The HIV/AIDS Asia Regional Pro...gram (HAARP) focuses on the prevention of HIV transmission associated with injecting drug use. HAARP comprises two layers of activity focused on HIV and drug use: at the national level in five countries, namely Burma, Cambodia, China (Yunnan and Guangxi), Laos and Vietnam; and at the regional level to support local-specific, technically sound harm reduction responses to HIV risk associated with drug use as part of national HIV programs. The total value of this initiative is $38 million over 10 years, starting 2005-06.

They provide this information to 2 specific target groups who are vulnerable to HIV/AIDS and STDs, namely female sex workers and lorry drivers. In addition, the members of staff of the clinic also treat STDs, test people for HIV and syphilis, and offer contraception. The specific goal of this project is to improve the access to high quality services for sexual and reproductive health and rights for female sex workers and their clients. The target group of the project consists of more than 4,500 female sex workers in the Tete region (Mozambique) and their clients, mainly long -distance lorry drivers. Female sex workers run a high risk of contamination by HIV/AIDS and disorders such as syphilis and gonorrhoea, but they often also fall victim to sexual violence, unwanted pregnancy, harmful vaginal practices and cervical cancer. Female sex workers are difficult to reach and have very limited access to social, legal and health services. These women are highly discriminated and stigmatised by their activities as sex worker. - Results: Year 1: The clinic and peer educator activities continued without major problems during the whole yearThe data collection and data entry of all components of the situational analysis (key informant interviews, in -depth interviews, focus group discussions and cross -sectional survey) is completed. An individual client monitoring system for FSW was developed and is operational. 15 FSW peer educators were identified and trained Contacts have been made with all other FSW programmes/ associations in the country and a first network meeting was held Year 2:The clinic and peer educator activities continued without major problems during the whole yearThe situational analysis was completed and the results were disseminated verbally (presentations) and in writing (report), at provincial and national levelThe final package of services to be offered at the clinic was defined and the clinic staff trained in the provision of these servicesA second FSW peer educator training was held on issues that arose from the situational analysisEstablishment -based educational activities have been introduced in at least 50% of establishmentsCouple counseling of FSW and their regular partners has started at the clinicA structure that represents FSW and defends their rights has been createdSkills building workshops were held with all FSW that are member of the structureSensitization workshops or meetings were held with all gate keepers, establishment managers, law enforcers, local government authorities and people of influence in the local communityA partnership was developed with at least one civil society organization for micro -financing and income -generation programs Year 3:The clinic, peer educator and structural activities continued without major problems during the whole yearThe results of the situational an

more
|
-Global objective: PSI/Mozambique will work with the Ministry of Health (MOH) in Tete Province to select two health facilities to scale up access to safe, quality adult male circumcision (MC) services among HIV negative men. MC services will help prevent the transmission of HIV between women and men as well as other sexually transmitted infections (STIs). PSI will train existing health providers, including nurses, doctors and health technicians, within the select MOH facilities to improve their capacity to provide minor surgical procedures, including MC. The MOH staff will be supported by 1-2 PSI staff during the first two years of the project to provide the entire MC package to ensure quality service delivery and to meet demand. The MC package will also include group education, individual HIV counseling and testing, MC counseling and a 2- and 7- day check up and with condom access and counseling. The proposed project will reach over 15,000 men over two years and avert over 16,800 disability adjusted life years (DALYs) and avert 836 cases of HIV -Specific objective : 15000 besnijdenissen uitgevoerd 836 gevallen van hiv opgespoord. PSI¿s work includes: managing high-quality, efficiency models to delivery MC services at public and private health centers; developing and implementing communication strategies and materials to create informed demand; providing client-centered HIV and MC counseling services to reduce post-procedure risk compensation; and developing and delivering MC kits to streamline procurement and reduce the risk of procedure-related infections. -

REDUCING HIV RISK IN BOTSWANA:... A NATIONAL CLUSTER RANDOMIZED CONTROLLED TRIAL

Reducing HIV Risk in Botswana:... A National Cluster Randomized Controlled Trial

Young women in Botswana bear t...he brunt of new HIV infections. This project aims to reduce the number of new HIV cases, particularly among women aged 15 to 29.While HIV/AIDS has affected most regions in the world, sub-Saharan Africa has felt its impact most severely, both in terms of lives lost and the economic and social consequences. Botswana has the second highest prevalence of HIV in the world after Swaziland. The country has made great strides in stopping the spread of HIV/AIDS. It was the first country in sub-Saharan Africa to implement a prevention of mother-to-child transmission program in 1999. It has since added other HIV/AIDS prevention and treatment programs. As a result, HIV infection rates have come down, particularly among children. Among adults, however, HIV incidence rates remain high. There are some 15,000 new cases annually despite a massive investment in biomedical and behavioural prevention. Women have a prevalence rate that is 10% higher than men.Building on results This project builds on previous research in Botswana which showed how interventions focused on the structural causes of HIV, such as poverty, poor education, and gender violence, can substantially reduce HIV infection among young women. The underlying premise: by addressing these factors, vulnerable groups can then choose and benefit from traditional prevention programs. The Government of Botswana took notice of these findings. It is actively restructuring and scaling up its existing national poverty programs to target those most vulnerable to HIV through district-wide interventions. These focus on:-providing social and economic support to young women;-coordinating existing structural support and HIV prevention programs in favour of the HIV vulnerable; and,-engaging the community to develop an enabling environment. Reducing poverty key to reducing HIV riskThe project will establish a randomized control trial design. Researchers will test the impact of Botswana's poverty alleviation program on HIV risk by comparing the impacts on young women in randomly selected intervention districts to those in districts without the intervention. The project will generate and apply lessons for the poverty alleviation program's ongoing rollout.CIET Trust and the National AIDS Coordinating Agency will lead the project and work in close collaboration with Botswana's Ministries of Health and Local Government. The research results will help strengthen inter-sector linkages that address the risk factors of people most vulnerable to HIV. The trial results will influence how structural interventions are established in other settings to address the HIV epidemic.

This project, which is part of... the renewed Canadian HIV Vaccine Initiative (CHVI), aims to improve country progress towards the elimination of pediatric HIV by identifying community-based interventions with the most potential to scale up prevention of mother-to-child transmission of HIV (PMTCT) and maternal-newborn-child health (MNCH) services in sub-Saharan Africa. These interventions aim to increase demand, uptake and retention of services. The focus on PMTCT under CHVI addresses the immediate prevention needs of those most at risk while researchers pursue a safe and effective vaccine.
