STRENGTHENING PRIMARY HEALTH C...ARE FOR RESULTS

Strengthening Primary Health C...are for Results

This PforR operation will supp...ort the Government’s primary health care (PHC) program for the
2015/16 – 2019/20 period, with a strong focus on a key government initiative, the Big Results Now in
Health (BRN in Health).
12. PHC under the Health Sector Strategic Plan IV (including BRN in Health) constitutes the
Government’s program. In the structure of the Tanzania health system, PHC is delivered at the district
level and below, involving district hospitals, health centers, dispensaries and community based health
services under the management of Council Health Management Teams (CHMTs). Over the next five
years, PHC will be guided by the HSSP IV, which will drive major reforms and implementation
including the BRN in Health.
13. The 2015-2018 BRN in Health program aims to accelerate the reduction of maternal and
neonatal mortality through improving performance, governance and accountability in PHC. It was
developed as part of Tanzania’s Development Vision 2025 and has four national key results areas
(NKRAs) as follows:
Performance Management: This result area aims to improve health workers’
performance. Interventions include: (i) a stepwise accreditation scheme for all PHC
facilities in the country (aka “Star Rating” initiative) which has both nation-wide
assessment and a subsequent facility improvement program (including incentives) to
help facilities improve their performances and star ratings, (ii) fiscal decentralization at
the facility level, (iii) performance contracts and targets at individual health worker
levels, and, (iv) social accountability mechanisms.
Human Resources for Health: This result area aims to improve the distribution of
skilled PHC workers especially in nine regions with critical shortages in human
resources for health (i.e. less than national averages). Interventions include: (i)
increasing PHC employment permits for such regions, (ii) engaging the private sector to
provide skilled HRH for public health facilities through PPPs, (iii) redistributing health
care workers within regions, and, (iv) optimizing the pool of new recruits through
“bonding” policy or compulsory attachments.
Health Commodities: This result area aims to improve the availability of essential
medicines in PHC facilities. Interventions tackle key issues along the health
commodities supply chain and include: (i) introducing new governance and
accountability mechanisms, (ii) developing new finance and business model for Medical
Stores Department (MSD), (iii) engaging private sector in procurement and distribution,
(iv) implementing quality improvement initiatives for inventory management, and, (v)
using innovative information and communication technology (ICT) to report stock-outs.
Maternal, Neonatal and Child Health: This result area aims to improve the coverage
and quality of MNCH along the continuum of care. Interventions include: (i) ensuring dispensaries and health centers meet Basic Emergency Obstetric and Neonatal Care
(BEmONC) requirements, (ii) expanding Comprehensive Emergency Obstetric and
Neonatal Care (CEmONC) to selected hospitals and health centers, (iii) strengthening
the corresponding satellite blood banks which serve facilities with CEmONC, and, (iv)
extending MNCH services to communities through the use of community health
workers (CHWs) and awareness campaigns. Five regions that are poorly performing on
maternal and neonatal mortality indicators will receive priority focus. (TZA P152736.IDA56430.crs1 Strengthening Primary Health Care for Results p.4-5)

