Center and its partners propose to collaborate with Christian Hospital Association of Malawi (CHAM) to achieve the following overall goal: To improve the quality of HIV care in Malawi by increasing the capacity of CHAM facilities to deliver care, sustained by integrating and strengthening linkages within the continuum of care, innovative mentored training to develop a model for workforce training, and through the performance of operational research to inform the future of the Malawi National programs and policies. The goal will be achieved through three complementary objectives: 1. Strengthening the continuum of care between different health services and between facilities and communities. 2. Develop Zonal Mentoring Teams. 3. Create a consortium of sites for operational research.

The Clinical and Community HIV.../AIDS Services Strengthening (CHASS) project is designed to improve the quality, coverage and effectiveness of high-impact, evidence-based HIV/AIDS interventions in four provinces in Mozambique: Manica, Niassa, Sofala and Tete. The project, which is funded by the U.S. Agency for International Development (USAID), works to ensure that the right HIV/AIDS interventions are implemented at the right time and in the right places, to contribute to the goal of controlling the country’s HIV epidemic.
CHASS will pursue these objectives across all four provinces:
Increase coverage of antiretroviral treatment (ART) to 90 percent
Increase retention of patients on ART to 80 percent at 12 months and 70 percent at 36 months
Increase the average CD4 count at initiation of ART from 350 to 500 cells per cubic millimeter
Complete tuberculosis treatment for 90 percent of patients with HIV who are diagnosed with tuberculosis
Operationalize viral load testing
In partnership with local organizations, district health teams, provincial health teams and the Ministry of Health, CHASS combines innovation, technical capacity strengthening of health facilities and community-based organizations, direct support for quality and integrated HIV/AIDS service delivery, and health systems strengthening at provincial and district levels. In line with USAID Forward principles, CHASS places more emphasis on technical assistance to the host government and local organizations to enhance local ownership of the national response. Additionally, the project supports the expansion of HIV prevention and treatment services in high-prevalence areas and among high-risk population groups. This approach aims to close existing coverage gaps and increase access by saturating services in districts and communities where HIV prevalence and unmet need are high.
FHI 360 is the prime partner of the initiative, which includes partnerships with Howard University, departments and agencies within the Mozambique Ministry of Health, and several regional and national nongovernmental organizations.

PEPFAR: HIV/AIDS - All health ...facility-based care for HIV-exposed children and their families aimed at extending and optimizing quality of life for HIV-infected clients and their families throughout the continuum of illness through provision of clinical, psychological, spiritual, social and prevention services. Clinical care should include early infant diagnosis, prevention and treatment of OIs (excluding TB) and other HIV/AIDS-related complications including malaria and diarrhea (providing access to commodities such as pharmaceuticals, insecticide-treated nets, safe water interventions and related laboratory services), pain and symptom relief, and nutritional assessment and support including food. Other services psychological, social, spiritual and prevention services should be provided as appropriate. Pediatric care and support services should be counted if they are provided at a facility; community services should be included within programs for orphans and vulnerable children (OVC). It is important that funding for pediatric care activities is not double-counted in OVC. - Assistance for HIV/AIDS

The BroadReach Healthcare (BRH...C) program supports the following objectives in support of GoSA: 1) improve HIV-related population outcomes by strengthening health and patient management systems at the district level; 2) build capacity of District Health Management Teams (DHMT)in coordination with provinces to support HIV/TB services and HSS; 3) support development and implementation of GoSA policies for HIV-related interventions; and 4) support transition to a sustainable national comprehensive HIV/TB program. The objectives and TA delivered are aligned to the USG-GoSA PFIP, NSP, NSDA, PHC Reengineering program, NCS and NHI objectives. The BRHC Team supports the following districts: Ekurhuleni (GP), Sisonke (KZN), West Coast (WC), Bojanala (NW), and Ngaka Modiri Molema (NW) covering a total population of over 6,000,000.

FPD is implementing the Compre...hensive District Health Team Support (CDHTS) TA model. The goal is to foster a public sector management culture that is based on effective leadership by professional managers who are committed to: providing integrated high quality services based on GoSA policy; working in partnership with civil society; using strategic information to guide decision making; and improving the quality of services and patient outcomes. In partnership with GoSA, FPD will: i) develop and inform strategies to help Districts realize the 10 Point Plan, including NSP on HIV/AIDS, PHC Re-Engineering Strategy, NHI and the Minister of Healths NSDA in line with the PIPF; ii) support district management to draft, implement and monitor progress against District Health Plans, related plans and expenditure reviews; and iii) support districts to achieve and maintain targeted levels of performance for PEPFARs priority areas, in particular TB/HIV.

This programme targets adolesc...ents living with HIV (ALHIV) between 10-19 years of age within region F of the City of Johannesburg and the Matlosana sub-district of the North West Province. The goal is to in collaboration with the Departments of Health, Basic Education and other partners to develop, pilot, evaluate and support the scale-up of a replicable model of targeted and linked interventions across clinics, schools and communities (the 3 streams of the PHC re-engineering programme) to improve the screening, diagnosis, treatment, adherence, retention in care, and psychosocial and mental health of ALHIV. The programme is highly aligned with the HIV & AIDS and TB National Strategic Plan 2012-2016, the National Youth Policy 2009-2014, the Draft Adolescent Youth Health Policy and the Integrated Schools Health Policy.

The purpose of INROADS is to p...romote innovation to improve patient outcomes in support of the NSP. The framework for INROADS is based on NSP Strategic Objective 3, which aims to reduce deaths and disability from HIV and TB through universal access to diagnosis, care, and treatment. By using the NSP as a guide for identifying priority activities, this framework ensures that program results will be relevant and useful to the GoSA, as well as to PEPFAR and other stakeholders. The INROADS framework identifies five technical focus areas for data analysis, program evaluation, cost-effectiveness analysis, policy recommendations, and capacity strengthening.
