A profound health reform is un...derway in Ukraine. The currently ill-prepared medical education system is a fundamental component of it. Training curricula are outdated and teaching staff lack essentials skills, capacities and motivation. This project supports the reform of the medical education system for family doctors and nurses, and the strengthening of competencies of healthcare managers. As a result, significant quality improvements of health care delivered to the population are expected.

Although health sector reforms... have prioritised family medicine, the medical education system continues to prepare narrow specialists, leading to a shortage of family doctors mainly in rural areas. The Ministry of Health initiated the medical education reforms with focus on family doctors specialty at pregraduate level and compulsory postgraduate clinical practice in the regions. Phase II of the project will finalize the medical education reform implementation process with focus on postgraduate and continuous medical education.

To provide up to £50m in 'mat...ched' funding over the lifetime of the Queen Elizabeth Diamond Jubilee Trust, from 2013/14 to 2018/19. The matched funding will come from a variety of governmental, non-governmental and private sector corporate sources from across the Commonwealth (including the UK), offering a unique opportunity for a diverse and collaborative partnership with global reach. Department for International Development (DFID) support is intended to be channelled mainly into the programmes to tackle avoidable blindness – a significant cause of disability worldwide. Visual impairments affect 285 million people worldwide, of whom 39 million are blind. There is strong coherence with the Global Goals, and with a range of other DFID work streams.

The Trust’s avoidable blindn...ess programme comprises four initiatives:
1. Trachoma management in Africa (£40.1m) and the Pacific (£2m). This is managed through a partnership with International Coalition for Trachoma Control (ICTC) who appointed Sightsavers to manage the initiative in 6 African countries working to support the elimination of trachoma in Malawi, Kenya and Uganda, and significant reductions in the burden of trachoma in Mozambique, Nigeria, and Tanzania. Trachoma is a neglected tropical disease which affects the eyes, if left untreated, it can lead to blindness. Elimination of trachoma is possible through the adoption of the World Health Organisation’s SAFE strategy (Surgeries, Antibodies, Facial cleanliness and Environmental components). In the Pacific, the Trust it is working with ICTC and the Fred Hollows Foundation to, support elimination in four countries; Fiji, the Solomon Islands, Vanuatu and Kiribati (the Trust also supports work in Australia but not with DFID funds).
2. Diabetic retinopathy (DR). An increase of 60% in DR is predicted in the Commonwealth by 2030 . Diabetes increases the risk of many eye health problems, with DR being the main cause of blindness. The Trust is supporting a partnership between the Public Health Foundation of India (PHFI) and the London School of Hygiene and Tropical Medicine (£4.2m) to improve prevention, detection and treatment of DR in India. The Trust is also supporting the Caribbean Council for the Blind to reduce rates of DR in the Caribbean (£1.4m), and the Fred Hollows Foundation to reduce rates of DR in Bangladesh, Pakistan and the Pacific (£2m).
3. Retinopathy of prematurity (ROP). The major stream of work is in India through a partnership with the Public Health Foundation of India (PHFI) and the London School of Hygiene and Tropical Medicine (£3.5m), who are working with federal and state governments to integrate services to prevent, detect and treat ROP within Special Newborn Care Units. These units are being rolled out at district level with a target of 1000 units operational in 2017. The initiative covers supporting strategy and guideline development, training physicians, new patient record and management systems and the development and testing of more affordable equipment for ROP diagnosis.
4. Strengthening of knowledge, tools and people to improve eye health care across the Commonwealth managed by the International Centre for Eye Health (CEHC), hosted by the London School of Hygiene and Tropical Medicine (£11.5m). The Consortium supports fellowships to increase eye care expertise in low income Commonwealth countries, provides a knowledge platform to facilitate learning and information exchange, and a technology programme to develop, validate and rollout low cost eye health diagnostic and management tools.

Upper limb loss can have devas...tating effects on an individual, particularly if that person is already surviving at a subsistence level. Prostheses can be used to replace the missing limb, offering both cosmetic and functional benefits. In lower and middle income countries (LMICs), conflict and road traffic accidents mean that demand for upper limb prostheses is high, however provision is sparse, and maintenance is a major challenge. Body-powered (BP) prostheses have seen little development since the early 20th century, despite high self-reported rates of rejection. Nevertheless, BP prostheses are well suited for use in LMICs, being potentially simple to manufacture and maintain. If the reasons for rejection (e.g. limited functionality, cost and heat-related discomfort) can be addressed, BP prostheses offer a potentially viable solution. Therefore, this project will bring together an experienced team from across the UK, Uganda and Jordan to create a new BP prosthesis that is optimised for adoption by LMIC prosthetic services and acceptable to LMIC users. This will include establishing methods of fabrication, fitting and evaluation of the prosthesis which are appropriate to LMICs. Our two LMIC partners (Uganda and Jordon) have been selected because of the unique challenges of prosthetic provision in these countries. Uganda is one of the least developed countries in the world, with poorly resourced and fragmented rehabilitation services. Jordan is an upper middle income country, with well-developed clinical training, but facing immense pressures on prosthetic services due, in part, to regional conflicts. To achieve our goals, the following work packages (WPs) are planned: WP1: The requirements of amputees in both Jordan and Uganda will be investigated using focus groups and questionnaires. WP2: We will develop an engineering/human factors specification for a BP prosthesis optimised for LMICs by identifying the key features of the conventional BP prosthesis that determine its functionality and usability. WP3: Informed by WP1 & 2 we will develop a new design, optimised for LMICs. We aim to restore a high level of functionality, in a culturally acceptable manner, and in a way that is well suited to local prescription, manufacture and fitting. To ensure local relevance, we will work closely with our partners in Jordan and Uganda. WP4: In parallel to the work in WP3, we will address more ambitious design challenges, creating and testing a highly novel prototype in the laboratory. This division of the work will allow us to move quickly towards a practical solution for LMICs and to also include more novel but higher risk research. WP5: There is currently no objective data on the extent to which prostheses are worn, how they are used, or the impact on daily life.
