The operation supports the gov...ernment’s system for the delivery of basic services, which
is supported by the Federal Government through the IGFT to the woredas—Ethiopia’s main
instrument to finance decentralized service delivery. Under this program of support, the woredas
are responsible for the delivery of basic education, primary health care, agriculture extension,
water supply, and rural road maintenance. The IGFT or block grant mechanism is primarily
focused on recurrent expenditure: salaries and operational and maintenance costs. Approximately
85 percent of local-level allocations finance recurrent expenditures, the majority of which are for
the salaries of front-line service providers, for example, teachers, agricultural development

The objectives of the Particip...atory Health, Population, and Nutrition Project for the Gambia are to improve quality in reproductive health services, in infant and child health services, in nutrition services for women of childbearing age, infants, and children; and in management and implementation of a family health program. There are four components. The first helps improve maternal health services, prevents unwanted pregnancies, and prevents and controls STDs/HIV (sexually transmitted diseases and the human immunodeficiency virus). The second component aids in integrating vertical programs to combat childhood diseases through introducing and implementing the integrated management of childhood illnesses (IMCI) approach. The third component supports nutrition policy formulation and institutional strengthening; as well as community and micronutrient approaches to improve the nutritional status of women and young children. The fourth component supports the management and implementation of a family health program by capacity building and policy development, upgrading and maintaining health infrastructure--rebuilding selected primary health care facilities, establishing a blood supply system, procuring equipment, maintenance support, a local fund initiative fund, and project management. Capacity will be built in the areas of cost recovery; information, education, and communication; monitoring and evaluation; and research application.

The project will focus on 1) the sustainable provision of adequate public infrastructure and services; and 2) the development of a lasting, enabling environment for municipal investment financing and for employment creation and income generation. The project has three main components: a) The investment component will concern subprojects of infrastructure or public facilities in urban areas, either to be built or rehabilitated. This component will also comprise the establishment of an area-wide, solid waste collection and disposal system. b) The capacity building component will finance i) training, technical assistance and support for the government, ii) training for local consultants and contractors, and iii) support for Gabian Agency for Management of Public Works. c) The studies component will benefit the local government authorities (LGAs) by getting physical and financial management audits ; Greater Banjul Area (GBA) by defining institutional and financial framework of a solid waste system; Banjul City Council (BCC) by carrying out an engineering study of the rehabilitation of a storm water drainage and sanitation system; and urban sector by conducting a feasibility study to create a Municipal Development Fund.

The overall objective of the p...roject is to improve health and nutrition outcomes among
women and children by focusing on the least served Regions. The proposed changes will
support the long term objectives of reducing maternal and child mortality and undernutrition and
thereby contribute to the attainment of MDGs 1c, 4 and 5. Thus, the AF will contribute to the
Health, Nutrition and Population Global Practice goal of ending preventable deaths and disability
through Universal Health Coverage. Summary of Proposed Changes
The proposed AF would help finance the costs associated with scaled-up activities to enhance the
impact of the project and new activities included as part of project restructuring in response to a
looming food and nutrition security crisis as a result of poor and erratic rainfall in 2014. The
Additional Financing will:
a. Include issues of food security in the Social and Behavior Change Communication (SBCC)
Strategy and food security indicators in the community RBF contracts under Component 1;
b. Introduce household food and nutrition security interventions in the Baby Friendly
Community Initiative (BFCI) strategy in the current Project Regions (North Bank West, Upper
River and Central River Regions), and scale up the food security-enhanced BFCI and Primary
Health Care (PHC) additionally to North Bank East and Lower River Regions under Component
2;
c. Expand performance-based financing (PBF) for health and nutrition service delivery to the
North Bank East and Lower River Regions under Component 2; d. Replenish the funds allocated to Ebola preparedness and control under the restructuring of
December 2014; and
e. Increase scale and intensity of ongoing capacity building for service delivery and RBF due
to the geographic expansion and expansion of scope of activities under Component 3.
The Project Development Objective (PDO) of the MCNHRP remains unchanged and is to increase
the utilization of community nutrition and primary maternal and child health services in selected
regions in the Recipient’s territory. One outcome indicator, being the same as an intermediatelevel
Core indicator, has been deleted. One new PDO outcome indicator and three new
intermediate outcome indicators have been added to reflect the additional activities aimed at
enhancing household food and nutrition security.
The current project is being implemented in three regions, accounting for one third of the total
population with an expectation of reaching approximately 183,000 children under five and
180,000 women aged 15-49 years, yielding a total of 363,000 direct beneficiaries. The proposed
AF will reach these individuals with the additional food security interventions. In addition, the AF
will reach an additional 114,000 new direct beneficiaries (52,700 children under five and 61,800

