In this project, we would like... to develop and evaluate a complex intervention for reducing deaths in mothers and babies, by improving birth planning and uptake of contraception after delivery in Uganda. Every year, thousands of women and children in Uganda die as a result of problems during pregnancy and childbirth. Our research in 2011-2015 showed that about 20% of these deaths could have been avoided by use of contraception, to prevent unwanted pregnancies. Another 25% or so could be prevented by ensuring mothers go to the most appropriate health facility for their delivery. In Uganda, many women will not take these decisions without the approval and support of their husband, 43% of women still do not give birth in a health facility. Even of those who do deliver in a health facility, many high-risk women deliver in a small health centre which is unable to offer emergency surgery if needed. If a woman does deliver in a health facility, although she may never return for postnatal check-ups, there is a key opportunity to provide contraception immediately after delivery. Although several projects have trained health workers to provide the contraceptive coil immediately after delivery, few women go home with a method of family planning in Uganda. To understand why, our team interviewed 80 women, men and health workers in Uganda in 2015. Many women wanted time to discuss contraception with their husband (who was often absent at the time of delivery) and to recover from the birth. Women feared side-effects such as bleeding, and the impact these would have on their marriage, men were also concerned about side-effects and the cost of managing these. Many people we interviewed suggested that more awareness-raising on family planning was needed. Several people also suggested that men should accompany their wives to antenatal clinics (which rarely happens in Uganda) to discuss future family planning with a health worker. This would also be an ideal opportunity to discuss the birth plan with the couple, and to gain the husband's understanding and cooperation in deciding on the safest place for the delivery. Thanks to a MRC-AHRC partnership development award, we have now developed health education documentaries and dramas on contraception, in two local Ugandan languages. The films have been edited to incorporate feedback from local audiences, and are now ready to be evaluated on a larger scale. However, it is likely that films alone would have a limited impact, unless they are followed up with counselling of couples to address their specific questions and concerns. We have sought feedback on the concept of antenatal couples' counselling in 18 focus groups with women, men and health workers in central and southern Uganda. Background: Globally 15 millio...n babies are born preterm or 'born too soon' each year. Nearly two-thirds of such births happen in sub-Saharan Africa and Asia. Premature birth is the cause for a third of all deaths in the first month of life. In Kenya, over 200,000 babies are 'born too soon' and despite recent reductions, deaths in the first month of life remain high at 20 per 1000, 7 times higher than in the UK. Kenya suffer from a shortage of resources including lack of trained neonatal doctors and nurses. Poor record keeping in hospital causes delays in recognising poorly babies, in providing treatment and leads to poor outcomes. Early warning scores (EWS) are a streamlined way to monitor patients and prompt treatment to prevent deterioration that could lead to death. The 'Newborn Early Warning Track and Trigger' (NEWTT) used in the UK uses a simple paper-based form, which has a 'traffic-light system' to easily plot infants' vital signs (temperature, heart rate and respiratory rate). This promptly alerts healthcare professionals to take actions if signs are in the red or amber zones. Closer monitoring of preterm infants with a system that more easily identifies sicker infants could help resource-limited staff to reduce deaths and illness in this very vulnerable group. Previous work: To understand how doctors and nurses care for preterm and low birth weight (LBW) infants in Kenya, we recently completed a study at a large hospital in Nairobi, Kenya. We collected data on 294 babies over 8 weeks and recorded vital signs on the NEWTT. We also spoke to 19 mothers and 20 stakeholders to explore their views of newborn care. The data were presented at a large meeting in Nairobi in July 2019 attended by around 80 people involved in care of preterm infants. The idea of implementing a EWS for preterm infants in Kenya was discussed and feedback was positive. They liked the simplicity of documentation using a traffic-light system for quick identification of high risk infants. Some barriers were discussed such as a recognition of need for training and resources such as nursing time and colour printed charts. Current study: This study aims to test how feasible and acceptable it is to implement an EWS in neonatal units in Kenya. We will implement the paper-based EWS in three neonatal units and ask staff to complete this for all preterm or LBW infants over a 4-week period. When a vital sign is entered onto one red or two amber trigger zones, they will alert a more senior member of staff to assess the infant and advise further action. The EWS will temporarily replace the current documents to avoid duplication of work. We will also conduct interviews a range of healthcare professionals involved in caring for preterm infants. A randomised controlled trial ...showed unequivocally that the Friendship Bench (FB) is the