Con el presente proyecto, Méd...icos del Mundo pretende continuar el trabajo iniciado en 2009 en el Distrito de Same, abarcando con cada fase de los proyectos ejecutados nuevas divisiones a modo tal de cubrir el 100% de las divisiones del Distrito de Same.La propuesta de proyecto continuará a contribuir a la reducción de la mortalidad y morbilidad de las mujeres en edad fértil por causas relacionadas con el embarazo, parto y post parto; así como de neonatos, orientándose a los colectivos más vulnerables, desarrollando un sistema de salud responsable hacia las necesidades de la población. Mejorando la calidad, accesibilidad y cobertura de la atención sanitaria básica con sensibilidad de género, sostenibles y proveídos para la consecución de mejoras de salud y estatus social.El proyecto forma parte del programa iniciado en 2009 que fortalece la capacidad del Distrito de promover los derechos de salud. El programa se ejecuta por fases, cada fase de trabajo en dos divisiones diferentes del mismo distrito. El programa se basa en un enfoque multisectorial que incluye no sólo el componente de salud, sino que también involucra a la comunidad y el trabajo con los alumnos, con el objetivo de mejorar el conocimiento de la población sobre temas de salud sexual y reproductiva, los derechos y de fortalecer la respuesta de las estructuras de la comunidad a través de los Comités de salud de la Comunidad con respecto a los problemas de salud sexual y reproductiva.

Asimismo, pretende fortalecerlas para que puedan incidir en la construcción de un proceso de paz en el país desde su contribución a la Memoria Histórica del conflicto y que aporten sus propuestas para el establecimiento de una plítica publica de usticia transicional.

El proyecto se desarrolla en e...l distrito sanitario de Fada (Región del Este) y tiene como objetivo mejorar la atención sanitaria pública de mujeres embarazadas y recién nacidos mediante el refuerzo de la calidad de los servicios de SSyR prestados por los CSPS (primer nivel de atención sanitaria), la formación de futuros profesionales sanitarios y la sensibilización comunitaria en DSDR. Sus componentes principales son:- Fortalecimiento de las capacidades de las estructuras de salud del distrito sanitario de Fada para ofrecer servicios de SSR mediante actuaciones de formación continua y dotación de materiales.- Sensibilización de la población de distrito de Fada en el ejercicio de los DSDR a través de la realización de actividades de IEC- Mejora de la calidad de la formación de alumna/os de enfermería (rama de matronas/es) de la ENSP de FadaPara su ejecución MMS actúa en partenariado con el sistema público de salud de Burkina, en concreto con el Ministerio de Salud.

El proyecto fortalecerá las c...apacidades institucionales y profesionales para la atención prenatal de comunidades rurales de Guatemala, contribuyendo a la reducción de la mortalidad materna y neonatal y al acceso de las mujeres a servicios de salud de calidad. La VI Encuesta Nacional de Salud Materno Infantil 2014-2015 de Guatemala señala que la Razón de Mortalidad Materna entre 2008 y 2015 fue de 140 muertes maternas, subrayando además que el 60% de las niñas y niños fallecidos durante su 1º año murieron antes de cumplir el 1º mes, lo que pone de relieve la grave incidencia de la mortalidad neonatal. La Encuesta evidencia que persisten dificultades de acceso a los servicios de salud prenatal y de atención por personal cualificado en el ámbito rural, lo que resulta determinante para evitar riesgos de mortalidad para la madre y sus hijos e hijas.

Santa Rosa de Copán, Honduras :: El Hogar Maternal de hospedaje y alimentación a mujeres con embarazos de alto riesgo, en los días previos a dar a luz. Se alojan un máximo de 30 personas en condiciones precarias, hacinadas, sin privacidad, con sanitarios insalubres, siendo la construcción inestable.la mortalidad es elevada cuando es difícil acceder al hospital. La carencia de infraestructuras y las costumbres locales conllevan largas esperas en el hogar antes del traslado al hospital. Debido al paso del tiempo y al constante uso de las instalaciones, el Hogar materno requiere una rehabilitación integral.

