RECAP will focus on acute and ...protracted humanitarian crises related to armed conflict, natural disasters, forced migration, and epidemics. Humanitarian crises and their impact on health and related sectors can pose major obstacles to international development and the achieving the SDGs. Effective decision-making by humanitarian actors is essential in humanitarian response. Accurate and timely information and evidence is essential in achieving effective decision-making. For example, through setting evidence-based standards and performance measurement against those standards, assessing needs and effectiveness in meeting those needs, ensuring value for money, and supporting accountability to recipients of humanitarian programmes and those programmes. Yet current interventions are impeded by limited data, capacity, guidance, and ethical preparedness with consequent implications for accountability. Work on protection is central to supporting accountability through seeking to ensure authorities and other humanitarian actors respect their obligations and the rights of individuals and vulnerable populations. RECAP's vision is to strengthen research capacity and capability to generate knowledge on how to improve decision-making and accountability to help support preparedness and response to humanitarian crises. RECAP will focus on the sectors of health and protection through overlapping research and capacity building activities. For the research, there are 6 interconnected, interdisciplinary research Objectives/work-packages. These research objectives will also act as platforms for capacity building through applied research methods training. The capacity building activities will be realised through 4 capacity building Objectives on: training on specific research methods linked to the RECAP research, building institutional capacity for a sustainable and supportive research environment, supporting individual researcher capability and career progression, and building UK capability in multi-disciplinary research on health and protection in humanitarian crises. It will be a four year programme. RECAP's strategy to grow research capacity and capability is to establish a unique network of leading academic and non-governmental organisations (NGOs) working on health and protection in the context of humanitarian crises. RECAP will be led by the London School of Hygiene and Tropical Medicine (LSHTM) and includes the University of Sierra Leone (USL), the American University of Beirut (AUB), the Refugee Law Initiative at the School of Advanced Study (SAS) in London University, Oxford University, and the London School of Economics and Political Science (LSE). NGOs are the front line actors in humanitarian preparedness and response and so are vital to RECAP. The RECAP NGO partners are Médecins Sans Frontières (MSF), the International Rescue Committee (IRC), Save the Children UK, and BRAC. Schizophrenia is a severe long...-term mental health condition. It causes a range of different psychological symptoms including hallucinations (hearing or seeing things that don't exist), delusions (unusual beliefs not based on reality), muddled thoughts based on hallucinations or delusions and changes in behaviour. Schizophrenia is a common cause of long-term disability in the 15-49-year-old group and whilst effective treatments are available, in developing countries around two thirds of patients receive no treatment. This lack of treatment, commonly known as the 'treatment gap' arises due to factors including poor treatment adherence, a lack of primary care involvement and poor access treatments. We have previously reported a new approach (Supervised Treatment in Out-Patients for Schizophrenia (STOPS)) that resulted in improved treatment adherence and functioning in a resource poor setting in Khyber Pakhtunkhwa (KP), Pakistan. In this study we trained and supported relatives and family members to monitor medication taking, building on the success of a similar approach used for patients with tuberculosis. We now aim to implement and evaluate a 'scaled-up' version of STOPS (STOPS+) which is based on the World Health Organisation mental health guidelines and maintain treatment adherence with the help of family members and text message reminders. We will work with primary care physicians and multipurpose primary are technicians to task shift care, under the supervision of mental health experts. The study has 3 phases: PHASE 1: PRE-IMPLEMENTATION PHASE involving community engagement, modification of STOP to STOPS+ (involving patient, healthcare and wider community partners) and the identification of potential study participants. PHASE 2: IMPLEMENTATION OF STOPS+ IN PRIMARY CARE SETTINGS USING A CLUSTER TRIAL DESIGN to determine the clinical and cost-effectiveness. 24 primary care centres in district Peshawar, KP will be randomised to deliver either STOPS+ or Enhanced Treatment As Usual (ETAU). We will recruit 526 patients (263 in each arm) suffering from Schizophrenia or schizoaffective disorder based on the International Classification of Diseases (ICD)-10 criteria. The outcomes of interest will include Global Assessment of Functioning and adherence to treatment regimen. PHASE 3: EVALUATION OF THE IMPLEMENTATION IN REAL WORLD SETTING will be measured using the standardised World Health Organization's Assessment Instrument for Mental Health Systems (WHO-AIMS), economic evaluation, secondary data from the trial and utilisation of primary health care, and in depth interview study involving all participants. If successful this trial will improve schizophrenia care, improving lives for patients and their families. This work will provide important information for policy makers and set the foundations for reducing disability and improving long term patient outcomes.

This project will build partne...rships towards developing selected health system interventions to address the impact of urban violence on youth access to health services in South Africa and Brazil. The objectives are: 1) Build collaborations with South Africa and Brazil, as countries affected by urban violence particularly in low-income neighbourhoods 2) Carry out a scoping review of the evidence on the influence of urban violence on treatment seeking and health systems and interventions to address this 3) Engage with stakeholders in local health services and socio-economic development, and youth in affected communities, to increase understanding of health systems issues and interventions to decrease the impact of urban violence on youth healthcare access 4) Through collaborative partnerships, work towards developing context appropriate specific health systems micro and meso interventions in South Africa and Brazil for proof-of-concept testing in a future proposal. The outputs from this activity can be applied/adapted for other DAC-list country/countries and therefore would benefit a wider geographical region.

