Sustainable Outcomes for Child...ren and Youth in Central and Western Uganda is implementing a household-based response to empower vulnerable children, youths and their caregivers to reduce their vulnerability and improve their wellbeing. The project is implemented in the Southwestern districts of Bushenyi, Rukungiri, Kiruhura, Kabale and Isingiro. The project targets to reach a total of 150453 direct beneficiaries (including vulnerable children and their caregivers) from 24654 households in a period of 3 years. The program activities involving at least 2268 Community volunteers (i.e. Para-social workers, VHTs and Para-legal workers) is implemented at sub-county level covering a total of 83 sub-counties. The project goal is to create an integrated and coordinated service delivery system that will ensure access to essential health, nutrition and social services by OVC, youth and their caregivers resulting in better outcomes and reduced HIV/AIDS vulnerability in the target districts.

The objectives of the activity are 1) Strengthen the capacity of communities and local service providers to support vulnerable families and children, 2) Strengthen the capacity of vulnerable households to care for children, 3) Strengthen the capacity of the public sector social service system to support vulnerable families and children, and 4) Strengthen the capacity of communities to support vulnerable children and their families to access prevention, care and treatment of HIV services. FACT Children Tariro OVC program employs a systemic approach which builds on Department of Child Welfare and Protection Services (DPWPS) strategy and FACTs integrated Community Care of OVC model of offering wrap around services to children and their families.

Canada's international respons...e to the HIV/AIDS epidemic is largely built around the work of the Canadian HIV Vaccine Initiative (CHVI). CHVI proposes to increase the capacity of Canada and low- and middle-income countries to respond to the HIV/AIDS pandemic by developing new HIV vaccines and other preventive approaches; investigating social and policy issues related to HIV/AIDS; conducting clinical trials; and building an HIV vaccine manufacturing facility in Canada. The third component, conducting clinical trials, requires the participation of research institutions from around the world, including those operating in high HIV-prevalence countries. However, such institutions - especially those in Africa - often lack the necessary research capacity to fully engage in clinical trials. In order to address this challenge, the Canadian International Development Agency (CIDA) has allocated CA$ 6 million to the Global Health Research Initiative (GHRI) to support building African capacity for clinical trials. This project will build on work carried out under an earlier effort, Capacity Building for HIV/AIDS Prevention Trials (104051).

This project is a joint initia...tive between the University of Nairobi, Kenya, and the universities of Manitoba and Toronto, Canada. The overall objective is to establish the Kenya Aids Vaccine Initiative (KAVI) as a centre of excellence for training healthcare professionals for HIV vaccine and other prevention trials in East Africa. KAVI will offer training in epidemiology, research design, immunology, good clinical practice, good clinical laboratory practice, data management and cohort establishment. Basic science training will take place in Kenya and Canada, with an emphasis on technology transfer. The project will also endeavor to build vaccine literacy and acceptance on the part of local communities, policymakers and other stakeholders; enhance the capacity of local ethics review boards and institutional administrators to handle issues raised by vaccine trials; and enhance the ability of KAVI to bid in competitions for independent research funding for clinical trials.

