Deaths from trauma, especially... due to road traffic collisions, are prevalent in lower and middle income countries (LMICs), trauma causes a substantial burden of disability in survivors. Equitable access to quality health care is essential to treat trauma patients. Expanding on our preliminary work in Rwanda, this project will: - Convene workshops with providers and policy makers to delineate barriers, knowledge, and priorities in access to care in three contrasting countries - Map access to trauma care as a complex health system problem - Collect data from patients on their experiences of barriers to and quality of care - Build research capacity in partner countries to enable further work - Develop a network of trauma researchers - Convene a meeting of investigators, patient representatives, and policy makers to discuss findings and, in combination with ongoing studies and literature review results, identify and agree upon priorities for a programme for future research

Essential technologies could i...mprove quality and outcomes of care if they are successfully adopted but benefits in poorer countries are threatened by workforce shortages and a failure to consider all the obstacles to their use up and down the system. Thus, many technologies fail, for example it is reported there are over 200,000 failed mHealth apps. This wastes valuable resources, or can even worsen care quality. Our proposal addresses this key emerging issue. In doing this we address the priority areas of Quality of Care, including families' experiences of care, Health Workforce Management and Planning, as we explore how technologies can support or undermine routine work in settings with few staff, and Integrating Health Services & Improved Data Quality and Use, by exploring how information technologies might best support post-discharge care for sick babies.

