This project, which is part of... the renewed Canadian HIV Vaccine Initiative (CHVI), aims to improve country progress towards the elimination of pediatric HIV by identifying community-based interventions with the most potential to scale up prevention of mother-to-child transmission of HIV (PMTCT) and maternal-newborn-child health (MNCH) services in sub-Saharan Africa. These interventions aim to increase demand, uptake and retention of services. The focus on PMTCT under CHVI addresses the immediate prevention needs of those most at risk while researchers pursue a safe and effective vaccine. / Ce projet, qui s inscrit dans l Initiative canadienne de vaccin contre le VIH (ICVV) renouvelée, a pour but d améliorer les progrès réalisés par les pays en vue d éliminer l infection pédiatrique au VIH en déterminant les interventions communautaires qui sont les plus susceptibles d accroître la prévention de la transmission du VIH de la mère à l enfant et la prestation de services en santé des mères, des nouveau-nés et des enfants (SMNE) en Afrique subsaharienne. Ces interventions visent à accroître la demande des services et l intérêt à l égard de ces derniers et de leur maintien. Le fait de mettre l accent sur la prévention de la transmission du VIH de la mère à l enfant dans le cadre de l ICVV contribue à répondre aux besoins immédiats des personnes les plus à risque, pendant que les chercheurs s efforcent de trouver un vaccin sécuritaire et efficace.

This project aims to improve g...lobal, regional and country progress towards the elimination of HIV infection in mothers and children by supporting the Secretariat of the Interagency Task Team on the Prevention and Treatment of HIV Infection in Pregnant Women, Mothers and Children (IATT on PMTCT). The IATT on PMTCT is an inter-agency technical partnership coordinated by UNICEF and the World Health Organization, which aims to improve accountability and increase the use of evidence to support the country-led implementation of the Global Plan Towards the Elimination of New HIV Infections Among Children by 2015 and Keeping their Mothers Alive. This project builds on the results of previous support from Canada towards the IATT on PMTCT for 2012-2014. It assists the IATT Secretariat in carrying out three distinct activities: (1) the management and hosting of virtual learning platforms, including a comprehensive prevention of mother to child transmission of HIV website, webinar series, and virtual discussions; (2) documenting best practices on the prevention of mother to child transmission of HIV, publishing implementation science findings, and updating and disseminating key tools and guidance; and (3) providing in-country technical assistance through IATT working groups and IATT country focal points in line with country needs. This support contributes to DFATD s broader efforts of reducing infant mortality and maternal deaths. / Ce projet a comme objectif d améliorer les progrès réalisés à l échelon mondial, régional et national dans le but d éliminer l infection au VIH chez les mères et les enfants, en appuyant le Secrétariat de l Équipe spéciale interinstitutions (ETII) pour la prévention et le traitement du VIH chez les femmes enceintes, les mères et les enfants. Cette équipe est un partenariat technique interinstitutions coordonné par l UNICEF et l Organisation mondiale de la Santé. Elle vise à accroître la responsabilisation et l utilisation de données probantes à l appui de la mise en oeuvre, par les pays, du Plan mondial pour l élimination des nouvelles infections au VIH chez les enfants à l horizon 2015, et le maintien en vie de leurs mères. Le projet fait fond sur les résultats liés au soutien que le Canada a apporté à l ETII au cours de la période 2012-2014. This project, also known as IN...SPIRE (INtegration and Scaling up Prevention of mother-to-child transmission through Implementation REsearch), aims to directly contribute to the global elimination of the mother-to-child transmission of HIV by providing sustained support in countries with a high prevalence of HIV/AIDS. Specifically, the project aims to: (1) increase the uptake and effective implementation of interventions to prevent mother-to-child transmission of HIV; (2) promote a more integrated and comprehensive approach to the prevention of mother-to-child transmission of HIV; (3) link district-level program effectiveness studies to the strengthening of national programs; and (4) reduce the number of infants becoming infected with HIV and improve the health of HIV-infected mothers. To achieve these objectives, the program supports six research teams (two research teams in Zimbabwe, two in Nigeria, and two in Malawi) that focus on integrating health programming approaches and communications technologies, such as text messaging, for better facility-level follow-up. The project builds on CIDA's longstanding support to the World Health Organization (WHO) HIV/AIDS department, and is part of the Canadian HIV Vaccine Initiative (CHVI). / Ce projet, connu également comme INSPIRE (INtegration and Scaling up Prevention of mother-to-child transmission through Implementation REsearch), vise à contribuer directement à l'élimination mondiale de la transmission du VIH de la mère à l'enfant grâce à un soutien constant offert aux pays à forte prévalence du VIH/sida. Plus particulièrement, le projet vise à : 1) accroitre l'adoption et la mise en oeuvre efficace d'interventions sur la prévention de la transmission du VIH de la mère à son enfant; 2) favoriser une approche plus globale et exhaustive à la prévention de la transmission du VIH de la mère à son enfant; 3) établir des liens entre des études sur l'efficacité des programmes à l'échelle des districts et le renforcement des programmes à l'échelle nationale; 4) réduire le nombre de nouveau-nés qui sont infectés par le VIH et améliorer la santé des mères infectées. Mobile phone technology, drive...n by local market forces rather than foreign assistance, is spreading rapidly through Africa. Starting in 2005, researchers in Nairobi developed a clinic-based program using SMS (text messaging) to support HIV-positive pregnant women taking antiretroviral therapy (ART). The system is currently under evaluation. This project will use a similarly structured mobile phone communications system to improve antenatal care, remind the participants to take their medication, and improve postpartum and neonatal follow-up, even when mothers deliver at home. Researchers will determine whether mobile phone text messages can enhance compliance with a known intervention (use of nevirapine) for preventing mother-to-child transmission of HIV. This will be demonstrated in terms of improved antenatal care (more than four visits), increased use of nevirapine in labour (from 60% to at least 70%), fewer HIV-infected infants born to HIV-positive mothers, and earlier identification and treatment of HIV-positive infants.