The goal of the Large-Team Gra...nts program under the Discovery and Social Research component of the Canadian HIV Vaccine Initiative (CHVI) is to make a significant contribution to HIV vaccine discovery and social research, and to train the next generation of HIV vaccine researchers. Funded teams are expected to strengthen links between researchers in Canada and those in low- and middle-income countries (LMIC) to increase integration and facilitate the sharing of research data and knowledge. The program, as part of the component, makes a significant contribution to research and research capacity focussed on: the discovery of HIV vaccines and related issues, and social and behavioural issues around HIV vaccines. Teams co-led by Canadian and LMIC researchers receive support for projects that build on their relevant skills and expertise and align with the Global Enterprise's Scientific Strategic Plan. / Le programme Subventions de grandes équipes dans le cadre du volet Découverte et recherche sociale de l'Initiative canadienne de vaccin contre le VIH (ICVV) vise à apporter une contribution importante à la découverte d'un vaccin contre le VIH et à la recherche sociale à cet égard, de même qu'à former la prochaine génération de chercheurs qui étudient les vaccins contre le VIH. On s'attend à ce que les équipes financées renforcent les liens entre les chercheurs canadiens et ceux des pays à revenu faible ou moyen pour accroitre l'intégration et faciliter la mise en commun des données de recherche et des connaissances. However, such institutions - especially those in Africa - often lack the necessary research capacity to fully engage in clinical trials. In order to address this challenge, the Canadian International Development Agency (CIDA) has allocated CA$ 6 million to the Global Health Research Initiative (GHRI) to support building African capacity for clinical trials. This project will build on work carried out under an earlier effort, Capacity Building for HIV/AIDS Prevention Trials (104051).

CPED is dedicated to promoting sustainable development and to reducing poverty and inequality through policy-oriented research and active engagement on development issues.CPED's research areas include globalization, democracy and governance, health (particularly HIV/AIDS), population, gender, education, environment, and the development of Nigeria's Niger Delta region. It receives its funding from the Think Tank Initiative (TTI).Quality research with policy influence TTI is a multi-funder program dedicated to strengthening independent policy research institutions, or think tanks, in developing countries. The program aims to enhance their ability to provide sound research that informs and influences policy. This second TTI phase (2014?2019) will fund 43 institutions. This will help them consolidate their role as credible development actors in their countries, and in some cases, regionally and internationally. For CPED, this project will help enhance its research quality, organizational performance, and policy engagement. The International Indigenous W...orking Group on HIV & AIDS (IIWGHA) was established in 2009. From inception IIWGHA has been interested in research as a resource to describe and understand the impact of HIV in Indigenous communities. United by the urgent need to respond to HIV and sexual health across cultures, IIWGHA established a sub-committee to develop and undertake research. After discussing diverse research ideas and agreed priorities for epidemiological research, the PROCESS for moving forward was unclear. How will ownership of research data be shared? What values are shared and what expectations must be met? How will IIWGHA approach research development and implementation? IIWGHA has a strong history of developing strategic documents. This ability to strategize led to the decision to develop a Protocol, to be ratified by all the member countries. A CIHR research grant was received and supported an in-person meeting in Guatemala, hundreds of emails and bi-monthly teleconferences, collaborative writing and English/Spanish translations and interpretation. In December 2017 the Sowing Seeds Protocol: Conducting Research on HIV Among Indigenous Peoples: Values, Approaches and Guidelines was publicly released in English. Spanish and French versions will be completed by March 2018. Parallel to finalization, a large multi-country team have been investigating Indigenous knowledge translation. Serving as a test of the protocol, team leads have referenced our work when guidance is needed. Canada has led IIWGHA's research activities, largely due to a favourable funding environment. Our shared International guidelines will be used to mobilize funders around the world. Therefore, it is essential to share our Protocol inviting more to learn about our values, approaches and guidelines. More resources and background materials will be developed to support the Protocol. IIWGHA's momentum will continue to promote and implement this process.