It is a United Nations Sustain...able Development Goal to achieve universal health coverage with access to quality healthcare for all. Our network plans to evaluate the contribution to this goal of mobile consulting (mConsulting). This is when patients consult with health providers about their health using digital communication. Our concern is remote, marginalised communities in Africa that have difficulty accessing quality health care. The digital infrastructure of Africa is its best infrastructure - much better than the roads, and most people own a mobile phone. We want to test whether using digital infrastructure for mConsulting improves access to quality healthcare for remote, marginalised communities and whether this is sustainable. mConsulting needs to be affordable to the community and for providers. However, improved healthcare can improve the health of the community and so increase economic activity and use of mConsulting can stimulate investment in digital infrastructure benefiting the community. Commercial companies are delivering mConsulting services in Africa. Currently they target urban communities but could extend their reach to remote communities. In addition to good mobile signal and sustainable business models they need community acceptance and integration with local providers for certain diagnostic tests and treatments. Existing health services (public or private) can add mConsulting to their services e.g. a health worker using a mobile phone for consulting. This needs planning to ensure professional standards and patient acceptability. We are academics/researchers, mConsulting providers and policy organisations. Four of the academic/research teams (Kenya, Nigeria, Tanzania and UK) have collaborated on research and research capacity building for ten years. We have extended our team to further organisations in our countries and in Uganda and Rwanda. Our expertise includes, technical, medical, nursing, education, social and behavioural science, health systems, economics and epidemiology. We aim to evaluate the impact of mConsulting on healthcare access and health outcomes for remote marginalised communities and explore how it can be optimally delivered. We will do this in varied contexts of East and West Africa so we learn what works well (or not) in different places, with different histories, policies and health systems. Our results will be relevant to diverse settings across Africa. To do this we will develop a research proposal for funding. We will first use the seed network funding to: 1. Extend and consolidate our networks to include all relevant and necessary people/organisations, 2. Work with remote community representatives, local healthcare providers, mConsulting providers, policy makers (and telecommunication providers where needed) to collaboratively identify plans for mConsulting including challenges to be overcome before implementation and evaluation, 3. The project involves two inter...related approaches: a) organising networking events & other in-depth stakeholder engagements to build an active research consortium of academics & practitioners focused on combating violence against women, & b) facilitating the co-creation of problem-solving digital innovations to be implemented & studied in a further phase of the project (post-October 2020). The primary intended beneficiaries of the work being women at risk of violence in South Africa a) The networking events & stakeholder engagements with 10 network partners (NGOs & tech companies) will explore 4 interrelated dimensions: - Understanding women's rights in a digital context: By means of inclusive stakeholder dialogues taking place at a Roundtable & a Public Forum, we will explore how women's rights are constructed in online spaces & collective movements in South Africa, in order to identify the tacit roots of discrimination & victimization, & to understand how these may be reproduced, but could also be addressed, in digital settings. - Access: Through focus groups, we will identify the primary local barriers to access that undermines the agency of women in realising their rights & protecting themselves against violence & intimidation. We aim to understand both practical impediments like access to electricity, affordable data & instruments, as well as the more tacit cultural, social & individual factors that could undermine active participation in digital spaces. - Participation: The potential of digital platforms to enable easier access to government services, especially around healthcare & justice, can play an important role in upholding women's rights, protecting them against violence & intimidation, & offering avenues for recourse & support. We will explore how the participation of women in relevant policy platforms could be strengthened & streamlined, as well as what government & corporate policy changes may be needed to protect women's rights & voices online. - Speak-out & Action: Studying the ways in which women communicate in digital spaces will allow us to identify the factors that make women feel safe, supported & empowered to speak out against violence & discrimination. The Affect Lab brings detailed expertise on participation in online chat forums, & one of their post-doctoral researchers will study engagements on a specific forum focused on violence against women that will be created within the Values Chat APP, a mobile tool which was designed & launched in a previous round of funding. b) Our engagements with our business & NGO partners will go beyond networking events towards the co-creation of potential digital innovations. The goal would be to understand these organisations' current business models, services & approaches to digitalization. The potential harm that increa...sing levels of antibacterial resistance (ABR) will have on human health is vast, as a consequence the effects of this will be felt across society and at the economic level. It is predicted that by 2050, 10 million lives per year and a cumulative 100 trillion US Dollars of economic output may be lost worldwide. In order to address this looming problem a co-ordinated global response is required to try and halt the rise of ABR. Efforts are underway to tackle the rise in resistance, however agenda-setting is dominated by High Income Countries (HICs) and may not reflect priorities or needs of Low and Middle Income Countries (LMICs), where the levels of resistance and also the types of disease caused by bacteria may be different. One of the most vulnerable regions to the increase in antibiotic resistance is Africa where, in comparison to other regions of the world, the burden of infectious diseases is highest. The economic, cultural and ethnic diversity of Africa mean that the problems surrounding ABR across African countries are likely be distinct from other regions of the world, and therefore require regional solutions and approaches. For example, the availability of antibiotics can be patchy, and the routes of access to antibiotics are variable (including traditional healers, public and private medical practitioners and over-the- counter antibiotic access). Social, cultural and lifestyle drivers of ABR in Africa also have specific features - closer communal living in cities with variable water/sanitation, and closer animal husbandry in rural communities. This project aims to address ABR in Africa and fill the gaps in knowledge. The research will target three main areas that comprise the problem: the bacteria that are antibiotic resistance and cause disease, the amount of antibiotics that are used to treat disease, and the behaviour of humans that governs how antibiotics are used and supplied. The three elements incorporate epidemiological, economic, cultural and societal factors that interact and contribute to the problem. Bringing together research covering these 3 elements will provide a holistic view of ABR in East Africa. The project will establish a surveillance network across Uganda, Kenya and Tanzania that is comprised of sentinel sites that monitors and characterized the ABR bacteria that cause disease at that location, maps the use of antibiotics in the sentinel hospitals and the surrounding communities, and captures the behaviour and attitudes of humans that is responsible for their use in these setting. The project also recognises antibiotics are not exclusively used for human medicine, therefore our surveillance will also cover the use of antibiotics in veterinary medicine in the linked communities to provide a 'One Health' view. Rheumatic heart disease (RHD) ...is a disease of poverty affecting the poorest countries in the world, with Sub-Saharan Africa bearing a heavy toll. In 2015, over 33 millions clinical cases and >300,000 deaths worldwide were estimated to be due to RHD. RHD is caused by an excessive immune reaction ('rheumatic fever' (ARF)) to throat or skin infections by Group A Streptococcus (GAS) which further damages the heart valves. Complications include heart failure and stroke that result in premature death, usually before 25 years old, and women with RHD are at higher risk of complication during delivery. RHD patients with early symptoms can be treated with monthly injections of benzathin penicillin G which stops the evolution of the heart lesions. However, in many countries, including The Gambia, RHD is usually diagnosed in patients attending health facilities at a late stage, with heart failure or other complications, when the efficacy of penicilllin treatment to prevent RHD adverse outcomes is limited. Ideally, penicillin treatment should be started as early as possible although its benefits in individuals with latent RHD, i.e. with cardiac lesions diagnosed by echocardiography but without symptoms, is unknown. Latent RHD represents a potential window of opportunity for early treatment with penicillin to prevent its progression towards overt clinical disease. This intervention needs to be evaluated with a large randomized clinical trial that will recruit hundreds of individuals with latent RHD. Therefore, considering the prevalence of latent RHD is probably 0.5-1% in children (up to 3% in young adults), the study team would need to rapidly screen thousands of individuals to attain the required sample size within a reasonable time period. Handheld echocardiography (HHEC) by non expert users has been proved as a reliable tool to detect RHD and is considered the best option for mass screening. This Trial Development Grant (TDG) will determine i) the prevalence of RHD (latent- and symptomatic) among school-aged children and young adults in peri-urban areas of The Gambia, Senegal and Nigeria, ii) whether non-experts users in the three study sites can be adequately trained for RHD screening using HHEC, and iii) the feasibility and acceptability of community-based screening by HHEC in the three study sites. After HHEC training (3 months), 3,000 children and 3,000 adults per country will be screened and all suspected RHD cases will be confirmed by standard EC before referral to the nearest teaching hospital for adequate management. The feasibility and acceptability of HHEC screenings will be evaluated as an ongoing process through early and regular engagement with participants and all stakeholders involved. Therefore, this TGD will provide useful information on RHD prevalence (both latent and symptomaticin different age groups) and the feasibility and acceptability of mass screening with HHEC, allowing to adapt accordingly the future trial design in the three W

This project aims to build a r...esearch network on Antibiotic Resistance (ABR) in the Nile Valley. ABR is a growing global concern, with an impact not only on the lives of patients, but also presents a massive economic burden on healthcare systems. This network (PRESAR) will to bring together researchers from diverse backgrounds to address ABR from different perspectives. We will work to seek solutions to decrease the burden of infectious diseases, thereby having a positive impact on the welfare needs and subsequently economic development. The project is underpinned by promoting and maintaining a strong partnership between the UK and African academic communities, enhancing capacity to becoming world leaders in ABR research.The overall aim is to build capacity, address multiple aspects of ABR transmission, and help create locally-relevant policies that will eventually reduce the disease burden. We aim to deliver impact regionally in Sudan, Ethiopia and Egypt, and globally by encouraging engagement, communication and dissemination of findings to the global research community.'
