The role of diabetes-associate...d pathology in the development of clinical tuberculosis: a collaborative project between Sweden and Bangladesh

An important but currently neg...lected threat for the development of active tuberculosis (TB) is diabetes, which increase the risk of clinical TB several fold. In this proposal, we take advantage of a well-established collaboration with Bangladesh to explore the role of diabetes in the pathogenesis and progression of chronic TB disease. We will use patients samples collected in well-designed clinical studies and advanced immunological techniques to carefully characterize macrophage function as well as the behaviour and relevance of peripheral lymphocyte responses involved in TB and how these are affected by concomitant diabetes disease. Our research constellation has also developed novel concepts for immunodiagnosis and immunotherapy of TB that will be evaluated in patients with diabetes. Here, a novel B cell test will be used as a diagnostic and prognostic biomarker of active TB in patient cohorts recruited in a high-prevalence setting in Dhaka, Bangladesh. In a clinical trial, we will also study if novel inducers of antimicrobial peptides, phenylbutyrate and vitamin D, can restore immune function and insulin sensitivity in diabetic patients with TB. Our ambition is to promote knowledge and technology transfer between the collaborating institutions, partly by local PhD and postdoc training, to gain synergistic effects on the discovery of clinically relevant targets that could be used to improve medical management and general health of patients with TB and diabetes.

We will carry out a comprehens...ive analysis of the effects of introducing and expanding publicly provided health insurance to those not covered, that is, those without a labor contract in Mexico. This is an expansion that aimed to reach half of the population, and, in particular, the poor. We aim to carry out several empirical studies to address different features associated with the implementation of such scheme. To which extent the introduction of universal health insurance coverage is effectively associated to improved access to preventive care among the most vulnerable? Who effectively benefits more from such policy? We will use differences-in-differences and regression discontinuity methods (typically used in policy evaluation), and also new econometric techniques to understand the marginal gains of expanding policies. It is also part of our plan to study whether there are non-health effects on children to learn to which extent publicly provided health investments in Latin America are effective in decreasing inequality not only in the current generation, but also in the next one. Finally, our project will contribute to a better understanding of the unintended consequences of health insurance, including the labor market, by distorting incentives to work in the formal sector, and on whether large scale programs can be used for political purposes.

This project will be carried o...ut in Nepal in collaboration with Nepalese partners. The objective is to develop capacity and collaboration on health care system research in and between Nepal and Sweden. This will be done through a series of intervention studies focusing on improvements in quality of care in the arena of maternal and child health care. This will be achieved through collaboration between UNICEF, Uppsala University (UU) and Patan Academy of Health Sciences (PAHS) in Kathmandu. UU and PAHS have since 2012 collaborated on a hospital-based intervention trial implementing a simplified neonatal resuscitation protocol (Helping Babies Breath, HBB) at a tertiary level delivery hospital in Kathmandu. The study period for the trial was completed in September 2013. Preliminary results indicate a 53% reduction of intra-partum stillbirths and considerable improvements in the quality of delivery care. Ministry of Health and Population (MoHP), which have commissioned UNICEF to scale up the HBB intervention at 40 district hospitals. We plan to design a series of research studies investigating different approaches on the implementation of QoC enhancing interventions. Considering the programmatic approach in the call for applications we intend to integrate a number of research trials over the program period, thus being able to use synergies and develop a critical mass for the Swedish-Nepalese research collaboration.

This project will focus on the... preparation of compounds that combine a fragment with known antiviral (anti-HIV) properties with the essential frameworks of known antimalaria or antituberculosis drugs, in order to prepare candidates for tandem drugs that may overcome the adverse effects of combination treatment. The efficacy of the complexes as anti-HIV and antituberculosis agents will be assessed by in vitro cell line studies in collaboration with researchers at the Universities of Johannesburg and Pretoria, South Africa. Similarly, antimalarial properties will be tested on Plasmodium falciparum malaria parasites in collaboration with a research team at the University of Cape Town, South Africa.

South Africa is heavily burden...ed by chronic diseases including HIV and Tuberculosis (TB). The South African health system struggles with a shortage of human resources, fuelled by a cumulative increase in individuals in need of life-long antiretroviral treatment (ART), often co-infected with TB. A solution to overcome these shortages is lay-health workers (LHWs). Shortfalls for a system relying on LHWs are e.g. the challenges related to a high work-load in rural areas, with long distances and barriers to timely and adequate reporting. The main purpose of this study is to analyse the effects of an integrated mobile phone managmenet intervention to improve lay health workers (LHWs) performance, case finding and health outcomes for HIV and TB in rural Soth Africa. A before and after comparison, applying an interrupted time series analysis will be used to assess the effects on primary outcomes (proportion of scheduled visits by LHWs occuring as planned, proportion of patients being referred for TB and HIV screening and testing, who access the clinic) together with survival analyses of secondary outcomes (retention in care for ART and TB treatment) as well as a cost-effectiveness study will be conducted. Knowledge generated by the project will assist in finding innovative ways to handle the increasing work-load faced by the lay-health workers, as well as contributing to improved case detection and treatment of the devastating HIV and TB co-epidemic in South Africa.

