Colorectal Cancer (CRC) is the... second most common cancer in Malaysia and CRC-related deaths are predicted to increase from 2,565 in 2015 to 4,562 in 2030. CRC-related deaths may be avoided by early presentation, detection and appropriate treatment. However, cancers in Malaysia present at late stages. The Ministry of Health recommends annual screening for Malaysians aged 50 years or above to diagnose CRC early and increase chances of survival. However, research indicates that over 90% of the Malaysia population in that age group do not follow these recommendations. Recently, our research group conducted a population representative survey of 953 adults and found that they reported that they did not know 'how to go about screening' and that they believed that they were 'not the right age to undergo routine CRC screening'. There is a need to design and develop or adapt a programme that will ensure that Malaysians will avail of CRC screening. In order to support earlier presentation and diagnosis and to improve survival rates, the 'Bowel Cancer Screening Programme' (BCSP) was launched in England in 2006 and, more recently in 2010, a BCSP was designed and implemented by Northern Ireland's Public Health Agency. Evaluation findings reported an increased screening uptake of CRC as well as a shift towards earlier stage disease. There is a clear need for a co-ordinated, population-based CRC screening programme in Malaysia. Previous population-based surveys indicate that particular attention should be paid towards targeting common cancer disparities among various ethnic, socio-economic and geographical groups in Malaysia. The main aim of the proposed project is to address the need to design and develop a programme that is theoretically informed, evidence-based and culturally appropriate for Malaysia by building upon the BCSP programmes in NI and England, and then to evaluate the feasibility and acceptability of the programme. The project comprises four phases. Phase 1 will review and synthesise the evidence and experiential learning regarding the lessons about implementation of CRC screening in NI and elsewhere, and will assess the views and preferences of key stakeholder groups particularly the views of the general population regarding CRC screening, how to facilitate easy access to screening and views about a population-based CRC Intervention in Malaysia (CRC-SIM). Phase 2 will involve further partnership working and the co-design of a culturally sensitive population-based CRC screening programme and the identification and agreement of metrics for the evaluation of implementation in Malaysia. Previous learning and perspectives will be presented and discussed in order to finalise the design and evaluation framework for the appraisal of the CRC-SIM and the assessment tools in the context of the Malaysian infrastructure and health system. Access to adequate, safe and a...ffordable housing plays a fundamental role in human health. This includes thorough limits our exposure to infectious diseases such as those that cause diarrhoea, which remains a leading cause of death in children under five. Conventional approaches to reducing environmental exposure to faecal pathogens include ensuring universal access to safe water and basic sanitation. Recent evidence has suggested that this alone may be insufficient to reduce the high levels of environmental contamination seen in poor rural communities, and that transformative cross-sectoral approaches will be required to see real impacts in child health. For example, these approaches fail to address the fact that most poor rural homes have rudimentary (earth, sand or dirt) floors that are difficult to sanitise, providing an ideal environment for the survival of faecal pathogens and other parasites. These floors can also host parasitic infections including intestinal nematodes and sand fleas, both of which are responsible for considerable morbidity and poor quality of life. We propose to examine flooring and its impact on enteric and parasitic diseases in three culturally and environmentally diverse settings in Kenya, and aim to address two related questions: 1. What is the importance of household flooring as a driver of enteric and parasitic infection risk in rural communities, and does this vary across wider social and environmental contexts? 2. Can infection risk be mitigated by replacing existing rudimentary (earth, sand or dirt) floors with improved (sealed, washable and durable) materials, and what additional behaviour changes are required to ensure impact? We expect that successful installation and ongoing maintenance of improved flooring will reduce the transmission of enteric and parasitic infections, by preventing direct exposure and through an intermediate effect of improved domestic hygiene. This will however be influenced by local context. A priority in each setting will therefore be to explore the interplay between domestic flooring, water and sanitation infrastructure, domestic hygiene behaviours, and the wider socio-cultural and environmental context. Our planned approach involves comprehensive formative research, intervention development conducted in collaborative partnership with recipient communities and key stakeholders, and then implementation trials to test the effects, feasibility and acceptability of the resulting intervention. We will assess the impact of the intervention on a range of child health outcomes, including prevalence of enteric and intestinal worm infections, prevalence of tungiasis, and incidence of gastrointestinal illness. We will also monitor levels of environmental contamination, and explore the impact of the intervention on domestic routines and self-reported wellness. In babies and toddlers worldwi...de, respiratory syncytial virus (RSV) is the most common cause of a type of chest infection called bronchiolitis and causes severe lung inflammation. 