The Integrated Regional Health... Development Project comprises the establishment of health regions at Jinhua, Jiujiang, and Baoji to experiment with new ways to improve capability for planning and delivery of health services by establishing policy criteria and systems for financing investment in the regions and supporting investments in the provincial capitals. The regional and provincial project components are: (i) health planning, management and reform; (ii) disease prevention and surveillance; (iii) health education; (iv) maternal and child health services; (v) emergency services; (vi) hospital services; (vii) rehabilitative medical services; (viii) medical education and training; and (ix) equipment management. Medical education, health education, and emergency service investments at the provincial level will receive additional support beyond that needed to just support the project region. A national component to strengthen the capability of the Ministry of Public Health to guide, assess and replicate the project and to begin to deal with issues which transcend the provincial and local levels is also being supported by the project.

Rural Health Workers Developme...nt Project

The goal of the project is to ...improve the quality of rural health manpower, thereby contributing to better quality health services and an improved health status of the rural poor in the six provinces. The project has three province-specific components and one central component. The Health Workers Planning component will strengthen the planning capability at the national, provincial and local levels. This component will define the tasks to be done to solve health problems and will identify the demand, the requirements and the utilization for all health worker categories. The Health Workers Training component will retrain large numbers of minimally trained rural health workers, train additional workers for underserved areas, and strengthen the training capability at the provincial, prefectural and county levels. Technical assistance will be provided for developing and implementing community oriented curricula, new teaching and learning methods, teacher training in pedagogy, integration of theory and practice, and evaluation of changes and innovations. The Rural Health Services Management component will improve the working conditions of rural health workers, develop alternative means for mobilizing financial resources to support rural health care delivery, for organizing and managing rural health services, and for compensating and stimulating rural health workers to emphasize preventive care; and provide support systems for supervising these workers effectively. A small Central Component will augment the institutional capacity of the Ministry of Public Health (MOPH) for coordinating and supporting project implementation activities. It will strengthen MOPH's capability to carry out its national mandate in planning and policy formulation, provide technical assistance to the project provinces, and evaluate project activities in order to disseminate the experience gained to the rest of the country.

This will be accomplished through policy and institutional improvements and by implementing pilot programs, including surveillance, staff training, health education and other health promotion interventions, at selected locations. The project comprises three main components: 1) immunization, which consists of four sub-components: (a) cold chain improvement; (b) health workers' training; (c) management and surveillance; and (d) information, education and communication; 2) health promotion, which will comprise the following sub-components: (a) institutional development and policy reform; (b) human resources development; (c) surveillance, especially behavioral risk factor surveillance; and (d) community-based interventions; and 3) the national-level programs.

The principal objective of the... Comprehensive Maternal and Child Health (MCH) project is to reduce maternal and child morbidity and mortality in the poorest areas of China. This will be achieved by: 1) improving the quality and effectiveness of MCH care at the primary and county levels; 2) increasing equitable access to and utilization of the basic care; and 3) improving MCH management. The project has four main components: basic health care delivery; health worker's training; management improvement; and a national-level component. Under the basic health care delivery component the project will improve services at the village, township, and county referral levels. The health worker's training component will support in-service training of all health workers concerned with MCH care at the village, township, and county levels in basic MCH, clinical care and management skills. The management improvement component will improve management of comprehensive MCH programs at different levels through training, supervisory support and improved management information. The national level component will oversee the overall management and monitoring of the project, carry out interdepartmental and interprovincial coordination, provide technical support to the provinces, prepare and appraise the second phase of this project and disseminate the project experiences to nonproject areas.

The principal objectives of th...e Southwest Poverty Reduction Project are to: 1) demonstrate the effectiveness of a focused, multisectoral rural development project approach to poverty reduction; 2) facilitate a market-friendly increase in labor mobility from the poor areas to better-off rural and rapidly growing urban areas; 3) upgrade poverty monitoring at the national and local levels; and 4) significantly reduce absolute poverty in 35 of the very poorest counties in southwestern China. The project will comprise the following six components: 1) social services, including increasing the access and quality of primary education and enhancing the availability and delivery of primary and curative health care; 2) labor mobility, through a voluntary system of enhanced rural labor mobility for the upland absolute poor; 3) rural infrastructure, including the labor-intensive construction of rural roads, safe drinking water supply systems, small-scale irrigation, agricultural drainage works, biogas digesters, and rural electrification; 4) land and farmer development, using menus of crop and livestock activities to increase upland agricultural productivity and reverse the trend of environmental degradation; 5) Township and Village Enterprise (TVE) development, through support for TVEs consisting of investment credit for labor-intensive, mineral, service and handicraft industries; and 6) institution building and poverty monitoring.