El proyecto se desarrolla en 4... áreas aisladas de la Comuna VI de Bamako y su objetivo es mejorar la atención sanitaria pública de mujeres embarazadas y recién nacidos reduciendo las altas tasas de morbimortalidad maternal y neonatal en la zona. Se han diseñado 3 resultados:- Mejorar la atención sanitaria a mujeres embarazadas y neonatos mediante un servicio de urgencia y reanimación en el CSREF y el refuerzo de las capacidades profesionales del personal sanitario encargado.- Incrementar los controles prenatales de mujeres embarazadas mediante el refuerzo de capacidades del personal sanitario de los CSCOM y promotores/as de salud comunitaria para prevenir complicaciones durante el embarazo, el parto y el puerperio. - Mejorar las capacidades comunitarias para promover el acceso de las mujeres a sus DSDR.El proyecto tiene un sólido componente de fortalecimiento de las capacidades locales en SSyR pues cuenta como socio locala con el sistema público de salud de Malí.

La intervención se desarrolla... en las moughataa de Bogué y de Bababé y tiene como objetivo mejorar la calidad de la atención sanitaria pre y post natal de mujeres embarazadas desde un enfoque de DDHH y género mediante el refuerzo del sistema público de salud materno ¿ infantil. Sus componentes principales son:1. Refuerzo de las competencias del personal de salud y mejora del equipamiento técnico de los centros de salud de Bababé y Bogue para asegurar la calidad de la atención sanitaria en salud sexual y reproductiva (SSyR).2. Fortalecimiento de capacidades de la Red de Mujeres Animadoras del MASEF que trabajan en la promoción de la SSyR en IEC desde un enfoque de género y DDHH. 3. Implicación y sensibilización de la población de Bababé y de Bogué en el ejercicio de los DSRD a través de la realización de un programa de IEC.El proyecto tiene un componente de fortalecimiento de las capacidades locales en SSyR pues se ejecuta en partenariado con el sistema público de salud.

Región de Sikasao, Mali. :: Este proyecto pretende mejorar las condiciones de vida de la población de Djelinfing, un pueblo a 250 km de Bamako, en Mali (África), centrándose fundamentalmente en las mujeres, haciendo especial hincapié en la disminución de la morbi-mortalidad materno-infantil con la creación de una maternidad anexionada al dispensario médico, que realiza labors de control prenatal, asistencia al parto y puerperio, así como la atención de los niños en el primer año de vida.Beneficiarios directos: 7.678 mujeres adultas y 17.118 niños y niñas.Beneficiarios indirectos: 6.505 hombres adultos.

Construcción y equipamiento de una maternidad en el Centro de Referencia de Salud de Dwe :: El proyecto pretende continuar una reforma del Centro de Salud de Referencia de Dwe, posibilitando progresivamente una atención médica completa en una zona situada entre dos hospitales alejados en 100 kilómetros de Dwe, Distrito del Kwilu, Provincia de Bandundu en la República Democrática del Congo.El Centro Rural de Salud de Referencia de Dwe tiene una población en el centro de 10.870 habitantes, pero su área de influencia alcanza a la población de otras zonas de salud atendiendo a una población potencial de 66.980 personas. El 90% de la población de la zona de Dwe, al igual que la población de la zona de salud de Djuma, vive de la agricultura de subsistencia.Inicialmente, fue sólo un pequeño centro de salud de las Hermanas de María en Kwango que incluían la maternidad, cirugía menor, y laboratorio precario. La población local estaba expuesta a un alto riesgo de vida para los casos graves en los que era necesario transferir el hospital Djuma más de 100 km. Por lo tanto, la población local, sus líderes políticos, administrativos y tradicionales, así como el Comité de Dirección y el médico jefe de toda la zona expresaron su derecho a la salud y solicitaron a la ONGD PRIAC para rehabilitar el centro de salud en el hospital secundaria Dwe. Desde esta visión se planteó un proyecto en varias fases para reformar el centro de salud existente, y así dotarle de las infraestructuras necesarias para que pueda prestar un servicio adecuado a las necesidades de la población. Estas reformas se están realizando gracias a la ayuda de diferentes Entidades Públicas y privadas desde el año 2011.En 2011 se empezó con la construcción del Pabellón Pediátrico y la Fosa para Placentas (Ayto. Pamplona), Construcción quirófano, depósito de agua y bloque de letrinas (Gobierno Navarra) y un primer pabellón de enfermos, depósito de agua y 2º bloque de letrinas (Fondo 0,7% Trabajadores/as GV y Osakidetza). En 2012 segundo pabellón de planta (Ayto Pamplona). En 2013, 2014, 2015 y 2016 se ha construido y equipado el pabellón de rayos y ecografías. En 2017 se construyó un corredor para unir los pabellones (Ayto. Pamplona) y un depósito de agua (Tu salario solidario). En 2018 y 2019 se ha centrado en la construcción de un vallado de saneamiento y seguridad. Todos estos edificios que componen el hospital de segundo nivel Dwe se han construido conforme a las construcciones locales (plantas bajas, acceso directo, sin escaleras) y a la realidad financiera (un solo edificio en esta cantidad de años con poca financiación hubiera sido inviable) en diferentes bloques, dispersos ente sí aunque cercanos. Fortalecimiento y puesta en valor de las capacidades organizativas y técnicas de las Comadronas Tradicionales de Santa María de Nebaj en la atención materno-infantil :: La estrategia del proyecto se dirige al reconocimiento de las Comadronas Tradicionales de Santa María Nebaj, al fortalecimiento de sus capacidades y al ejercicio de sus derechos para la mejora de la atención a la salud materno-infantil con pertinencia cultural.La cobertura geográfica del proyecto se extiende a 15 comunidades rurales del municipio de Santa María Nebaj, en departamento de El Quiché, con una población beneficiaria directa de 60 comadronas (30 pertenecientes a las comunidades y 30 vinculadas al centro de salud), con un promedio de 13 mujeres indígenas atendidas al año, lo que supone un total de 780 mujeres (840 teniendo en cuenta a las propias comadronas) que verán mejorada la atención materno-infantil que reciben. Por ello sus familias se verán beneficiadas de forma indirecta siendo un total de 2.100 personas (1.100 mujeres y 1.000 hombres).