2-3% of all babies in the UK have to be admitted to hospital with RSV bronchiolitis and some of them develop very severe and sometimes life-threatening disease. This happens particularly when very high numbers of virus particles are present after infection. Due to treatment costs and costs for the wider society (e.g. days lost at work for parents/ carers) RSV is responsible for a major financial burden. Despite all of this, no specific treatment or effective, widely available preventative interventions exist and novel approaches are urgently required. Palivizumab, a prophylactic antibody against RSV, can prevent hospital admissions by about 50% but due to high cost its use is limited to small groups of high-risk infants in affluent countries. We have previously reported that infection with a gut parasite worm can reduce the number of viral particles in the lungs and disease severity in a mouse model of RSV infection. More recently, we have found that protection from severe RSV infection in this model is associated with increased production of immune cells called monocytes in the bone marrow and their accumulation in the lung. Monocytes are thought to be important in the immune response to RSV, but how they exert their antiviral effect is not fully understood. Importantly, all the above effects of parasite infection can be recapitulated with cell-free blood serum from infected mice, unless it has been heated. This suggests a central role for a protein in the serum, such as an immune system messenger molecule, as the soluble RSV-protective factor. If we can identify this factor, and work out what it is doing, we may in the future be able to develop a novel approach to protection from severe RSV disease in young children. Here, we will initially use our mouse model to study which subgroups of monocytes occur and which monocyte genes are 'switched on' during parasite infection, in order to define the mechanisms by which monocytes limit RSV infection. We will then use two approaches to identify the RSV-protective factor from blood serum, one where we measure, block and replace known candidate immune mediators in the serum, and another where we test groups of serum proteins of different sizes for their anti-RSV effect, followed by measurement of the proteins within the effective group and identification of candidate factors. These will then be tested individually for their antiviral effect and the RSV-protective factor will be identified. Finally, to translate our findings from the mouse model to humans, we will use existing blood samples from Ugandan children with and without gut parasite worms. Plasmodium is the causative ag...ent of malaria and is responsible for about 584,000 human deaths annually, mostly children under the age of 5. The increasing resistance of the parasite to existing drugs and the lack of an efficient vaccine represent the main obstacles to combat this burden. Therefore, there is an urgent need for the development of innovative therapeutics. Plasmodium has a complex life cycle with diverse host environments, changes in cell shape, size and motility and an atypical mode of cell proliferation. The symptoms of the disease are manifest when malaria parasites invade host red blood cells, wherein the parasite divides and multiplies many times (endomitosis), eventually leading to destruction of the host cell. Some of the parasite cells may cease to divide and they become precursor sex cells (male and female gametocytes). When a female mosquito bites an infected person they ingest parasites along with the blood and this acts as a trigger to activate the precursor sex cells within the mosquito gut. The male gametocytes undergo rapid cell division (endoreduplication) to produce eight male gametes, which then fertilise the female gametes and the parasite life cycle continues in the mosquito gut. After further development and multiplication the parasite moves to the mosquito's salivary glands and is passed again to a new human host when the mosquito feeds. The process of Plasmodium cell division in host red blood cells, and during sexual development in the mosquito vector is very different, but both are essential for parasite growth and transmission. If any of these stages are blocked then both proliferation and transmission are curtailed. Therefore, it is critically important to understand how the parasite multiplies and divides at these stages so that we can devise ways to interfere with them by developing appropriate drugs. The molecules that control these types of atypical cell division in the parasite are very poorly understood. The project proposed here is to study the role of two important protein complexes: condensin and cohesin. Although these complexes are known to be involved in cell division in many model systems, there is no knowledge of how they function during malaria parasite cell division and proliferation. We can start by using the knowledge gathered in model systems and applying this to the malaria parasite. For example, we have recently identified one such protein (CDC20) in the malaria parasite and showed that it has a key role in regulating male gamete formation. In preliminary work showing that our approach is feasible, we have obtained evidence for the presence of condensin and cohesin in the parasite multiplying within the red blood cell, in the male sex cell and in the meiotic cell (an ookinete). Recent advances in analysing genes in malaria allow us to study the function of these molecules. Leishmaniasis is an important ...'Neglected Tropical Disease' caused by protozoan parasites of the genus Leishmania. There are five clinical forms which range from localized cutaneous leishmaniasis (CL) to fatal visceral leishmaniasis (VL). With over 350 million people world wide considered at risk, 12 million people currently infected and an economic cost that can be estimated in terms of 2 million DALYs the health challenge is only surpassed amongst parasitic diseases by malaria and lymphatic filariasis. Consequently, the leishmaniases have been classified by the World Health Organisation as Category I: emerging or uncontrolled diseases. In particular, the spread and severity of infection is exacerbated by its status as an important co-infection of AIDS patients and the overlap in prevalence of HIV and Leishmania species. In India, despite considerable government effort VL is still a major health hazard in 90 regions whilst CL, not being a notifiable disease, is a neglected 'neglected disease'. The treatment of leishmanial infections is difficult with all forms requiring a long, painful and costly course of drug therapy. The challenge presented by these disease states is heightened by the fact that the few efficacious drugs available often exhibit serious, potentially fatal, side-effects. New medicines with better modes of delivery are therefore required. In this UK-India collaborative project, that brings together chemistry, pharmaceutics, formulation science, parasitology and medical anthropology, we will address these challenges by developing novel active antileishmanial compounds with better modes of administration. We will use state-of-the-art crystallisation and formulation technologies to enable new oral and topical delivery methods for each compound. This key aspect will be directed by community engagement within endemic communities and local health care professionals to define what makes a drug acceptable to the affected populations. Significantly, to help overcome the potential for resistance to arise, the design of delivery devices that enable combination therapies in which two different drugs are dosed simultaneously will be undertaken.
