cross border disease surveillance and disease outbreak preparedness; carrying out annual peer audits of participating laboratories; implementing joint
training and research activities; and sharing/disseminating good practices. Over two-thirds of project-supported laboratories have reached three stars
and on the Stepwise Laboratory Improvement Process towards Accreditation and three have attained international accreditation. Training and capacity
building activities continue to boost human resources capacity with a cumulative total of over 12,000 health personnel trained. Roll out of new molecular
technologies has improved access to TB diagnostic services, strengthened turnaround time, and picked up missing cases, with the introduction of the
GeneXpert machines project-supported facilities were able to conduct nearly 104,500 tests, accurately diagnosing multi-drug resistant TB within several
hours, rather than waiting months for culture results. The Rwanda project is completed but stakeholders continue to stay engaged on several key

The Second Health Project for ...Uzbekistan aims to improve the quality and overall cost-effectiveness of health care services. The project has the following four components: Component 1) will extend further support for development of Primary Health Care (PHC) services. In concert with the Asian Development Bank, all Primary Care Centers (SVPs), as well as some remaining SVPs not covered under Health I, will be supplied with a package of equipment. More remote SVPs will be supplied with telecommunication equipment and transport to improve patient services, referrals, and overall management of these facilities. Component 2) will support broad activities to continue to improve the health care financing and management system, to improve efficiency in the delivery of services and to help increase sustainability of primary health care reforms. Activities to improve the health care financing and management system will comprise the scaling up of financing and management pilots initiated under the first Health project, and extending and geographically expanding the rural PHC financing model of the first project nation-wide following some adaptations. Component 3) aims at contributing to the control of communicable and non-communicable diseases, and to improve public health services, including surveillance, and health promotion. Component 4) The project will be implemented through the Ministry of Health, its Central Project Implementation Bureau (CPIB), and Oblast Project Implementation Bureaus (PIBs).

RMUs and 15 selected CMUs with up-to-date equipment (including, inter alia, diagnostic and waste
management equipment) and medical furniture; and (b) providing training to hospital managers and staff
on, inter alia, hospital design, planning and management.

18. This subcomponent will support primary health care development by expanding the following
activities to additional urban and rural PHC facilities: (a) providing medical equipment; and (b) training
of medical personnel in early diagnosis, screening, and treatment of priority NCDs.
Subcomponent 1.3: Clinical Quality Enhancement
19. This subcomponent aims at enhancing clinical quality by (a) providing on-site training of
additional RMUs' and selected CMUs' pediatric and internal medicine doctors and nurses in new clinical
treatment standards; (b) training of staff of the additional RMUs and selected CMUs in clinical case
management and hospital administration; and (c) establishing quality improvement mechanisms to
monitor implementation of the new clinical treatment standards.
Component 2: Strengthening Health Financing and Management Reforms
20. This component aims at supporting the development of a health information system for the
hospital financing pilot in preparation for potential national level rollout, and expanding the training
program for PHC facility managers in financial and health care management. Component 3: Institutional Strengthening on NCD Prevention and Control
21. This component aims at strengthening the capacity in the control and prevention of NCDs by
expanding surveillance and health promotion activities in selected regions in preparation for national level
rollout.
Component 4: Project Management

National Health Support
|
The National Health Support Pr...oject's overall objective is to improve the health status of the rural population of Viet Nam in the poorer areas of the country. Specifically, it will aim to: 1) provide high quality, reliable primary health care on a sustainable basis in fifteen of the poorer provinces; 2) reduce nationwide mortality and morbidity due to malaria, tuberculosis and acute respiratory infections, as well as the adverse socio-economic impacts associated with these diseases; and 3) strengthen the capacity of the ministry of health (MOH) in policy-formulation, planning and management. To achieve these objectives, the project consists of the following components: 1) service delivery in the areas of: (a) infectious and communicable diseases; (b) maternal and child health care; (c) family planning; (d) reproductive health services; and (e) other basic health care; 2) national program assistance for the Malaria, Tuberculosis and Acute Respiratory Infection Programs including provision of drugs, insecticides, transport, equipment and technical assistance; and 3) strengthening institutional health planning and management.