In Tanzania, medicinal plants ...are an important source of primary healthcare. Molecular identification of plant material is an expressed critical interest for the research capacities at the Institute of Traditional Medicine, ITM, as it allows for the identification and monitoring of trade in Tanzania's rich biological and genetic resources, which is crucial to implementing the CBD and CITES conventions. Our lab in Uppsala has been at the forefront of the development of DNA barcoding of plant species in trade and forensic material, and our work with next-generation sequencing for analysis of species complexes and mixtures is state-of-the-art. The objectives of the project are to: 1) make a quantitative investigation of wild-harvested medicinal plants; 2) identify diversity and volumes of medicinal plants traded in Tanzania; 3) use DNA barcoding for the identification of medicinal plants traded from local markets, border checkpoints and Dar es Salaam Airport; 4) use Ion-Torrent NGS for molecular identification of species in mixtures and processed products; 5) and use standard and innovative NGS-generated markers for the identification of wild-harvested orchids. This project will help our partners to take directed measures to protect consumers from toxic adulterants, protect endangered species from destructive overharvesting, curb biopiracy and trade in endangered wildlife, and facilitate controlled sustainable exploitation of medicinal plant resources.

Antimalarial and antimicrobial drugs constitute the routine treatment basis, but the role of supportive therapies, involving the maintenance of a normal glucose and electrolyte balance, has been given limited attention. While hyperglycemia is the most common blood glucose anomaly seen in infectious disease patients in high-income settings, hypoglycemia is not uncommon in low-income settings. 1.8-7.3% of children admitted with fever present with hypoglycemia, a known predictor of poor clinical outcome. A number of studies also indicate an increased mortality among children with blood glucose levels that are currently considered within the normal range, e.g. 2.5 - 5.0 mmol/l, compared to children with higher blood glucose levels (>5.0mmol/l). This randomized clinical trial will evaluate the benefit of glucose administration already ay blood glucose levels <5.0mmol/l in children presenting to a Ugandan hospital with an acute febrile illness. It will further explain the metabolic response, through the measurement of metabolic markers, in pediatric acute infections and how this influences the final outcome in children with hypo-, normo- and hyperglycemia. Our results will highlight the potential need to revise current hypoglycemia cut off values in order to increase child survival in acute infections.

2016 - The second EDCTP programme (EDCTP2) is a highly ambitious programme and 2016 was a transformational year for consolidating the organisation itself and accelerating focussed implementation of the programme.As an organisation, EDCTP began the implementation of a results-based managementapproach and further worked on ensuring coherence of the EDCTP programme through afocused Strategic Business Plan. Guided by its Scientific Advisory Committee, EDCTP developed an annual strategic research agenda which identifies specific research gaps and opportunities, and sets specific funding priorities within its extended programme scope. The research agenda also drew on the results of the two stakeholder meetings on diarrhoeal diseases and lower respiratory tract infections held in July 2016. In 2016, EDCTP launched nine new calls for proposals while proceeding with the selection and grant agreement processes for the eleven calls for proposals from 2014-2015. By the end of2016, EDCTP2 had a portfolio of 60 grants with a total value of €97.09 million; €75.11 million of thisamount was invested in large-scale clinical trials and clinical research projects. 70% of grants are coordinated by an African institution and 33% of grants are led by a female project coordinator, 26 reserach fellows granted are from Africa.