The African Development of AID...S Prevention Trials (ADAPT2) capacity building initiative is an African-Canadian partnership that aims to increase the number and quality of HIV prevention trials led by African researchers. Building on experience gained during ADAPT1 - funded by the Global Health Research Initiative (GHRI) - ADAPT2 will mentor researchers in 12 southern African countries through the process of designing, implementing, analyzing the data from and communicating the results of HIV/AIDS prevention trials. Activities will include organizing a series of short courses for researchers on epidemiology and randomized controlled trials; building training capacity (for HIV/AIDS prevention trials) at two African universities (University of Namibia and University of Western Cape) and one nongovernmental organization (CIET); and offering training to policymakers (parliamentarians) on interpreting research findings. In addition to gaining practical experience in ongoing HIV/AIDS-prevention trials, a number of research fellows are expected to obtain graduate degrees as part of the project.

This project is based on the premise that many of the required skills are already present at additional locations, but need further development. The grant will facilitate interaction between developed and developing sites in sub-Saharan Africa, and with North American institutions. It will involve knowledge transfer, infrastructure development (including support for laboratory equipment and data management) and collaborative pilot projects. The results are expected to enhance the design and implementation of African-originated clinical and laboratory studies, and to optimize capacity building in the required areas. They are also expected to provide a foundation of expertise that can be readily extended to studies at other sites, and to the control of other diseases such as tuberculosis and malaria.

The Canada-Africa Prevention T...rials Network (CAPT Network) was formed through a capacity building grant from the Global Health Research Initiative (GHRI). The Network comprises eight African centres (four in Uganda, one in Kenya and three in South Africa) and seven Canadian partner institutions. The first round of GHRI funding served to strengthen African capacity through South-South and North-South Partnerships, infrastructure support and training. Under the new 2010-2014 funding proposal, the Network will enhance the prevention research program at each of the African centres through targeted workshops and training, while continuing to provide infrastructure support for research personnel. This grant will complement the hands-on training with both merit-based academic scholarships for advanced research degrees and mentorship programs, both to be offered on a yearly basis. The aim is to prepare the Network to offer a well-coordinated, African-led research agenda, and to respond to HIV prevention funding opportunities.