Reduction Project's development objective is that poor villagers in the region, may profit from improved, sustainable infrastructure, and social services, within an improved institutional capacity in the upland communes, and districts. To this end, the components will assess community participation to identify development priorities, focused on the development of rural transport, and market networks, by connecting the administrative center to the road system. Construction of new, and upgraded existing rural roads will be financed in district-to-commune roads; in commune-to-village roads; and, in village-to-village roads; and, in addition, facilities for rural markets will be financed. Capacity building will be provided to support the Ministry of Transport, and local authorities. Additionally, and following the participatory approach, small-scale irrigation systems, rural water supply, and agriculture development will be financed, to include an agricultural program of applied demonstrations and training, and an on-site research program; irrigation schemes will serve a total of 12,500 hectares of paddy fields; while water supply infrastructure will build drinking water schemes, with designs relating both to available water sources, and to cultural preferences of ethnic groups. Furthermore, the project will facilitate the development of education and health systems, through improved access to, and delivery of primary education, construction of schools, and improved teacher qualifications; while increased health service provision will focus on basic equipment, and medicine supply, as well as on improved information, training, and communication activities. Community development budgets, planning and management, and institutional capacity building will be funded based on community priorities.

Improving the quality of district hospitals through the following: (a) human resources development,
including strengthening health workforce capacity and expertise, increasing staff retention, and provision of
long-term training for doctors and specialists; (b) improving the quality of district hospitals through the
provision of basic medical equipment and selective facility repair and refurbishment; and (c) improving
hospital management by creating a management environment that would sustain project outcomes.
Component 2: Increasing Financial Access to Healthcare Services for Decision 139 Beneficiaries
(Appraisal US$10.0 million; Actual US$10.03 million).
Addressing demand-side constraints of access to services, the component included: (a) support for direct
catastrophic and nonmedical expenditures of health care for Decision 139 beneficiaries; (b) strengthening
the capacity of Health Care Funds for the Poor through institutional capacity-building activities to increase
the financial capacity of the district hospitals to enroll beneficiaries and recover the medical expenses
incurred by them; and (c) strengthening local access to health services through promoting health seeking
behavior, and increasing knowledge and understanding of the rights, entitlements, and benefits covered by
Health Care Funds for the Poor among the beneficiary population through information, education and
communication campaigns.
Component 3: Monitoring, Evaluation, and Project Management (Appraisal US$13.10 million; Actual
US$9.71 million).
Setting up and managing the Central Project Management Unit and the Provincial Project Management
Units through: (a) consulting services for technical issues, procurement, financial management, and
disbursement; (b) training of project management staff; (c) provision of necessary office equipment; (d)
financing of incremental operating costs; and (d) M&E activities, including baseline data collection,
indicator updates, midterm review, and end-of-project completion report and audits. This component also

Component 1. Supporting health... insurance for the near poor (US$9.1 million at appraisal; US$9.8

Expand coverage and access to health insurance for near-poor households in the Central North Region,
and improve the system’s capacity to manage health insurance, through: (a) subsidizing premiums for the
near poor; (b) conducting social marketing and information, education, and communication (IEC)
campaigns and providing technical assistance to the Health Information and Education Centers of the
Vietnam Social Security Offices (VSSs); and (c) strengthening the institutional capacities for its
administration.
Component 2. Strengthening district health services (US$32.2 million at appraisal; US$29.3 million
at closing).
Improve the capacity of district hospitals to provide basic curative health services to the population, and
strengthen basic public health functions of the District Preventive Health Centers (DPHCs), through: (a)
upgrading medical equipment in the district hospitals; (b) providing equipment and training to 30 DPHCs;
(c) building the technical and administrative blocks of DPHCs; and (d) piloting performance-based
financing mechanisms to incentivize health care providers to perform better and more efficiently.
Component 3. Improving supply and quality of human resources for health (US$15.9 million at
appraisal; US$11.6 at closing).
Strengthen the capacity of existing medical educational institutions, and improve skills and knowledge of
already-practicing medical personnel, through: (a) supporting the transformation of the Nghê An medical
college into a medical university; (b) improving teaching facilities in five other medical colleges; (c) training
district hospitals’ clinical staff, DPHCs’ personnel, and hospital administrators (short-term training, Level 1
specialty training, on-the-job training, and Master’s degree training); and (d) supporting the implementation
of Decision 1816 of the Ministry of Health (MoH), which targeted the rotation of qualified medical personnel
from higher-level facilities to district hospital to temporarily address health personnel shortages and at the
same time ensure the transfer of knowledge and skills to district-level hospital human resources.
Component 4: Project management, Monitoring, and Evaluation (US$7.7 million at appraisal; US$3.9 at closing)
Ensure adequate management structures, processes, and human resource capacities for the project, and
set up mechanisms for effective monitoring of activities and evaluation of results, through financing: (a) the
operation of provincial project management units (PPMUs) at both the central and provincial levels, (b)
health information systems and data collection, (c) M&E and audit mechanisms, and (d) implementation