The development objective of t...he Maternal and Child Health (MCH) and Nutrition Services Support Project for Togo is to increase utilization of selected MCH and nutrition services for pregnant women and children. The project has two components. The first component is improved utilization of malaria and nutrition services to address malaria in pregnancy through the provision of intermittent preventive treatment for malaria in pregnancy (IPTp) with sulfodoxinepyrimethamine (SP) and free long-lasting insecticide nets (LLINs), to pregnant women attending antenatal care (ANC). It has following two sub-components: (i) support for: (a) malaria control in pregnancy through provision of LLINs and SPs as part of the basic package of ANC services; (b) community-based diagnosis and treatment of malaria; and (c) management, supervision, and behavioral changes to ensure effective utilization; and (ii) community- based nutrition services for pregnant women and young children under five years of age. The second component is strengthening health monitoring and evaluation systems; project management. It has following two sub-components: (i) strengthening the health monitoring and evaluation system, particularly the health management information system (HMIS), including the government's capacity to monitor its MCH programs; and (ii) project coordination and implementation support. Component 1. Supporting health... insurance for the near poor (US$9.1 million at appraisal; US$9.8
million at closing).
Expand coverage and access to health insurance for near-poor households in the Central North Region,
and improve the system’s capacity to manage health insurance, through: (a) subsidizing premiums for the
near poor; (b) conducting social marketing and information, education, and communication (IEC)
campaigns and providing technical assistance to the Health Information and Education Centers of the
Vietnam Social Security Offices (VSSs); and (c) strengthening the institutional capacities for its
administration.
Component 2. Strengthening district health services (US$32.2 million at appraisal; US$29.3 million
at closing).
Improve the capacity of district hospitals to provide basic curative health services to the population, and
strengthen basic public health functions of the District Preventive Health Centers (DPHCs), through: (a)
upgrading medical equipment in the district hospitals; (b) providing equipment and training to 30 DPHCs;
(c) building the technical and administrative blocks of DPHCs; and (d) piloting performance-based
financing mechanisms to incentivize health care providers to perform better and more efficiently.
Component 3. Improving supply and quality of human resources for health (US$15.9 million at
appraisal; US$11.6 at closing).
Strengthen the capacity of existing medical educational institutions, and improve skills and knowledge of
already-practicing medical personnel, through: (a) supporting the transformation of the Nghê An medical
college into a medical university; (b) improving teaching facilities in five other medical colleges; (c) training
district hospitals’ clinical staff, DPHCs’ personnel, and hospital administrators (short-term training, Level 1
specialty training, on-the-job training, and Master’s degree training); and (d) supporting the implementation
of Decision 1816 of the Ministry of Health (MoH), which targeted the rotation of qualified medical personnel
from higher-level facilities to district hospital to temporarily address health personnel shortages and at the
same time ensure the transfer of knowledge and skills to district-level hospital human resources.
Component 4: Project management, Monitoring, and Evaluation (US$7.7 million at appraisal; US$3.9 at closing)
Ensure adequate management structures, processes, and human resource capacities for the project, and
set up mechanisms for effective monitoring of activities and evaluation of results, through financing: (a) the
operation of provincial project management units (PPMUs) at both the central and provincial levels, (b)
health information systems and data collection, (c) M&E and audit mechanisms, and (d) implementation
of the results-based financing (RBF) pilot. The M&E activities consisted of following the project’s
implementation progress and tracking the project’s results indicators and performance of the pilot. (VNM P095275.IDA46880.crs1 Central North Region Health Support Project p.2)

Component One: Strengthening t...he capacity of lower level hospitals to deliver
quality services (estimated US$118.0million of which US$116.0 million IDA): Component
One aims to support the implementation of the government’s Masterplan on Reducing Hospital
Overcrowding14 through increasing the capacity of the health system to provide more and better
quality health services in the project provinces. It would consist of two subcomponents. Subcomponent 1a - Provincial Subprojects for the Development of Medical
Services and Quality Improvement (estimated US$109.0 million of which US$107.0
million IDA). This subcomponent would provide technical and financial support directly
to the Provinces to increase their capacity to deliver quality health services at their
general provincial hospitals or specialized pediatric or obstetrics and gynecology
hospitals as well as at least three district hospitals per province. Subcomponent 1b - National Policy and Central Level Technical Support for
Reducing Hospital Overcrowding and Quality Improvement (estimated US$9.0 million
of which US$9.0 million IDA). This subcomponent would support the MoH and the
other national level stakeholders to provide quality technical support to the Provinces to
achieve the Component goals. Component Two: Reducing the financial barriers to access health services by the
economically vulnerable (US$29.0 million of which US$29.0 million IDA). Component Two
aims to support the implementation of the government’s Masterplan on Universal Health
Coverage (UHC) through reducing the financial barriers to accessing health services,
particularly for the poor and near poor. It would achieve this through three subcomponents: Subcomponent 2a - Direct subsidy to support the purchase of health insurance by
the near poor (estimated US$23.0 million of which US$23.0 million IDA). Subcomponent 2b – Supporting catastrophic health care expenditures at
provincial hospitals (US$2.0 million of which US$2.0 million IDA). Sub-component 2c - Information, Education and Communication (IEC)
support towards increased and effective health insurance coverage (estimated US$4.0
million of which US$4.0 million IDA). Component Three: Project management, monitoring and evaluation (estimated
US$7.0 million of which US$5.0 million IDA) (VNM P122629.IDA52590.crs1 North East and Red River Delta Regions Health System Support Project p.22-25)