This grant will support the de...velopment of HIV/AIDS prevention trial expertise in Botswana, Tanzania and Zambia using existing collaborations between Africa, Canada and the United States. The Botswana-Tanzania-Zambia Capacity Building Network (TanZamBo) is composed of two African institutions with fairly well established facilities for HIV/AIDS prevention trials (Botswana Harvard AIDS Institute Partnership and Kilimanjaro Christian Medical Centre), one institution with an emerging environment for HIV/AIDS prevention trials (Tropical Diseases Research Centre, Ndola, Zambia) and two institutions with advanced facilities for HIV/AIDS prevention research and clinical trials (McGill University, Montreal, and Harvard University School of Public Health, Boston). Team members will use their comparative advantages in terms of expertise and technology to build the capacity of the weaker institutions through learning by doing. By the end of the project, it is expected that TanZamBo will have built a strong cadre of young researchers capable of designing and conducting HIV/AIDS prevention trials, publishing in peer-reviewed journals and competing for HIV/AIDS clinical trials grants, as well as a pool of competent laboratory scientists.

This project aims to improve g...lobal, regional and country progress towards the elimination of HIV infection in mothers and children by supporting the Secretariat of the Interagency Task Team on the Prevention and Treatment of HIV Infection in Pregnant Women, Mothers and Children (IATT on PMTCT). The IATT on PMTCT is an inter-agency technical partnership coordinated by UNICEF and the World Health Organization, which aims to improve accountability and increase the use of evidence to support the country-led implementation of the Global Plan Towards the Elimination of New HIV Infections Among Children by 2015 and Keeping their Mothers Alive. This project builds on the results of previous support from Canada towards the IATT on PMTCT for 2012-2014. It assists the IATT Secretariat in carrying out three distinct activities: (1) the management and hosting of virtual learning platforms, including a comprehensive prevention of mother to child transmission of HIV website, webinar series, and virtual discussions; (2) documenting best practices on the prevention of mother to child transmission of HIV, publishing implementation science findings, and updating and disseminating key tools and guidance; and (3) providing in-country technical assistance through IATT working groups and IATT country focal points in line with country needs. This support contributes to DFATD s broader efforts of reducing infant mortality and maternal deaths. / Ce projet a comme objectif d améliorer les progrès réalisés à l échelon mondial, régional et national dans le but d éliminer l infection au VIH chez les mères et les enfants, en appuyant le Secrétariat de l Équipe spéciale interinstitutions (ETII) pour la prévention et le traitement du VIH chez les femmes enceintes, les mères et les enfants. Cette équipe est un partenariat technique interinstitutions coordonné par l UNICEF et l Organisation mondiale de la Santé. Elle vise à accroître la responsabilisation et l utilisation de données probantes à l appui de la mise en oeuvre, par les pays, du Plan mondial pour l élimination des nouvelles infections au VIH chez les enfants à l horizon 2015, et le maintien en vie de leurs mères. Le projet fait fond sur les résultats liés au soutien que le Canada a apporté à l ETII au cours de la période 2012-2014. This project, also known as IN...SPIRE (INtegration and Scaling up Prevention of mother-to-child transmission through Implementation REsearch), aims to directly contribute to the global elimination of the mother-to-child transmission of HIV by providing sustained support in countries with a high prevalence of HIV/AIDS. Specifically, the project aims to: (1) increase the uptake and effective implementation of interventions to prevent mother-to-child transmission of HIV; (2) promote a more integrated and comprehensive approach to the prevention of mother-to-child transmission of HIV; (3) link district-level program effectiveness studies to the strengthening of national programs; and (4) reduce the number of infants becoming infected with HIV and improve the health of HIV-infected mothers. To achieve these objectives, the program supports six research teams (two research teams in Zimbabwe, two in Nigeria, and two in Malawi) that focus on integrating health programming approaches and communications technologies, such as text messaging, for better facility-level follow-up. The project builds on CIDA's longstanding support to the World Health Organization (WHO) HIV/AIDS department, and is part of the Canadian HIV Vaccine Initiative (CHVI). / Ce projet, connu également comme INSPIRE (INtegration and Scaling up Prevention of mother-to-child transmission through Implementation REsearch), vise à contribuer directement à l'élimination mondiale de la transmission du VIH de la mère à l'enfant grâce à un soutien constant offert aux pays à forte prévalence du VIH/sida. Plus particulièrement, le projet vise à : 1) accroitre l'adoption et la mise en oeuvre efficace d'interventions sur la prévention de la transmission du VIH de la mère à son enfant; 2) favoriser une approche plus globale et exhaustive à la prévention de la transmission du VIH de la mère à son enfant; 3) établir des liens entre des études sur l'efficacité des programmes à l'échelle des districts et le renforcement des programmes à l'échelle nationale; 4) réduire le nombre de nouveau-nés qui sont infectés par le VIH et améliorer la santé des mères infectées. Pour atteindre ces objectifs, le programme appuie six équipes de recherche (deux au Zimbabwe, deux en Norvège et deux au Malawi) dont le travail est axé sur l'intégration des programmes de santé et des technologies de communication, notamment la messagerie texte, pour assurer un meilleur suivi médical dans chaque établissement. This project, which is part of... the renewed Canadian HIV Vaccine Initiative (CHVI), aims to improve country progress towards the elimination of pediatric HIV by identifying community-based interventions with the most potential to scale up prevention of mother-to-child transmission of HIV (PMTCT) and maternal-newborn-child health (MNCH) services in sub-Saharan Africa. These interventions aim to increase demand, uptake and retention of services. The focus on PMTCT under CHVI addresses the immediate prevention needs of those most at risk while researchers pursue a safe and effective vaccine. / Ce projet, qui s inscrit dans l Initiative canadienne de vaccin contre le VIH (ICVV) renouvelée, a pour but d améliorer les progrès réalisés par les pays en vue d éliminer l infection pédiatrique au VIH en déterminant les interventions communautaires qui sont les plus susceptibles d accroître la prévention de la transmission du VIH de la mère à l enfant et la prestation de services en santé des mères, des nouveau-nés et des enfants (SMNE) en Afrique subsaharienne. Ces interventions visent à accroître la demande des services et l intérêt à l égard de ces derniers et de leur maintien. Le fait de mettre l accent sur la prévention de la transmission du VIH de la mère à l enfant dans le cadre de l ICVV contribue à répondre aux besoins immédiats des personnes les plus à risque, pendant que les chercheurs s efforcent de trouver un vaccin sécuritaire et efficace.