The Ninth Health Project for C...hina aims to 1) reduce maternal and child mortality and morbidity and improve child survival and development in the poorest areas of China; and 2) improve prevention and control of the Human Immunodeficiency Virus (HIV), Acquired Immune Deficiency Syndrome (AIDS), sexually transmitted diseases (STDs), and other blood-borne infections. There are three components. The first has five subcomponents: 1) It funds development and dissemination of clinical protocols and procedures, equipment, incremental costs, and health facility repair to help focus on providing systematic prenatal, obstetric, labor and delivery, and post-natal care; on improving well-child and new-born care; and on integrating sick child care for priority childhood illnesses. 2) It promotes pre-marital counseling and family health education materials. 3) It improves supervisory and monitoring support for maternal and child health services by funding management procedures and information systems. 4) It funds training including materials and equipment. 5) It provides a variety of poverty relief funds. The second component comprises four components, which improve and expand the policy environment to prevent and control STDs, improve interventions targeted at changing behavior and informing and educating the general public, improve the disease surveillance system, and improve management of blood transfusion services. The third component consists of project coordination and support.

The Yangtze Flood Emergency Re...habilitation Project aims to rebuild social and economic infrastructure damaged by the devastating floods in Hubei, Hunan, and Jiangxi provinces, thereby rapidly restoring economic production and social services. In the project design, priority has been given to restoring and rebuilding the water supply and health facilities to prevent the spread of diseases after the floods, and roads to restore key access to the flooded areas so as to regenerate economic activities. The main components are restoration and reconstruction of roads, water supply systems, county and township secondary and primary schools, hospitals, health centers and clinics, and irrigation systems. The project also provides for each province to upgrade existing or developing flood forecasting, and simulation and dispatch systems, taking into account the recent flood experience.

The project development object...ive of the Health Project for the Comoros is to reduce mortality from common diseases by ensuring a better use of health facilities for the delivery of quality health care, and by organizing mosquito control activities to reduce the incidence of malaria. There are three components: 1) rehabilitating and equipping selected health facilities to facilitate the delivery of good quality health care to the population, which will also have a direct favorable impact on cost recovery by those facilities; 2) controlling malaria by improving the case management at all levels of the health service, educating the population for a better use of services and to improve case management at home, and preventive activities including mosquito control to reduce the incidence of the disease; and 3) institutional strengthening to facilitate and support the increased use of quality care by the population and to further implement health sector reforms, including a) training the staff of the public health and population ministry (MOH) and the health facilities staff in the delivery of quality health care, management, and accounting; b) studies to develop alternative mechanisms for health financing; and c) technical assistance, logistical support, and equipment for MOH's central and regional directorates and for PNAC (the agency in charge of marketing drugs).

The objectives of the Integrat...ed Health Services Development Project, within the framework of the government's National Health Development Plan, are to: 1) expand access to health services and improve their utilization; 2) make health services available on a sustainable basis; and 3) improve the ability of the Ministry of Public Health (MSP) to analyze health problems, and to formulate, monitor and evaluate policies, strategies and programs. The project components are as follows: 1) the minimum package of health services (PMA); 2) reproductive health; 3) health planning and management; and 4) information and health. The first two components - service delivery - include the following: (a) investment and start-up costs for PMA introduction in about 14 districts, including nutrition, pharmaceuticals, training, reproductive health, and information, education and communication services; and (b) an accelerated program of reproductive health services, including family planning and sexually transmitted diseases. The last two components - institutional strengthening activities - cover the following areas: (a) organization development of the MSP; 2) information systems, including epidemiological surveillance and continuous monitoring and feedback of user perspectives on services; 3) training of prescribers in use of generic drugs and strengthening of quality control laboratory services; 4) health care financing; 5) financial management; and 6) review of training programs and strengthening of personnel management.