Improvement of the services of... the staff through information and training, also advice for the administrative body of the station, especially for accounting, contribution to better hygiene and COVID-19 infection protection, repair of the drinking water supply through wells, pumps and pipe rehabilitation, renovation of the building and procurement of examination and treatment material and of simple furniture and beds of metal. Power supply guarantee through solar systems for drug cooling. The measure will provide adequate health care and obstetrics for approx. 1,000 women secured. That corresponds to the enormously increased demand. A further 1,600 women indirectly benefit from the measures through community-wide information campaigns on health and hygiene practices, including COVID-19. In addition, vaccination campaigns are carried out, for which a simple motorcycle is procured for transport.The responsible municipal administration and the village community actively support the station and without remuneration. A trainer is hired for the implementation, who also oversees the implementation.

Tuberculosis (TB) is a chronic... infectious disease. Every year approximately 10 million people fall ill with TB and 1.6 million die from it. TB often coexists with other chronic diseases including chronic non-infectious conditions (e.g. lung disease and diabetes), mental health conditions (e.g. depression), and chronic infectious diseases (e.g. HIV/AIDS). This coexistence (or multimorbidity) is common because these illnesses share similar risk factors. For example, smoking can lead to both chronic lung disease and TB. Having one condition also increases the risk of another, for example, people with diabetes have poor defences against infections making them vulnerable to developing TB. Coexisting conditions also interact adversely with each other, worsening health, quality of life and survival for each, compared with having just one condition alone. Healthcare services for people with TB should, therefore, offer prevention, detection and treatment of other coexisting chronic illnesses. Traditionally, however, services, including TB programmes, typically focus only on a single condition. Our overall goal is to support TB programmes in low and middle-income countries to improve the prevention, screening, and management of the most common and most devastating chronic illnesses that coexist in people with TB. This approach may improve quality of life and survival for individuals and also help control the rapidly increasing epidemics of chronic conditions. We focus on conditions coexisting with TB due to their common occurrence and their significant negative health consequences. We think that there may be opportunities to integrate care for multimorbidity within relatively well-resourced and functioning TB programmes (compared with other parts of the health system). Knowledge gained about managing multimorbidity could also be transferable to other services. Multimorbidity can manifest in many patterns or combinations of conditions (clusters). In this 'seed-funding' project, we will first estimate how common are the various co-existing illnesses, and identify the clusters that contribute the greatest burden in terms of shortening life expectancy, causing ill health and incurring healthcare costs for people with TB. We will then explore potential interventions for these conditions. To do this, we will review published academic literature on coexisting chronic conditions in people with TB and analyse data from a large world health survey. This will allow us to estimate how common is multimorbidity and how it affects the health, quality of life and life expectancy of people with TB. It will also help to identify the clusters of conditions that cause the most burden for patients, and to understand the sorts of interventions that are effective for these target conditions, which may be feasible to deliver integrated within TB programmes. Die FZ-Maßnahme dient der Unt...erstützung der nepalesischen Regierung bei der Bewältigung der Covid-19 Pandemie sowie der Implementierung des neuen nationalen Gesundheitsstrategie (NHSS) 2015/2016- 2020/2021 in Form einer Ko-Finanzierung über einen gemeinsamen, durch internationale Geber finanzierten Korbmechanismus.
Für Nepal als Least Developed Country bedeutet die Corona-Pandemie und der Versuch deren Eindämmung notgedrungen eine große - auch finanzielle - Herausforderung für das Gesundheitssystem, zu deren Bewältigung Deutschland mit der Erhöhung seiner Beteiligung in Höhe von 10 Mio. EUR an der laufenden Korbfinanzierung zur Implementierung der Gesundheitsstrategie beitragen möchte. Mit Hilfe dieser Maßnahme wird in Ergänzung der Beiträge durch andere Geber der Mehrbedarf auf nepalesischer Seite reduziert und damit ein sinnvoller Beitrag zur Umsetzung der nepalesischen Gesundheitsstrategie und zur Verbesserung des Gesundheitszustands der Gesamtbevölkerung Nepals durch ein verantwortungsvolles und gerechtes Gesundheitssystem erreicht.
Die Zielgruppe des FZ-Vorhabens ist identisch mit der Zielgruppe der Gesundheitsstrategie NHSS und umfasst die Gesamtbevölkerung Nepals, insbesondere überproportional vulnerable Gruppen, wie chronisch Kranke, Arme und ältere Personen. Der an die Korbfinanzierung geknüpfte Politikdialog gewährleistet eine hohe Mitgestaltung bei der Formulierung der Sektorpolitik und Umsetzung von Reformen und trägt so zu einer umfassenden Stärkung des nepalesischen Gesundheitssystems bei. Die FZ-Maßnahme leistet einen entwicklungspolitisch wichtigen Beitrag zur Verbesserung der Gesundheitssituation der Bevölkerung, zur Geberharmonisierung sowie zur Eindämmung der Covid-19 Pandemie.