Component One: Strengthening t...he capacity of lower level hospitals to deliver
quality services (estimated US$118.0million of which US$116.0 million IDA): Component
One aims to support the implementation of the government’s Masterplan on Reducing Hospital
Overcrowding14 through increasing the capacity of the health system to provide more and better
quality health services in the project provinces. It would consist of two subcomponents. Subcomponent 1a - Provincial Subprojects for the Development of Medical
Services and Quality Improvement (estimated US$109.0 million of which US$107.0
million IDA). This subcomponent would provide technical and financial support directly
to the Provinces to increase their capacity to deliver quality health services at their
general provincial hospitals or specialized pediatric or obstetrics and gynecology
hospitals as well as at least three district hospitals per province. Subcomponent 1b - National Policy and Central Level Technical Support for
Reducing Hospital Overcrowding and Quality Improvement (estimated US$9.0 million
of which US$9.0 million IDA). This subcomponent would support the MoH and the
other national level stakeholders to provide quality technical support to the Provinces to

The Social Fund for Developmen...t Project seeks to reduce poverty by improving living conditions and providing income generation opportunities to the poor. To achieve this objective, the project will establish a demand-driven mechanism to: (1) provide community development services to poor communities; and (2) support income generation activities through development of small and microenterprises. The project has three components: (1) community development; (2) small and microenterprise development; and (3) capacity building. The first component comprises small-scale labor-intensive works that provide income generation opportunities and temporary employment while improving basic services in disadvantaged communities; and the delivery of community services involving government, nongovernmental organizations (NGOs), and communities through subprojects that will serve as models for larger sectoral operations. The second component will deliver credit, technical assistance, and training to microenterprises through NGOs and to banks and other financial intermediaries in order to assist the latter in developing a portfolio of small enterprise loans. The third component will develop capacity in NGOs, community groups, and the private sector for involvement in small-scale development activities; help strengthen training, procurement, and accounting capacity; and monitor poverty nationwide.

Public Works Project

The overall objective of the P...ublic Works Project is to mitigate potential adverse effects of economic adjustment. Specific objective is to generate an estimated 75,000-96,000 work-months of employment at a cost range of US$260 to US$350 per work-month. The project will also help deliver much needed infrastructure services, improve environmental and sanitary conditions and enhance community participation in preparing and implementing sub-projects for civil works. The project will comprise the following components: 1) small scale civil works in areas with above average unemployment rates for rehabilitation and/or extension in the following sectors: waste management, urban development, rural development soil conservation, and health and education; 2) development of a project management capability comprising a steering committee (SC) and a project management unit (PMU) to be funded as the executive arm of the SC; and 3) provision of technical assistance services for: (a) the establishment of the PMU; (b) investment programming, monitoring and review; (c) sub-project preparation and implementation; (d) support to local consulting and construction companies; and (e) auditing.

The development objectives of ...the Health Reform Support Project (HRSP) are to: 1) increase access for women and children to a package of integrated maternal and child health services provided in district hospitals and health centers in selected districts; and 2) to improve the effectiveness of national public health programs and resource allocation within the public health sector in Yemen. There are three main project components. The first component strengthens health services delivery at the district level, as part of the policy of the Ministry of Public Health and Population to operationalize a District Health System (DHS) by upgrading district hospitals and health centers in eight districts and ensuring the effective delivery of a package of integrated maternal and child (PIMAC) health services together with supporting the decentralization of resource management. The second component improves the effectiveness of public health programs by strengthening two critical programs, namely, malaria and health education. It finances civil works for minor renovations of referral laboratories, proivides equipment and supplies for laboratories and vector control and disease surveillance, computers and vehicles, emergency stocks of essential drugs and insecticide treated materials, staff training, etc. The third component supports human resources development in the areas of financial analysis and health systems management.

The Social Investment Fund for... Zambia (ZAMSIF) supports, as part of the core Country Assistance Strategy (CAS), two main strategic objectives of the Government of Zambia: 1) decentralization and empowering local authorities to improve governance and efficiency in service delivery; and 2) increasing access to basic social services through direct poverty interventions. There are four main project components. The Community Investment Fund component consists of community-based small projects including social mobilization, infrastructure, improved natural resources management, service delivery activities, and capacity building for communities. The second component establishes a Direct Investment Fund, comprising larger projects benefiting more than one community to be managed and executed by district authorities; and builds capacity and training for local governments and administrations. The third component, Poverty Monitoring and Analysis, funds data collection and training and capacity building on financial, technical, and institutional issues; and sponsors workshops and information dissemination. The fourth component finances institutional support for ZAMSIF, supplying administration and operating costs, assessments, poverty maps, training, and capacity building.