This paper is for an additiona...l grant to the Democratic Republic of Congo (DRC) for the Emergency Multi-sector Rehabilitation and Reconstruction Project which was approved in June 2002. The additional grant would help finance the costs associated with completion of certain priority activities included in the original Project scope, which are subject to unanticipated cost overruns and financing gaps. The focus of the support will be for: (i) urgent rehabilitation of key elements of electricity sector infrastructure, and (ii) project execution and supervision across the infrastructure component of the Project in order to ensure that activities are completed by the Project closing date. As a result, the expected outcome of the proposed second Additional Financing is that: (i) the original development objectives of the electricity sector component continue to be achieved, namely to rehabilitate and reconstruct the electricity supply networks serving major cities, and (ii) that overall execution and supervision of the Project continues smoothly until Project completion.

As part of a continuing effort... to ensure a social safety net associated with economic reform, and in response to the adverse effects of the recent Gulf crisis, the proposed project will be the first phase of an overall program aimed at addressing the immediate and pressing needs of those most vulnerable to the reform process, in addition to facilitating the reintegration of Egyptian workers returning from Kuwait and Iraq. The project's main objective is to reinforce prospects of success of the reforms to be undertaken by the economic reform and structural adjustment program (ERSAP). In line with this objective, the project will support six core programs : 1) the provision of essential municipal services; 2) improvements to essential public transport services; 3) a community development program including improvements in basic and primary health care provision, immunization programs, and mother and child nutrition programs; 4) an enterprise development program; 5) a labor mobility program; and 6) an institutional development program.

The Third Social Fund for Deve...lopment Project's objective is to help create jobs and provide community infrastructure and services through labor intensive works. The project consists of two components. 1) Public works program funds small-scale public works programs in partnership with communities and local government, which are executed by small local private contractors. Projects funded include construction, rehabilitation and repair of irrigation and drainage canals, water supply and sewerage systems, rural roads and streets, and public buildings. Maintenance include canal cleaning, public buildings, and solid waste collection and disposal. 2) Community development program will support the development of community support infrastructure and services, in partnership with local groups, to reduce poverty and improve the quality of life, and to promote community and nongovernmental organization participation.

HEALTH IMPROVEMENT PROJECT

Health Improvement Project
|
The project has four major com...ponents: (i) Institutional strengthening of the Ministry of Health (MOH) to provide restructuring of the MOH services, training workshops and seminars for staff at all levels, health center reconstruction and MOH office rehabilitation works, furniture and medical equipment. Reforms aimed at management and financial autonomy for the main hospitals of Malabo and Bata are to be initiated in collaboration with French aid. (ii) Health strategy development and implementation establishing a management information system and a health planning unit in MOH. (iii) Human resource development to train health sector personnel and improve MOH's technical and management capacity at all levels of the health system. (iv) Strengthening of priority health programs to improve MOH's operational capabilities and testing of new approaches to health care delivery by strengthening field work in malaria control, mother and child health and family planning, sexually transmitted diseases, and by providing essential generic drugs, with a special focus on most vulnerable groups. The project finances the rehabilitation of four rural health centers in the remote rural districts of Micomiseng and Acurenam; equipment for the mother and child and family clinics in Malabo and Bata, and supplies of essential medical materials and drugs to all these facilities. Cost recovery is introduced through a differentiated fee structure.

To achieve these goals, ESRDF will strengthen community capacity in project identification, implementation, and maintenance. The project will also support the establishment of a Welfare Monitoring System to strengthen capacity to collect and analyze information related to causes, patterns, and incidence of poverty; and improve the basis for developing poverty reduction strategies, assessing the impact on the poor of economic policy reform, and evaluating the benefits of other poverty reductions interventions. The components of this project will be as follows: 1) construction and rehabilitation of social and economic infrastructure and improvement of related services including basic health and education, small-scale irrigation, urban sanitation and rural water supply; 2) welfare monitoring system; and 3) capacity building, training and research.
