Addressing Key Bottlenecks in ...Mozambique’s Primary
Health Care System
The IC provides a comprehensive and technically sound roadmap
to improve RMNCAH‐N by identifying the most critical health
system bottlenecks and proposing evidence‐based interventions
to overcome them. The Program will support these interventions,
including:
Service availability and readiness: IC interventions will improve
the availability of emergency obstetric and neonatal care, essential
RMNCAH‐N medicines/supplies, and facility access to water and
electricity. The IC also orients multi‐sectoral engagement for
Sexual and Reproductive Health (SRH) services, in particular,
through secondary schools to reach adolescents.
Health Human Resources (HRH): IC interventions will help retain
and increase the number of health professionals at the primary
level to overcome critical shortages (e.g. through leave
management, career progression, and learning, especially for
remote postings). Through a country‐wide platform for
community‐based service delivery, there is a national plan to train
and assign APEs to provide RMNCAH‐N services.
Quality of care: There will be systematic efforts to monitor and
improve quality of care at different levels. Service Delivery
Indicators (SDIs) will be collected to gauge national progress.
Quality of care will be assessed at health centers and district
hospitals using a balanced scorecard (BSC) to hold facilities
accountable for results, incentivized through performance‐based
payments. Community consultations will be part of the BSC in
health centers to better understand and respond to users’
perspectives and concerns.
Health financing and PFM: Actions will protect and increase the
currently low share of health in domestic spending. To address
inequitable resource allocation, the government will increase
health funding for lagging and under‐resourced districts. A series
of reforms will strengthen fiduciary oversight and fiscal
decentralization to facilitate service delivery.
Information for decision‐making and accountability: The IC will
improve birth and death data in the health information system
(DHIS2‐SISMA) and strengthen the interface with CRVS, critical to
generate routine data for decision making and to guarantee rights.
Change Management Support for the above will include a robust
capacity development program for frontline workers and
managers, with quality TA and coaches to support incremental
changes.

children;
Micronutrient
supplementation); (iv) immunization (Routine
Childhood
Immunization;
Tetanus toxoid
for mothers;
polio eradication); (v) malaria
control (ITN
distribution;
diagnosis &
treatment
with ACTs); and (vi) the Elimination of Mother to Child Transmission (EMTCT) of HIV. The
objective is to dramatically improve the coverage of these interventions that currently suffer
from poor access and utilization. In addition, to its six “pillars” the SOML Program also includes
two “enablers”: (i) promoting innovation and the use of information and communications

Introduction of Nutrition Serv...ices. It will support introduction of preventive nutrition
services in all 36 districts by scaling up the provision of micronutrients and deworming (see
details in Annex 2) and expanding the delivery of behavior change communication, focusing
on the prevention of malnutrition during pregnancy and in the first two years of life. In 12
districts and urban slums in 9 cities with the highest prevalence of acute malnutrition, therapeutic nutrition services will be delivered, including commodity support micronutrients
and Ready-to-Use Therapeutic Foods. The project will improve the capacity of health service
providers to deliver nutrition interventions at the facility and community level and
coordinating structures to implement multi-sectoral response to address nutrition.
iii) Expanded Coverage of HIV/AIDS Preventive Services. The component will support
expanding coverage of preventive, treatment, and care services for population subgroups
vulnerable to HIV infection [IDUs and Men Who Have Sex with Men (MSM)] in targeted
cities; and support provincial integrated biological and behavioral surveillance. Service