This project aims to support B...urundi's national AIDS policy by helping to ''Prevent Mother-to-Child HIV Transmission'' (PMCT) in 4 communities. To do so, the project is strengthening existing health services by opening a PMCT specialized health centre. It focuses on ensuring effective health services for HIV-positive pregnant and breastfeeding women, and awareness of the importance of avoiding transmission of HIV from mothers to their children. Activities include: establishing a program with a comprehensive monitoring plan for the prevention of mother-to-child HIV transmission; training female beneficiaries in health, hygiene and nutrition; training home care workers; and, broadcasting radio announcements concerning mother-to-child HIV transmission. Reaching more than 150 HIV-positive pregnant or breastfeeding women and 150 infants, and providing training to 15 home care workers, the project is in line with the priorities of the Government of Burundi's national health plan. L'AMIE is working in partnership with ''Association de prise en charge des orphelins du sida'' (APECOS) to implement this project. This project is part of Canada's Maternal, Newborn and Child Health commitment. / Le projet vise à appuyer la politique nationale relative au VIH/sida du Burundi, en aidant à réduire la transmission du VIH de la mère à l'enfant dans quatre communautés du pays. This grant will support the de...velopment of HIV/AIDS prevention trial expertise in Botswana, Tanzania and Zambia using existing collaborations between Africa, Canada and the United States. The Botswana-Tanzania-Zambia Capacity Building Network (TanZamBo) is composed of two African institutions with fairly well established facilities for HIV/AIDS prevention trials (Botswana Harvard AIDS Institute Partnership and Kilimanjaro Christian Medical Centre), one institution with an emerging environment for HIV/AIDS prevention trials (Tropical Diseases Research Centre, Ndola, Zambia) and two institutions with advanced facilities for HIV/AIDS prevention research and clinical trials (McGill University, Montreal, and Harvard University School of Public Health, Boston). Team members will use their comparative advantages in terms of expertise and technology to build the capacity of the weaker institutions through learning by doing. By the end of the project, it is expected that TanZamBo will have built a strong cadre of young researchers capable of designing and conducting HIV/AIDS prevention trials, publishing in peer-reviewed journals and competing for HIV/AIDS clinical trials grants, as well as a pool of competent laboratory scientists.