first effective psychological treatment programme in primary care to be tested so far in Sub-Saharan Africa. The auspicious results persuaded the government to scale-up the FB to 72 clinics in and around Harare about one year ago. Additional scaling-up has been planned for later this year. Up until now, there has not been an evaluation of the results of this scaling-up initiative. This study aims to evaluate the scaling-up so far but most importantly to identify the factors that have influenced the outcome of the implementation of the FB with a view to design an enhanced facilitation intervention to improve the implementation in clinics that have not performed so well. We will use two well-known implementation science frameworks (RE-AIM and CFIR) to guide our work. Whilst RE-AIM will provide us with a framework for the outcomes, CIFR will guide us through the process of exploring the 'core' factors that determine the success or failure of the implementation of the FB. We will start with an evaluation of what has been achieved so far using all the data that has been routinely collected during the scale-up effort until now. This will be supplemented with additional data captured through a survey and other qualitative methods. The results will allow us to evaluate RE-AIM outcomes and classify clinics according to their implementation outcomes. Subsequently we will conduct a workshop with the main stakeholders to discuss and agree on the CFIR framework to use for the second phase of exploration. The main aim of the second phase is to identify key or core variables deemed to be key predictors of the successful implementation of the FB. In order to do this we will focus on comparing low and high performing clinics to identify those factors that might explain implementation outcomes. During the second phase, most research will be of a qualitative nature, through focus groups and semi-structured interviews, except for small surveys and the collection of additional economic data. At the end of this phase, we will invite main stakeholders again to review our results and conclusions and to help us identify the components of an enhanced facilitation intervention that we will test in those clinics whose implementation achievements fall into the lowest quartile. We will conduct a hybrid type 3 study to ascertain if implementation outcomes improve four months after this facilitation takes place in comparison with control clinics. The information generated in this study is vital to continue the successful expansion of the FB and to consolidate the implementation currently underway. Besides it will provide essential information for other neighbouring countries in terms of using implementation science methodology as well as learning about potential problems in the route to implementation as well as possible approaches to improve their implementation efforts.

The Covid-19 pandemic has had ...significant adverse effects on university students, whose education and training has been severely disrupted and their social contacts and job prospects lost. Even before the pandemic around of a third of students would had had symptoms of depression, with students who are economically disadvantaged at greatest risk. Depressed students do less well academically which has negative impacts on their employment prospects and they are also less likely to follow health advice. Thus depressed students are particularly vulnerable to the health and economic impacts of the COVID-19 pandemic. This is a particular concern for students in less developed countries such as Zambia where access to mental health services is limited by the lack of resources and by the stigma associated with psychiatric illness. This study aims to address this cycle of disadvantage by providing targeted access to an effective, online treatment programme for depression (moodgym) to a 1000 students who identify themselves as having symptoms of depression. These students will be recruited from universities in Zambia, Malawi and Botswana: all countries identified as eligible for overseas development assistance. Moodgym is based on principles of cognitive behavioural therapy and aims to reduce the risk of depression by helping users to recognize and change those negative thoughts and behaviour patterns which can drive and sustain low mood. The 5 modules are particularly aimed at young people aged 15 to 25 and include exercises, practical assignments and quizzes. This study will investigate whether combining moodgym with a university-wide online COVID-19 prevention programme will improve students' mental health and enhance their ability to withstand the health and economic challenges of COVID-19. The online COVID-19 prevention program, adapted for each local context, will portray health-promotion behaviours such as social distancing and face coverings as a normal part of student life. We will collect feedback data from the prevention programme and survey data before and after the moodgym/ COVID-19 prevention programme intervention to look for improvements in depression, academic performance and COVID prevention behaviours and to check whether benefits are felt equally by men and women. We will also interview participants to try to understand how moodgyam helped them and to explore their feelings about the impact of COVID-19 on their mood and their studies. We also want to find out which factors are associated with improved mental health and academic outcomes so that we can ensure the intervention's sustainability and successful implementation in other less developed countries.