El presente proyecto pretende ...mejorar las condiciones de vida de la población de Djelinfing, unpueblo a 250 kms de Bamako, en Malí (África), centrándose fundamentalmente en la mujer, haciendoespecial hincapié en la disminución de la morbi-mortalidad materno-infantil con la creación de unamaternidad anexionada al dispensario médico, que realice labores de control prenatal, asistencia alparto y puerperio, así como la atención de los niños en el primer año de vida. La iniciativa surge apartir de la petición de una asociación de jóvenes que se congregan para fines no lucrativos y quepersiguen el bienestar de su pueblo. Mediante la creación de este dispensario, se buscará daratención e impacto a los pueblos y localidades de un amplio radio en la región, y así evitardesplazamientos largos de las personas necesitadas de atención médica (que muchas vecesprovocan la muerte), en especial las mujeres gestantes. Se va a realizar una sensibilización sobreel COVID, como prevención a través de visitas, mensajes radiados, panfletos y posters, entrega dejabón y clorina a la población, además se hará fortalecimiento del hospital de referencia de la zonay apoyo a centros de salud dependientes de él con material de protección, medicamentos yequipamiento sanitario y preparación del pabellón de infecciosos del hospital, apoyando a lapoblación más vulnerable (ancianos, gestantes, desnutridos) con alimentación y vitaminas.

