- a detailed description of the clinical course of dengue illness, which reflects the dynamic and sys...temic nature of dengue that has crucial bearing on the patient’s management;
- a detailed description of the basic pathophysiological changes of severe dengue (i.e. plasma leakage and hypovolaemia/shock) and guidance on the recognition of these changes and appropriate action of management;
- a brief discussion on WHO classification (1997) and its limitations;
- guidance on the differential diagnoses that can be confused with dengue or vice versa; they were described according to the stage of disease;
- a more focused guide on the disease monitoring in accordance with the dynamic changes as the disease progresses;
- emphasis on the importance of monitoring the plasma leakage (haemodynamic status of the patient, clinical signs of plasma leakage and haematocrit);
- a clearer algorithm for fluid management in cases of severe dengue; and
- emphasis on the importance of recognizing or suspecting significant occult bleed. Keep the facilitator’s manual with you each day as you prepare and deliver the information. Use it as a reference when delivering classroom presentations, but avoid reading directly from it during sessions.

Delivery of comprehensive arrhythmia care requires the simultaneous presence of many resources. These include complex hospital infrastructure, expensive implantable equipment, and expert personnel. In many low- and middle-income countries (LMICs), at least 1 of these components is often missing, res...ulting in a gap between the demand for arrhythmia care and the capacity to supply care. In addition to this treatment gap, there exists a training gap, as many clinicians in LMICs have limited access to formal training in cardiac electrophysiology. Given the progressive increase in the burden of cardiovascular diseases in LMICs, these patient care and clinical training gaps will widen unless further actions are taken to build capacity. Several strategies for building arrhythmia care capacity in LMICs have been described. Medical missions can provide donations of both equipment and clinical expertise but are only intermittently present and therefore are not optimized to provide the longitudinal support needed to create self-sustaining infrastructure. Use of donated or reprocessed equipment (eg, cardiac implantable electronic devices) can reduce procedural costs but does not address the need for infrastructure, including diagnostics and expert personnel. Collaborative efforts involving multiple stakeholders (eg, professional organizations, government agencies, hospitals, and educational institutions) have the potential to provide longitudinal support of both patient care and clinician education in LMICs.

This article describes WHO's efforts to combat a cholera outbreak in South Sudan in July 2017. The organization received 500,000 doses of oral cholera vaccine (OCV) and was working with South Sudan's Ministry of Health to launch a vaccination campaign from July 28 to August 3, 2017. At the time, the... country had reported 17,785 cholera cases and 320 deaths since the outbreak began in June 2016. The vaccination campaign targeted four counties with high transmission rates: Tonj East, Kapoeta South, Kapoeta North, and Kapoeta East. South Sudan was implementing an integrated approach to control cholera, combining patient care, surveillance, social mobilization, water and sanitation improvements, and vaccination. The article notes that approximately 6 million people in South Sudan were facing starvation, with food insecurity and drought exacerbating the risk of cholera spread as people resorted to using contaminated water sources.
