement and business skills, and assisted in the development of a quality and business improvement plan. They receive regular mentoring visits and are also connected with the PharmAccess Medical Credit Fund, a social investment fund which facilitates access to bank loans to finance implementation of the plan. The aim is to create a series of virtuous circles, with improvements in SafeCare assessment scores signalling improved quality of care to individual and institutional purchasers, thus increasing demand for health services, and improved business performance ensuring further access to credit, and the sustainability of quality gains. The research will begin with an analysis of secondary data collected routinely by PharmAccess in Tanzania and Kenya to date to examine changes in the SafeCare assessment scores over time and factors associated with these changes. We will then undertake a randomised controlled trial to evaluate prospectively the impact on quality of care of the roll out of the PharmAccess model to additional private for-profit and faith-based facilities in Tanzania. The trial will assess the effects of the intervention on the quality of care received by patients through comparison of 120 intervention and 120 control facilities after 2 years of implementation. These data will also be used to assess the relationship between SafeCare assessment scores (which measure availability of inputs such as infrastructure, technology and standard operating procedures) and measures of technical and perceived quality of patient care (which concern the interaction between caregivers and patients).