Component 1: Addressing genera...l malnutrition in women and children (total
estimated cost US$31.19 million – IDA US$26.08 million, PPIN US$5.11 million) - This
component will support key nutrition interventions that address general malnutrition, mainly in
pregnant and lactating women and children less than two years of age. This component will
include:
i. Infant and young child feeding (IYCF): A set of IYCF behavior change communications at
community level will target a few key behaviors to improve nutritional outcomes. The IYCF
interventions will include providing micronutrient powders. The IYCF interventions will be
phased into priority districts, 7 districts in Balochistan and 9 districts in Sindh.
ii. Community management of acute malnutrition (CMAM): The project will support the
treatment of severe acute malnutrition (SAM) in affected children 6-59 months old, as per
Pakistan’s guidelines. CMAM will be introduced in the same geographical areas as IYCF
interventions.
iii. Maternal malnutrition: The project will support scaling-up of well proven maternal
nutrition interventions for women of child-bearing age and sharpening the nutrition focus of
ante-natal visits and provision of daily IFA supplementation during pregnancy. Component 2: Addressing micronutrient malnutrition (total estimated cost US$5.96
million – IDA US$4.46 million and PPIN US$1.50 million) - This component will support
vitamin and mineral interventions for women and young children. The focus is on delivery of
key micronutrient supplementation (vitamin A, iron, iodine, folic acid and zinc) and, in
Balochistan, in developing the legislative/enforcement mechanisms for food fortification.
24. Component 3: Communication for development (total estimated cost US$4.51
million – IDA US$3.65 million and PPIN US$0.86 million) - This component includes three
types of cross-cutting communications activities that will support all the other project
interventions:
i. Advocacy: The project will enhance the capacity of the provincial Departments of Health to
undertake activities to familiarize key stakeholders about the magnitude of the malnutrition
challenge in Pakistan and how to address it.
ii. Mass media campaigns for behavior change: The project will support behavior change
communications through mass media to improve knowledge and attitudes relating to
nutrition and thus increase demand for nutrition services.
iii. Inter-personal communications: LHWs and other health workers will be trained and
provided communications tools to facilitate inter-personal communication for behavior
change in areas such as exclusive breastfeeding.
25. Component 4: Strengthening institutional capacity (total estimated cost US$6.27
million – IDA US$2.04 million and PPIN US$4.23 million). The project will strengthen
existing institutional capacity for nutrition at the provincial and district levels. Specifically, this
component will address the following areas:
i. Staff complement: The Provincial Nutrition Cells and District Health Offices will be
strengthened with a few additional staff to cover key skills and knowledge areas such as
planning, monitoring, specific technical areas (e.g. IYCF, micronutrients), etc.
ii. Accountability for results: Systems for effective accountability between the district and
provincial levels for nutrition will be strengthened.
iii. Capacity building: New and existing staff will be supported by training on priority technical
knowledge as well as in management skills. One priority area for capacity building is
contracting out of service delivery to NGOs.
iv. Technical assistance for service delivery: The provinces will outsource technical
assistance (TA) to NGOs, individual consultants and development partners, in order to
support the delivery of services.
v. Monitoring and evaluation: The project will build internal capacity of the Department to
monitor programs and manage in a data-driven manner and to contract out evaluations to
firms.
vi. Social accountability: The project will support the provincial Departments of Health to
establish mechanisms to enhance social accountability, which will include stakeholder
consultation and complaint redress mechanisms.
vii. Multi-sectoral coordination: The project will build the capacity of provincial inter-sectoral
structures which are currently being instituted to oversee the implementation of provincial

Component 2. Improvement to ac...cessibility of maternal, nutrition and child health services
(US$14 million, including US$12 million IDA and US$2 million MDTFHRI)
39. This component will support several mechanisms whose main purpose is to subsidize
demand for healthcare and nutrition services. These mechanisms are (i) the Universal Health
Insurance Scheme (i.e. Couverture Maladie Universelle or CMU), including the Health
Equity Fund and the national identification system of the poorest; (ii) the national program for
improving demand for nutrition services; and (iii) the maternal health vouchers.
Sub-component 2.1. Support to the implementation of the Universal Health Insurance
scheme (US$7 million)
40. The objectives of this sub-component are to help the Government implementing the
Universal Health Insurance Coverage scheme, called Couverture Maladie Universelle
(CMU). As mentioned earlier, the CMU consists mostly in creating at least one mutuelle (i.e.
community-based health insurance scheme) in each commune of Senegal and in subsidizing
heavily (at a 50% rate) the premiums. An additional mechanism (i.e. Equity Fund) will ensure
that the households identified as poorest will pay neither a premium nor copayments. This
CMU is therefore based on an ambitious strategy of voluntary enrollment in communitybased