Viral load (VL) monitoring for... patients with HIV/AIDS is necessary to assess treatment efficacy and confirm suspected treatment failure. Yet in 2015, fewer than 30% of the 15.8 million people accessing antiretroviral therapy had ever had a VL test; this is because routine VL testing is not available in most high-burden HIV countries. When VL testing is available, it is performed in large batches using expensive, infrastructure demanding equipment that prohibit its use at the point-of-care. Delay in receipt of results translates to delayed clinical decision-making, which may lead to worsened clinical outcomes. The Xpert HIV-1 VL assay is a newly developed, automated and rapid test that can be performed easily at the point-of-care. A study I published last year in Lancet Global Health showed that Xpert HIV-1 VL correlates extremely well with a current gold standard assay. However, there is currently no published data on the clinical impact of implementing Xpert HIV-1 Viral Load at the point-of-care. The objective of my current project is to assess the impact of this same-day VL test in a primary healthcare facility in Manipal, India. We hypothesize that the implementation of point-of-care same-day VL testing will decrease the turn-around-time for VL tests, reduce delays in initiation of second-line treatment regimens, and increase the proportion of patients receiving enhanced adherence counselling. Delayed switching to second-line drug regimens among patients with treatment failure contributes significantly to HIV/AIDS-related mortality. Access to point-of-care VL testing may reduce this delay thereby improving clinical outcomes. The knowledge gained from this study will inform national policy on VL testing for HIV-positive patients.

Some pregnant women under PMTC...T, a programme that has been successfully integrated and influenced other maternal and child health services at primary health care (PHC) in South Africa, are also diagnosed with gestational diabetes (GDM). GDM is glucose intolerance recognised for the first time during pregnancy. It increases risk for type 2 diabetes (T2D) for women and their babies. GDM is managed at tertiary level in South Africa but how GDM is managed among HIV infected women has not been studied. Integrating PMTCT with GDM screening and intervention in routine antenatal care (ANC) and postnatal care (PNC) could potentially reduce T2D. This study explored how the PMTCT integration experience in South Africa could bridge gaps in managing GDM and T2D for women and their exposed babies and assessed the possibility of integrating GDM and T2D prevention into the PMTCT cascade at PHC level. Mixed methods were used. PMTCT data of 2012-2017 for Western Cape province, complemented by analysis of policy documents on PMTCT and PNC were analysed along with semi-structured interviews from 10 national and local key informants, 9 clinic managers, nurses and midwives and 10 HIV-infected women diagnosed with GDM (N=29), in Cape Town. GDM and other main non-communicable diseases screenings were not adequately included in both ANC and PNC policies that emphasized only on HIV services. All participants underlined the importance of integrated PMTCT, through which women who tested positive reached 84064 (235% increase) between 2014-2017, while those who delivered under PMTCT increased by 13% and MTCT reduced by 16% between 2012-2017 in Western Cape. All women interviewed wanted their GDM screening and management through PMTCT services. Most of the key informants and clinic staff said that GDM and T2D integration at PHC level could be feasible and can improve experienced quality of care upon more staff recruitment, adequate training and managerial support and infrastructure expansion.

On average 2,000 young women i...n South Africa acquire HIV weekly, representing the highest new acquisition rates globally. In recognition of young women's disproportionate risk of HIV, South Africa has initiated a national campaign entitled She Conquers , which has called for researchers and program implementers to listen to the needs, voices, and priorities of young women. Due to prevailing age, gender, racial and power inequities, young people are too often excluded from the decision-making processes that affect their lives. In response to a growing demand for youth- and client-centered approaches, we have proposed a knowledge translation and exchange (KTE) plan to help address poor engagement of HIV-affected youth in research, including KTE stages. The objectives of this grant are (1) To support a community-collaborative research team to plan and develop a KTE strategy to disseminate research findings to HIV-affected youth, policy makers, care providers, and other change agents in an accessible, inclusive and youth-centred format; (2) To build youth capacity in KTE focused on HIV and sexual and reproductive health; and (3) To support youth to plan, develop, and present KTE strategies that attend to pervasive gender and social inequities in access to information, access to care, and HIV risk and consequence. This proposal is a Canadian-South African collaboration of researchers, youth leaders, and community advocates with expertise and experience in HIV prevention research. As we advance towards the deadline to end AIDS by 2030 , we have a responsibility to better engage with and prioritize needs of communities who are the main participants and targets of HIV prevention efforts. This project is in line with, and can be used to inform, the Canadian government's feminist international assistance policy that recognizes that empowering women and girls is key to promoting gender equity and improving health and well-being around the world.