As the Syrian conflict enters ...its ninth year, 13.2 million Syrians are in need of health assistance. Displacement continues to be a major challenge for the country with 5.6 million refugees and 6.6 million internally displaced persons (IDPs) according to UNHCR as of April 2019. The conflict has caused a severe disruption in health services leading to a collapse of the health system in the most conflict-affected areas. Attacks on healthcare have been a major threat in Syria, in what has been described as a weaponisation of health care. The UN estimates that half of the health facilities in Syria are either only partially functional or destroyed. Physicians for Human Rights have documented 588 attacks on health facilities and 914 medical personnel killed between March 2011 and November 2019. Essential health services have been further disrupted by the increasing number of health professionals fleeing the country. This has left populations with limited access to healthcare leading to increased vulnerability to communicable and non-communicable diseases.Recently, there have been three distinctive territories in Syria corresponding to the different areas of control. About 60% of the Syrian territories, including the capital Damascus and most central and southern areas, are controlled by Assad regime. Kurdish armed groups, supported by the US, control around 25% of north east Syria, with a population of approximately 4 million. The remainder of opposition armed groups, supported by Turkey, control around 15% in the north west with a population of approximately 3.5 million. The health system in each territory developed different adaptation mechanisms to the conflict. For example, after the collapse of the health system in north west Syria, local medical networks - relying on limited local resources, the remanence of health infrastructure and equipment, and the humanitarian health resources - adopted a new bottom up approach to build a hybrid and kinetic health system that is currently functioning as an autonomous local health authority in the region. Whereas, the health system in north east Syria maintained a level of functionality throughout the conflict with a similar structure to the pre-conflict health system and some ties to the Damascus Ministry of Health.Little is understood about how contemporary conflicts shape emergent health 'systems'. It is important to study these different adaptation mechanisms and explore various strategies to strengthen these health systems and ways to integrate them into a national health system in what will now be the post conflict phase. Throughout the Syrian conflict, research has been focused largely on Syrian refugees in neighbouring countries and in Europe with very little focus on health issues within the country. Liberia Agricultural Upgrading..., Nutrition and Child Health Project (LAUNCH) is working to reduce chronic malnutrition levels in food insecure, vulnerable populations in Bong and Nimba Counties. They are working with smallholders to sustainably improve production practices, increase farm management skills, build resilience, improve infrastructure and expand economic opportunities through a value chain approach. The health and nutrition strategy focuses on the prevention of malnutrition, the early identification and treatment of acute malnutrition to prevent further deterioration of health/nutritional status, and the promotion of high-impact health and nutrition interventions at household, community and facility levels. Households receiving supplementary food rations are targeted for inclusion in the agricultural and livelihoods activities. The ration has been carefully designed to reduce malnutrition without craeting beneficiary dependencies and undermining a market approach. Education activities develop community capacities to support education and increase access by youth to livelihoods education that increases their employability.

LAUNCH is a
USAID/Food for Pea...ce (FFP) project,
managed by ACDI/VOCA in
collaboration with Project Concern ,
working in the Bong and Nimba
counties of Liberia to improve food
security and reduce chronic
malnutrition of vulnerable women and
children under five years old4. The
project aims to reduce chronic
malnutrition by providing food
supplementation and nutritional
information and improve maternal,
infant and young child feeding and
hygiene practices (breastfeeding). Each
month about 9,000 beneficiaries
participate in the food distribution, with
the number fluctuating by season.
Under LAUNCH there are food
distributions once a month, at a
designated point on a designated day.
Although this project is primarily
focused on nutrition, it does contain an
agriculture component. In addition, its
use of mobile data collection even
within its nutrition component is
relevant to agriculture development

This project will research a c...ritical but entirely neglected aspect of the prison system in Guyana, which certainly contributes to violence and instability: the definition, extent, experience and treatment of MNS disorders among inmates and the people who work with them. A UK-Guyana team will co-create new academic perspectives from Economic and Social History and Criminology, and use them to produce policy-relevant materials on mental health, cognitive impairment, addiction and substance abuse among prisoners and prison officers. The research will be shared with the project's partner, the Guyana Prison Service, which has an appetite for change and is seeking to effect transformations in prisoner well-being, health and education, during a time of unprecedented penal crisis. The project will not only model a new interdisciplinary way of working in this field, it will impact on prisoner and officer well-being and education, and on the rehabilitation of inmates. The project's perspective is historical, social and cultural. It covers the period from 1825, when the British opened the colony's first jail in Georgetown, to the present day, following Guyana's independence in 1966. It is rooted in the hypothesis that the existence of MNS disorders in jails today can be traced back to the British colonial period, and that they cannot be disconnected from the country's history as a sugar colony that employed and controlled indigenous people (Amerindians), enslaved Africans and indentured Indian labourers. Empire created particular forms of trauma, shaped demography and religious practice, and instituted patterns of population control including through institutionalisation. We seek to render this history actively part of the process of change, by connecting new historical work to new criminological research on Guyana since independence. We are especially interested in assessing and evaluating different definitions and management regimes for people afflicted with MNS disorders in Guyana's jails, among the colony/ country's multi-ethnic and multi-religious population of indigenous, African and Asian descent, and including women and juveniles. Guyana has serious resource constraints, in terms of GDP it is the second poorest country (after Haiti) in the southern hemisphere. Following the destruction of the country's largest jail during a prison riot in 2017, the challenges facing Guyana's prison system are unparalleled in the Caribbean region. This project is urgent and timely. The research team will conduct research on colonial-era and post-1966 archives and records, and conduct focus groups, workshop and interviews with prison personnel, communities living near prisons, and prisoners' families. The project seeks to enhance academic, practitioner and public understanding of MNS disorders in the jails context, build robust relationships between academics, practitioners and policy makers, and stimulate behavioural change among key actors.