This PforR operation will supp...ort the Government’s primary health care (PHC) program for the
2015/16 – 2019/20 period, with a strong focus on a key government initiative, the Big Results Now in
Health (BRN in Health).
12. PHC under the Health Sector Strategic Plan IV (including BRN in Health) constitutes the
Government’s program. In the structure of the Tanzania health system, PHC is delivered at the district
level and below, involving district hospitals, health centers, dispensaries and community based health
services under the management of Council Health Management Teams (CHMTs). Over the next five
years, PHC will be guided by the HSSP IV, which will drive major reforms and implementation
including the BRN in Health.
13. The 2015-2018 BRN in Health program aims to accelerate the reduction of maternal and
neonatal mortality through improving performance, governance and accountability in PHC. It was
developed as part of Tanzania’s Development Vision 2025 and has four national key results areas
(NKRAs) as follows:
Performance Management: This result area aims to improve health workers’
performance. Interventions include: (i) a stepwise accreditation scheme for all PHC
facilities in the country (aka “Star Rating” initiative) which has both nation-wide
assessment and a subsequent facility improvement program (including incentives) to
help facilities improve their performances and star ratings, (ii) fiscal decentralization at
the facility level, (iii) performance contracts and targets at individual health worker
levels, and, (iv) social accountability mechanisms.
Human Resources for Health: This result area aims to improve the distribution of
skilled PHC workers especially in nine regions with critical shortages in human
resources for health (i.e. less than national averages). Interventions include: (i)
increasing PHC employment permits for such regions, (ii) engaging the private sector to
provide skilled HRH for public health facilities through PPPs, (iii) redistributing health
care workers within regions, and, (iv) optimizing the pool of new recruits through
“bonding” policy or compulsory attachments.
Health Commodities: This result area aims to improve the availability of essential
medicines in PHC facilities. Interventions tackle key issues along the health
commodities supply chain and include: (i) introducing new governance and
accountability mechanisms, (ii) developing new finance and business model for Medical
Stores Department (MSD), (iii) engaging private sector in procurement and distribution,
(iv) implementing quality improvement initiatives for inventory management, and, (v)
using innovative information and communication technology (ICT) to report stock-outs.
Maternal, Neonatal and Child Health: This result area aims to improve the coverage
and quality of MNCH along the continuum of care. Interventions include: (i) ensuring dispensaries and health centers meet Basic Emergency Obstetric and Neonatal Care
(BEmONC) requirements, (ii) expanding Comprehensive Emergency Obstetric and
Neonatal Care (CEmONC) to selected hospitals and health centers, (iii) strengthening
the corresponding satellite blood banks which serve facilities with CEmONC, and, (iv)
extending MNCH services to communities through the use of community health
workers (CHWs) and awareness campaigns. Five regions that are poorly performing on

The development objective of t...he Maternal and Child Health (MCH) and Nutrition Services Support Project for Togo is to increase utilization of selected MCH and nutrition services for pregnant women and children. The project has two components. The first component is improved utilization of malaria and nutrition services to address malaria in pregnancy through the provision of intermittent preventive treatment for malaria in pregnancy (IPTp) with sulfodoxinepyrimethamine (SP) and free long-lasting insecticide nets (LLINs), to pregnant women attending antenatal care (ANC). It has following two sub-components: (i) support for: (a) malaria control in pregnancy through provision of LLINs and SPs as part of the basic package of ANC services; (b) community-based diagnosis and treatment of malaria; and (c) management, supervision, and behavioral changes to ensure effective utilization; and (ii) community- based nutrition services for pregnant women and young children under five years of age. The second component is strengthening health monitoring and evaluation systems; project management. It has following two sub-components: (i) strengthening the health monitoring and evaluation system, particularly the health management information system (HMIS), including the government's capacity to monitor its MCH programs; and (ii) project coordination and implementation support. En tant qu'organisation de dé...veloppement des églises protestantes suisses, Pain pour le Prochain a le mandat d'informer et de former, de s'engager dans la politique de développement et d'assurer la qualité. Pain pour le Prochain et les onze œuvres partenaires forment une communauté de coopération (KoGe)[1] qui est un réseau d'apprentissage avec des objectifs spécifiques. Ce réseau permet notamment de joindre le travail de politique de développement en Suisse et le travail au Sud. Son objectif commun est de contribuer à une transformation du système et des comportements économiques, sociaux, politiques et écologiques. [1] Les œuvres sont les suivantes : Pain pour le prochain, cfd (Christlicher Friedensdienst), Mission 21, International Blue cross, Connexio, DM-échange et mission, Heilsarmee, Horyzon, MEB-handicap visuel, SME (service de missions et d'entraide) , Tear Fund Schweiz, HEKS

La stratégie de coopération ...internationale 2020 de Caritas s'intitule « End Poverty ». Pour y contribuer Caritas Suisse s'engage en faveur de la justice sociale et de la dignité humaine dans 40 pays avec 250 projets. Le programme de Caritas considère trois aspects dans son programme qui permettent la durabilité du développement humain: couvrir les besoins de base, promouvoir la résilience et créer des opportunités d'égalité en améliorant l'équité. Dans sa conception, Caritas considère la coopération au développement et l'aide humanitaire comme intégrées dans une seule démarche.

EPER/HEKS concentre son progra...mme international sur le développement rural, la gestion des conflits et la paix et les droits humains. La terre en tant que ressource est au cœur du programme de EPER/HEKS. Ceci que ce soit au niveau de l'accès à la terre, son utilisation agricole et la commercialisation des produits. L'intégration économique et sociale des hommes et femmes marginalisés est un aspect important du programme de EPER/HEKS.
