Many prevention of mother-to-child HIV transmission programmes across Africa initiate HIV-infected (HIV positive) pregnant women on lifelong antiretroviral therapy (ART) on the first day of antenatal care (“same-day” initiation ... ). However, there are concerns that same-day initiation may limit patient preparation before starting ART and contribute to subsequent non-adherence, disengagement from care and raised viral load. We examined if same-day initiation was associated with viral suppression and engagement in care during pregnancy.
The data suggest that same-day ART initiation during pregnancy is not associated with lower levels of engagement in care or viral suppression through 12 months post-delivery in this setting, providing reassurance to ART programmes implementing Option B+.

The report was prepared by a World Bank Task Team led by Iris Semini (seconded to the World Bank until July 2013, and now back with UNAIDS), and concluded by Emiko Masaki and Marelize Görgens (World Bank), with support and guidance provided by Daniel Dulitzky, Paolo Belli, Alejandro Cedeno, Alona Goroshko and Lombe Kasonde. Administrative support was provided by Anna Goodman, Mario Mendez and Uma Balasubramanian. When draft results were ready, an in-country workshop was held where stakeholders provided their inputs. on people with urgent health needs. Thirty-six per cent of countries reported disruptions to ambulance services; 32% to 24-hour emergency room services; and 23% to emergency surgeries.
Elective surgeries have also been disrupted in 59% of countries, which can have accumulating consequences on health and well-being as the pandemic continues. Disruptions to rehabilitative care and palliative care were also reported in around half of the countries surveyed.
Major barriers to health service recovery include pre-existing health systems issues which have been exacerbated by the pandemic as well as decreased demand for care.
