The World Bank team, in co-ope...ration with the Swiss Agency for Development Cooperation, has concentrated its efforts on supporting the
Government of Moldova (GoM) and the Ministry of Health in conducting a thorough analysis of health sector spending, focusing on technical and
allocative efficiency in public healthcare expenditures to identify potential areas for optimization including health promotion and disease prevention
activities. Key findings of this exercise will be publicly presented as part of a mid-term review of the project in September 2017. This review will
provide much needed evidence towards the optimization of inpatient healthcare services as an integral part of the much anticipated hospital reform.
At present, MoH is working on the finalization of the hospital reform concept which shall be subsequently approved by the Parliament. Based on this

Addressing Key Bottlenecks in ...Mozambique’s Primary
Health Care System
The IC provides a comprehensive and technically sound roadmap
to improve RMNCAH‐N by identifying the most critical health
system bottlenecks and proposing evidence‐based interventions
to overcome them. The Program will support these interventions,
including:
Service availability and readiness: IC interventions will improve
the availability of emergency obstetric and neonatal care, essential
RMNCAH‐N medicines/supplies, and facility access to water and
electricity. The IC also orients multi‐sectoral engagement for
Sexual and Reproductive Health (SRH) services, in particular,
through secondary schools to reach adolescents.
Health Human Resources (HRH): IC interventions will help retain
and increase the number of health professionals at the primary
level to overcome critical shortages (e.g. through leave
management, career progression, and learning, especially for
remote postings). Through a country‐wide platform for
community‐based service delivery, there is a national plan to train
and assign APEs to provide RMNCAH‐N services.
Quality of care: There will be systematic efforts to monitor and
improve quality of care at different levels. Service Delivery
Indicators (SDIs) will be collected to gauge national progress.
Quality of care will be assessed at health centers and district
hospitals using a balanced scorecard (BSC) to hold facilities
accountable for results, incentivized through performance‐based
payments. Community consultations will be part of the BSC in
health centers to better understand and respond to users’
perspectives and concerns.
Health financing and PFM: Actions will protect and increase the
currently low share of health in domestic spending. To address
inequitable resource allocation, the government will increase
health funding for lagging and under‐resourced districts. A series
of reforms will strengthen fiduciary oversight and fiscal
decentralization to facilitate service delivery.
Information for decision‐making and accountability: The IC will
improve birth and death data in the health information system
(DHIS2‐SISMA) and strengthen the interface with CRVS, critical to
generate routine data for decision making and to guarantee rights.
Change Management Support for the above will include a robust
capacity development program for frontline workers and
managers, with quality TA and coaches to support incremental
changes.

The leadership of MOHS is comm...ended on continuing to make significant advances with the implementation of the National Health Plan (NHP), which
was endorsed by the Government of Myanmar in March 2017. The Annual Operation Plan for the first year was widely shared and discussed with all
the key stakeholders and the World Bank Group (WBG) is pleased to be able to support through the IDA financed Essential Health Services Access
Project (EHSAP) and the Advisory Services and Analytics Program on health systems strengthening for Universal Health Coverage.
Proposed additional financing for EHSAP through another $100 million IDA credit and $10 million grant from the Global Financing Facility is currently
undergoing the Government of Myanmar internal clearance and approval process. Once clearances have been provided, the World Bank and
Government of Myanmar intends to negotiate the financing agreement and related documents. The proposed additional financing would further support
the implementation of the NHP. Furthermore, the additional financing, which also includes restructuring of EHSAP, to address some key bottlenecks to
implementation.
Implementation of EHSAP with already approved IDA credit is progressing, with Component 1 on track and Component 2 with delays.
Component 1 is advancing as planned and implementation is considered satisfactory. MOHS has submitted the Year 2 Disbursement Linked Indicator
Achievement report, which has been shared with the Department of Medical Research for independent verification. MOHS and World Bank have also
convened an Oversight Committee to provide additional independence to the verification. MOHS report indicated that targets for all DLIs--with the
exception of DLI 7--were met (at least 100 townships meeting the service readiness level of 14 out of 20, receiving health facility funds, making outreach
visits for antenatal and postnatal care, having township health plans prepared with community engagement, and conducting supervision visits by
State/Region health departments and by Township officials). As regards DLI 7, the achievement is 87%; i.e., 87 townships have 60% of the midwives
trained in both Basic Emergency Obstetric Care and Integrated Management of Childhood Illnesses. The independent verification is still ongoing and
WB expects to receive the report at the beginning of September.
Implementation of Component 2 lags significantly with moderately unsatisfactory progress, due to the delays with reviews and clearance for work

