Being one of the post-Soviet c...ountries, Armenia inherited many of the problems intrinsic to the health systems in the region. The country is engaged in reforming its health care system towards a primary care orientation and community approach. With this grant, Armenia became the first country in the region to receive direct assistance to improve its health system. The country aims to use the funding to strengthen and develop its health sector workforce, integrate palliative care into the national health system and implement a national blood safety strategy. The program supported by this grant seeks to revise the curricula of medical personnel in accordance with the current needs and standards and to incorporate modern approaches to TB and HIV/AIDS diagnostics, treatment, and management. Among other objectives, the program is tasked with capacity-building in procurement and supply of medicines and commodities for addressing priority diseases.

In the 1990s the government of... Cambodia introduced health system reforms to improve and extend primary health care through the implementation of a district health system. The overarching goal of the program financed by this Round 9 grant is to contribute to strengthen the national health system through the identification and strengthening of complementarities between interventions for the three diseases and across the health system to achieve efficiency gains. The program also aims to increase demand for health services at the community level; reduce maternal and newborn mortality by increasing uptake of antenatal care and delivery and newborn care via linkages with the HIV and AIDS program; and improve the availability and quality of blood supply. The program will be implemented under sector-wide management (SWiM), which has most of the characteristics of a sector-wide approach (SWAp) but does not require a common funding basket or the complete adoption of common implementation arrangements. Sector-wide management significantly increases opportunities for coordination of efforts and provides flexibility for the allocation of scarce resources. The program targets the Department of Planning and Health Information, the Ministry of Health, the Department of Drugs and Food, the Provincial Health Department, national and local nongovernmental organizations, pregnant women, migrants and rural populations.

Swaziland faces the challenge ...of inadequate health and community systems capacity to support an effective response that includes scaling-up and strengthening existing interventions. One of the most important factors that affects access to prevention, treatment and care services for HIV, tuberculosis (TB), malaria and other health programs in Swaziland is the production, attrition and deployment of health workers. A health workforce assessment conducted by the European Union in 2007, found that the overall number of medical personnel declined by nearly 13 percent over the preceding two years. These losses are particularly severe in the rural areas of Swaziland. Limited capacity of nursing training facilities, lengthy hiring processes, growing burden on existing staff and poor supervision in the regional areas are some of the key reasons for shortages of health staff. The cross-cutting HSS Program proposes to address these shortages through strengthening the health workforce, improving service delivery and enhancing monitoring and information systems. Staff shortages in the health sector will be addressed through improvement in recruitment and worker retention policies, health workforce wellness programs and the expansion of human resource capacity in the health sector. The Program will improve service delivery and accessibility by capacity building and decentralizing services such as HIV care and treatment, the provision of Antenatal Care services (ANC), Prevention of Mother to Child Transmission of HIV (PMTCT) programs, and TB diagnosis at the clinic level. Diagnostic services are currently concentrated in the national hospitals and diagnostic centers which are less accessible to vulnerable and rural populations. Moreover, the inability of the national health information system to routinely provide the data necessary for effective monitoring and evaluation of disease-specific programs has led to the development of parallel systems of data collection. The Program will focus on the creation of a common national database which will facilitate access to information and a harmonized monitoring and evaluation system. The integrated monitoring and evaluation system will also incorporate data generated by community-based and private sector monitoring and evaluation systems into the overall national health information system. These interventions will strengthen data collection from two sectors with historically weak monitoring and evaluation functions in Swaziland and add to the completeness of the national health information system.

Ethiopia, the largest country ...in Africa in terms of both size and population, is among the most affected by the HIV and AIDS pandemic, with an estimate of 2.2 million citizens currently living with HIV; it has the world's seventh highest burden of Tuberculosis (TB), and approximately 68 percent of the population is considered to be at risk of contracting Malaria. Although Ethiopia's policy and strategy are strong, the implementation is impeded by disease-specific and system-wide weaknesses which leave remote and high-risk populations under-served due to lack of management and leadership capacity, inadequate HR motivation and supportive supervision, weak delivery of health services to remote and pastoral populations, poor condition of community-based structures and organizations, and unsatisfactory drug procurement, quality control, storage and distribution systems. The Federal Ministry of Health's strategy for meeting HIV/AIDS, TB and Malaria program targets is by means of implementing Health Systems Strengthening (HSS) approaches that support broader health sector goals. The program aims to improve the health status of the Ethiopians by decreasing the burden of preventable morbidity and mortality. This is expected to be achieved by strengthening the health system to impact the three priority diseases including building management, service delivery, and supply management.

Indonesia's 238 million people... are spread over 17,000 islands grouped into 34 administrative provinces and then subdivided into 497 districts. Various aspects of the health system in Indonesia warrant strengthening. The disease part of the proposal attempts to address some of these including health workforces. The selection of cross cutting Health System Strengthening (HSS) interventions is guided in part by the priorities outlined in the national health sector strategy 2010-2014 and potential to complement other strategic initiatives aimed at eliminating bottlenecks in the delivery of primary health care (PHC) services, including those targeted at AIDS, tuberculosis (TB) and malaria. The goal of the Program is to contribute to reducing morbidity and mortality from infectious diseases including AIDS, TB and malaria. The Program will target 138 underserved and remote districts that are situated in 24 provinces and cover a total population of 42 million. Three strategies will be implemented; reinforcing the national health information system for better integration, data management and use for decision making; scaling-up the Sample Registration System (SRS) and promoting health system research; and improving pharmaceutical and supply chain management, drug safety and pharmacovigilance.This is a stand-alone single stream of funding (SSF) Round 10 HSS grant.

Uganda faces severe challenges..., which result in a multitude of issues reflecting poorly on the overall performance of the health sector. Exacerbating these challenges is the limited level of human resource development as well as availability of laboratory infrastructure and disease specific commodities. In line with the National HIV/AIDS Strategic Plan (2007/8-2012/13), the country-wide targeted Health System Strengthening (HSS) and community-systems strengthening interventions will provide essential equipment and supplies, as well as put emphasis on human and institutional capacity building of public, civil society, faith and community-based organizations in planning, implementation and management, including monitoring and evaluation of responses to disease-specific services as well as other complementary health services. These HSS interventions across all levels will benefit prevention and control efforts for HIV/AIDS as well as contribute to efforts that target Tuberculosis (TB) and malaria and the whole complex of primary health care services. The Program goal is to strengthen health and community systems to enable the delivery of a more responsive and accountable health sector response to these three diseases. Among others, the Program will target HIV/AIDS, TB and malaria patients; general population; Ministry of Health and; National Programs. The program activities relating to this HSS grant will be implemented by two Principal Recipients (PR) under dual-track financing.