The overall aim of this projec...t is to increase access to safe and comprehensive managmenet of second trimester incomplete abortion (referred to as Post Abortion Care (PAC)) at district health care level in Uganda. Complications from spontaneous and unsafely induced abortions add significantly to maternal mortality and morbidity. The problem is most pronounced in the 2nd trimester of pregnancy. In low resource settings including Uganda where abortion is restrictive and often self-induced, it is imperative to provide PAC. PAC services include treatment of abortion complications such as incomplete abortion and infection and provision of contraception if required. To date, there is no established medical regimen for second trimester PAC and the recommended method is surgical manual vacuum aspiration (MVA) despite that equipment, and training is frequently lacking. Unsafe and outdated methods such as sharp curettage are frequently used. If available a medical method would incerase access to safe PAC. Therefore we propose to study the efficacy, safety and acceptability of misoprostol treatment versus MVA in a randomised controlled non-inferiority trial (RCT) including 1030 women seeking treatment for incomplete second trimester spontaneous or induced abortion at six District Health Centres in central Uganda. Primary outcome is the efficacy of the treatment to expel the retained tissue without need for additional surgical intervention. Secondary outcomes will include the safety, and acceptability of PAC provision by treatment group (MVA and misoprostol) and contraceptive uptake. All health care providers (doctors and midwives (contraceptive counselling)) providing PAC will be trained according to standardized PAC training modules. The WHO recommended misoprostol regimen for second trimester induced abortion will be used for PAC: 400mcg sublingual every 3 hours up to 5 doses Inclusion criteria: Bleeding and contractions during pregnancy. Exclusions criteria: Women < 15 years old, known allergy to misoprostol, a uterine size < 12 weeks or > 15 weeks, unstable hemodynamic status and chock, signs of pelvic infection and/or sepsis. Main and secondary outcomes will be evaluated at follow up visit between 14-28 days by a obstetrician/gynecologist blinded to treatment allocation. A study cocordinating centre will be established at Mulago Hospital, Kampala. Preparations, permissions, and training will be ready by mid 2016. It is estimated that recruitment will start in 2016 and continue thorugh 2017. In 2018 data cleaning, analyses, and dissemination will be performed. Significance. We have shown that misoprostol is highly effective, acceptable and can safely be administered by midwives for first trimester PAC. Grossesses non désirées – ...soins aux jeunes femmes enceintes. Situation initiale: Plus d'un tiers des femmes ukrainiennes en âge de procréer sont stériles ou présentent des problèmes de fertilité. Dans 80 % des cas, c'est la conséquence d'avortements pratiqués de manière hasardeuse. Un programme de traitement médical des grossesses non désirées viendra donc s'ajouter aux programmes de médecine périnatale (médecine des nouveau-nés, obstétrique, etc.) et de promotion de la santé de la mère et de l'enfant (voir le projet numéro 7F-05620), déjà en place. Objectifs: La réglementation nationale et la formation médicale dans le domaine de l'avortement seront actualisées afin de tenir compte des connaissances les plus récentes. Pour mettre en oeuvre ce projet, l'organisation non gouvernementale (ONG) Women's Health and Family Planning (WHFP) collabore étroitement avec l'OMS.

The procurement of EmONC and t...he construction and setup of the Baobab Maternity Hospital are meant to achieve concrete outputs such as strengthening the referral and reducing the high rate of maternal and neonatal mortality. In addition, CCBRT's efficient management system will serve as model for other hospitals (e.g. St Francis) and health facilities. Similar to the CCBRT Disability Hospital, the new Baobab Maternity hospital will aim to make quality services accessible to women of poor background with an innovative and sustainable financing system. The project will link with Swiss support to St. Francis Hospital (strengthening management system), the Emergency Obstetric care project (increasing basic and emergency obstetric and new born care), as well as complementing the efforts of the health basket to reduce maternal and neonatal mortality rates. The proposed 4 year phase will focus first on a contribution to the construction and equipment of the hospital (2011-12), and then on the strengthening of the management by developing a performance based management system which will serve as a model for PPP on referral structures (eg. St. Francis and others).

Successful scaling-up of commu...nity-based social or systems interventions for improving healthcare processes and outcomes are lacking. We have recently completed the NeoKIP (Neonatal health - Knowledge Into Practice) trial, a community-based participatory intervention that resulted in a 50% reduction in neonatal mortality in 44 Vietnamese communes. In NeoKIP, laywomen from the local Women?s Union acted as facilitators, supporting local stakeholder groups in improving perinatal health. Facilitators stimulated group-members to have a useful dialogue in applying a structured approach to identify problems and accomplish actions using the PDSA-cycle. Similar to other community-based trials we found that this bottom-up-strategy preferably should be combined with efforts to improve service-delivery at hospitals. The aim of NeoKIP+ is to analyse the effectiveness of facilitation of community-level maternal-and-newborn stakeholder groups, and similar groups of staff at district hospitals, on neonatal mortality in six provinces in Vietnam. Secondary aims include the influence of the health system context on the process of change. The project is implemented in six provinces (28 districts) with neonatal mortality 24/1000 by a randomly allocated stepped-wedge design; birth and neonatal death monitoring is maintained over the total project duration of 4 years. This project will provide answers whether this kind of intervention is appropriate for scale-up in countries like Vietnam.