The AIDS epidemic continues to... affect Africa more than any other continent, and HIV/AIDS prevention remains a global health priority. This project will support continued HIV/AIDS research and information-sharing at the Afri-Can Forum 2.0 in February 2015.Life-saving researchThe forum will allow researchers who are part of the HIV/AIDS Prevention Trials Capacity Building Grants Phase 2 program to exchange results with Canadian Institutes of Health Research grantees and others, such as participants from the Bill & Melinda Gates Foundation and the European and Developing Countries Clinical Trials Partnership. The Global Health Research Initiative (GHRI) manages the program with funding from the Foreign Affairs, Trade and Development Canada. The goal is to help create sustainable African skills, knowledge, and leadership to conduct future HIV/AIDS prevention trials. This program seeks to support collaboration among African researchers and research users in HIV/AIDS prevention trials. It also aims to increase global HIV/AIDS prevention trial collaboration among Canadian and African researchers and institutions. About the Afri-Can ForumThis project builds on the success of the first Afri-Can Forum attended by close to 130 participants, including representatives from research organizations and international donors. The Wits Health Consortium in South Africa will organize this second forum, which features plenary and abstract presentations, panel discussions, and poster viewing sessions. Participants will share the results of GHRI-supported research. The forum will serve to reinforce inter-regional collaboration for greater HIV prevention impact.

Young women in Botswana bear t...he brunt of new HIV infections. This project aims to reduce the number of new HIV cases, particularly among women aged 15 to 29. While HIV/AIDS has affected most regions in the world, sub-Saharan Africa has felt its impact most severely, both in terms of lives lost and the economic and social consequences. Botswana has the second highest prevalence of HIV in the world after Swaziland. The country has made great strides in stopping the spread of HIV/AIDS. It was the first country in sub-Saharan Africa to implement a prevention of mother-to-child transmission program in 1999. It has since added other HIV/AIDS prevention and treatment programs. As a result, HIV infection rates have come down, particularly among children. Among adults, however, HIV incidence rates remain high. There are some 15,000 new cases annually despite a massive investment in biomedical and behavioural prevention. Women have a prevalence rate that is 10% higher than men. Building on results This project builds on previous research in Botswana which showed how interventions focused on the structural causes of HIV, such as poverty, poor education, and gender violence, can substantially reduce HIV infection among young women. The underlying premise: by addressing these factors, vulnerable groups can then choose and benefit from traditional prevention programs. The Government of Botswana took notice of these findings. It is actively restructuring and scaling up its existing national poverty programs to target those most vulnerable to HIV through district-wide interventions. These focus on: -providing social and economic support to young women; -coordinating existing structural support and HIV prevention programs in favour of the HIV vulnerable; and, -engaging the community to develop an enabling environment. Reducing poverty key to reducing HIV risk The project will establish a randomized control trial design. Researchers will test the impact of Botswana's poverty alleviation program on HIV risk by comparing the impacts on young women in randomly selected intervention districts to those in districts without the intervention. The project will generate and apply lessons for the poverty alleviation program's ongoing rollout. CIET Trust and the National AIDS Coordinating Agency will lead the project and work in close collaboration with Botswana's Ministries of Health and Local Government. The research results will help strengthen inter-sector linkages that address the risk factors of people most vulnerable to HIV. The trial results will influence how structural interventions are established in other settings to address the HIV epidemic.

These focus on: -providing social and economic support to young women; -coordinating existing structural support and HIV prevention programs in favour of the HIV vulnerable; and, -engaging the community to develop an enabling environment. Reducing poverty key to reducing HIV risk The project will establish a randomized control trial design. Researchers will test the impact of Botswana's poverty alleviation program on HIV risk by comparing the impacts on young women in randomly selected intervention districts to those in districts without the intervention. The project will generate and apply lessons for the poverty alleviation program's ongoing rollout. CIET Trust and the National AIDS Coordinating Agency will lead the project and work in close collaboration with Botswana's Ministries of Health and Local Government. The research results will help strengthen inter-sector linkages that address the risk factors of people most vulnerable to HIV. The trial results will influence how structural interventions are established in other settings to address the HIV epidemic.
