COVID-19 is challenging everyw...here because many of the measures to control the disease also produce negative effects. This is especially the case in Sub-Saharan Africa (SSA) where health systems are relatively under-resourced, where poverty rates are high, and where countries have to simultaneously deal with other health challenges such as those presented by HIV/AIDS, TB, diarrhoeal disease, malaria and malnutrition. Previous research conducted by ourselves has shown strong anecdotal evidence of health systems disruption and other unintended harms. Concerns include the effects of school closures, reduced household food income, worsening food security, reduced vaccination coverage rates and the effects of certain COVID-19 control measures having a worse impact on the health of girls and women. We also found countries adopting different approaches and actions to control COVID-19. Building upon these findings, we intend to: 1) describe and evaluate the impact of COVID-19 and COVID-19 control measures on health systems functioning, child health and women's health in Ghana, Tanzania, Uganda and Zimbabwe, 2) identify how COVID-19 control measures can be improved, and 3) enable the sharing of lessons and information across countries. The main deliverables will be: - a set of country case study reports incorporating new knowledge about how COVID-19 control measures impact on the health of children and girls/women, with recommendations for how children and women may be better protected, - a report describing the different approaches and strategies taken to control COVID-19 across the four countries, with recommendations on how research and evidence can be produced and harnessed to strengthen future policy making and implementation These reports will form the basis for shorter policy briefs, lay summaries and peer-reviewed publications. The project will also deliver: - a series of in-country stakeholder discussions and meetings and establish opportunities for cross-country dialogue, - a collaborative research network that will set the foundations for future research studies. The activities and outputs proposed here will complement the efforts of government agencies and non-governmental organisations (including UNICEF, Save the Children and UNFPA) working to promote and protect child and women's health. And it will identify critical knowledge gaps and priorities for further research, including in-depth epidemiological studies, intervention studies and in-depth social science inquiries.

The United States Agency for I...nternational Developments (USAID) Bureau for Global Healths flagship Maternal and Child Survival Program (MCSP) focuses on 25 high-priority countries with the ultimate goal of preventing child and maternal deaths. The Program is introducing and supporting high-impact, sustainable reproductive, maternal, newborn and child health (RMNCH) interventions in partnership with ministries of health and other partners. MCSP places a greater emphasis on key cross-cutting issues such as innovation, e/mHealth, equity, quality, gender, public-private partnerships and involvement of civil society, community approaches, health systems strengthening, and behavior change interventions. While maintaining a focus on technical, high-impact interventions, the Program works toward sustainable scale-up to include the health systems that deliver these interventions. In addition to contributing to high-level technical and policy dialogue at the global level, the Program provides tailored technical assistance to help countries meet specific priorities and contextual needs of local RMNCH programs.

USAID's new MCH/nutrition acti...vity will contribute to global health goals and achieve nutrition targets in Tajikistan's Feed the Future (FTF) geographic focus area by: (1) improving maternal nutritional status, (2) enhancing delivery and postpartum care for mothers and children, and (3) expanding access to health interventions to improve child survival and address child under-nutrition and malnutrition. This three-to-five year community based activity will include interventions such as capasity building of the primary health care health providers, including family practicioners, nurses, midviwes, ob-gynsto deliver quality service as well as community-based health and nutrition education, hygiene and sanitation, and the promotion of increased use of health services provided through a network of local village organizations and volunteer health educators. SHOPS Plus supports the achievement of US government priorities, including preventing child and maternal deaths, an AIDS-free generation, and supporting the goals of FP2020. The project improves the equity and quality of the total health system, accelerating progress toward universal health coverage.

With an aim of providing equitable access to cost effective quality health care, the Tanzania Results-Based Financing Trust Fund will improve the accessibility and utilization of health care services in primary health care facilities, the quality of health services at all facilities within the local government authority, the productivity and efficiency of service delivery by health care providers, the quality and use of data for evidence-based decision making, and accountability and responsiveness of health management teams and facility governing committees. The geographic coverage for the coming year includes Shinyanga and Mwanza regions.

Social Enterprise Support (USE...SA) will improve the health status of Tanzanians by increased access to needed commodities through a total market approach (TMA) strategy. Focusing on HIV and family planning, this activity will increase uptake of preventive health services and products in support of Government of Tanzania and U.S. Government health priorities. Major activities include strengthening and expanding the sale of oral contraceptive pills (Flexi P) and male condoms (Dume) to targeted populations with a willingness to pay. It will also increase demand among target groups for these commodities. USESA will also work to transition its health products marketing from a grant dependency activity to a self-sustaining enterprise. Additionally, the activity will participate in Government of Tanzania forecasting and quantification exercises for health products and promote the implementation of a TMA in the country.

Specifically, USAID intends to support interventions consistent with a strategic approach of using social behavior change communication interventions to improve the health of Tanzanians, demonstrating a clear link between program interventions and health status outcomes. USAID seeks to support a program where messages are tailored to geographic, epidemiological and audience segments at the regional level. Because regional radio programs have proven successful in the past, USAID seeks to support a program that incorporates this type of radio as a core media channel. Beyond traditional media, USAID seeks to support a program that is expected to use innovative techniques and mobile technologies to support program quality, reach, and effectiveness.

The suite of MOMENTUM awards s...eeks to accelerate reductions in maternal, newborn, and child mortality and morbidity in high-burden, USAID-supported countries by increasing the capacity of host country institutions and local organizations to introduce, deliver, scale up, and sustain the use of evidence-based, quality maternal, newborn, and child health (MNCH) services, voluntary family planning (FP), and reproductive health (RH) care. MOMENTUM Round 2A (project name TBD) is focused on (1) providing targeted technical assistance and capacity-development to USAID s Missions, partner governments, and local organizations, and, (2) contributing to global technical leadership and policy dialogue on improving measurable outcomes for reproductive, maternal, newborn, and child health, and voluntary family planning.

Covalima Municipality is the model for improvements in the provision of health services. Assistance improves health worker skills in maternal and newborn care through structured training and coaching, the quality of customer service and marketing to enhance communitiy awareness and utilization of services, establish a clinical training center for certification linked to the human resource management system, recording individual health worker professional development, deliver a full range of Family Planning (FP) and sexual and reproductive health services in targeted facilities, and work with communities to create demand for FP and maternal and child health services with health promotion and education tools such as a pre-conception care, and encourage men to support and accept their families' utilization of available health services.

Health Improvement Project

The Timor-Leste Health Improve...ment Project (TL HIP) will support TL in improving the quality and impact of health services. Guided by the vision that health care quality can be significantly improved by applying proven health improvement methods, the TL HIP will assist programs to scale up evidence-based interventions to improve outcomes in Maternal Newborn Child Health, Nutrition, and Family Planning. The project will build on current strategies of the MoH and USAID successes including, Basic Service Package implementation, support to SISCas, assistance with guidelines and protocols, training, development and piloting of job aids( flip charts, flow-charts, instructional videos) and supportive supervision. TL HIP will complement the strengths identified by partner organizations throughout the Ministry of Health system. The project will support the National, District, Sub-district, and community levels, but will primarily be focused at the district level.

The programme is being implemented by Malaria Consortium in partnership with Jhpiego, Banyan Global, Communication for Development Foundation Uganda (CDFU), Deloitte Uganda and Infectious Diseases Institute (IDI).