In particular, the project aim...s at financing food expenditures and agricultural production
assets. Food expenditures will be effectively targeted towards selected beneficiaries by setting a
prioritization list within the contract with WFP for the use of WB funds (in descending order of
priority) for (i) blanket supplementary feeding (BSF) for under-five child nutrition; (ii) targeted
supplementary feeding (TSF) for pregnant and lactating women (PLW) and children aged 6-59
months in areas that suffer from moderate acute malnutrition; (iii) emergency school feeding
(ESF); and (iv) general food distribution (GFD). In the contract with FAO, agricultural assets
will cover subsidized seeds, equipment and training to seeds producers and women groups and to

This component seeks to strengthen the quality and supply of the public health care services
delivered nationwide through the following subcomponents: Subcomponent 1.1 Improvement of
quality health care delivered at municipal level through marginal funding of per capita health care
service fee; and (b) Subcomponent 1.2 Strengthening and expanding the provision of basic health
care services.
Component 2. Strengthen the MOH response to the country’s changing epidemiological profile
Comments (optional)
The purpose of this component is to contribute to adapting the public health system to the country’s
changing epidemiological profile. It consists of two sub-components supporting interventions to
address the increased mortality and morbidity caused by chronic diseases and traumas, as follows:
(a) Subcomponent 2.1 Strengthen the MOH to effectively prevent and respond to chronic diseases;
and (b) Subcomponent 2.2 Strengthening the MOH to effectively prevent and respond to trauma
cases.
Component Name
Component 3. Contingency Funds for Public Health Alerts and Public Health Emergencies
Comments (optional)
This component would provide funding to prevent and respond to a public health alert or a public
health emergency. It also includes funding to prevent and control outbreaks, epidemics, and other
potential risk factors involving the handling of general and medical waste. It has three
subcomponents: (a) Subcomponent 3.1 Financing operating costs related to a Public Health Alert or
a Public Health Emergency; (b) Subcomponent 3.2 Strengthening Health Epidemics and Outbreaks
Preventive Actions; and (c) Subcomponent 3.3 Strengthening capabilities and implementing
measures to reduce environmental pollution affecting the population’s health.

Component 1: Results-based fin...ancing for quality improvement in prevention and provision of
health care services to the poorest 66 municipalities (US$14 million). The purpose of this Component is
to ensure quality in the prevention of disease and the provision of care for communicable diseases and
NCDs at the primary health care level in the public sector for the 66 most vulnerable municipalities of the
country, including Alto Wangki Bocay and the municipalities of the Caribbean Coast. Targeted
interventions will include: (a) the early identification of health risk factors; and (b) monitoring of the
provision and quality of health care services.
Component 2: Support to the implementation of national health strategies for the provision of
quality health services under the MOSAFC (US$44.5 million). This Component will support the
implementation of several national strategies aimed at improving the quality of health care provision
nationwide, the expansion of coverage in the Caribbean Coast, and the provision of public goods targeted
to vulnerable population groups. These national health strategies are a key instrument for implementing
the MOSAFC health care model. This Component will finance goods, consulting, and non-consulting
services, minor works, training and operation costs to implement the strategies.

implementation of several national strategies aimed at improving the quality of health care provision
nationwide, the expansion of coverage in the Caribbean Coast, and the provision of public goods targeted
to vulnerable population groups. These national health strategies are a key instrument for implementing
the MOSAFC health care model. This Component will finance goods, consulting, and non-consulting
services, minor works, training and operation costs to implement the strategies.