through Universal Health Coverage. Summary of Proposed Changes
The proposed AF would help finance the costs associated with scaled-up activities to enhance the
impact of the project and new activities included as part of project restructuring in response to a
looming food and nutrition security crisis as a result of poor and erratic rainfall in 2014. The
Additional Financing will:
a. Include issues of food security in the Social and Behavior Change Communication (SBCC)
Strategy and food security indicators in the community RBF contracts under Component 1;
b. Introduce household food and nutrition security interventions in the Baby Friendly
Community Initiative (BFCI) strategy in the current Project Regions (North Bank West, Upper

e. Increase scale and intensity of ongoing capacity building for service delivery and RBF due
to the geographic expansion and expansion of scope of activities under Component 3.

The project has four components: 1) Rehabilitating small-scale infrastructure, about 400 microprojects, including civil works improving water supply, sanitation, solid waste management; and rehabilitating local clinics, primary schools, and feeder roads; 2) Providing equipment, vehicles, salaries, operating costs, training, technical assistance; and financing for monitoring and evaluating implementation; 3) Building the capacity of small contractors to prepare bids and execute work contracts; providing training and technical assistance to communities; financing contracts with individuals, firms, and nongovernmental organizations to provide others with technical assistance; and funding regional launch workshops; and 4) Financially supporting the statistics department's (SDS) budget for conducting living standards monitoring surveys (LSM); and offering it technical assistance.

The project is an integral part of the Government of Georgia's health sector strategy and builds upon the investments in preventive health care supported through the Bank-funded First Health Project. The project consists of three components. First, the PHC service delivery component will support a two-phased development of PHC services in urban and rural areas through rehabilitation of facilities and provision of basic medical and office equipment. It will establish PHC clinics and referral laboratories, conduct a PHC referral pilot, and enhance community-based information, education, and communication. Second, the institutional development component will support capacity building and institutional development training, policy framework, and regulatory environment of PHC. It will also support the management of PHC services through an integrated health management information system. Third, the project management support component will support project implementation.

The Reform Support Credit Proj...ect aims to provide initial assistance to the new Government of Georgia, in its efforts to define, and implement a program of economic, legal, and social reforms. The Project will support actions in the following areas: 1) improvement in governance and anti-corruption; 2) improvement in public finance management; and, 3) steps to resolve crucial issues in the energy sector. Actions taken in these areas will significantly contribute to the implementation of broader structural reforms over the medium term. At the same time, the project aims to define, in broad preliminary terms, a framework for future policy-based lending, and policy dialogue by the International Development Association (IDA), to support the country's medium-term economic growth, and poverty reduction efforts. This will further support fiscal stability and poverty reduction directly. First, the proposed project will address issues of governance, corruption, and the business environment, to facilitate new businesses, and create employment. Second, a stable fiscal environment should allow for the timely payment of salaries, pensions, and social expenditures. It will also foster Government links between the Economic Development and Poverty Reduction Framework programs and the budget process. Despite risks, the new political environment, the overwhelming electoral victories at the Presidential and Parliamentary elections, and the recent return of Adjara under the authority of the Government, will help ameliorate these risks, while lessons of past adjustment lending, in particular the weak government implementation capacity, have been carefully factored into the design: a narrow focus on priority issues, and the requirement for reform implementation.

The World-Bank supported Mater...nal and Child Health and Nutrition Improvement Project (MCHNP) (Cr No.4563-GH) started implementation in
February 2015. About 23% of project funds have been disbursed by Ghana Health Service (GHS) to the regions to enable service delivery at the
Community-Based Health Planning and Services (CHPS) level. The funds released have facilitated outreach services in the communities, home visits
to follow up on cases, durbars (community level meetings on health-related topics), meetings of community health committees and service delivery in
over 2,400 CHPS communities. The funds have also been used to support capacity building and enhance supportive supervision of the CHPS zones by
Sub District, District and Regional level officials in the country. Community-based Performance Based Financing (cPBF) has been launched in 8 pilot
districts in the Upper East, Upper West, Northern and Volta Regions to test the model of financial motivation as a tool for self-directed service delivery
improvement. An impact evaluation baseline has been concluded and contracts have been issued for service delivery to start.
Procurement and capacity building have been initiated under the project to enhance availability of logistics and personnel. This will include the provision
of vehicles, anthropometrics equipment, record books and the training of all community, Sub District, District and Regional level Health Teams. The