The International Centre for D...iarrhoeal Disease Research, Bangladesh (icddr,b) is an international health research institution located in Dhaka, dedicated to addressing the health challenges that disproportionately affect people living in poverty in Bangladesh and other low- and middle-income countries. Sida has supported icddr,b since 1981, and the institute has evolved considerably over time. The institute is today a centre of excellence producing research of global relevance, such as child and maternal health, including reproductive health. Icddr,b provides unique opportunities to address health challenges in low-income settings through low-cost, practical, lifesaving solutions. The track record includes the oral rehydration solution (ORS), zinc for diarrhoea treatment and tetanus vaccines for newborns, which have saved and continue to save millions of lives worldwide. icddr,b has the capacity to undertake a full spectrum of research spanning population-based studies and demographic surveillance, large-scale clinical trials, hospital-based clinical studies, and laboratory research. icddr,b has recently had some turbulent years with concerns of both organizational nature and regarding research sustainability, especially with regards to the regrowth of researchers at the senior level. These challenges are being addressed in different ways and the institute is currently at a transitional stage.icddr,b's Strategic Plan 2015-2018 reinforces icddr,b's legacy of focusing not just on discovery, but also on strengthening the organisation's ability to shape policy at the national, regional and global level, as well as advocating for the use of highly effective products and evidence-based discoveries. icddr,b reinforces these efforts through publishing and South-South collaboration, training and capacity-building, and continues to build a cadre of public health experts and capacities for low- and middle-income countries. The proposed support is a continuation of previous support, last for the agreement period 2014 - 2015. The total proposed amount is 121 million SEK for the biennium 2016 - 2017. The Sida support is mainly disbursed as programme support and is therefore of particular importance to the programmes as this enables them to develop their programmes to address emerging research needs and types of support. The share of core (undesignated ) support of the total budget from all funders to the programmes were for AHPSR 78% (2015), for HRP 60% (2014) and for TDR 81% (2014). Sida's contribution during the proposed agreement period is estimated to 14% of AHPSR's overall budget, approx. 7% of the HRP's ditto, respectively 11% of TDR's ditto. The main reason to that the proposed support is for a two year period (and not four years) is that that the TDR's current strategy expires at the end of 2017 and preparations have started for the development of a new strategi for the period 2018 - 2023. The support to TDR is nearly half of the total Sidas support to the three WHO research programmes and it is considered to be valuable to await the planned evaluation of TDR aswell as the new strategy before a longer agreement period is entered into. Sida has extensive experience, since the mid-1970s of support to WHO for research carried out either by special research programmes or as separate research projects. Expericence shows that the WHO has the ability to engage leading researchers for poverty related health issues and that the stand-alone research programmes are able to adapt to changes in the research environment and take leading role in the global research agenda. Experience has also shown that the research results produced in the programs are spread globally as a result of the WHO normative mandate. The overall assessment is that the three programmes over time have proven results in line with targets. For 2014 some of the results reported are summarized below (results from 2015 will be reported at the beginning of 2016): AHPSR published the flagship report Medicines in Health Systems: Advancing access, affordability and appropriate use , which is a significant contribution to the understanding of a previously neglected area of concern. The proposed intervention is p...rogramme support for the period 2016 - 2017 to three research programmes hosted by the World Health Organisation (WHO): i) Alliance for Health Policy and Systems Research, AHPSR or the Alliance ; ii) The UNDP-UNFPA-UNICEF-WHO-World Bank Special Programme of Research, Development and Research Training in Human Reproduction, HRP; och iii) The UNICEF-UNDP-World Bank-WHO Special Programme for Research and Training in Tropical Diseases,TDR. The proposed support is a continuation of previous support, last for the agreement period 2014 - 2015. The total proposed amount is 121 million SEK for the biennium 2016 - 2017. The Sida support is mainly disbursed as programme support and is therefore of particular importance to the programmes as this enables them to develop their programmes to address emerging research needs and types of support. The share of core (undesignated ) support of the total budget from all funders to the programmes were for AHPSR 78% (2015), for HRP 60% (2014) and for TDR 81% (2014). Sida's contribution during the proposed agreement period is estimated to 14% of AHPSR's overall budget, approx. 7% of the HRP's ditto, respectively 11% of TDR's ditto. The main reason to that the proposed support is for a two year period (and not four years) is that that the TDR's current strategy expires at the end of 2017 and preparations have started for the development of a new strategi for the period 2018 - 2023. The support to TDR is nearly half of the total Sidas support to the three WHO research programmes and it is considered to be valuable to await the planned evaluation of TDR aswell as the new strategy before a longer agreement period is entered into. Sida has extensive experience, since the mid-1970s of support to WHO for research carried out either by special research programmes or as separate research projects. Expericence shows that the WHO has the ability to engage leading researchers for poverty related health issues and that the stand-alone research programmes are able to adapt to changes in the research environment and take leading role in the global research agenda. Experience has also shown that the research results produced in the programs are spread globally as a result of the WHO normative mandate. The overall assessment is that the three programmes over time have proven results in line with targets. For 2014 some of the results reported are summarized below (results from 2015 will be reported at the beginning of 2016): AHPSR published the flagship report Medicines in Health Systems: Advancing access, affordability and appropriate use , which is a significant contribution to the understanding of a previously neglected area of concern. The report is based on work conducted in a multi-disciplinary team consisting of medical and pharmaceutical expertise, economists, social scientists and sociologists.HRP produced the hitherto most comprehensive global overview of sexual and reproductive health and rights
